Purpose: The aim of this study was to analyze the clinical characteristics and survival rates of our patients in our center in order to reach the basic epidemiologic data of region.Materials and Methods: Epidemiological features of 599 patients with childhood cancer were evaluated retrospectively. The 440 of them were followed-up in our hospital more than one month and they were included to our retrospective evaluation of survival analysis. The data were obtained by scanning the patient record files of the pediatric hematology and oncology department and analyzed with special statistical programs.Results: Median age of patients was 6 years (1 day-20,4 years) and male to female ratio was 1,2. The diagnosis was leukemia in 8,4%, lymphoma in 21.2%, central nervous systems tumors in 22.2 %. Event-free and overall survival analysis were also estimated for followed patients (n=440). Five year overall and event-free survival rates of the whole group were calculated as 75.3% and 66.9% respectively. Conclusion: Epidemiological data except incidence of leukemia were similar with many epidemiological studies. Leukemia is majorly being folllowed up by our department since 2010. Multidisciplinary treatment protocols and a professional team are important in high survival rates in pediatric oncology., Amaç: Bu çalışmada amacımız bölgemizde merkezimize başvuran malign tümörlü hastaların epidemiyolojik özellikleri ve sağkalımlarını değerlendirmektir.Gereç ve Yöntem: Malign tümör tanısı alan, 599 çocukluk çağı kanseri olan hasta epidemiyolojik özellikleri açısından retrospektif olarak değerlendirildi. Bu hastalardan; bir aydan uzun süre merkezimizde takip edilmiş olan 440 hasta sağkalım açısından ayrı olarak analiz edildi. Veriler, çocuk hematoloji ve onkoloji bölümü hasta kayıt dosyaları taranarak elde edilmiş olup, özel istatistik programları ile analiz edilmiştir.Bulgular: Hastanemize başvuran malign tümör tanısı alan kayıtlı toplam 599 hastanın, ortanca yaşı 6 yıl (1 gün-20,4 yıl), erkek kız oranı (E/K) 1,2 saptandı. Tümör tanılarına göre görülme sıklığı sırası ile hastaların %8,4’inde lösemi; %21,2’sinde lenfoma; %22,2’sinde santral sinir sistemi tümörleri idi.. Tüm kayıtlı 599 hasta ve primer takip ve tedavi edilen 440 hastanın epidemiyolojik özellikleri benzer saptanmıştır. Bir aydan uzun takipli 440 hastanın beş yıllık genel ve olaysız sağkalım hızı sırası ile %75,3 ve %66,9 saptandı. Sonuç: Hastalarımızın epidemiyolojik verileri lösemi sıklığı dışında diğer epidemiyolojik çalışmalardaki sonuçlarla benzer saptanmıştır. Merkezimizde lösemi hastaları 2010 yılından itibaren izlenmeye başlanmıştır. Pediatrik onkoloji hastalarında yüksek sağkalım hızları multidisipliner tedavi protokolerinin ve profesyonel ekibin önemini göstermektedir.

Background and AimsIn children, complaints of a respiratory disorder are very frequent. Etiology of respiratory illness is a broad spectrum that varies from a simple viral infection to a malignant disorder. Pulmonary Langerhans cell histiocytosis (PLCH) is one of these entities and it is truly rare in children. The aim of this study is to evaluate our patients with PLCH.

N. Yazıcı, Tamamı Cilt altına Yerleştirilen Kalıcı Santral Venöz Kateter Uygulamaları ve Komplikasyonlarının Retrospektif Değerlendirilmesi. Hacettepe Üniversitesi Tıp Fakültesi, Çocuk Sağlığı ve Hastalıkları, Pediatrik Onkoloji Bölümü, Ankara, 2008.Hacettepe Üniversitesi Çocuk Onkolojisi Bölümü'nde takip ve tedavi edilen hastalarda tamamı cilt altına yerleştirilen kalıcı santral venöz kateterlerin (port kateterlerin) uygulama ve kullanımı sırasında gelişen komplikasyonları belirlemek amaçlandı. Şubat 1991 ve Ocak 2005 tarihleri arasında takip edilmiş pediatrik kanserli hastalardan 283'üne uygulanmış 301 port kateter retrospektif olarak incelendi. Komplikasyonlar gözden geçirildi. İstatistiksel analizde yüzde oranlar, ortalama değerler ve literatür bilgileri gözden geçirilerek kateter kullanım süresine göre hesaplanan komplikasyon hızları ve Mann-Whitney U ve One-Way Anova testi ile gruplar arası ortalama karşılaştırmaları kullanıldı. Toplam port kateter kalım süresi minimum 2 maksimum 1835 gün olup kümülatif süre 153757 gün olarak hesaplandı. Ortanca kalış süresi bir kez uygulanmış port kateterlerde 467; ikinci kez uygulanmış olanlarda 266,5 gündü. İzlem süresi boyunca 301 port kateterin 140'ında hiçbir komplikasyon saptanmadı. Geri kalan kateterlerde komplikasyon sayıları ve kümülatif süreye göre hesaplanan komplikasyon hızlarına bakıldığında toplam 371 komplikasyon görüldü. Bunlardan 282'si sadece enfeksiyon, 69'u sadece mekanik, 20'si her ikisinin birlikte görüldüğü epizotlardı. Toplam komplikasyon hızı 2,41/1000 kateter günü olarak hesaplandı. Enfeksiyona ait komplikasyon hızı 1,96/1000 kateter günü, mekanik komplikasyon hızı ise 0,58/1000 kateter günüydü. Enfeksiyon alt tiplerine göre bakıldığında 19 cep enfeksiyonu (0,12/1000 kateter günü), 5 tünel enfeksiyonu (0,03/1000 kateter günü), 190 kateter ilişkili kan dolaşım enfeksiyonu (1,24 /1000 kateter günü), 74 kateter ilişkili sepsis atağı (0,48 / 1000 kateter günü) ve 14 çıkış yeri enfeksiyonu (0,09/1000 kateter günü) saptandı. Mekanik komplikasyonlar; 43 port kateterde tıkanma (0,27/1000 kateter günü), 10'unda yer değiştirme (0,07/1000 kateter günü), 8'inde kırık-ayrılma (0.05 /1000 kateter günü) ve 9'unda rezervuar üzerinde cilt defektiydi (0.06/1000 kateter günü). Çalışma tamamlandığında 301 port kateterin 58'i elektif olarak kemoterapi tamamlandıktan sonra; 37'si enfeksiyon; 14'ü mekanik komplikasyon (kıvrılma, delinme, ayrılma, yer değiştirme, cilt defekti); 5'i tromboz, 5'i de hem mekanik hem enfeksiyon komplikasyonu nedeniyle çıkartıldı. Hastaların 91'inde port hala kullanımdaydı. Seksen altı hasta port cihazı üzerinde iken çeşitli nedenlerle kaybedildi. Dört hasta izlemden çıktığı için portun son durumu ile ilgili bilgi edinilemedi. Bir vakanın ise analiz sırasında portu çalışmamakta olup çıkartılması planlanmaktaydı. Enfeksiyon ilişkili 302 komplikasyonun 154 tanesi (%51) antibiyotik tedavisi ile giderildi. Tıkanma-tromboz sonucu gelişen 43 komplikasyonun 25'inde trombolitik tedavi uygulandı. Bunların 4'ünün rekombinan doku plasminojen aktivatör (rTPA), 8'inin ürokinaz ve 13'ünün de heparinli yıkama sonucu çalışır duruma geldiği görüldü. Mekanik komplikasyonların 13'ünde cerrahi revizyonla kateter çalışır hale getirildi. Port kateter takılma yıllarına göre komplikasyon hızlarına bakıldığında en yüksek enfeksiyon hızının 2001-2004, en yüksek mekanik komplikasyon hızının ise 1991-1995 yılları arasında olduğu görüldü. Komplikasyon hızı yüksek olmasına karşın az sayıda kateterin bu nedenle çıkartılmış olduğu görüldü. En önemli kateter çıkartılma nedeninin enfeksiyon olduğu saptandı. Pratikte oldukça önemli olan bu araçların daha hijyenik, dikkatli ve özenli kullanılmasının gerektiği ve komplikasyon durumlarında ise konservatif tedavi yaklaşımlarıyla kullanım süresinin uzatılabileceği sonucuna varıldı.Anahtar Kelimeler: Tamamı cilt altına yerleştirilen santral venöz kateterler (port kateterler), pediatrik kanser The aim of the study is to determine the infectious and mechanical complications of the totally implantable venous access devices (TIVADS) and to see the results of conservative management in the case of device removal. Three hundred and one TIVADS which had been implanted to 283 pediatric patients with solid tumors were retrospectively evaluated between February 1991 and January 2005. The complications were overviewed. Percent ratios, mean values and complication rates calculated according to cumulative time of port usage, comparison of mean values by Mann-Whitney U and One-Way Anova tests are used for statistical analyses. The total duration of implantation varied from 2 to 1835 days with a cumulative experience of 153757 days. The mean duration time was 467 days for the first applications. During the follow-up time 140 of the 301 devices continued to have full function without any complication. The total complication rate was 2.41/1000 catheter days. The incidence of infectious complications of the devices was 1.96/1000 days and the mechanical complications was 0.58/1000 days. The incidences of the types of infectious complications were as follows: 19 pocket infections (0.12/1000 catheter days); 5 tunnel infections (0.03/1000 catheter days); 190 catheter associated bloodstream infections (1.24/1000 catheter days); 74 sepsis burdens (0.48/1000 catheter days); 14 exit site infections (0.09/1000 catheter days). The incidences of the types of mechanical complications were as follows: 43 occlusion (0.27/1000 catheter days); 10 migration-displacement (0.07/1000 catheter days); 8 fracture-disconnection (0.05/1000 catheter days) and 9 skin necrosis (0.06/1000 catheter days). After the end of the study, the reasons which had been detected for the removal of the TIVADS were end of chemotherapy in 58, infection in 37, thrombotic occlusions in 5, and other mechanical problems in 15 and both infectious and mechanical complications in 5. In 91 patients the catheters were still in use. The 86 of the patients were died with catheters due to several reasons associated with primary disease. Four of the patients were lost to follow-up. In 1 patient the catheter was not working properly and planned to be removed at the time of the analysis. The major cause for catheter removal was infection. Fifty one percent of the catheter associated infections were treated with antimicrobial agents. Trombolytic therapy was administered in 25 conditions and restored the function in all of the catheters. Thirteen surgical revisions led to the salvage of the catheters with full function. According to the implantation years, the highest infection rates were between the years of 2001-2004, and the highest mechanical complications occured between the years of 1991-1995. Although the complication rate was relatively high, limited number of catheters were removed due to complications. These devices are important in practice, so they should be used more carefully and managed conservatively in the case of complications.Keywords: Totally implantable venous access devices (TIVADS), pediatric cancer 98

Hacettepe Üniversitesi Tıp Fakültesi Çocuk Onkoloji, Beyin Cerrahisi, Patoloji, Radyasyon Onkolojisi 06100 Sıhhiye/ ANKARA Giriş: Diensefalon beyinde anatomik olarak derin bir bölge olup pek çok histopatolojik tipte tümörlerin yerleşebildiği bir bölgedir. Bu tümörlerin klinik davranış, tedavi biçimleri ve prognozları histolojileriyle ve yerleşim yerleri ile ilişkili olarak birbirinden oldukça farklıdır. Hastalar ve metot: Retrospektif olarak 1971 - 2002 yılları arasında bir referans merkezi olan hastanemize başvurmuş olan diensefalonda tümörü bulunan toplam 165 vaka tesbit edildi. Bütün hastaların demografik özellikleri, tümör histopatolojik tipleri klinik uygulamaları ile bir grup vakanın genel ve hastalıksız yaşam hızı analizleri ortaya konarak bu bölge tümörlerinin daha iyi anlaşılıp izlem ve tedavilerinin gözden geçirilmesi amaçlandı. Bulgular: Tüm hastaların yaş ortalaması 7.7 ± 4.4 yıl, Erkek/ Kız oranı 1.1 'di. Düşük 'grade'li glial tümörlerin, diensefalik tümörlerin %41.2'sini oluşturduğu görüldü. Hastaların %22.4'ünde doku tanısı yoktu. Tümörlerin %40.6'sı optik sinir, optik kiazma ve optik traktuslardan oluşan optik yollarda yerleşmişti. Hastaların %17.6'sında nörofibromatozis tip 1 vardı. Vakaların %29.1'ine kemoterapi, %52.4'üne radyoterapi, %78.2'sine değişik rezeksiyon derecelerinde cerrahi uygulanmıştı. Yüzkırkiki hasta yaşam analizine uygun bulunmuş olup bunların 10 yıllık genel yaşam hızı %76.8, 10 yıllık hastalıksız yaşam hızı ise %42.2 olarak hesaplandı. Düşük `grade`li glial tümörlerin 10 yıllık genel ve hastalıksız yaşam hızları sırasıyla %98 ve %52.7 idi. Yüksek 'grade'li glial tümörlerde beş yıllık genel yaşam hızı % 21.6 iken hastalıksız yaşam oranı % 0 olarak tesbit edildi. Yerleşim yeri gruplarına göre optik myollarda yerleşmiş olan tümörler için 10 yıllık genel ve hastalıksız yaşam hızları sırasıyla %92.7 ve %55.5 olarak bulundu. Sonuç: Bu bölgede en fazla düşük 'grade'li glial tümörlerin bulunduğu ve öncelikli olarak optik yollarda yerleştiği görülmüştür. Hastaların önemli bir kısmında derin yerleşimli bir bölge olması nedeniyle doku tanısına gidilmediği saptanmıştır. Yaşam analizlerinde histopatolojik gruplar arasında belirgin istatistiksel fark saptanmış olup bunun nedeni olarak bu bölgede benign ve malign nitelikli değişik tümör gruplarının heterojen olarak bulunması düşünülmüştür. Bu histopatolojik gruplar içerisinde düşük gradeli glial tümörler için cerrahi dışında uygulanmış tedaviler literatürle benzerlik göstermekte olup bu grup hastaların tüm dünyada tedavisi halen tartışmalıdır. The clinical behaviour of these tumors are different from each other affected mainly by their histopathological type and the site of origin. Materials and Method: It had been aimed to investigate retrospectively the histopathological types, demographic properties, clinical management and outcome of the patients with diencephalic tumors who admitted to Hacettepe University Faculty of Medicine between 1971-2002. The median age of the patients was 7.7 ± 4.4 years, male to female ratio was 1.1. The ratio of low 'grade' glial tumors was 41.2% in the whole group of 165 diencephalic tumors. The tumors in the optic-chiasmatic-hypothalamic region were about 40.6% of all. The 17.6% of all patients had neurofibromatosis type 1. Chemotheraphy and radiotheraphy had been applied to 29.1% and 52.4% of all patients respectively. Survival analyses has been made to 142 eligible patients of all histopathological types. Ten-year overall survival was %76.8, and the 10-year event-free survival was 42.2% for the whole group. In the case of high grade glial tumors the five year overall and the event free survival rates were 21.6% and 0% respectively. Ten year-overall and event free survival rates were 92.7% and 55.5% respectively in the tumors of the optic, chiasmatic and hypotalamic region.Conclusion: It had been observed that low grade gliomas were the most common type of tumors in optic pathways of the diencephalic region. Histopathological confirmation could not be made in the majority of the cases because the diencephalon is a deep seated region anatomically. In the survival analyses a statistically significant relationship had been found between the histopathological groups because the diencephalon is a region where many types of benign and malignant tumors might occur. Although we know that the current management of these tumors are still contoversial, in the case of low grade gliomas the treatment strategies were similar with the literature except surgery. VI 98

Amaç: Son 4 yıl içerisinde ITP tanısı almış pediatrik hastalarda başvuru ve klinik özelliklerinin tanımlanması, klinik gidişat, tedavi şekli ve komplikasyonların değerlendirilmesi; infeksiyon öyküsü, yaş, cinsiyet, tedavi şekli ve başvuru trombosit sayısının kronikleşme ve relaps üzerine etkilerinin belirlenmesi amaçlandı.Gereç ve Yöntem: Ocak 2005-Haziran 2009 tarihleri arasında yaşları 4 ay ile 11 yaş arasında ortalama 64.2±39.9 ay değişen akut ITP tanısı almış toplam 40 çocuk hasta dosya kayıtlarından geriye dönük olarak değerlendirildi. Bulgular: Sekiz hasta % 20 kronik ITP geliştirdi. Akut ve kronik ITP grupları arasında cinsiyet, yaş, infeksiyon öyküsü ve trombosit sayısı açısından fark saptanmadı. Relaps 12 hastada % 30 , tanı sonrası ortalama 22±25.8 gün içerisinde gelişti. Relaps olanların % 66.7’si 8 hasta kronik ITP geliştirirken, % 33.3’ü 4 hasta akut İTP olarak değerlendirildi. Relaps gözlenmeyen hastaların hiçbirinde kronikleşme görülmedi. Relaps oranının istatistiksel olarak anlamlı şekilde kronik grupta yüksek olduğu saptandı. Relaps gösteren grupta tanı sonrası 15. gün trombosit sayısı ortancalarının anlamlı olarak düşük olduğu görüldü. Tedavi türünün relaps üzerine etkisinin olmadığı saptandı.Sonuçlar: Tanı sonrası 15. gün trombosit değerinin relaps olacak hastaların belirlenmesinde kullanılabileceği, ancak vaka sayısı kısıtlı olduğundan daha geniş hasta populasyonunda yapılacak prospektif çalışmalarda bu sonucun değerlendirilmesinin daha uygun olacağı sonucuna varıldı, Aim: To describe presenting features and clinical characteristics of ITP and evaluate clinical course, treatment modalities, and complications and determine the effects of preceding infection history, age, gender, treatment modality, and admission platelet count on relapse and chronicity in pediatric patients treated over a 4 year period. Material and Method: Forty cases with an age range of 4 months to 11 years mean 64.2±39.9 months who were diagnosed with ITP between January 2005 and June 2009 were retrospectively evaluated from medical records. Results: Eight patients 20 % progressed to chronic ITP. No difference was found between acute and chronic ITP groups with respect to gender, age, preceding viral infection and platelet count. Relapse developed in 12 patients 30 % , after a mean of 22±25.8 days. While 66.7 % 8 patients of the relapsed cases developed chronic ITP, 33.3 % 4 patients were evaluated as acute ITP: None of the cases who have not relapsed progressed to chronic ITP. Relapse rate was statistically significantly higher in the chronic group. Median platelet count at day 15 after diagnosis was significantly lower in the relapse group. It was found that treatment modality had no effect on relapse. Conclusions: Although platelet count at day 15 after diagnosis can be used in determining the patients who will relapse, since the number of cases are limited it is concluded that it would be better to evaluate this result in a prospective study including a larger study population

Acute cerebral venous sinus thrombosis is a rare disorder in children, but is often secondary to infections. The clinical features of cerebral venous sinus thrombosis are based on the localization and size of the affected vessel and age of the patient. In this article we encountered two different presentations of acute cerebral venous sinus thrombosis in two children aged 8 and 12 years old. [Cukurova Med J 2014; 39(4.000): 922-926]
