Competition is often cited as a central force that affects the distribution and performance of organisms. Ecological stoichiometry is the balance of elements within animal bodies that can be affected by resource acquisition and processing, as well as by intra‐ or interspecific interactions. Though relatively underexplored for mosquitoes, stoichiometry may provide a wealth of information linking ecological interactions to body nutrient content, and potentially on to pathogen transmission. Detritus, which often varies in nutrient content, forms the base of the food web within the small aquatic habitats occupied by larval mosquitoes, and detrital nutrient content can alter mosquito growth, survival, and population growth. The invasive mosquitoes Aedes albopictus (Diptera: Culicidae) and Aedes aegypti (Diptera: Culicidae) interact as larvae in aquatic systems, often altering their adult populations. Herein, we investigated how different detritus combinations as well as how intra‐ and interspecific densities of Ae. albopictus and Ae. aegypti would affect coexistence; we also measured how nutrient composition (carbon and nitrogen) and stoichiometry (C:N) of adults would vary with those interactions. Ae. albopictus survival, population growth, and stoichiometry were not affected by intra‐ or interspecific competition; nutrient values did vary with detritus ratios. However, Ae. aegypti nutrient content and stoichiometry and survival were negatively affected within the lowest nutrient environments in the presence of Ae. albopictus, but in the highest nutrient environments, both species showed high survival rates and population growth. This is the first study to show that adult mosquito body nutrients can be altered by interspecific interactions, and as nutrient content in adults has been linked to pathogen transmission, it provides a novel role of competition in affecting disease dynamics. Understanding the environmental effects of chemicals used in pest control on aquatic systems is crucial to ensure the conservation of beneficial non-target organisms. Mosquito larvicides are commonly applied to aquatic habitats; however, their non-target effects are not well understood. Our goal was to determine lethal and behavioral effects of insect growth regulators (IGRs) and surface films (SFs) on predaceous diving beetle adults and larvae, and damselfly and dragonfly nymphs in roadside ditch habitats. Behavioral effects were determined via bioassays quantifying predator movement, location, and larval mosquito predation. Using Principal Components Analyses, correlated behaviors were determined for each taxa, and compared within SF and IGR treatment levels. Surface films were lethal to beetle adults in the genus Laccophilus (Dytiscidae) at recommended and high concentrations. Laccophilus adult behavior changed in response to IGRs, those exposed to recommended and high concentrations were more active than controls. Larval Laccophilus mosquito consumption varied between SFs and IGRs. We conclude that SFs can harm aquatic organisms that require atmospheric oxygen, and both larvicides may have sublethal effects on hunting behaviors of aquatic insects. Background: The exact number of mosquito species relevant to human health is unknown, posing challenges in understanding the scope and breadth of vector–pathogen relationships, and how resilient mosquito vector–pathogen networks are to targeted eradication of vectors. Methods: We performed an extensive literature survey to determine the associations between mosquito species and their associated pathogens of human medical importance. For each vector–pathogen association, we then determined the strength of the associations (i.e., natural infection, lab infection, lab dissemination, lab transmission, known vector). A network analysis was used to identify relationships among all pathogens and vectors. Finally, we examined how elimination of either random or targeted species affected the extinction of pathogens. Results: We found that 88 of 3578 mosquito species (2.5%) are known vectors for 78 human disease-causing pathogens; however, an additional 243 species (6.8%) were identified as potential or likely vectors, bringing the total of all mosquitos implicated in human disease to 331 (9.3%). Network analysis revealed that known vectors and pathogens were compartmentalized, with the removal of six vectors being enough to break the network (i.e., cause a pathogen to have no vector). However, the presence of potential or likely vectors greatly increased redundancies in the network, requiring more than 41 vectors to be eliminated before breaking the network. Conclusion: Although < 10% of mosquitoes are involved in transmitting pathogens that cause human disease, our findings point to inherent robustness in global mosquito vector–pathogen networks. Animals with complex life cycles have traits related to oviposition and juvenile survival that can respond to environmental factors in similar or dissimilar ways. We examined the preference-performance hypothesis (PPH), which states that females lacking parental care select juvenile habitats that maximize fitness, for two ubiquitous mosquito species, Aedes albopictus and Culex quinquefasciatus. Specifically, we examined if environmental factors known to affect larval abundance patterns in the field played a role in the PPH for these species. We first identified important environmental factors from a field survey that predicted larvae across different spatial scales. We then performed two experiments, the first testing the independent responses of oviposition and larval survival to these environmental factors, followed by a combined experiment where initial oviposition decisions were allowed to affect larval life history measures. We used path analysis for this last experiment to determine important links among factors in explaining egg numbers, larval mass, development time, and survival. For separate trials, Aedes albopictus displayed congruence between oviposition and larval survival, however C. quinquefasciatus did not. For the combined experiment path analysis suggested neither species completely fit predictions of the PPH, with density dependent effects of initial egg number on juvenile performance in A. albopictus. 1. The preference-performance hypothesis ( PPH) predicts that organisms lacking parental care should oviposit in habitats that optimise offspring performance. Preference-performance relationships were investigated for the Asian tiger mosquito ( Aedes albopictus Skuse) and the southern house mosquito ( Culex quinquefasciatus Say) ( Diptera: Culicidae), two medically important container-breeding species, in response to an organic chemical blend mimicking decaying plant matter. Additionally, the effects of long-term laboratory colonisation of Cx. quinquefasciatus using wild and laboratory strains were evaluated. 2. Oviposition bioassays were conducted by releasing gravid mosquitoes into field enclosures with automobile tires containing low and high concentrations of the chemical blend, and water controls. The offspring were then reared in water collected from the tires in which they were deposited. 3. Aedes albopictus and wild Cx. quinquefasciatus laid more eggs in the chemical blend than water controls but did not differentiate between the low and high concentrations. Conversely, laboratory Cx. quinquefasciatus only preferred the high concentration to the low concentration. No statistical associations between oviposition preference and larval survival were found, as the chemical blend did not affect survivorship of either species. 4. The oviposition preference for the chemical blend over water controls suggests that both species oviposit in the best available resource environment, but further studies are needed before conclusions regarding preference-performance relationships can be drawn. 5. Study Objective, Design, Setting, Participants, Interventions, and Main Outcome Measures Bleeding disorders (BD) occur in up to 50% of adolescents with heavy menstrual bleeding (HMB). This presents unique challenges to health care providers because of the complexity of treating the condition and such complexity can result in difficulty with patients understanding basic information about their condition, limit communication with medical providers, and patient compliance. The aim of the study was to use an electronic approach to enhance patient compliance with medications used to treat their HMB, and to provide educational access to adolescents with BD. This was a prospective cohort study involving patients in a Young Women's Bleeding Disorder Clinic at a single children's hospital. Subjects were given an iPod Touch (Apple Inc, Cupertino, CA) device (ITD), preloaded with the iPeriod (Winkpass Creations) application. Participants recorded information about their BD that they learned about on BD Web sites, and menses, and medications. Electronic and charted data were collected to monitor compliance with prescribed treatment regimens. All ITD allowed Wi-Fi access to allow teens to explore BD Web sites and knowledge was assessed. Results Twenty-three of 45 subjects completed the study. The mean age was 14.1 ± 1.9 years. Subjects who were compliant with the ITD (group 1), charted on baseline symptoms, menstrual flow (83.3%), cramps (100%, 23/23), breakthrough bleeding (95.6%, 22/23), mood (95.6%, 22/23), and medication use (91.7%) for a mean of 9.3 ± 3.1 months. Subjects who were nonusers (group 2) did not report on symptoms, their condition, or medication use in the device (n = 22). More than 75% (17/23) of subjects in group 1 used hormones alone or hormones with antifibrinolytic agents to control HMB. No subjects stopped or missed medications who were in group 1 intentionally, and also there were 9 enrollees within this same group who missed a medication related to awaiting the prescription to be filled from pharmacy. In group 2, 17 enrollees missed medications, resulting in 19% (4/22) of these enrollees being admitted to hospital for 1-2 days. In addition, enrollees in group 2 missed more medications on average compared with group 1. No subjects in group 1 required admission for HMB treatment failure during the study period, compared with those in group 2 ( P = .006). All subjects in group 1 reported accessing Web sites using their ITD to learn about their BD. Groups 1 and 2 did not differ in the number of medications that were prescribed during the time frame ( P = .77) or the number of follow-up clinic visits ( P = .49). Furthermore, those in group 1 reported fewer breakthrough bleeding episodes than those in group 2 according to clinic notes ( P = .03). Users of the ITD were given a set of knowledge questions. Group 2 subjects were not consistent users of the ITD use and did not complete the knowledge questions. Group 1 and 2 could not be compared with regard to knowledge as a result. Conclusion ITD is an excellent tool for adolescents with HMB and BD to allow self-monitoring, provider monitoring, and improve educational access through engaging technology; compliance with device use was associated with several parameters suggestive of improved clinical outcomes. Increasing global temperatures may affect many ectotherms, including insects, although increasing temperatures are thought to benefit future populations through effects on adult size, fecundity, or populations. However, the way that temperature may interact with photoperiod is not well understood. We study this problem using the Asian tiger mosquito Aedes albopictus, an important worldwide invasive whose future spread is thought to be affected by changes in climate. We investigated how mass at maturity varied with temperature (21°C, 25°C) across short and long photoperiods, using laboratory populations from the extreme ends of this species' current US range (Florida, New Jersey). These values were used to parametrize a model to predict optimal mass based on development times; the results of a second laboratory experiment under the same treatments were compared to model predictions. Warmer conditions shortened development times in females from all locations leading to either higher or lower mass depending on the photoperiod. We then used published mass-fecundity relationships to determine the consequences of mass on fecundity under our conditions. Under the majority of scenarios warming decreased predicted fecundity under long photoperiods, but proved beneficial under short photoperiods because the costs of fast development were offset by increased survival in the face of late-season freezing risk. However, fecundity was always low under short photoperiods, so the marginal benefit of warming appears negligible given its cost under long photoperiods when the majority of reproduction occurs. Thus, with northern range expansion, where colder weather currently limits this species, detrimental effects of warming on fecundity are likely, similar to those identified for mass. Unlike previous work that has shown benefits of a warming planet to insects like Aedes albopictus, our work predicts lower performance under warming conditions in summer across the current range, a prediction with implications for range expansion, disease dynamics and populations. Allocation patterns of carbon and nitrogen in animals are influenced by food quality and quantity, as well as by inherent metabolic and physiological constraints within organisms. Whole body stoichiometry also may vary between the sexes who differ in development rates and reproductive allocation patterns. In aquatic containers, such as tree holes and tires, detrital inputs, which vary in amounts of carbon and nitrogen, form the basis of the mosquito-dominated food web. Differences in development times and mass between male and female mosquitoes may be the result of different reproductive constraints, which could also influence patterns of nutrient allocation. We examined development time, survival, and adult mass for males and females of three co-occurring species, Aedes albopictus, Ae. aegypti, and Culex quinquefasciatus, across environments with different ratios of animal and leaf detritus. We quantified the contribution of detritus to biomass using stable isotope analysis and measured tissue carbon and nitrogen concentrations among species and between the sexes. Development times were shorter and adults were heavier for Aedes in animal versus leaf-only environments, whereas Culex development times were invariant across detritus types. Aedes displayed similar survival across detritus types whereas C. quinquefasciatus showed decreased survival with increasing leaf detritus. All species had lower values of 15N and 13C in leaf-only detritus compared to animal, however, Aedes generally had lower tissue nitrogen compared to C. quinquefasciatus. There were no differences in the C:N ratio between male and female Aedes, however, Aedes were different than C. quinquefasciatus adults, with male C. quinquefasciatus significantly higher than females. Culex quinquefasciatus was homeostatic across detrital environments. These results allow us to hypothesize an underlying stoichiometric explanation for the variation in performance of different container species under similar detrital environments, and if supported may assist in explaining the production of vector populations in nature. Freshwater predatory insects can exert strong effects on prey, although how multiple similar predators may coexist is not well understood. Larval predaceous diving beetles are often numerically and taxonomically abundant predators in lentic systems, but the proximate mechanisms that explain their high abundance remain unknown. Field surveys were conducted twice in June in ponds in Alberta, Canada to assess the associations between larvae of two genera ( Graphoderus, Rhantus), their spatial locations, and correlations with potential prey. Both larvae were common and positively correlated within wetlands although neither varied with pond depth nor distance from edge. Laboratory trials indicated that Graphoderus consumed more prey (corixids) at the surface, whereas Rhantus killed benthic prey (chironomids) and corixids at an equal rate; damselflies were the least consumed prey. Predation also varied with depth, with both larvae feeding at higher rates in the shallowest environments compared to Graphoderus at an intermediate depth. Predator-prey correlations from ponds were mostly congruent with predation trials; Graphoderus was positively correlated with corixids, Rhantus was positively correlated with corixids and chironomids; beetles were uncorrelated with damselflies. Given the rising incidence of thrombotic complications in paediatric patients, understanding of the pharmacologic behaviour of anticoagulant drugs in children has gained importance. Significant developmental differences between children and adults in the haemostatic system and pharmacologic parameters for individual drugs highlight potentially unique aspects of anticoagulant pharmacology in this special and vulnerable population. This review focuses on pharmacologic information relevant to the dosing of unfractionated heparin, low molecular weight heparin, warfarin, bivalirudin, argatroban and fondaparinux in paediatric patients. The bulk of clinical experience with paediatric anticoagulation rests with the first three of these agents, each of which requires higher bodyweight-based dosing for the youngest patients, compared with adults, in order to achieve comparable pharmacodynamic effects, likely related to an inverse correlation between age and bodyweight-normalized clearance of these drugs. Whether extrapolation of therapeutic ranges targeted for adult patients prescribed these agents is valid for children, however, is unknown and a high priority for future research. Novel oral anticoagulants, such as dabigatran, rivaroxaban and apixaban, hold promise for future use in paediatrics but require further pharmacologic study in infants, children and adolescents. Episodic resource inputs (i.e., pulses) can affect food web properties and community dynamics, but detailed mechanistic understanding of such effects remain elusive. Natural aquatic microsystems (e.g., tree holes, human-made containers) are colonized by invertebrates that form complex food webs dependent on episodic and sometimes sizeable inputs of allochthonous detritus from adjacent terrestrial environments. We investigated how variation in pulse frequency, amount, and resource type interacted to affect richness, abundance, composition, and population sizes of colonizing invertebrates in water-filled tires and tree hole analogs in a forest habitat. Different container types were used to assess the generality of effects across two environmental contexts. Containers received large infrequent or small frequent pulses of animal or leaf detritus of different cumulative amounts distributed over the same period. Invertebrates were sampled in June and September when cumulative detritus input was equal for the two pulse frequencies. Pulse frequency and detritus type interacted to affect the responses of richness and abundance in both months; pulse frequency alone in June affected the relationship between richness and abundance. Richness and abundance were also greater with more detritus regardless of detritus type. One group, the filter feeders, were most important in driving the response of abundance and richness to pulses, especially in June. This work highlights the potential complex nature of responses of communities and populations to resource pulses and implicates the ability of certain groups to exploit pulses of detrital resources as a key to understanding community-level responses to pulses. 1. Inputs of animal and plant detritus are the main energy sources for food webs in a number of isolated container systems, including discarded automobile tyres and tree holes. Containers are dominated by mosquitoes in the genera Culex and Aedes, which among other differences often engage in different foraging behaviours. We hypothesised that because Aedes feed more by browsing surfaces, whereas Culex often filter the water column, these mosquitoes would show variation in performance and differentially affect detritus. Effects of different ratios of animal and plant detritus on survival, mass, and development time for two common container mosquito species, Culex restuans L. and Aedes albopictus Skuse, were examined. We also quantified detrital contribution to biomass via isotopic and nutrient analysis and the effect of larvae on detrital decay. 2. Adult male and female mass of both species was highest with some animal detritus and lowest in only leaf detritus. Aedes albopictus survival was higher than C. restuans across most detritus ratios. 3. Aedes albopictus had higher values of 15N and in some cases 13C across all detritus ratios compared with C. restuans; A. albopictus had lower nitrogen in tissue. Aedes albopictus appeared to be more efficient at obtaining potentially limiting nutrients and had a greater overall effect on detrital decay - a possible consequence of greater foraging effort. 4. Findings further support the view that mosquito performance can be influenced by detritus type, and provide a more precise hypothesis (i.e. lower need for nitrogen) that may explain the superior competitive ability of A. albopictus over other container mosquitoes. The San Francisco Bay (California, USA) is a water body listed as impaired because of Hg contamination in sport fish for human consumption, as well as possible effects on resident wildlife. A legacy of Hg mining in local watersheds and Hg used in Au mining in the Sierra Nevada (USA) has contributed to contamination seen in the bay, with additional more recent and ongoing inputs from various sources. Methylmercury is the species of Hg most directly responsible for contamination in biota, so better understanding of its sources, loads, and processes was sought to identify the best means to reduce impacts. A regional scale model of San Francisco Bay was developed to characterize major methylmercury inputs and processes. The model was used to evaluate the potential impact of uncertainties in estimates for methylmercury loading pathways and environmental processes, identify major data gaps, and explore management prospects for reducing methylmercury contamination. External loading pathways considered in the mass balance include methylmercury loads entering via atmospheric deposition to the bay surface, and discharges from the Sacramento/San Joaquin Delta, local watersheds, municipal wastewater, and fringing wetlands. Internal processes considered include exchange between bed and suspended sediments and the water column, in situ production and demethylation, biological uptake, and losses via hydrologic transport to the ocean through the Golden Gate. In situ sediment methylation and demethylation were dominant sources and losses determining ambient steady-state concentrations in the model, with changes in external loads and export causing smaller changes. Better information on methylation and demethylation is thus most critical to improving understanding of methylmercury balances and management. Environ. Toxicol. Predation among aquatic invertebrate predators can have important effects on patterns of exclusion and coexistence in aquatic habitats, especially if these predators also act as intraguild predators. Such patterns may be explained by variation in predator foraging mode and in the extent and overlap of habitat use. Predaceous diving beetles (Coleoptera: Dytiscidae) are abundant in isolated bodies of water and are effective predators on many aquatic organisms, including other dytiscids. The under-investigated role of hunting behavior and habitat use in altering outcomes of predation under different plant densities may offer insights into patterns of coexistence among larval dytiscids. I performed experiments that quantified behavior of larvae of three common genera of dytiscids that share common prey and then measured predation among genera in the presence or absence of aquatic plants. Behavioral analyses concluded that there were significant differences in foraging modes, with Dytiscus primarily exhibiting sit-and-wait tactics, Graphoderus engaging in active, open water searching, and Rhantus displaying combinations of these behaviors. Predation among larvae was common and occurred when predators were larger than the prey, with no indication of prey preference. Incidence of predation among generic combinations depended on the presence of plants and appeared to be related to behavioral differences among genera. The presence or absence of plants and differences in larval behavior may help to mitigate predation by reducing negative interactions in natural aquatic systems. Dispersal can influence population dynamics, species distributions, and community assembly, but few studies have attempted to determine the factors that affect dispersal of insects in natural populations. Consequently, little is known about how proximate factors affect the dispersal behavior of individuals or populations, or how an organism’s behavior may change in light of such factors. Adult predaceous diving beetles are active dispersers and are important predators in isolated aquatic habitats. We conducted interrelated studies to determine how several factors affected dispersal in two common pond-inhabiting species in southern Alberta, Canada: Graphoderus occidentalis and Rhantus sericans. Specifically, we (1) experimentally tested the effect of plant and beetle densities on dispersal probabilities in ponds; (2) surveyed ponds and determined the relationships among beetle densities and plant densities and water depth; and (3) conducted laboratory trials to determine how beetle behavior changed in response to variation in plant densities, conspecific densities, food, and water depth. Our field experiment determined that both species exhibited density dependence, with higher beetle densities leading to higher dispersal probabilities. Low plant density also appeared to increase beetle dispersal. Consistent with our experimental results, densities of R. sericans in ponds were significantly related to plant density and varied also with water depth; G. occidentalis densities did not vary with either factor. In the laboratory, behavior varied with plant density only for R. sericans, which swam at low density but were sedentary at high density. Both species responded to depth, with high beetle densities eliciting beetles to spend more time in deeper water. The presence of food caused opposite responses for G. occidentalis between experiments. Behavioral changes in response to patch-level heterogeneity likely influence dispersal in natural populations and are expected to be important for observed patterns of individuals in nature. • 31 species of mosquitoes were collected on the islands of Puerto Rico and Vieques. • One new species record was identified Aedes obturbator (Dyar and Knab). • The invasive Aedes albopictus (Skuse) was not collected from either island. We conducted an island-wide survey of the Caribbean islands Puerto Rico and Vieques, U.S.A. during late 2018 and early 2019 to document the current richness of mosquitoes (Diptera: Culicidae). We used a combination of larval surveys and adult trapping using baited CDC light traps and BG-Sentinel traps across 41 of the 78 municipalities. We collected 9 genera, 12 subgenera, and 31 species, which when combined with past studies yields 44 species on the islands. We also note species occurrences across habitat types and elevations from around the islands. One new record, Aedes (Ochlerotatus) obturbator Dyar and Knab, is noted. However we found no evidence of the presence of Aedes albopictus (Skuse), an invasive found throughout the Caribbean, or Aedes (Fredwardsius) vittatus (Bigot), an exotic species recently reported in the Dominican Republic and Cuba. Habitat associations and information regarding the medical importance of species are also included. Given that the islands often experience outbreaks of several arboviruses, obtaining a complete picture of the species present is of high importance. [ABSTRACT FROM AUTHOR]

1. Colonisation of ephemeral aquatic habitats via oviposition by invertebrates may be influenced by a variety of factors, such as the quality of aquatic habitat and the characteristics of the surrounding terrestrial environment. The water-holding bracts of Heliconia caribaea, a subtropical herb that produces ephemeral aquatic habitats, are colonised by a variety of aquatic invertebrates. To date, no experiments have been conducted to identify the cues that affect colonisation patterns via oviposition selection in Heliconia. 2. Artificial bracts were used to assess the influence of two types of resources found in bracts (plant produced carbohydrates and terrestrial snail faeces) on oviposition site-selection by invertebrate taxa via a replicated factorial design at four locations in the Luquillo Experimental Forest of Puerto Rico, U.S.A. Eleven microsite characteristics thought to affect oviposition were measured for each experimental container. 3. Most taxa responded in a minor way to microsite characteristics, whereas site selection by the most numerically dominant groups (e.g. Syrphidae) were influenced principally by resources within artificial bracts. Overall, the greatest response by particular taxa was to the presence of snail faeces. At the community level, total abundance, richness, and evenness of invertebrates increased with increasing biomass of faeces. Variation in sugar produced a more complex response. 4. In general, the terrestrial matrix surrounding these aquatic habitats was only a secondary determinant of population and community attributes; the principal factor affecting site selection was the quality of the aquatic habitat. 1. Investigations of competitive interactions emphasize non-detrital resources, even though detritus is a major component of most food webs. Studies of competing species focus usually on single resource types, although consumers in nature are likely to encounter mixtures of resource types that may affect whether competition results in exclusion or coexistence. 2. The invasive mosquito Aedes albopictus is capable of excluding the native mosquito Ochlerotatus triseriatus in competition for single detritus types in laboratory and field microcosms. In this study, we used nine ratios of two detritus types (animal and leaf) common in natural containers to test whether detritus ratios affect the outcome of competition. 3. Under intraspecific and interspecific competition, A. albopictus attained higher survival and estimated population growth rate than did O. triseriatus. Unlike past studies, both species had positive growth and high adult survival, with little evidence of competitive effects, under one resource ratio (10 : 1 ratio of leaf : animal detritus) regardless of mosquito densities, suggesting potential coexistence. 4. Path analysis showed that densities of larvae had negative effects on population growth for O. triseriatus but not for A. albopictus, indicating competitive superiority of A. albopictus. Population growth of both species was affected strongly by the direct paths from animal (positive) and leaf (negative) detritus, and the indirect effect of leaf detritus via bacterial production (positive). 5. Field sampling established that detritus entered real tree holes in ratios similar to those in our experiment, suggesting that natural variation in detritus ratios may influence local coexistence of these species. Most theoretical and empirical studies of productivity–species richness relationships fail to consider linkages among trophic levels. We quantified productivity–richness relationships in detritus-based, water-filled tree-hole communities for two trophic levels: invertebrate consumers and the protozoans on which they feed. By analogy to theory for biomass partitioning among trophic levels, we predicted that consumer control would result in richness of protozoans in the lower trophic level being unaffected by increases in productivity, whereas richness of invertebrate consumers would increase with productivity. Our data were consistent with this prediction: consumer richness increased linearly, but protozoan richness was unrelated to changes in productivity. The productivity–richness relationships for all taxa combined were not necessarily consistent with relationships within each trophic level. We used path analysis to investigate the mechanisms that may produce the observed responses of trophic levels to changes in productivity. We tested the importance of the direct effect of productivity on richness and the indirect effect of productivity mediated by effects on total abundance. For protozoans, only direct effects of productivity on richness were important, but both direct and indirect effects of productivity on richness were important for invertebrates. Protozoan richness was strongly affected by top-down impacts of abundance of invertebrates. These results are consistent with theory on biomass partitioning among trophic levels and suggest a strong link between richness and abundance within and between trophic levels. Understanding how trophic level interactions determine productivity–richness relationships will likely be necessary in order for us to achieve a comprehensive understanding of the determinants of diversity. Distributions of mosquito larvae likely are a consequence of multiple factors, although two commonly studied factors (quality of the larval environment and the terrestrial matrix in which these habitats reside) have rarely and simultaneously been varied in the field to understand distributions of larvae. We monitored aquatic containers of two leaf detritus levels within a forest, prairie, and industrial habitat across five months to understand the temporal and spatial colonization of aquatic invertebrates in Northcentral Illinois, USA. Data were collected monthly on mosquito populations and the composition of other invertebrates colonizing containers. Overall, six species of mosquitoes colonized containers, with Culex restuans and Aedes triseriatus having the highest relative abundances. There were strong seasonal abundance patterns for these two mosquito species, with the dominant species changing over time in the forest habitat. The responses of other mosquito taxa were more variable, with abundances reflective of either the terrestrial matrix or larval habitat quality. High detritus containers supported the highest abundances of most species encountered, regardless of habitat. Non-mosquito taxa were less common numerically, but analyses suggested that some taxa, such as syrphid larvae, often co-occurred with mosquitoes. Nested subset analysis indicated communities were strongly nested, and that both habitat type and detritus level were important in explaining nested patterns of aquatic invertebrates. Our data show that both the larval habitat and the surrounding terrestrial matrix shape patterns of container mosquitoes, and that other container invertebrates vary in similar ways as mosquitoes. The more individuals hypothesis (MIH) postulates that productivity increases species richness by increasing mean equilibrium population size, thereby reducing the probability of local extinction. We tested the MIH for invertebrates colonizing microcosms that simulated tree holes by manipulating productivity through additions of leaf or animal detritus and subsequently determining the relationships among richness, total abundance, abundance per species, and measures of productivity. We quantified productivity as the rate of microorganism protein synthesis, microorganism metabolic rate, nutrient ion concentration, and type and amount of detritus. Microcosms with animal detritus attracted more species, more individuals per species, and more total individuals than did microcosms with similar amounts of leaf detritus. Relationships between richness or abundance and productivity varied with date. Richness in June increased as a linear function of productivity, whereas the power function predicted by the MIH fit best in July. Abundance in June and July was best described by a power function of productivity, but the linear function predicted by the MIH fit best in September. Abundance per species was best described by a power function of productivity in June and July. Path analysis showed that the indirect effect of productivity through abundance on richness that is predicted by MIH was important in all months, and that direct links between productivity and richness were unnecessary. 1. Variation in detritus quality and quantity can have significant effects on aquatic invertebrate food webs. Allochthonous inputs of detritus are the principal energy source for organisms in aquatic tree hole microsystems. We compared the effects of two major detritus types found in tree holes, senescent leaves (Sugar Maple and White Oak) and invertebrate carcasses (dead adult fruit flies and crickets), on several water quality characteristics of laboratory microcosms as well as on mass, survival and population performance of the dominant tree hole consumer, Ochlerotatus triseriatus (Diptera: Culicidae). To date, no study has documented the effects of animal detritus in tree hole microsystems or on resident consumers. 2. Aquatic environments receiving invertebrate carcasses had significantly greater total nitrogen, total reactive phosphorus and higher pH, than leaf-based environments. Decay rate of invertebrate carcasses was greater compared to leaf material. Consumption of O2 by micro-organisms increased with increasing detritus amounts, but we detected no difference between detritus types. 3. Ochlerotatus triseriatus larvae grew faster in animal-based treatments, and mean mass of larvae was significantly greater when more animal detritus was used. The effect of animal-based treatments on larvae translated into higher performance for adults, which were three times heavier than counterparts from plant-based containers. Survivorship and estimated population growth rates were significantly greater for O. triseriatus reared on animal-based versus plant-based detritus. 4. We hypothesise two mechanisms for the pronounced effect of invertebrate carcasses on mosquito performance relative to that associated with leaf detritus: (i) invertebrate carcasses decompose more quickly and release nutrients more effectively into the aquatic environment; or (ii) O. triseriatus larvae may directly ingest nutrient-rich components of invertebrate carcasses. Because even relatively small animal detritus additions can have strong effects on O. triseriatus populations, studies need to be conducted to explore the overall role of animal detritus in tree holes in nature. Context-Patients with sickle cell disease suffer from a variety of vaso-occlusive events that may be related to activation of the hemostatic system. Thromboelastography assesses the functionality of this system from a global standpoint and has demonstrated some utility in detecting hypercoagulable states in varied clinical settings, but it has not been systematically evaluated in patients with sickle cell disease. Objective.-To characterize the findings of thromboe-lastography in patients with sickle cell disease during periods of steady state and illness, to compare these results with those of healthy controls, and to correlate these profiles with other measured hemostatic parameters. Design.-In this cross-sectional study, we obtained thromboelastographic and other hemostatic data on specimens from 46 patients with sickle cell disease (35 with hemoglobin SS, 7 with hemoglobin SC, and 4 with hemoglobin S-β thalassemia) and 20 healthy race-matched controls. Data were obtained from patients with sickle cell disease at baseline conditions (n = 41) and in the setting of acute illness (n = 5). Results.-Patients with hemoglobin 55 had lower reaction time and higher angle, maximum amplitude, and coagulation index values on thromboelastography than the control group. Hemoglobin SC patients had higher angle, maximum amplitude, and coagulation index values than controls. Hemoglobin S-β thalassemia patients showed no significant differences compared with controls. Five hemoglobin SS patients with recent or current illness demonstrated increased maximum amplitude and coagulation index compared with hemoglobin SS patients at baseline conditions. Conclusions.-Patients with sickle cell disease demonstrated a significant hypercoagulable state in thromboelastography profiles, with the degree of abnormality dependent on the type of sickle cell disease and perhaps the presence of acute illness, continued follow-up of this patient cohort, as well as further study of larger and more homogeneous patient groups, is required to adequately assess the utility of thromboelastography in predicting complications of sickle cell disease. Aedes albopictus has replaced Aedes aegypti in much of the latter species' historic range within the United States. The leading hypothesis for this displacement is exclusion via resource competition; however, the proximate mechanism producing a competitive advantage for A. albopictus over A. aegypti has not been identified. We performed laboratory experiments to test the hypotheses that these species differ in feeding behavior, and that these differences result in differences in survival when resources are scarce. Differences in feeding behavior were assessed in three environments with food (growing microorganisms) available: 1) in fluid only; 2) on leaf surfaces only; 3) or both in fluid and on leaf surfaces. We determined behavior of larvae in these environments, recording their positions (bottom, wall, leaf, top, or middle) and activities (browsing, filtering, resting, or thrashing) using instantaneous scan censuses. A. albopictus spent significantly more time at leaf surfaces, whereas A. aegtdpti spent more time engaging in nonfeeding activities. Both species showed a significant shift in foraging activity toward leaves when leaves were available. In a second experiment, we recorded survivorship for individuals raised in two treatment combinations: whole or half 17-mm disks of live oak leaves, with or without direct access to the leaf surface (controlled using nylon mesh, which allowed movement of microscopic organisms, but prevented mosquito larva movement between container sides). After 31 d, survivorship of A. albopictus was significantly greater than that of A. aegypti regardless of treatments. Moreover, A. albopictus showed significantly greater survivorship compared with A. aegypti when deprived of access to leaf surfaces and in whole leaf disk treatments, suggesting superior resource-harvesting ability for A. albopictus. Background: Ectotherms, like mosquitoes, have evolved specific responses to variation in environmental conditions like temperature, and these responses could confer a fitness benefit or cost when carried-over to different life stages. However, effects of temperature on animals with complex life-cycles often only focus on part of their life-cycle, or only consider how single aspects of life-history may carry over to new stages. Herein we investigated how temperature affects intraspecific larval competition and carry-over effects from larval to adult stages in the widespread invasive Asian tiger mosquito Aedes albopictus. Methods: For larval competition, larvae were reared at three densities (10, 20, and 40 individuals) across three source temperatures (21 °C, 27 °C and 34 °C). To test carry-over effects, adult survival was measured for individuals crossed with adult temperatures of 21 °C, 27 °C and 34 °C from the larval density of 20 individuals at each source temperature. Fecundity data also were obtained from mated females. Results: For competition, there was a significant interaction between larval density and temperature, with the smallest females, who took the longest to develop, produced in the highest temperatures; density generally accentuated this effect. Regarding carry-over effects, adults exposed to higher temperatures lead to greater differences in fecundity and survival of adult populations. Conclusions: Temperature appears to affect life-history of developing larvae under competitive interactions and can also alter adult fitness as the disparity between larval rearing and adult habitat temperatures increases. Abstract: Rare bleeding disorders can cause significant bleeding in children and adolescents. These encompass rare clotting factor deficiencies, and fibrinolytic pathway defects. Vascular anomalies can cause recurrent and refractory bleeding, and are included in this review of rare causes of abnormal bleeding in children and adolescents. Menorrhagia is often reported as a manifestation of these conditions. Succinct knowledge about these disorders, their clinical presentation, diagnostic work-up, and therapeutic options are crucial to the accurate diagnosis and optimal management of affected patients. This review provides an overview of these infrequently encountered disorders, discusses their recognition based on results of suggested screening tests and offers a general guideline for approach to therapy. Venous thromboembolism (VTE) is increasingly diagnosed in pediatric patients, and anticoagulantusein thispopulationhas become common, despite the absence of US Food and Drug Administration (FDA) approval for this indication. Guidelines for the use of anticoagulants in pediatrics are largely extrapolated from large randomized controlled trials (RCTs) in adults, smaller dose-finding and observational studies in children, and expert opinion. The recently FDA-approved direct oral anticoagulants (DOACs), such as dabigatran, rivaroxaban, apixaban, and edoxaban, provide potential advantages over oral vitamin K antagonists and subcutaneous low-molecular-weight heparins (LMWHs). However, key questions arise regarding their potential off-label clinical application inpediatric thromboembolic disease. In this Perspective, we provide background on the use of LMWHs such as enoxaparin as the main stay of treatment of pediatric provoked VTE; identify key questions and challenges with regard to DOAC trials and future DOAC therapy in pediatric VTE; and discuss applicable lessons learned from the recent pilot/feasibility phase of a large multicenter RCT of anticoagulant duration in pediatric VTE. Hydroxylated polybrominated diphenyl ethers (OH-BDEs) are a new class of contaminants of emerging concern, but the relative roles of natural and anthropogenic sources remain uncertain. Polybrominated diphenyl ethers (PBDEs) are used as brominated flame retardants, and they are a potential source of OH-BDEs via oxidative transformations. OH-BDEs are also natural products in marine systems. In this study, OH-BDEs were measured in water and sediment of freshwater and coastal systems along with the anthropogenic wastewater-marker compound triclosan and its photoproduct dioxin, 2,8-dichlorodibenzo-p-dioxin. The 6-OH-BDE 47 congener and its brominated dioxin (1,3,7-tribromodibenzo-p-dioxin) photoproduct were the only OH-BDE and brominated dioxin detected in surface sediments from San Francisco Bay, the anthropogenically impacted coastal site, where levels increased along a north-south gradient. Triclosan, 6-OH-BDE 47, 6-OH-BDE 90, 6-OH-BDE 99, and (only once) 6’-OH-BDE 100 were detected in two sediment cores from San Francisco Bay. The occurrence of 6-OH-BDE 47 and 1,3,7-tribromodibenzo-p-dioxin sediments in Point Reyes National Seashore, a marine system with limited anthropogenic impact, was generally lower than in San Francisco Bay surface sediments. OH-BDEs were not detected in freshwater lakes. The spatial and temporal trends of triclosan, 2,8-dichlorodibenzo-p-dioxin, OH-BDEs, and brominated dioxins observed in this study suggest that the dominant source of OH-BDEs in these systems is likely natural production, but their occurrence may be enhanced in San Francisco Bay by anthropogenic activities. Study Objective To compare the efficacy of oral tranexamic acid (TA) with combined oral contraceptives (COC) in reducing menstrual blood loss (MBL) and improving quality of life in adolescent heavy menstrual bleeding (HMB). Design, Setting, and Participants A prospective randomized crossover trial with 17 postmenarchal girls aged 21 years and younger with HMB who were seen at our institution. Interventions Patients were randomized to group A (TA arm) or group B (COC arm), each for 3 cycles, with crossover to the second arm after 1-month washout. Main Outcome Measures The primary end points were difference in improvement in MBL and QOL, from baseline to end of each therapy. Results Seventeen patients were enrolled (mean age 14.2 years, range 11.7 to 16.8 years). Nine patients completed both arms; 8 patients withdrew from the study due to adverse events or noncompliance. Compared with baseline, significant improvement ( P < .05) was demonstrated by TA and COC in MBL (mean Pictorial Blood Assessment Chart score decrease: TA, 536.4; COC, 430.6) and quality of life (mean Pediatric Quality of Life Inventory TM version 4.0 Generic Scales score increase: TA, 15.6; COC, 16.75), but no significant difference was noted between TA and COC ( P > .05). There was statistically significant reduction in the length of menstrual cycle for COC only (mean reduction 5.3 days; P = .04) and not for TA ( P = .18). Ten patients (58%) experienced adverse events that were possibly drug related (TA: n = 3, 30%; COCP: n = 7, 64%). The negative effects of urbanization on water quantity and quality and the performance of bioretention for treating runoff and reducing concentrations and loads of sediment, nutrients, pathogens, and selected trace metals have been well documented. This study evaluated the efficiency of a bioretention system installed in semiarid Daly City, California, for a large suite of pollutants, including some that have been rarely evaluated in such systems. Stormwater flow and pollutant concentrations were measured before and after the construction of a bioretention system consisting of rain gardens and a bioswale. After installation, concentrations of most pollutants were reduced and met water quality guidelines. Water quality improvements (concentration or load reductions) were demonstrated for total Hg, polychlorinated biphenols (PCBs) and dioxins ranging between 18 and 100%, and a suite of more conventional pollutants [SSC, Cd, Cu, Ni, Pb, Zn, and polycyclic aromatic hydrocarbons (PAHs)] ranging between 20 and 90%. Results were less favorable for methylmercury. The balance of evidence suggests that such systems, when designed and functioning correctly, can be highly effective in treating pollutant concentrations and loads in stormwater runoff in semiarid environments. California has implemented unique consumer product flammability standards. Polybrominated diphenyl ether (PBDE) flame retardants were once widely incorporated into products to meet these standards, but concerns regarding toxicity and accumulation in humans and biota led to nationwide phase-outs and state bans. A decade of PBDE monitoring in San Francisco Bay has resulted in a data set that covers periods during and after PBDE use and consists of hundreds of measurements of water, sediment, and biota. While PBDEs remain widely detected in biota, levels have declined by nearly half in sport fish and 74-95% in bivalves and bird eggs. Concentrations of BDE-47 in sediment have dropped by over one-third from 2002 to 2012; in water, a decline is not yet evident. The dominant congener in sediment, DecaBDE component BDE-209, showed no temporal trend. U.S. productmn ot DecaBDE ended in 2013; future monitoring may reveal declines. Overall, the data indicate that reduced production can result in relatively rapid reductions in the concentrations of some hydrophobic contaminants in biota and sediment, particularly when implemented after only a few decades of heavy use. Abstract: Extensive evidence of the adverse impacts of polychlorinated biphenyls (PCBs) to wildlife, domestic animals, and humans has now been documented for over 40years. Despite the ban on production and new use of PCBs in the United States in 1979, a number of fish consumption advisories remain in effect, and there remains considerable uncertainty regarding ongoing environmental sources and management alternatives. Using a blind sampling approach, 25 caulk samples were collected from the exterior of ten buildings in the San Francisco Bay Area and analyzed for PCBs using congener-specific gas chromatography–mass spectrometry (GC–MS) and chlorine using portable X-ray fluorescence (XRF). PCBs were detected in 88% of the caulk samples collected from the study area buildings, with 40% exceeding 50ppm. Detectable PCB concentrations ranged from 1 to 220,000ppm. These data are consistent with previous studies in other cities that have identified relatively high concentrations of PCBs in concrete and masonry buildings built between 1950 and 1980. Portable XRF was not a good predictor of the PCB content in caulk and the results indicate that portable XRF analysis may only be useful for identifying caulk that contains low concentrations of Cl (≤10,000ppm) and by extension low or no PCBs. A geographic information system-based approach was used to estimate that 10,500kg of PCBs remain in interior and exterior caulk in buildings located in the study area, which equates to an average of 4.7kg PCBs per building. The presence of high concentrations in the exterior caulk of currently standing buildings suggests that building caulk may be an ongoing source of PCBs to the San Francisco Bay Area environment. Further studies to expand the currently small international dataset on PCBs in caulking materials in buildings of countries that produced or imported PCBs appear justified in the context of both human health and possible ongoing environmental release. We analyzed eight sediment cores from wetland and subtidal locations around San Francisco (SF) Bay, terrace sediment from the Yuba River, CA and precipitation from the SF Bay region. We defined the Hg isotopic composition of uncontaminated SF Bay sediment, two contributing endmember sediments contaminated by distinct Hg sources, and regional precipitation. Deep subtidal sediments with pre-mining THg concentrations (less than 60ng/g) have δ202Hg of −0.98±0.06‰ and Δ199Hg of 0.17±0.03‰ (1 s.d.; n=5). The δ202Hg of subtidal sediment in SF Bay systematically changed between pre-mining, circa 1960, and surface sediment. In circa 1960 sediment δ202Hg ranges from −0.63 to −0.32‰ (±0.10‰) with higher δ202Hg in the south and lower δ202Hg in the north; however in surface sediment δ202Hg is nearly constant and averages −0.52±0.04‰ (1 s.d.; n=6). These latter values for SF Bay surface sediment are similar to those for terrace sediment along the Yuba River that have δ202Hg of −0.57±0.10‰ and Δ199Hg of 0.05±0.01‰ (1 s.d.; n=2) and are consistent with sediment contaminated by a metallic Hg source that was derived from Hg ore in the California Coast Ranges and used in Sierra Nevada gold mining. Wetlands adjacent to south and central SF Bay have high THg in deeper sediment layers (>3000ng/g) that vary by ~0.6‰ in δ202Hg, presumably reflective of various anthropogenic Hg sources. The three sediment endmembers (metallic Hg, Hg mining and background Hg) were defined using δ202Hg and THg concentrations. Based on the estimated contribution of endmember sediment to subtidal and intertidal locations, we suggest that the subtidal Hg stable isotope record in sediment cores is consistent with historical sediment transport to, and deposition in, SF Bay. Thus, Hg stable isotopes can be used to trace sediment transport in locations that are impacted by multiple anthropogenic Hg sources. [ABSTRACT FROM AUTHOR]

- Understanding food-web ecology is valuable to conservation by linking interactions of multiple species together and illustrating the functionality of trophic exchange. Alosa alabamae (Alabama Shad), an anadromous species, reproduces in northern Gulf of Mexico drainages from February through May, and for this study, the Pascagoula and Apalachicola rivers were chosen to sample juvenile Alabama Shad. The age-0 fish mature within these rivers and have the potential to impact the food web of the systems in which maturation occurs. The focus was to determine if diet changes as Alabama Shad mature, and to identify diet differences between drainages. Diets of Alabama Shad <50 mm standard length (SL) consisted primarily of a dark, almost black material labeled as unidentifiable organics, while larger Alabama Shad, >50 mm SL, fed almost exclusively on insects. Many groups of aquatic and terrestrial insects were found in the stomachs of this species. Alabama Shad diets also differed among drainages, with the Apalachicola River being dominated by terrestrial insects, and the Pascagoula River having both terrestrial and aquatic insects. Diet and trophic placement of Alabama Shad may allow managers to understand the importance of this fish within its natal rivers. [ABSTRACT FROM AUTHOR]

1. Detritus that forms the basis for mosquito production in tree hole ecosystems can vary in type and timing of input. We investigated the contributions of plant- and animal-derived detritus to the biomass of Aedes triseriatus (Say) pupae and adults by using stable isotope (15N and 13C) techniques in laboratory experiments and field collections. 2. Laboratory-reared mosquito isotope values reflected their detrital resource base, providing a clear distinction between mosquitoes reared on plant or animal detritus. 3. Isotope values from field-collected pupae were intermediate between what would be expected if a single (either plant or animal) detrital source dominated the resource base. However, mosquito isotope values clustered most closely with plant-derived values, and a mixed feeding model analysis indicated tree floral parts contributed approximately 80% of mosquito biomass. The mixed model also indicated that animal detritus contributed approximately 30% of mosquito tissue nitrogen. 4. Pupae collected later in the season generally had isotope values that were consistent with an increased contribution from animal detritus, suggesting that this resource became more nutritionally important for mosquitoes as plant inputs declined over the summer. A study was conducted to measure atmospheric concentrations of PAHs and PCBs and estimate their fluxes between air and water in the northern San Francisco Estuary. Ambient air samples were collected once every 12 days at a single sampling site in Concord, CA, from June to November 2000, using a modified high-volume air-sampling device equipped with glass fiber filters and polyurethane foam. Concentrations of total PAHs and PCBs ranged from 5.7 to 56 and 0.17 to 0.32 ng/m³, respectively. PAHs and PCBs in the ambient air were predominantly in the vapor phase (83-99%). Gaseous fluxes of PAHs in the estuary showed high seasonal variation, ranging from 110 ng·m[sup -2]·day[sup -1] efflux in August to 1050 ng·m[sup -2]t·day[sup -1] influx in November. Gaseous PCBs showed consistent net volatilization (2.2-24 ng·m[sup -2]·day[sup -1]) for this period. Particle settling contributed estimated net deposition fluxes of 45960 ng·m[sup -2]-day[sup -1] for PAHs and 0·39-2.1 ng·m[sup -2]·.day[sup -1] for PCBs. Combining these fluxes, PAHs were either deposited to or lost from the estuary via the atmosphere, depending on the month. In contrast, there consistently was net emission of PCBs from the estuary to the atmosphere. Mosquito community dynamics in urban areas are influenced by an array of both social and ecological factors. Human socioeconomic factors (SEF) can be related to mosquito abundance and diversity as urban mosquito development sites are modified by varying human activity, e.g. level of abandoned structures or amount of accumulated trash. The goal of this study was to investigate the relationships among mosquito diversity, populations of Aedes aegypti , and SEF in a tropical urban setting. Mosquitoes were collected using BG Sentinel 2 traps and CDC light traps during three periods between late 2018 and early 2019 in San Juan, Puerto Rico, and were identified to species. SEFs (i.e. median household income, population density, college-level educational attainment, unemployment, health insurance coverage, percentage of households below the poverty line, amount of trash and level of abandoned homes) were measured using foot surveys and U.S. Census data. We found 19 species with the two most abundant species being Culex quinquefasciatus (n  = 10 641, 87.6%) and Ae. aegypti (n  = 1558, 12.8%). We found a positive association between Ae. aegypti abundance and mosquito diversity, which were both negatively related to SES and ecological factors. Specifically, lower socioeconomic status neighborhoods had both more Ae. aegypti and more diverse communities, due to more favorable development habitat, indicating that control efforts should be focused in these areas. Food quality and quantity serve as the basis for cycling of key chemical elements in trophic interactions; yet the role of nutrient stoichiometry in shaping host–pathogen interactions is under appreciated. Most of the emergent mosquito-borne viruses affecting human health are transmitted by mosquitoes that inhabit container systems during their immature stages, where allochthonous input of detritus serves as the basal nutrients. Quantity and type of detritus (animal and plant) were manipulated in microcosms containing newly hatched Aedes aegypti mosquito larvae. Adult mosquitoes derived from these microcosms were allowed to ingest Zika virus-infected blood and then tested for disseminated infection, transmission, and total nutrients (percent carbon, percent nitrogen, ratio of carbon to nitrogen). Treatments lacking high-quality animal (insect) detritus significantly delayed development. Survivorship to adulthood was closely associated with the amount of insect detritus present. Insect detritus was positively correlated with percent nitrogen, which affected Zika virus infection. Disseminated infection and transmission decreased with increasing insect detritus and percent nitrogen. We provide the first definitive evidence linking nutrient stoichiometry to arbovirus infection and transmission in a mosquito using a model system of invasive Ae. aegypti and emergent Zika virus. OBJECTIVES The purposes of this study are to perform a large-scale evaluation of the standardized dosage adjustment nomogram recommended by the American College of Chest Physicians (CHEST) for the management of enoxaparin in hospitalized pediatric patients and to determine the necessity of routine and repeated anti--factor Xa (anti-Xa) levels. METHODS A retrospective cohort study was designed, and charts were reviewed in a single tertiary care institution for all patients who received enoxaparin between October 1, 2010, through September 30, 2016. Patients were included if they were receiving treatment doses of enoxaparin according to the pediatric CHEST guidelines, had a subtherapeutic or supratherapeutic anti-Xa level drawn at 3.5 to 6 hours after a dose, had a dose changed in an attempt to attain a therapeutic anti-Xa level, and had a second anti-Xa level drawn 3.5 to 6 hours after the dose change. Descriptive statistical methods were used to characterize the ability of dose adjustment via a nomogram to attain an anti-Xa of 0.5 to 1 unit/mL. RESULTS A total of 467 patients were identified who received the appropriate initial dose and dosage adjustment and whose levels were drawn according to the CHEST guidelines. In patients who had an initial anti-Xa level of <0.35 units/mL and received the nomogram recommended dose increase of 25% ± 5%, 28 out of 96 patients (29.2%) reached therapeutic levels. Of 197 patients who had an initial anti-Xa level between 0.35 and 0.49 units/mL and who received the nomogram recommended dose increase of 10% ± 5%, 116 (58.9%) reached therapeutic levels. Of 50 patients with an initial anti-Xa level between 1.1 and 1.5 units/mL and who received the nomogram dose decrease of 20% ± 5%, 31 (62%) reached therapeutic levels. CONCLUSIONS The current dosage adjustment nomogram recommended by the CHEST guidelines does not reliably lead to therapeutic anti-Xa levels when used to adjust enoxaparin doses in pediatric patients. Urban watersheds are significantly anthropogenically-altered landscapes. Most previous studies cover relatively short periods, without addressing concentrations, loads, and yields in relation to annual climate fluctuations, and datasets on Ag, Se, PBDEs, and PCDD/Fs are rare. Intensive storm-focused sampling and continuous turbidity monitoring were employed to quantify pollution at two locations in the Guadalupe River (California, USA). At a downstream location, we determined loads of suspended sediment (SS) for 14 yrs., mercury (HgT), PCBs, and total organic carbon (TOC) (8 yrs), total methylmercury (MeHgT) (6 yrs), nutrients, and trace elements including Ag and Se (3 yrs), DDTs, chlordanes, dieldrin, and PBDEs (2 yrs), and PCDD/Fs (1 yr). At an upstream location, we determined loads of SS for 4 yrs. and HgT, MeHgT, PCBs and PCDD/Fs for 1 yr. These data were compared to previous studies, climatically adjusted, and used to critically assess the use of small datasets for estimating annual average conditions. Concentrations and yields in the Guadalupe River appear to be atypical for total phosphorus, DDTs, dieldrin, HgT, MeHgT, Cr, Ni, and possibly Se due to local conditions. Other pollutants appear to be similar to other urban systems. On average, wet season flow varied by 6.5-fold and flow-weighted mean (FWM) concentrations varied 4.4-fold, with an average 7.1-fold difference between minimum and maximum annual loads. Loads for an average runoff year for each pollutant were usually less than the best estimate of long-term average. The arithmetic average of multiple years of load data or a FWM concentration combined with mean annual flow was also usually below the best estimate of long-term average load. Mean annual loads using sampled years were also less than the best estimate of long-term average by a mean of 2.2-fold. Aims Antithrombin is increasingly used in paediatric patients, yet there are few age-specific pharmacokinetic data to guide dosing. We aimed to describe the pharmacokinetic profile of human (plasma-derived) antithrombin concentrate in paediatric patients. Methods A 5-year retrospective review was performed of patients <19 years of age admitted to our institution who received antithrombin concentrate, were not on mechanical circulatory support and had baseline (predose) and postdose plasma antithrombin activity levels available for analysis. Demographic and laboratory variables, antithrombin dosing information and data on the use of continuous infusion unfractionated heparin were collected. Population pharmacokinetic analysis was performed with bootstrap analysis. The model developed was tested against a validation dataset from a cohort of similar patients, and a predictive value was calculated. Results A total 184 patients met the study criteria {46.7% male, median age [years] 0.35 [interquartile range (IQR) 0.07-3.9]}. A median of two antithrombin doses (IQR 1-4) were given to patients (at a dose of 46.3 ± 13.6 units kg-1), with median of three (IQR 2-7) postdose levels per patient. Continuous infusion unfractionated heparin was administered in 87.5% of patients, at a mean dose of 34.1 ± 22.7 units kg-1 h-1. A one-compartment exponential error model best fit the data, and significant covariates included allometrically scaled weight on clearance and volume of distribution, unfractionated heparin dose on clearance, and baseline antithrombin activity level on volume of distribution. The model resulted in a median −1.75% prediction error (IQR -11.75% to 6.5%) when applied to the validation dataset ( n = 30). Objective To study the prevalence of hemostatic abnormalities, including bleeding disorders and risk factors, in young females referred to a multidisciplinary clinic for evaluation of heavy menstrual bleeding (HMB). Methods Retrospective chart review was undertaken for 131 post-menarchal girls with HMB, 7 to 17 years of age, enrolled in the institutional ‘Menorrhagia Data Registry’ protocol. The diagnostic approach included: (1) complete blood count, prothrombin time, partial thromboplastin time, fibrinogen, von Willebrand panel (2) platelet aggregometry, specific clotting factor assay, fibrinolytic pathway analysis, and factor XIII level as needed. The prevalence of hemostatic abnormalities and the prognostic significance of clinical variables associated with hemostatic abnormalities in young girls with HMB were evaluated. Results A hemostatic abnormality was identified in 69 (53%) young girls with HMB. Of these, 27 (21%) had an underlying bleeding disorder and 42 (32%) had a risk factor for bleeding, namely low von Willebrand factor activity. A larger number of girls with underlying bleeding disorder had personal history of other bleeding symptoms (48% vs 31%) and bleeding after surgical or dental procedure (25% vs 8%) when compared to females without hemostatic abnormality. Furthermore, girls with risk factor for bleeding (low vWF activity) were more likely to have bleeding after surgical or dental procedure (15% vs 8%) and family history of bleeding (79% vs 60%) than patients without hemostatic abnormality. Conclusions There is high prevalence of hemostatic abnormalities, including bleeding disorders and risk factors, in young girls with HMB. Abstract: Novel methods utilizing liquid chromatography–tandem mass spectrometry and gas chromatography–mass spectrometry were validated for low-level detection of 104 pharmaceuticals and personal care products ingredients (PPCPs) and four alkylphenols (APs) in environmental samples. The methods were applied to surface water, sediment, and mussel tissue samples collected from San Francisco Bay, CA, USA, an urban estuary that receives direct discharge from over forty municipal and industrial wastewater outfalls. Among the target PPCPs, 35% were detected in at least one sample, with 31, 10, and 17 compounds detected in water, sediment, and mussels, respectively. Maximum concentrations were 92ng/L in water (valsartan), 33ng/g dry weight (dw) in sediments (triclocarban), and 14ng/g wet weight (ww) in mussels (N,N-diethyl-m-toluamide). Nonylphenol was detected in water (<2–73ng/L), sediments (22–86ng/g dw), and mussels (<0.04–95ng/g ww), and nonylphenol mono- and diethoxylates were detected in sediments (<1–40ng/g dw) and mussels (<5–192ng/g ww). The concentrations of PPCPs and APs detected in the San Francisco Bay samples were generally at least an order of magnitude below concentrations expected to elicit toxic effects in aquatic organisms. This study represents the first reconnaissance of PPCPs in mussels living in an urban estuary and provides the first field-derived bioaccumulation factors (BAFs) for select compounds in aquatic organisms. Abstract: Background: Although von Willebrand disease is the most common inherited bleeding disorder, platelet function disorders are less well recognized as a cause of bleeding. We report a case of menorrhagia caused by an unsuspected platelet secretion defect. Case: A 13-year-old Asian female, with unknown family history, presented with menorrhagia not responsive to intravenous conjugated estrogens, requiring transfusion of 7 units of packed red blood cells. Initial screening tests for bleeding disorders were normal; however, due to high clinical suspicion, further specific testing with platelet aggregometry was performed, which revealed a platelet secretion defect. Summary and Conclusion: The prevalence of platelet secretion defects in adolescents with menorrhagia is unknown, but may be higher than currently recognized. When screening tests are normal, yet suspicion remains high for an underlying hemostatic disorder, platelet aggregometry must be performed. [Copyright &y& Elsevier]
