Computer Science - Distributed, Parallel, and Cluster Computing - Abstract
We describe matrix computations available in the cluster programming framework, Apache Spark. Out of the box, Spark provides abstractions and implementations for distributed matrices and optimization routines using these matrices. When translating single-node algorithms to run on a distributed cluster, we observe that often a simple idea is enough: separating matrix operations from vector operations and shipping the matrix operations to be ran on the cluster, while keeping vector operations local to the driver. In the case of the Singular Value Decomposition, by taking this idea to an extreme, we are able to exploit the computational power of a cluster, while running code written decades ago for a single core. Another example is our Spark port of the popular TFOCS optimization package, originally built for MATLAB, which allows for solving Linear programs as well as a variety of other convex programs. We conclude with a comprehensive set of benchmarks for hardware accelerated matrix computations from the JVM, which is interesting in its own right, as many cluster programming frameworks use the JVM. The contributions described in this paper are already merged into Apache Spark and available on Spark installations by default, and commercially supported by a slew of companies which provide further services.

FIBROUS composites - Abstract
Aligned discontinuous fibre-reinforced composites are becoming more popular because they have the potential to offer stiffness and strength comparable to their continuous counterparts along with better manufacturability. However, the modelling of highly aligned discontinuous fibre composites is still in its infancy. This paper aims to provide a comprehensive review of the available literature to understand how modelling techniques have developed and consider whether all aspects which could affect the performance of aligned discontinuous fibre composites have been addressed. Here, for the first time, a broad view of the advantages, perspectives, and limitations of current approaches to modelling the performance and behaviour of aligned discontinuous fibre composites during alignment, forming, and mechanical loading is provided in one place as a route to design optimisation. The initial aim of this work was to elucidate the mutual influence of different single-stranded segments (loops and caps) on the thermodynamic stability of RNA G-quadruplexes. To this end, we used a new NAB-GQ-builder software program, to construct dozens of two-tetrad G-quadruplex topologies, based on a designed library of sequences. Then, to probe the sequence–morphology–stability relationships of the designed topologies, we performed molecular dynamics simulations. Their results provide guidance for the design of G-quadruplexes with balanced structures, and in turn programmable physicochemical properties for applications as biomaterials. Moreover, by comparative examinations of the single-stranded segments of three oncogene promoter G-quadruplexes, we assess their druggability potential for future therapeutic strategies. Finally, on the basis of a thorough analysis at the quantum mechanical level of theory on a series of guanine assemblies, we demonstrate how a valence tautomerism, triggered by a coordination of cations, initiates the process of G-quadruplex folding, and we propose a sequential folding mechanism, otherwise dictated by the cancellation of the dipole moments on guanines.

Transplantation ,Nephrology - Abstract
Background Data on use of interleukin (IL)-1 blockers in kidney transplant recipients (KTRs) with familial Mediterranean fever (FMF) are very limited. We aimed to evaluate the efficacy and safety of anakinra and canakinumab in the transplantation setting. Methods In this retrospective cohort study, we included KTRs who suffered from AA amyloidosis caused by FMF and treated with anakinra or canakinumab (study group, n = 36). Using propensity score matching, we selected 36 patients without FMF or amyloidosis from our database of 696 KTRs as the control group. Primary outcomes were patient and graft survival. Biopsy-confirmed graft rejection, changes in estimated glomerular filtration rate (eGFR), high-sensitivity CRP (hsCRP), erythrocyte sedimentation rate (ESR), proteinuria and number of monthly attacks were secondary outcomes. Results All KTRs with FMF began IL-1 blocker therapy with anakinra and nine (25%) were switched to canakinumab. Overall death was more frequent in the study group (19.4% vs 0%) (P = .005); however, overall graft loss was comparable between study (27.8%) and control groups (36.1%) (P = .448). Five- and 10-year graft survival rates were significantly higher in the study group (94.4% and 83.3%, respectively) than in the control group (77.8% and 63.9%, respectively) (P = .014 and P Conclusions Anakinra and canakinumab are effective in KTRs suffering from FMF; however, the mortality rate may be of concern.

In this article, we investigate the association between testicular cancer and hematological paraneoplastic syndrome, which is seen infrequently among all types of tumors in the literature. This case report provides a comprehensive evaluation of a 28-year-old patient admitted to the emergency department with symptoms of vomiting, nausea, and a medical history of epilepsy. Although the initial presentation did not exhibit specific symptoms or signs associated with anemia, subsequent blood tests were interpreted in favor of hemolytic anemia. Our study provides detailed radiological findings and laboratory tests that provide comprehensive information about the progression of the lesions and the patient’s response to our treatment. Original Article - Abstract
Objective: Severe acute respiratory syndrome (SARS) coronavirus 2 (SaRS-Cov-2) associated respiratory disease (COVID-19), announced as a pandemic, is a multisystem syndrome. SARS-CoV-2 directly infects and damages vascular endothelial cells, which leads to microvascular dysfunction and promotes a procoagulant state. Dipyridamole (DP) acts as a reversible phosphodiesterase inhibitor and is used mainly as an antiplatelet agent. It is hypothetised that it has possible activities in COVID-19. Design and Methodology: We report our retrospective, real-world results of DP added to low-molecular weight heparin (LMWH) in the treatment of 462 clinically diagnosed and hospitalized COVID-19 patients. We compared anticoagulation with and without DP addition with no administration of anticoagulation in the same time frame. The primary outcome was proven or highly suspected coagulopathy within 30 days of hospitalization. Results: Definitive coagulopathy has been diagnosed in 3 (3.5%) of 85 LMWH administered patients and 7 (2.13%) of 328 DP + LMWH received patients (P=0.456). Five cases with definitive coagulopathy were not initiated any anticoagulation at the time of the event. The multivariate analysis showed that DP addition to the anticoagulant approach did not have any impact on the risk of demonstrated coagulopathy and highly-suspected coagulopathy. Conclusion: We think that our clinical experience is valuable in showing the real-life results of DP + LMWH treatment in COVID-19. This approach did not affect the coagulopathy rate. Our data did also not document an additive effect of DP in the COVID-19 outcome. Prospective controlled trials would give more convincing results regarding the role of DP in COVID-19 endothelial dysfunction and clinical outcome.

MULTISYSTEM inflammatory syndrome , *LOW-molecular-weight heparin , *SARS-CoV-2 , *ADULTS , *PEPTIDES , *LYMPHOPENIA - Abstract
Objective: Multisystem Inflammatory Syndrome (MIS) is a condition seen in the early post-COVID-19 period and thought to develop with an impaired immune response. It has been usually reported in children but rarely in adults. Here we report the first adult MIS (MIS-A) case series from Turkiye. Material and Methods: Six patients who met the Centers for Disease Control and Prevention’s MIS-A diagnostic criteria were included in the study. The demographic, clinical, laboratory, radiological characteristics and therapy regimes and outcomes of the patients were recorded. Results: All of our cases had a history of mild COVID-19. They presented with fever, severe fatigue and hypotension. Abnormal echocardiography findings were detected in five patients. Only one patient had multiple mucocutaneous findings. Common laboratory features were lymphopenia, markedly increased C-Reaktive Protein, procalcitonin, pro-brain natriuretic peptide (pro-BNP), D-dimer, and ferritin. All patients had positive SARSCoV-2 antibody result. Corticosteroids and/or anakinra were used in five, and intravenous immunoglobulin was used in two patients. Low-molecular-weight heparin (LMWH) was used for all cases. Empirically initiated antibiotic treatments were discontinued after cultures were negative. After anti-inflammatory treatment, the hypotension of the patients resolved, they did not need intensive care follow-up and no mortality was seen in our cases. Conclusions: MIS-A is a severe and mortal condition that causes various clinical pictures and can be confused with sepsis. Anakinra, a recombinant IL-1 receptor antagonist, is a significant agent that can be used in the treatment of MIS-A since it blocks the cytokine cascade at an early stage. The satisfactory responses will be obtained with early diagnosis and anti-inflammatory treatment. In this period when the pandemic is not over yet, it is necessary to increase the awareness of clinicians about MIS-A, which can be fatal. [ABSTRACT FROM AUTHOR]

Introduction: Coronavirus has caused a pandemic since it was first detected in Wuhan in December 2019. The mortality rate is high in moderate and severe cases. Our study aimed to screen the CBC parameters as a useful predictive factor for COVID-19 resulting in critical illness. Methods: A total of 285 patients with positive PCR results were analyzed. The median age was 55 (24-90), and 64.2% of patients were male. Sixty-eight percent of cases were hospitalized with moderate, 32% with severe disease at initial admission. Results: We found that lymphocyte count 6, and platelet to lymphocyte ratio (PLR) >350 were predictive of the outcome. We scored our cohort 0-3 for these three parameters. Patients with a score of 2-3 were more likely to have progressive disease, anti-cytokine treatment, intensive care admission, intubation, and death, compared to patients with a score of 0-1. Additionally, they tended to be hospitalized for longer (median 11.5 days, mean 15.6), compared to those with a score 0 or 1 (median 9 days, mean 11.3). Twenty-eight of 38 cases with scores of 2-3 were discharged (73.6%), whereas the rate was 89% for patients with a score of 0-1 (P=0.009). Conclusion: Based on the absolute lymphocyte count (6, PLR >350), our three-parameter score was able to predict disease progression, and the likelihood of anti-cytokine treatment, intubation, and death. We think that COVID-19 patients presenting with moderate to severe pneumonia, and having scores of 2 or 3 on our scale, should be closely monitored and robustly supported.

The paper presents a multi-phase approach for selecting a country in which to locate a global manufacturing facility. An influence diagram is used to frame the decision. A decision tree then analyzes uncertainties regarding cost and generates a risk profile. The risk profile becomes one of the measures in an MAUT model that incorporate a wide range of factors. This sequential approach of using the output from a decision tree as input to MAUT is demonstrated with an example involving an auto supplier locating a new plant in one of five countries. Three decision makers were interviewed to determine the weights and the shape of the individual utility curves. The paper identifies, clearly defines, and incorporates a variety of measures for which national data are readily available. This list is broader and less subjective when compared to other examples reported in the literature. Keywords: Decision support systems; Location; Multicriteria; Risk, 1. Introduction Business globalization has led to global manufacturing [1]. There are many reasons why companies establish manufacturing facilities in other countries, among them: tariff and trade concessions, cheap labor, [...]

Deficiency of vitamins and minerals is an important public health problem in both developing and industrialized countries. Consequently, food fortification practices are often used to combat nutritional deficiencies in the general populace. In Turkey, women in almost every age group do not meet the recommended daily iron intake. Furthermore, iron intake was found to be insufficient in males and in children (2-5, 6-8 and 9-11 ages). Folate intake is also under daily requirement in women of childbearing age. Serum vitamin D levels of 81.7% of mothers were deficient (19.9ng/mL and below) and 11.6% were insufficient (20-29.9 ng/mL). In a study of 6 to11 month-old infants, 26.8% of children were deficient (14.99ng/mL and below), 14.7% were slightly deficient (15-19.9 ng/mL) and 26.2% were insufficient (20-29.9 ng/mL). The median levels of serum vitamin D in 10,160 samples was found to be 23.8 ng/mL. Of the participants, 6.9% had below 20 ng/mL and 66.7% below 30ng/mL respectively. Fortification is important for eliminating these deficiencies and combating stunting, which is still relatively common in Turkey. The protection and prevention of diseases are easier and more cost-effective than treatment and rehabilitation practices. This report was produced through examining national study results and international practices, with the aim of contributing to regulatory practices to be developed around nutritional deficiencies. Regulations should be made to fortify flour with iron and folic acid; milk and oils with vitamin D and facilitate access to these products., Çeşitli vitamin ve minerallerin yetersizliği; gelişmekte olan ve endüstrileşmiş ülkelerde sağlık ve ekonomik sonuçları ile önemli bir halk sağlığı sorunudur.

Delamination is one of the main failure mechanisms in composite structures. Initiation and propagation of delamination are controlled by interface parameters including the interlaminar strength and fracture toughness, respectively. Hence, it is crucial to use the correct values of these parameters in the design and analysis of components such as composite tapered beams and curved beams. In this study, the effect of ply orientation on the interlaminar tensile strength and Mode I fracture toughness for CFRP, and GFRP laminates are investigated. For this purpose, curved beam strength (CBS) experiments are conducted on CFRP and GFRP laminates with 0˚/0˚, 25˚/-25˚, 45˚/-45˚ and 65˚/-65˚ interfaces and double cantilever beam (DCB) experiments are conducted on CFRP, and GFRP laminates with 0˚//0˚, 25˚//-25˚, 45˚//-45˚, 45˚//45˚, 65˚//65˚, and 90˚//90˚ interfaces. 186
