Background and Objectives: Tracheal intubation using laryngoscopy is a fundamental skill, for an anesthesiologist. However, teaching this skill is difficult since Macintosh direct laryngoscope (DL) allows only one individual to view the larynx during the procedure. Hence, this study aimed to determine whether King Vision® videolaryngoscope (KVL) provides any advantage over direct laryngoscopy in teaching this skill to airway novices. Materials and Methods: In this prospective randomized crossover study, Ethical Committee clearance was obtained from the institutional review board (MSRMC/EC/2017) and the study was registered with Clinical Trial Registry. After informed consent, 53 medical students were allotted to perform laryngoscopy and endotracheal intubation on a manikin by using either KVL or Macintosh DL. The participants first performed laryngoscopy with either KVL or Macintosh DL following a brief instruction and then crossed over to the second arm of the study to perform laryngoscopy using the other scope. The primary outcome measure was the time for successful endotracheal intubation. The secondary outcome measures were incidence of esophageal intubation (EI), excess application of pressure on maxillary teeth excess maxillary pressure, and success rate. Results: Mean time for endotracheal intubation was significantly faster using KVL than in DL (44.64 vs. 87.72 s; P < 0.001). No significant difference was found in the incidence of esophageal intubation 15.1% in KVL group versus 24.5% in DL group (P = 0.223). In the KVL group, 81.1% did not apply pressure on maxillary teeth versus 26.4% in the DL group (P < 0.001). The success rate of intubation was 100% in the KVL group versus 86.8% in the DL group (P = 0.006). Conclusion: The KVL is a more effective tool to teach endotracheal intubation in comparison to Macintosh laryngoscope in airway novice medical students. Background: Caudal analgesia is the most popular regional anesthesia technique in pediatric population for intraoperative and postoperative analgesia. Clonidine, an α2 agonist, prolongs analgesia without causing significant respiratory depression after systemic or neuraxial administration. However, the most beneficial route of its administration is still controversial. Thus, we compared the effects of caudal and intravenous (i.v) clonidine on postoperative analgesia produced by caudal levobupivacaine in children undergoing infraumbilical surgery. Methods: A comparative three group study was carried out in seventy-five pediatric patients who underwent elective surgery for infraumbilical procedures, under general anesthesia with caudal block. Group A (n = 25) received levobupivacaine 0.25% 1 mL/kg caudally and 5 mL of normal saline i.v, Group B (n = 25) received levobupivacaine 0.25% 1 mL/kg with 1 μg/kg clonidine caudally and 5 mL of normal saline i.v, and Group C (n = 25) received levobupivacaine 0.25% 1 mL/kg caudally and 1 μg/kg clonidine in 5 mL normal saline i.v. Postoperative pain was assessed for 24 h using the Children and Infants Postoperative Pain Scale Score. Ramsay sedation scale and modified Bromage scale were assessed at predetermined time intervals for sedation and motor blockade, respectively. Results: The mean duration of postoperative analgesia was significantly longer in Group B (16.68 ± 4.7 h) than in Group A (4.24 ± 1.42) and Group C (9.44 ± 3.88 h): P < 0.001. The number of patients not requiring rescue analgesia in Group B was 5, which was significantly higher than in Group C (one patient) and Group A (zero patient): P < 0.001. No motor blockade or sedation was observed in any of the groups. Conclusions: Clonidine in a dose of 1 μg/kg added to 0.25% levobupivacaine for caudal analgesia significantly prolongs the duration of analgesia, without any side effects.

Background and Objectives: The laryngeal mask airway (LMA) ProSeal is most commonly used supraglottic airway device; it is routinely inserted by blind technique. Although blind insertion technique is most widely used, there are many techniques which are available such as priming the drain tube with a guiding instrument such as a suction catheter, a gum elastic bougie, a Flexi-Slip Stylet, direct laryngoscopy, and even a fiber-optic bronchoscope (FOB). The present study was undertaken to compare and assess the placement of LMA ProSeal using blind versus direct laryngoscopy techniques using FOB. Materials and Methods: A prospective randomized comparative study of 110 patients divided into two groups of 55 each as Group I (blind insertion) and Group II (direct laryngoscopic insertion) after satisfying the inclusion criteria. The anatomical position was assessed by flexible FOB and evaluated based on fiber-optic scoring system. Results: In the present study, demographic characteristics, vital parameters, Mallampati score, and Wilson's score were comparable in both the groups (P > 0.05). The fiber-optic score (FOS) 1 in Group II was 78.18% compared to 60% in Group I, but the difference was statistically not significant (P > 0.05). Furthermore, the mean FOS in Group II was slightly high (3.84 ± 0.87) compared to Group II (1.62 ± 0.87), but the difference was statistically not significant (P > 0.05). Further hemodynamic parameters (P > 0.05) and complications (P > 0.05) were comparable in both the groups. Conclusion: The LMA placement scoring was similar in both blind and direct laryngoscopic techniques. Blind insertion technique is a simpler, easier, and has stood the test of time.

Background and Aims: The management of post-cesarean delivery pain is of utmost importance to prevent undesirable outcomes. Local anesthetic wound infiltration and bilateral ilioinguinal-iliohypogastric (ILIH) nerve block are two potential techniques to provide better postoperative analgesia. In this study, these two techniques have been compared for the management of postoperative pain in the elective cesarean section. Materials and Methods: After approval from the institutional ethics committee and informed consent from patients, this study was conducted on 150 patients who underwent elective cesarean section under spinal anesthesia. Patients were allotted into three groups: group C (postoperative sham injection), group L (postoperative infiltration of incision site with 20 mL of 0.5% ropivacaine), and the group I (postoperative bilateral ILIH block with 10 mL of 0.5% ropivacaine on each side under ultrasound guidance). The objectives of our study were to evaluate the duration of analgesia, visual analog scale (VAS) score, and the cumulative analgesic requirement for pain relief and a number of analgesic demands. Student t-test and Mann-Whitney U test were used to compare the analgesic parameters among the groups. Results: Group I had a significantly longer duration of analgesia (515.64 ± 82.87 min) compared to group L (280.87 ± 39.47 min), and group C (246.89 ± 37.85 min). Group I had significantly lower VAS scores compared to the groups L and C. Group I (1.72 ± 0.68) had lower analgesic demands compared to group L (3.26 ± 0.64) and group C (4.62 ± 0.65). The cumulative analgesic requirement was significantly lower in group I. Conclusion: ILIH nerve block has a longer duration of postoperative pain relief in cesarean delivery patients compared to local infiltration and placebo.

Background: The current bedside predictors of the difficult airway are not perfect. A new test, the acromio-axillo-suprasternal notch index (AASI), has been found to be superior to conventional predictors. In this study, we have compared the accuracy of AASI with upper lip bite test (ULBT) and modified Mallampati (MMP) test to predict difficult laryngoscopy and the time taken to complete each test. Methods: Institutional ethical committee clearance was obtained, and written informed consent was taken from 150 patients posted for elective surgery under general anesthesia with endotracheal intubation. Preoperative airway examination was carried out with AASI, ULBT, and MMP score. AASI ≥0.49, ULBT Class III, and MMP score III/IV were considered as predictive of difficult visualization of larynx (DVL). After the induction of anesthesia, the laryngeal view was recorded according to Cormack–Lehane (CL) grade. Sensitivity, specificity, predictive values, and accuracy were calculated for all the three tests. Results: DVL (CL Grades 3/4) was observed in 18 (12%) patients. AASI was found to have higher specificity (93.2%), positive predictive value (PPV) (55%), and accuracy (89.3%) when compared to MMP and ULBT. MMP was found to have the highest sensitivity (77.8%), and ULBT was found to have least sensitivity (50%). Time taken for AASI was higher (13.01 ± 1.03 s) when compared to ULBT (7.49 ± 1.95 s) and MMP (3.97 ± 0.49 s). Conclusion: We conclude that the MMP is the most sensitive and fastest test to predict DVL when compared to AASI and ULBT. AASI is a better predictor for DVL as it has higher specificity, PPV, accuracy, and odds ratio when compared to standard tests such as MMP and ULBT.

Background: Pneumoperitoneum in laparoscopic procedures is associated with hemodynamic response, due to the release of catecholamines and vasopressin. Magnesium and clonidine have been used to attenuate such hemodynamic responses by inhibiting release of these mediators. We conducted this randomized, double-blinded study to assess which of the two attenuates hemodynamic response better. Materials and Methods: Ninety American Society of Anesthesiologists health status Classes I and II patients posted for elective laparoscopic cholecystectomy were randomized into three groups of thirty patients each. Group C received injection clonidine 1 μg/kg diluted in 10 mL normal saline over 10 min, prior to pneumoperitoneum. Group M received injection magnesium sulfate 50 mg/kg diluted in 10 mL normal saline over 10 min, prior to pneumoperitoneum. Group NS received 10 mL normal saline intravenously over 10 min, prior to pneumoperitoneum. Hemodynamic parameters were recorded before induction (baseline values), at the end of magnesium sulfate/clonidine/saline administration and before pneumoperitoneum (P0), 5 min (P5), 10 min (P10), 20 min (P20), 30 min (P30), and 40 min (P40) after pneumoperitoneum. Results: Systolic blood pressure, diastolic blood pressure (DBP), mean arterial pressure (MAP), and heart rate (HR) were all significantly higher in the normal saline group compared to magnesium and clonidine. On comparing patients in Group M and Group C, DBP, MAP, and HR were significantly lower in the magnesium group. Mean extubation time and time to response to verbal commands were significantly longer in the magnesium group. Conclusions: Both magnesium and clonidine attenuated the hemodynamic response to pneumoperitoneum. However, magnesium 50 mg/kg, attenuated hemodynamic response better than clonidine 1 μg/kg.

Background and Aims: Spinal anesthesia is a safe and reliable technique for surgeries on the lower abdomen and lower limbs. Some of its characteristics like delayed ambulation and pain after block regression may limit its use, especially for short duration surgeries. 2-chloroprocaine is an amino-ester local anesthetic with an approximate duration of action of 40 minutes, which is ideal for short duration surgeries. This study aims to compare the effect of adding intrathecal buprenorphine to 2-chloroprocaine with regard to spinal anesthesia characteristics. Material and Methods: After obtaining the institutional ethical committee clearance and clinical trial registration, informed consent was taken from 90 patients who were undergoing either lower abdominal or lower limb surgeries of less than 60 minutes duration and were then randomized into two groups. Group C received 40 mg of 1% 2-chloroprocaine and Group B received 40 mg of 1% 2-chloroprocaine with 60 mcg of buprenorphine. Sensory/motor block characteristics, first analgesic requirements, time to void, and unassisted ambulation were assessed. Student t test was used to analyze the metric parameters and Fisher's exact test was used to compare the categorical variables. Results: The time of onset of sensory and motor blocks, peak sensory block, readiness for surgery, and complete regression of both sensory and motor blocks were comparable between the groups. Group B showed significantly prolonged duration of postoperative analgesia (855.82 ± 667.09 vs. 359.07 ± 253.3 minutes). 91.1% patients were able to ambulate within 100 minutes in our study. Conclusion: We conclude that addition of buprenorphine to 2-chloroprocaine has a significant synergistic effect on prolonging postoperative analgesia.

BACKGROUND: Spinal anesthesia is the more commonly administered procedure for pelvic, lower abdominal and lower limb procedures. Hypotension is the most common complication of spinal anesthesia. It is a common practice to give intravenous fluids, usually crystalloids, prior to centro neuraxial block in order to prevent hypotension and this is usually referred as preloading. AIMS: Compare the efficacy of preloading with crystalloid to that of colloids in reducing the incidence and severity of hypotension in spinal anesthesia for lower abdominal surgery and also to find out if hydroxyl ethyl starch (HES) is a better colloid than polygelatin. STUDY DESIGN: The study population included 150 patients belonging to either sex undergoing elective lower abdominal surgeries under spinal anesthesia. METHODS AND MATERIAL: One hundred and fifty ASA I and II patients posted for lower abdominal surgeries under spinal anesthesia were randomly allocated to receive either 10mL/Kg of HES (Group A), 10mL/Kg of polygelatin (Group B) or 10mL/Kg of Ringer's lactate solution as preloading solution. Heart rate, systolic blood pressure (SBP), diastolic blood pressure (DBP) and mean arterial pressure (MAP) were recorded once in two minutes for the first ten minutes and once in five minutes for the next fifty minutes. The incidence of hypotension and the mean dose requirement of mephentermine were noted after spinal anesthesia. STATISTICAL METHODS: The analysis of variance to find the significance difference of pulse rate, SBP and MAP between the three groups of patients. The Post hoc test by Tukey to find the pair wise significance between the three groups. The Chi-square test to find the significance of proportions of mephentermine dose requirement between the three groups. RESULTS: The incidence of hypotension was 9 of 50 (18%) in Group A, 17 of 50 (34%) in Group B and 27 of 50 (54%) in group C. The mephentermine bolus requirements were also less in group A (9 times) when compared to group B (18times) and group C (32 times), which was statistically significant. INTERPRETATION AND CONCLUSION: HES reduced the incidence of hypotension after spinal anesthesia and required lesser mean dose requirements of mephentermine when compared to polygelatin and ringers lactate. It can be concluded from the present study that colloids offset hypotension and hypovolemia more effectively than crystalloids. Also it can be concluded that hydroxyethyl starch is a better

Celiac plexus block is a time tested method for management of pain. It is conventionally done via posterior approach. We report successful use of anterior celiac plexus to manage pain in a case of chronic pancreatitis with necrotising fasciitis of the back.

Regional anaesthesia is challenging in surgical patients with anatomical abnormalities. Patients who have undergone total laryngectomy have a permanent tracheostomy, and these individuals are termed as neck breathers. These patients have distorted anatomy due to radiation and chemotherapy. We are report the successful management of a neck breather with distorted neck anatomy for surgery of the arm with an ultrasound guided interscalene block.

Background: Dexamethasone is a potent corticosteroid when administered alone or in combination alone has proven efficacious in preventing nausea and vomiting (PONV) perioperatively. However, the administration of even a single dose has been associated with hyperglycemia. This is the first study that evaluates the effect of two low-doses of dexamethasone (4 and 8 mg) on blood glucose concentrations among diabetics and nondiabetics in patients who have received spinal anesthesia. Materials and Methods: After obtaining ethical clearance and patient consent, 180 American Society of Anesthesiologists 1–3 patients undergoing the elective infraumbilical surgeries under spinal anesthesia aged between 18 and 70 years were included in this study. Ninety diabetic patients were allotted to the diabetic group (DM), and ninety nondiabetic patients were allotted to the nondiabetic group (ND). Group DM was divided into three subgroups DM0, DM4, and DM8. Group ND was divided into three subgroups ND0, ND4, and ND8. The patients in groups DM0 and ND0 served as controls. The patients in groups DM4 and ND4 received 4 mg dexamethasone. The patients in groups DM8 and ND8 received 8 mg dexamethasone. The blood glucose concentrations were monitored at 0 (baseline), 1, 2, 3, 4, 5, 6, and 8 h after giving the drug. Results: The baseline blood glucose values were higher in diabetics compared to nondiabetics (128.57 ± 22.26 vs 94.99 ± 12.82 mg/dL). There was a statistically significant increase in blood glucose concentrations in both diabetics and nondiabetics who received dexamethasone. The rise of blood glucose from baseline was similar in both diabetics and nondiabetics. Conclusion: The maximum rise in blood glucose was in the range of 40–45 mg/dl in the patients who received dexamethasone. The clinician should use his clinical judgment before administering dexamethasone for PONV prophylaxis/treatment.

The management of a neonate with a known difficult airway is a challenge to any clinician. We report a four-day-old neonate with a known difficult airway, who presented to us for rigid bronchoscopy. We used an innovative, economical and easily available adult central venous line guidewire to secure the airway and reintubate the child.

Local anesthesia is commonly used to perform cataract surgery. We present a case in which local anesthesia systemic toxicity, which occurred after peribulbar block. It is a life-threatening complication if it is not recognized early and appropriately resuscitated can be fatal. This case report attempts to highlight the need for trained personnel, with suitable monitoring and recent guidelines in the management of local anesthesia systemic toxicity surgery under a regional technique.

Anterior mediastinal mass is a rare pathology that presents considerable anesthetic challenges due to cardiopulmonary compromise. We present a case that was referred to us in the third trimester of pregnancy with severe breathlessness and orthopnea. An elective cesarean delivery was performed under combined spinal epidural anesthesia with a favorable outcome. We discuss the perioperative considerations in these patients with a review of the literature.

We report a case of venous air embolism which occurred during intraoperative endoscopy in a five-year-old boy who had undergone Kasai procedure in his infancy. The child had a cardiac arrest during the procedure from which he could not be resuscitated. The awareness about this complication would allow rapid diagnosis, which is vital to provide specific treatment and prevent fatal outcome.
