Abstract

Background: The aim of this study was to evaluate the efficacy and safety of retrograde intrarenal surgery (RIRS) in patients with renal calculi with solitary kidneys (SKs). Materials and Methods: In this retrospective, multicenter study, a matched case–control study was carried out using the data from 522 RIRS patients treated between 2014 and 2021. Patients' demographic data, stone characteristics, operative outcomes, perioperative and postoperative complications, and surgical success were analyzed. All patients were evaluated with noncontrast-enhanced computed tomography (NCCT) preoperatively and 1 month after the surgery. Surgical success was defined as no evidence of remaining residual fragments of <3 mm in the first-month postoperative NCCT images. The case group of 29 patients with SKs (Group 1) treated with RIRS were matched with 76 control patients (Group 2) with bilateral kidneys, who underwent unilateral RIRS by propensity score–matched (PSM) analysis. Results: After PSM analysis, the demographic and clinical data did not differ significantly between the groups. The stone burden was similar between the groups: 733.6 mm3 (range: 50.4–7565.9) versus 991.1 mm3 (range: 201.2–4380.6) (P = .09), respectively. The perioperative complication rates were 13.8% (n = 4) in Group 1 and 11.8% (n = 9) in Group 2 (P = .78). There was no statistically significant difference between the groups for postoperative complication rates (minor complications, classified as Clavien 1 or 2), (6.9% [n = 2] versus 13.2% [n = 10; P = .34]), respectively. Surgical success was 82.8% (n = 24) in Group 1 and 83.6% in Group 2 (P = .92). There was no significant difference between preoperative and postoperative glomerular filtration rate and creatinine values (P = .005). Conclusions: Our results support that RIRS is a safe and effective treatment method in SK patients with similar complication and stone-free rates compared to patients who had bilateral functional kidneys and underwent unilateral RIRS. Abstract

I- ÖZET: Metastatik veya hormona dirençli prostat kanserinde değişik tedavi alternatifleri bulunmasına rağmen, hastalarda uzun dönem sağkalım sağlanamamaktadır. Bu nedenle, ileri evre prostat kanseri olan hastalar için farklı tedavi seçenekleri araştınlmaktadır. Tümör hücrelerine karşı bağışıklık sisteminin uyarılması, araştırılmakta olan tedavi yöntemleri arasında bulunmaktadır. Bu çalışmada, sıçan prostat kanseri modelinde, tümör lizatı ile uyarılmış dendritik hücre tedavisinin etkinHği araştırılmıştır. Çalışma toplam 17 adet `Copenhagen` sıçanı ile yapılmıştır. Sıçanların 5 'i kontrol grubu, 5'i birinci tedavi kolu ve yine 5'i ikinci tedavi kolu olarak ayrılırken, 2 sıçan da dendritik hücre elde edilmesi amacıyla kullanılmıştır. Feda edilen 2 sıçarım ön ve arka bacak kemikleri ve vertebral kolumu çıkartılarak labaratuvar ortamında dendritik hücreler elde edilmiştir. Eş zamanlı olarak kontrol ve tedavi grubundaki sıçanlara `MAT-ly-lu` prostat kanseri tümör hücreleri enjekte edilerek tümör oluşturulmuştur. Ardından kontrol grubuna serum fizyolojik, 1. tedavi grubuna 10.000 adet/ml. ve 2. tedavi grubuna 15.000 adet/ml. dendritik hücre enjeksiyonu yapılarak izlenmiştir. Yirmi günlük izlem sonunda sıçanlar giyotin yöntemi ile feda edilerek, tümör dokuları çıkartılmış ve laparatomi yapılarak metastatik değerlendirme yapılmıştır. Çıkartılan dokularda apoptoz değerlendirilerek aşı tedavisinin etkinliği değerlendirilmiştir. Her üç grupta da izlem süresi boyunca ortalama tümör yüklerinde artış olduğu gözlenirken, bu artışın kontrol grubunda, tedavi gruplarına göre istatistiksel olarak anlamlı derecede fazla olduğu tespit edilmiştir. Tedavi grupları arasında da 1. ve 2. tedavi grubu arasında anlamlı fark olduğu gözlenmiştir. Tümör dokusundaki apoptoz oranlan değerlendirildiğinde benzer ilişkiler tespit edilmiştir. Toplam 15.000 adet/ml. dendritik hücre aşısı uygulanan sıçanlarda diğer gruplara oranla istatistiksel olarak anlamlı etki sağlanabildiği gösterilmiştir. Bu çalışmada, tümör lizatı ile olgunlaştınlan sıçan dendritik hücrelerinin, prostat kanseri sıçan modelinde oldukça etkili bir tedavi yöntemi olduğu gösterilmiştir. Bu konu hakkındaki çalışmaların artması, dendritik hücre aşı tedavisinin çok daha başarılı olmasına olanak sağlayacaktır. H- ABSTRACT Although, there had been several treatment alternatives for metastatic and hormone resistant prostate cancer, long term survival of the patients has not been established. For this reason, different treatment strategies has been underinvestigation for advanced stage prostate cancer. One of this investigations is the stimulation of immune system against tumor cells. The aim of this study is to investigate the effect of tumor lysate activated dendritic cell vaccine in prostate cancer rat model. The study was performed with a total of 17 `Copenhagen` rats. There were 5 rats in control group, 5 in first treatment arm and 5 in second treatment arm. Other 2 rats were used to obtain dendritic cells. Dendritic cells were obtained from anterior and posterior extremities and vertebral column of the rats. Prostate cancer was formed by injection of `MAT-ly-lu` prostate cancer tumor cells to the rats. Serum physiologic solution was injected to control group, while 10.000 unit/ml. and 15.000 unit/ml dendritic cells were injected to first and second treatment arm respectively. The rats were sacrificed by guillotine technique after a 20 days of follow-up. First the tumors was excised and then a laparotomy was performed to identify the possible metastasis. The effect of the vaccine therapy was determined by apoptotic evaluation of the excised tissues. Although there was an increase in mean tumor mass of each group, this was statistically significantly higher in control than in treatment groups. There was also a statistically significant difference between treatment groups. The results of apoptotic evaluation was similar. There was a statistically significant effect in tumor regression in rats who had 15.000 unit/ml. dendritic cells injection. In this study, tumor lysate activated dendritic cell vaccine was defined to be an effective treatment modality in prostate cancer rat model. By the advancing of clinical studies about this subject, success of dendritic cell vaccine will be more effectively established. 50. Tissue and Plasma Endocan and Endoglin Levels in Patients with Bladder Cancer.

Abstract

Objective: In this study, we aimed to compare plasma endoglin and endocan levels between bladder cancer patients and the control group. The secondary aim of the study was to compare plasma and tissue endoglin and endocan levels between high grade and low grade bladder cancers. Methods: A total of 36 patients with bladder cancer and 20 patients as control group were included. Eight milliliters of peripheral venous blood sample was taken from both study and control group to determine plasma Endocan and Endoglin levels. A tissue sample of 0.5 cm3 was obtained by cystoscopy and resection of papillary lesion of bladder to determine tissue Endocan and Endoglin levels. Results were compared between bladder cancer and control group. Result: The mean plasma Endoglin levels were 10.2 (4.5-13.7) ng/mL in low grade bladder cancer, 14.6 (8.4-22.3) ng/mL in high grade bladder cancer and 8.8 (6.3-19.4) ng/mL in the control group. The mean plasma Endocan levels were 520.4 (346.9-647.6) ng/L in low grade bladder tumors; 728.5 (311.4-1518.1) ng/L in high grade bladder tumors and 479.9 (140.6-864.7) ng/L in the control group. Plasma Endoglin level of control group was significantly lower than bladder cancer group (p<0.001). Plasma Endocan level was not different between bladder cancer and control group (p=0.117). Conclusions: Plasma Endoglin level was up-regulated in bladder cancer patients whereas plasma Endocan level did not show any difference. [ABSTRACT FROM AUTHOR]
