Abstract

Abstract A disposable screen-printed sensor has been crafted specifically for therapeutic drug monitoring purposes, particularly for detecting ofloxacin in biological fluids. To enhance selectivity toward ofloxacin, a supramolecular calix [6] arene serves as the ionophore of choice. The sensor incorporates a graphene nanocomposite as an ion-to-electron transducer layer, which not only boosts potential stability but also mitigates potential drift. The developed ofloxacin sensor underwent rigorous characterization following IUPAC guidelines. The linearity range spans from 1 × 10–6 to 1 × 10–2 M, with a measured slope of 59.0 mV/decade. Impressively, it boasts a percentage recovery of 100.18 ± 1.60 and a low detection limit (LOD) of 6 × 10–7 M. Stability assessments indicate reliable performance over an extended period of 8 weeks. The versatility of this sensor extends to various applications, including the determination of ofloxacin in pharmaceutical formulations, bulk powder, and biological fluids. Notably, it has demonstrated efficacy post-bioanalysis validation, adhering to Food and Drug Administration regulations. This advancement holds promise for personalized therapeutic drug monitoring in clinical pharmacy studies and quality control laboratories, thereby optimizing patient care at the point-of-care. 2. Adverse events following immunisation (AEFI) of COVISHIELD vaccination among healthcare workers in Ghana

Abstract

Objective To describe the incidence of adverse events following immunisation (AEFI) and determine the factors that affect the onset and duration of AEFI after COVISHIELD vaccination among healthcare workers.Design Prospective cohort study.Setting Tertiary healthcare, Korle-Bu, Ghana.Participant Three thousand and twenty-two healthcare workers at least 18 years of age were followed up for 2 months after receiving two doses of the COVISHIELD vaccine.Primary outcome The occurrence of the AEFI was identified by self-reporting to the AEFI team members.Results A total of 3022 healthcare workers had at least one AEFI (incidence rate of 706.0 (95% CI 676.8 to 736.1) per 1000 doses) with an incidence rate of 703.0 (95% CI 673.0 to 732.0) per 1000 doses for non-serious AEFI and an incidence rate of 3.3 (95% CI 1.6 to 6.1) per 1000 doses for serious AEFI. The most commonly reported systemic adverse events were headache (48.6%), fever (28.5%), weakness (18.4%) and body pains (17.9%). The estimated median time to onset of the AEFI following the first-dose vaccination was 19 hours and the median AEFI duration was 40 hours or 2 days. Delayed-onset AEFI occurred in 0.3% after first dose and 0.1% after second dose. Age, sex, previous SARS-CoV-2 infection, history of allergies and comorbidity were not significantly associated with the onset and duration of AEFI. However, participants who used paracetamol seemed to be significantly protected (HR 0.15; 95% CI 0.14, 0.17) from having a long duration of AEFI.Conclusion The results of our study indicate a high incidence of non-serious AEFI and the rare occurrence of serious AEFI after COVISHIELD vaccination in healthcare workers. The rate of AEFI was higher after the first dose than after the second dose. Sex, age, previous SARS-CoV-2 infection, allergies and comorbidity were not significantly associated with the onset and duration of AEFI.

Abstract

Objective(s): Mesenchymal stem cells are viewed as the first choice in regenerative medicine. This study aimed to elucidate the influence of BM-MSCs transplantation on angiogenesis in a rat model of unilateral peripheral vascular disease. Materials and Methods: Twenty-one rats were arbitrarily allocated into three groups (7/group). Group I: control sham-operated rats, Group II: control ischemic group: Rats were subjected to unilateral surgical ligation of the femoral artery, and Group III: ischemia group: Rats were induced as in group II, 24 hr after ligation, they were intramuscularly injected with BM-MSCs. After scarification, gastrocnemius muscle gene expression of stromal cell-derived factor-1 (SDF-1), CXC chemokine receptor 4 (CXCR4), vascular endothelial growth factor receptor 2 (VEGFR2), von Willebrand factor (vWF), and hypoxia-inducible factor-1α (HIF-1α) were analyzed by quantitative real-time PCR. Muscle regeneration and angiogenesis evaluation was assessed through H&E staining of the tissue. Furthermore, Pax3 and Pax7 nuclear expression was immunohistochemically assessed. Results: Rats treated with BM-MSCs showed significantly raised gene expression levels of SDF-1, CXCR4, VEGFR2, and vWF compared with control and ischemia groups. H&E staining of the gastrocnemius showed prominent new vessel formation. Granulation tissue within muscles of the ischemic treated group by BM-MSCs showed cells demonstrating nuclear expression of Pax3 and Pax7. Conclusion: BM-MSCs transplantation has an ameliorating effect on muscle ischemia through promoting angiogenesis, detected by normal muscle architecture restoration and new blood vessel formations observed by H&E, confirmed by increased gene expression levels of SDF-1, CXCR4, VEGFR2, and vWF, decreased HIF-1α gene expression, and increased myogenic Pax7 gene expression.

Abstract Background In low- and middle-income countries, the paucity of conventional health services means that many people with mental health problems rely on traditional health practitioners (THPs). This paper examines the possibility of forging partnerships at the Primary Health Care (PHC) level in two geopolitical regions of Ghana, to maximize the benefits to both health systems. Methods The study was a qualitative cross-sectional survey. Eight (8) focus group discussions (FGDs) were conducted between February and April 2014. The views of THPs, PHC providers, service users (i.e. patients) and their caregivers, on the perceived benefits, barriers and facilitators of forging partnerships were examined. A thematic framework approach was employed for analysis. Results The study revealed that underlying the widespread approval of forging partnerships, there were mutual undertones of suspicion. While PHC providers were mainly concerned that THPs may incur harms to service users (e.g., through delays in care pathways and human rights abuses), service users and their caregivers highlighted the failure of conventional medical care to meet their healthcare needs. There are practical challenges to these collaborations, including the lack of options to adequately deal with human rights issues such as some patients being chained and exposed to the vagaries of the weather at THPs. There is also the issue of the frequent shortage of psychotropic medication at PHCs. Conclusion Addressing these barriers could enhance partnerships. There is also a need to educate all providers, which should include sessions clarifying the potential value of such partnerships.

Abstract Background Four rapid, accurate, and validated stability-indicating spectrophotometric methods have been described in the present work for the analysis of trimebutine maleate (TM) in existence of its degradation products in its authentic form and in pharmaceutical formulations excluding any separation steps. Results These methods were a dual-wavelength (DW) method which allows the determination of TM in existence of its degradation products at 243 nm and 269 nm, second derivative (D 2) method measured at peak amplitude at 268 nm, ratio difference (RD) method at 242 nm and 278 nm, and constant center coupled with spectrum subtraction (CC-SS) method at 242 nm and 278 nm versus 278 nm. By applying the suggested methods, TM could be quantified in the range of 5.0–60.0 μg/mL with percentage recoveries 99.97 ± 0.40, 100.36 ± 0.58, 99.90 ± 0.42, and 100.15 ± 0.45 for DW, D 2, RD, and CC-SS methods, respectively. International Conference on Harmonization guidelines were followed for validation of the described methods, and the application of laboratory-prepared mixtures along with different pharmaceutical drugs including the target drug showed favorable results without any contribution from additives. Conclusions Statistical comparison was used to compare the proposed and official methods, and satisfactory results for both accuracy and precision were obtained. The results confirm the applicability of the suggested methods for the determination of TM in quality control laboratories.

Abstract

Abstract Background Emerging researchers in low- and middle-income countries (LMIC) face many barriers, including inadequacies in funding, international exposure and mentorship. In 2012, the National Institute of Mental Health (NIMH) funded five research hubs aimed at improving the research core for evidence-based mental health interventions, enhancing research skills in global mental health, and providing capacity building (CB) opportunities for early career investigators in LMIC. In this paper emerging researchers contextualize their experiences. Case presentation Each of the five hubs purposively selected an emerging researcher who had experienced more than one hub-related CB opportunity and actively participated in hub-related clinical trial activities. The five ‘voices’ were invited to contribute narratives on their professional backgrounds, CB experience, challenges and successes as an emerging mental health researcher, and suggestions for future CB activities. These narratives are presented as case studies. CB activities provided broader learning opportunities for emerging researchers. Benefits included the receipt of research funding, hands-on training and mentorship, as well as exposure to networks and collaborative opportunities on a global scale. To overcome ongoing challenges of access to funding, mentoring, networking and global exposure, the emerging voices recommend making mentorship and training opportunities available to a wider range of emerging mental health researchers. Conclusions Investing in CB is not enough to ensure sustainability and leave a legacy unless it is accompanied by ongoing mentorship and international exposure. Financial investment in building research capacity, promotion of mentorship and supervision, and international networking are essential to yield well-prepared young investigators in LMIC as experienced by these rising stars. Governments and policymakers should prioritize educational policies to support the continuous development and international engagement of emerging researchers. This can advance strategies to deal with one of most important and costly problems faced by healthcare systems in LMIC: the mental health treatment gap.

Da mesma forma, utiliza-se referencial bibliográfico sobre a política externa brasileira de Lula buscando-se avaliar sua aproximação com os países emergentes e, principalmente, a escolha do multilateralismo como estratégia de inserção internacional.

Abstract

The vast majority of research on mental health has been undertaken in high income countries. This study aimed at investigating the long-term course of maternal depressive symptoms and its association with various mother- and child-related characteristics in two West African lower middle income countries with focus on the relationship with long-term anxiety symptoms.In the Child Development Study, a prospective birth cohort study in Côte d'Ivoire and Ghana, the 9-item Patient Health Questionnaire (PHQ-9) was answered by N = 776 women 3 months antepartum, and 3, 12, and 24 months postpartum between April 2010 and March 2014. Growth mixture modeling was used to identify distinct trajectories of depressive symptoms. Several psychosocial, obstetric, and sociodemographic characteristics were assessed and multinomial regression analysis was performed to investigate the influence of these variables on the different depression trajectories.We found three distinct classes of depressive symptoms that were characterized by an asymptomatic trajectory (91.5%), by recurrent risk (4.3%) and by postnatal risk (4.3%). The longitudinal course of depressive symptoms was strongly associated with anxiety symptoms (χ2 = 258.54, df = 6, p < 0.001; φ = .577). Among other factors, higher levels of anxiety, new pregnancy 2 years after birth, economic stress, and family stress were associated with the risk classes.A substantial proportion of West African women in our sample developed unfavorable patterns of depressive symptoms during the vulnerable phase of pregnancy and early motherhood. Psychosocial factors, especially antepartum anxiety symptoms, played a decisive role in this process. Perceived economic hardship further exaggerated the mental health burden.

Abstract

هدفت هذه الدراسة التعرف إلى تأثير الدخل القومي المتاح والاستثمار الاجمالي على الاستهلاك الكلي في اليمن خلال الفترة (1991- 2017م)، اعتمدت هذه الدراسة على المنهج الوصفي التحليلي والمنهج الكمي القياسي، باستخدام أداة حديثة متمثلة في نموذج الانحدار الذاتي للإبطاء الزمني الموزع (ARDL) لتقدير العلاقة في المدى القصير والطويل بين متغيرات الدراسة، باستخدام بيانات الاقتصاد اليمني الصادرة عن الجهاز المركزي للإحصاء، وقد توصلت الدراسة الى النتائج الآتية: - تبين اختبارات الاستقرار باستخدام جذر الوحدة على أن المتغيرات غير مستقرة في المستوى العام (Level)، إلا أنها مستقرة في الفروق الأولى، كما أوضحت نتيجة اختبار الحدود للتكامل المشترك وجود علاقة توازنية طويلة الأجل تتجه من المتغيرات المستقلة (الدخل القومي المتاح والاستثمار الاجمالي) الى المتغير التابع (الاستهلاك الكلي). أفضل نموذج وفق معيار (AIC) لمحددات الاستهلاك الكلي هو (1 ,2 ,4)ARDL الدخل القومي المتاح والاستثمار الاجمالي تفسر حوالي 99.9% من التغير في الاستهلاك الكلي خلال فترة الدراسة. أغلب المعلمات معنوية في الأجل القصير، وكذلك ثبوت معنوية بعض المعلمات في الأجل الطويل. بينت نتائج تحليل نموذج تصحيح الخطأ وجود الية لتصحيح الخطأ في النموذج، وأن دالة الاستهلاك الكلي الاجمالي في الأجل القصير تتجه للتوازن في الأجل الطويل، وأن معلمة تصحيح الخطأ سالبة ومعنوية. خلو النموذج المقدر من مشاكل القياس الاقتصادي المتمثلة في الارتباط الذاتي وعدم ثبات تباين حد الخطأ، أظهرت نتائج اختبارات استقرار المقدرات أن مقدرات النموذج مستقرة.

15. Office Hysteroscopy: King Hamad University Hospital Experience

Abstract

Background & Objective: Office hysteroscopy (OH) is an efficient alternative for diagnosis and management of intrauterine pathology. The main goal of this study is to determine the usefulness and tolerability of OH in the patient population of a tertiary teaching hospital in the Kingdom of Bahrain. Methods: The total population consists of 101 patients who were prospectively included in the study from 2018 to 2020. The main indication for OH were abnormal uterine bleeding followed by post-menopausal bleeding. Results: The procedure was tolerable without analgesia in 93% of cases, analgesia was required in 3.96% of cases, and minor complications consisted of abdominal pain and dizziness (0.99%), as well as uterine bleeding (0.99 %). Given a success rate of 90% using a 1.8mm diameter hysteroscope, it can be concluded that OH with Etonox (N2O2) as an analgesic is a safe and feasible procedure for patients of varying ages and parities. Conclusion: OH is particularly useful in the workup of abnormal uterine bleeding, post-menopausal bleeding, assisted IUCD removal, endometrial biopsy, and the removal of small polyps.

19. Students' perception of private supplementary tutoring during medical undergraduate study in some Egyptian universities

PurposeThe current work aimed to investigate the private tutoring phenomenon among Egyptian medical faculty students.Design/methodology/approachThe present work is a cross-sectional observational study using an online, anonymous questionnaire disseminated to Egyptian medical students and instructors via social platforms and university e-mails. All subjects involved in the survey gave informed consent to begin the questionnaire. No financial incentives were awarded to finish the questionnaire.FindingsIn total, 79.2% of the surveyed students (n = 198) admitted taking private medical courses during their medical study courses till the date of the survey. The Egyptian students, 68.4% (n = 171), markedly surpassed the non-Egyptian participants (n = 79, 31.6%). Males were nearly double the female participants (n = 162 and 88 consecutively).The highest academic-level-seeking private medical tutoring was the fifth-year students (n = 66, 26.4%).Research limitations/implicationsA large sample size is needed to strengthen the statistical power and permit the generalization over the population, so more research work in this aspect is recommended. Also, subject-specific data in private medical tutoring need to be investigated in future works. Similar global work is recommended to allow better comparison of data worldwide.Originality/valueWhen conceptualizing medical education processes and developing its regulations, the dynamics of private medical instruction should be taken into account, especially concerning socioeconomic inequities and efficiency in medical school systems. This work has been the first to investigate the private tutoring phenomenon among Egyptian medical students to the authors' best knowledge.

Abstract

T he present research aimed at evaluating computer and information and communication technology courses of the preparatory stage in the light of the values ​​of digital citizenship according to Egypt's Vision 2030 and at developing a proposed vision for their enrichment. To achieve the objectives of the study, the descriptive analytical method was followed. Then the researcher transformed it into a content analysis card that included (52) sub-indicators distributed over nine main criteria. The results indicated that the values of digital citizenship came in the first and second preparatory grades with a medium degree (51%), while they came in low rates in the third intermediate grade curricula (24%). The results showed that all Computer, information technology and communication subject of the preparatory stage achieved the two elements of digital access and digital literacy. The educational axis is also available in all subjects at the highest percentage compared to the other two axes. There are also some elements of digital citizenship that are not addressed, such as digital commerce, based on previous recommended findings. The study conducted a comprehensive review of achievement in order to include more educational standards of digital natives.

Abstract

The current research aims to verify the effectiveness of flipped classroom in developing achievement motivation in a sample consisting of (40) students from third preparatory class in Al-Minshawi Preparatory School. The importance of this study lies in its being the first In this field in Egypt (according to the researcher's knowledge), in addition to providing decision makers, and Responsible for the educational process with basic information related to the effectiveness of using modern teaching methods, then developing the educational process in line with Requirements of the modern era. The Descriptive method approach based on the design (Pre x post) of two groups (experimental x control) was followed. The experimental group consisting of (20) students studied course using flipped classroom, while the control group consisting of (20) students also studied the same course in the traditional way, and to achieve research aims the achievement motivation scale was applied before and after. The data were collected and statistically processed, and the results revealed a statistically significant difference at the level of (α ≤05,0) in the post measurement in favor of the experimental group in the achievement motivation, the researchers refer to teaching in the flipped classroom.

High-performance liquid chromatographic method was developed, validated and applied for miconazole, mometasone and gentamicin in Momenta® cream. Physical separation was applied using two extraction procedures due to different solubility of the three components. First, a methanolic extract of the cream contained miconazole and mometasone was chromatographed on ODS-3 Inertsil C18 column (150 × 4.6, 5 μm) using acetonitrile: water (80:20, v/v) as a mobile phase, flow rate 1.5 mL·min−1, scanned at 230 nm, showing tR 2.817 and 6.808 min for mometasone and miconazole, respectively. Second, an aqueous extract of the cream containing gentamicin was derivatized with o-phthalaldehyde in order to enhance the gentamicin UV detection and subjected to ion pairing chromatography on Inertsil ODS-3 C18 column (150 × 4.6, 5 μm), using methanol: 0.025 M heptane sulphonic acid: glacial acetic acid (75:20:5, by volume) as a mobile phase, flow rate 0.8 mL·min−1, scanned at 330 nm where the three active gentamicin isomers were separated at tR 11.7, 15.6 and 18.3 min. Suitability of this method for quantitative estimation of the drugs was proved by validation according to ICH guidelines. Abstract

The study aimed toidentifythe motives of teachers to use the Internet of Things in the educational process, as well as toidentifythe acceptance of high school pupils in using this technology in the educational process, and revealingthe obstacles facing them. During its use, the researcher used a surveyform (with 14 items), and a questionnaire (with 10 items). The following statistical methods were followed by frequency, percentage, and arithmetic mean of analyzing the results of the questionnaires. Objective This study describes a comparison between colorimetric and chromatographic (RP-high performance liquid chromatography (HPLC) and thin-layer chromatography (TLC)-densitometric) methods concerning selectivity and specificity for the determination of VAL in all possible degradation products (alkali- and acid-induced degradation products, namely aciclovir [ACI] and guanine [GUA], respectively) in their synthetic mixture and pharmaceutical formulations. Method The colorimetric method was accomplished by forming a highly colored complex with ferric hydroxamate reagent measured at 493 nm in the concentration range (0.20–1.60 mg/mL). Both chromatographic methods were successfully applied using ultraviolet (UV) detection at 256 nm in the concentration range (2.00–5.00 µg/mL) for the RP-HPLC method and (10.00–900.00 ng/band) for the TLC-densitometric method. Results The linearity studies, regression equations, assay parameters, and validation sheet of the proposed colorimetric and chromatographic methods to determine VAL were obtained with highly acceptable values. Conclusions The International Council for Harmonization (ICH) guidelines were followed to validate the described methods and the statistical comparison regarding both accuracy and precision, and satisfactory results were accomplished. Highlights In this study, we configure a full comparative study between different analytical methods for the analysis of challengeable mixture containing the drug of interest, VAL, along with its degradation products, ACI and GUA.

Abstract

Obstetric fistula leaves women with few opportunities to earn a living and worsens their poverty. The death of the baby, inabil­ity to carry a child, and the stigma that accompanies fistula results in significant emotional & psychosocial damage. Vesico-vaginal fistula (VVF) is a medical condition that involves an abnormal opening between the bladder and the vagina, leading to uncontrolled leakage of urine which is resulted mostly from an unattended prolonged labor. Semi-structured in-depth interview was used to explore the lived in experience of eight women with obstetric fistula who were admitted in Jimma University Medical Center selected by purposive sampling technique. The data were analyzed using thematic analysis. Seven themes illustrating the lived in experiences of women with Vesico-vaginal fistula were emerged. These were awareness of altered identity, depression, anxiety, marital problems, disrupted social life, economic problems and coping strategy. Problem focused coping strategies mentioned were having family support, orientating to reality, financial independence and religiousness. Emotion focused coping strategies were also noted, such as, isolation, suicidal thoughts and anger. Altered psychological, physical, social and economic functioning were some of the disruption in the life of women with VVF observed. Promoting their positive coping strategies, providing physical and psychological support is crucial to reverse debilitating effects of women with VVF.

Abstract

Background In low- and middle-income countries, the paucity of conventional health services means that many people with mental health problems rely on traditional health practitioners (THPs). This paper examines the possibility of forging partnerships at the Primary Health Care (PHC) level in two geopolitical regions of Ghana, to maximize the benefits to both health systems. Methods The study was a qualitative cross-sectional survey. Eight (8) focus group discussions (FGDs) were conducted between February and April 2014. The views of THPs, PHC providers, service users (i.e. patients) and their caregivers, on the perceived benefits, barriers and facilitators of forging partnerships were examined. A thematic framework approach was employed for analysis. Results The study revealed that underlying the widespread approval of forging partnerships, there were mutual undertones of suspicion. While PHC providers were mainly concerned that THPs may incur harms to service users (e.g., through delays in care pathways and human rights abuses), service users and their caregivers highlighted the failure of conventional medical care to meet their healthcare needs. There are practical challenges to these collaborations, including the lack of options to adequately deal with human rights issues such as some patients being chained and exposed to the vagaries of the weather at THPs. There is also the issue of the frequent shortage of psychotropic medication at PHCs. Conclusion Addressing these barriers could enhance partnerships. There is also a need to educate all providers, which should include sessions clarifying the potential value of such partnerships.

Abstract

Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung's disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung's disease) from 264 hospitals (89 in high-income countries, 166 in middle-income countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p

Abstract

Background The conflict in Yemen has devastated the health system, with only 51% of health facilities classified as fully functional and 19.7 million people lacking access to health care. To address the urgent need for primary health care services in rural communities, Save the Children launched an iCCM program in Lahj and Taiz Governorates. A qualitative study was conducted to document the challenges to iCCM service delivery and to aid in developing strategies for overcoming service delivery bottlenecks in conflict-affected rural areas. Methods Qualitative data were collected in Aden City, Lahj Governorate, and Taiz Governorate. Twenty-three IDIs and six FGDs were conducted with iCCM stakeholders at all levels. Results Key findings included: 1) Policy, coordination, and funding were challenged by the fact that iCCM was not integrated into the national health system and was implemented as a short-term emergency program. 2) Villages that received services from a CHW who was based in a different community experienced reduced access to services, especially during times of heightened conflict and insecurity, when CHWs could not travel. 3) Supervision, supply chain, and monitoring were all challenges that were exacerbated by difficulties in travel due to the conflict. Potential solutions to these included the use of mobile technology for supervision and data collection and pre-positioning of buffer stocks in locations closer to CHWs. 4) Travel was seen as the primary threat to the safety of CHWs and supervisors. Measures taken to reduce the risk included limiting travel during periods of heightened insecurity, safety training for CHWs, and use of mobile technology for communication. Conclusions CHWs were able to provide iCCM services in a challenging and insecure context. The challenges in delivery of services were related to both a weak health system and the conflict. Several adaptations to service delivery to overcome the bottlenecks have been identified and should be considered for future community health programs. Abstract

One of the critical aspects of an individual’s life is career choice as a career chosen will determine the role pattern need to be played by the individual in the society in the future and the career choice is one of the important processes in life. The aim of this study is to investigate the factors that influencing the career choice of the students. This study is focus on undergraduate students in University of Aden. Undergraduate students were chosen because right upon graduation, they were the one who will be dealing with the decision on career choice. A total number of 321 students responded to the questionnaire. Statistical Package software for Social Science (SPSS) version 22.0 and Smart PLS version 2 were used to analyse the collected data. Using Smart PLS measurement model was examined to test the reliability and validity of the study instruments. In addition, structural model was used to test research hypotheses. The findings of the study largely supported the hypothesized relationships proposed in the theoretical model and showed that the family, self-efficacy and economic considerations exerted significant influence on the choice of a career. Moreover, the results showed that the influence of family, self-efficacy and economic considerations have positive relationships to career choices. With a view to investigating student’s career choice that is affected by other important factors further studies are recommended strongly

Abstract

Background: Little attention has been paid to investigate the effect of orchidectomy on hepatic ischemia/reperfusion (I/R) injury, which is lacking. The Aim of This Study: is to evaluate the possible impact of orchidectomy and/or testosterone treatment on the function and structure of liver in rat model of hepatic I/R injury and, also, to explore the possible underlying mechanism(s). Materials and Methods: 65 male Wistar albino-rats were divided into 5 groups; sham- operated control group (Sham), hepatic ischemia/reperfusion group (hepatic I/R), orchidectomized hepatic ischemia/reperfusion group (Orch+I/R), Testosterone- treated hepatic ischemia/reperfusion group (T+I/R) and Orchidectomized testosterone-treated hepatic ischemia/reperfusion group (Orch+T+I/ R). All rats were subjected to determination of serum levels of alanine transaminase (ALT), aspartate transaminase (AST), testosterone and estradiol, and hepatic tissue level of malondialdehyde (MDA), superoxide dismutase (SOD), tumor necrosis factor-alpha (TNF-α) and gene expression of nuclear factor-kappa B (NF-κB) and nuclear factor erythroid 2-related factor 2 (Nrf2). Also, hepatic tissues were histopathologically assessed. Results: Hepatic I/R was associated with significant rise of serum ALT and AST, and hepatic tissue TNF-α, NF-κB and MDA, and significant reduction of serum testosterone and estradiol, and hepatic tissue SOD and Nrf2. Histopathological analysis showed vacuolization of the cytoplasm, dilated sinusoids, inflammatory cells infiltration, increased collagen fibers and significantly increased caspase-3. Hepatic I/R -mediated effects were aggravated following orchidectomy, however preischemic treatment with testosterone protected the liver as shown by the significant decline in ALT and AST, and hepatic tissue MDA, TNF-α and NF-κB, and significant enhancement of serum testosterone and estradiol, and hepatic tissue SOD and Nrf2, and restoration of hepatic architecture. Conclusion: Orchidectomy hypersensitizes the liver to ischemic injury, due to exaggeration of oxidative and inflammatory states and apoptosis, while testosterone treatment improved the hepatic I/R detrimental effects on liver function and morphology due to its suppressive effect on oxidative and inflammatory states and apoptosis. Abstract

Background: Emerging researchers in low- and middle-income countries (LMIC) face many barriers, including inadequacies in funding, international exposure and mentorship. In 2012, the National Institute of Mental Health (NIMH) funded five research hubs aimed at improving the research core for evidence-based mental health interventions, enhancing research skills in global mental health, and providing capacity building (CB) opportunities for early career investigators in LMIC. In this paper emerging researchers contextualize their experiences.Case presentation: Each of the five hubs purposively selected an emerging researcher who had experienced more than one hub-related CB opportunity and actively participated in hub-related clinical trial activities. The five 'voices' were invited to contribute narratives on their professional backgrounds, CB experience, challenges and successes as an emerging mental health researcher, and suggestions for future CB activities. These narratives are presented as case studies. CB activities provided broader learning opportunities for emerging researchers. Benefits included the receipt of research funding, hands-on training and mentorship, as well as exposure to networks and collaborative opportunities on a global scale. To overcome ongoing challenges of access to funding, mentoring, networking and global exposure, the emerging voices recommend making mentorship and training opportunities available to a wider range of emerging mental health researchers.Conclusions: Investing in CB is not enough to ensure sustainability and leave a legacy unless it is accompanied by ongoing mentorship and international exposure. Financial investment in building research capacity, promotion of mentorship and supervision, and international networking are essential to yield well-prepared young investigators in LMIC as experienced by these rising stars. Governments and policymakers should prioritize educational policies to support the continuous development and international engagement of emerging researchers. This can advance strategies to deal with one of most important and costly problems faced by healthcare systems in LMIC: the mental health treatment gap.

Abstract

The Phonocardiography is a graphical method of recording of the tones and noise generated by the heart with the help of the phonocardiogram machine. Cardiovascular disease (CVD) and heart failure (HF) are considered life-threatening and mostly cause death. The phonocardiograph signal (PCG) considers an indicator of abnormalities in the cardiovascular system. It provides the ability to carry out qualitative information and quantitative analysis of different tones and heart murmurs. PCG plays a major role in treatment, diagnosis and decision making of the clinical examination and biomedical research fields. The use of simple stethoscope for diagnosis the heart problem requires an experienced physician or doctors. Many people with CVD and HF are dying every day because of the lack of facilities that analysis the heart defects. Most of the low come countries suffer from a severe shortage in the Electrocardiogram (ECG) and PCG devices and trained physicians and doctors. The Poor healthcare system in these countries needs to be improved especially the problem of heart disease diagnostics. The PCG is a technique for recording and monitoring the cardiac acoustics by using a transducer and microphone. This paper attempts to design a cheap and simple biotechnical system for recording and monitoring PCG signal. The hardware was designed and implemented using the stethoscope, electret microphone, amplifiers, DC source, and jack for transmitting the PCG signal to the computer. The software was codded for monitoring and processing the PCG signal.

Abstract

Background Little is known about the course of perinatal anxiety, particularly in low and middle income countries. This study aimed at examining trajectories of ante- and postpartum generalized anxiety symptoms in West-African women and their associations with mother and child characteristics. Methods 778 women from Cote d’Ivoire and Ghana were investigated between 04/2010 and 03/2014. Anxiety symptoms were measured using the seven-item Generalized Anxiety Disorder scale (GAD-7) at three months antepartum and three, 12 and 24 months postpartum. Growth mixture modeling was applied to identify latent trajectory classes of anxiety. Multinomial logistic regression was used to investigate the associations of psychosocial, sociodemographic, obstetric and clinical characteristics with different trajectories. Results Four distinct trajectories of anxiety were identified. The majority of women (79.8%) had consistent low anxiety symptoms, while 11.4% had elevated anxiety scores before and around childbirth that decreased gradually. 5.4% of women showed increasing anxiety symptoms over time. Few women (3.3%) had transient anxiety with elevated scores at three and 12 months postpartum. Risk factors for elevated anxiety levels around childbirth were antepartum depressive symptoms, higher levels of stress (economic, marital and social stress), lower child birth weight, and multiparity. Partner support was found to be protective. Limitations Anxiety symptoms were assessed using a screening instrument and not through a formal diagnostic classification system. Some putative risk factors were not investigated, and some psychosocial factors were assessed retrospectively. Conclusion The presence of different trajectories underline the importance of monitoring anxiety symptoms in pregnant women and in mothers with infants/toddlers.

Abstract

Purpose Smokers have increased denture stomatitis caused primarily by Candida albicans. The primary aim of this study was to demonstrate the impact of a wide range of nicotine and cigarette smoke condensate (CSC) concentrations on biofilm formation and metabolic activity of C. albicans on acrylic denture material. Materials and methods C. albicans (ATCC strain 10231) was used. Standardized denture acrylic (PMMA) specimens (total of 135 specimens) were incubated with C. albicans and exposed to nicotine and CSC at different concentrations (0, 0.25, 0.5, 1, 2, 4, 8, 16, and 32 mg/ml) and (0, 0.25, 0.5, 1, 2, and 4 mg/ml), respectively. For each experiment, 3 samples per nicotine and CSC concentration and a total of 45 specimens (27 specimens for the nicotine and 18 specimens for the CSC-treated samples) were used and were selected randomly for each group. The control group consisted of 0 mg/ml of nicotine or CSC. The viability of C. albicans was measured using spiral plating on blood agar plates. The effect of nicotine and CSC concentrations on planktonic cells was were measured using a microplate reader. Metabolic activity of 24-hour-old established C. albicans biofilm exposed to nicotine and CSC for 24 hours in microtiter plates was determined using a 2,3-bis (2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-carboxanilide (XTT) reduction assay. Results The viability of C. albicans increased concomitant with increasing concentrations of CSC and nicotine, particularly at 0.5 and 2 mg/ml, respectively. Concentrations of CSC and nicotine above this resulted in an inhibitory effect on C. albicans viability. CSC and nicotine at 4 and 16 mg/ml, respectively, increased C. albicans biofilm metabolic activity. Conclusion Nicotine and CSC up to certain concentrations caused increases in biofilm formation, metabolic activity, viability, and planktonic cell absorbance of C. albicans. This in vitro study demonstrates the effectiveness of tobacco on promoting the growth of C. albicans and suggests their potential contributing factor in C. albicans biofilm related infections in smokers.
