PURPOSE To examine the trends and quality metrics of publications by radiation oncologists in Saudi Arabia. METHODS PubMed was searched using names of all Saudi radiation oncologists to retrieve published articles between January 2010 and December 2019. International collaboration, journal impact factor and country of origin, and number of citations were collected. Each article was assessed for epidemiologic type and independently assigned a level of evidence (LOE) by two authors. The trend in publications was examined and compared in the first and second 5-year periods (2010-2014 and 2015-2019) using relevant parameters. RESULTS A total of 186 publications were found and included. The most common type of research was cohort studies followed by case reports and case series in 24%, 14%, and 13% of all publications, respectively. Dosimetry, clinical, and preclinical studies formed 7%, 8.6%, and 7.5% of the total publications, respectively. The LOE was I, II, III, IV, and not applicable in 8.6%, 22%, 25.8%, 29%, and 14.5% of the included publications, respectively. Comparing the first and second 5-year periods, there was an increase in international collaboration ( P < .001) in the second period. The number of citations ( P < .001) and journal impact factor ( P = .028) were lower in the second period. LOE and publications in international journals were not statistically different between the two periods. CONCLUSION Although radiation oncology research activity in Saudi Arabia has gained momentum in terms of volume and international collaboration over time, the LOE has not improved. This calls for a national effort to make the contribution to the literature a priority, allocate adequate resources, and apply appropriate measures to enhance research productivity and quality.

Objectives: To report the ipsilateral lung dosimetry data of breast cancer (BC) patients treated with loco-regional hypofractionated radiotherapy (HFRT). Methods: Treatment plans of 150 patients treated in the Radiotherapy Unit, King Abdulaziz University Hospital, Jeddah, Kingdom of Saudi Arabia between January 2012 and March 2015 by HFRT for BC were retrospectively reviewed. All patients received 42.4 Gy in 16 fractions by tangential and supra-clavicular fields with 6 MV, 18 MV, or mixed energies. Ipsilateral lung dosimetric data V20Gy and mean lung dose (MLD) were recorded. Correlations between lung dose, patient characteristics, and treatment delivery parameters were assessed by a logistic regression test. Results: The mean ipsilateral lung V20Gy was 24.6% and mean MLD was 11.9 Gy. A weak, but statistically significant correlation was found between lung dose and lung volume ( p =0.043). The lung dose was significantly decreasing with patient separation and depth of axillary lymph node (ALN) and supra-claviculary lymph nodes (SCLN) ( p less than 0.0001), and increasing with ALN ( p =0.001) and SCLN ( p =0.003) dose coverage. Lung dose significantly decreased with beam energy ( p less than 0.0001): mean V20Gy was 27.8%, 25.4% for 6 MV, mixed energy, and 21.2% for 18 MV. The use of a low breast-board angle correlates with low lung dose. Conclusion: Our data suggest that the use of high energy photon beams and low breast-board angulation can reduce the lung dose. Aim This study evaluated the survival outcome, pattern of failure and prognostic factors in cervix uteri cancer patients. Methods We reviewed the data of 60 patients with stages IB-IVA cancer who were treated between January 2004 and December 2010. Results Most patients (n = 50; 83%) had squamous cell carcinoma. Stage IIB was the most common presentation (n = 41; 68%). Forty-seven patients (78%) received Cisplatin concurrent with radiotherapy (CRT). The 2- and 4-year overall survival (OS) was 82% and 79%, respectively. Purpose: To retrospectively compare the potential dosimetric advantages of a multichannel vaginal applicator vs. a single channel one in intracavitary vaginal high-dose-rate (HDR) brachytherapy after hysterectomy, and evaluate the dosimetric advantage of fractional re-planning. Material and methods: We randomly selected 12 patients with endometrial carcinoma, who received adjuvant vaginal cuff HDR brachytherapy using a multichannel applicator. For each brachytherapy fraction, two inverse treatment plans (for central channel and multichannel loadings) were performed and compared. The advantage of fractional re-planning was also investigated. Results: Dose-volume-histogram (DVH) analysis showed limited, but statistically significant difference (p = 0.007) regarding clinical-target-volume dose coverage between single and multichannel approaches. For the organs-at-risk rectum and bladder, the use of multichannel applicator demonstrated a noticeable dose reduction, when compared to single channel, but statistically significant for rectum only (p = 0.0001). For D 2cc of rectum, an average fractional dose of 6.1 ± 0.7 Gy resulted for single channel vs. 5.1 ± 0.6 Gy for multichannel. For D 2cc of bladder, an average fractional dose of 5 ± 0.9 Gy occurred for single channel vs. 4.9 ± 0.8 Gy for multichannel. The dosimetric benefit of fractional re-planning was demonstrated: DVH analysis showed large, but not statistically significant differences between first fraction plan and fractional re-planning, due to large inter-fraction variations for rectum and bladder positioning and filling. Conclusions: Vaginal HDR brachytherapy using a multichannel vaginal applicator and inverse planning provides dosimetric advantages over single channel cylinder, by reducing the dose to organs at risk without compromising the target volume coverage, but at the expense of an increased vaginal mucosa dose. Purpose: To retrospectively assess the incidence and magnitude of air pockets around vaginal cylinders and its impact on dose distribution in vaginal cuff image-guided high-dose-rate (HDR) brachytherapy. Material and methods: Fifty endometrial carcinoma patients treated by postoperative HDR vaginal cuff brachytherapy were included in the study. The average age of patients was 58.3 ± 11.8 years (range: 31-87 years). Brachytherapy was performed using cylindrical applicators, and the dose prescribed to 0.5 cm from the applicator’s surface, over a length of 5 cm from the applicator’s tip. Computed tomography (CT) simulation was used for each brachytherapy fraction. The incidence, vaginal mucosa displacement, volume, and dosimetric effect of air pockets around the vaginal cylinder were evaluated. Results: A total of 78 air pockets were found in 29/50 patients (58%) and 45/135 (33%) brachytherapy plans. They were located at the apex: 16/78 (20%) and lateral to the applicator: 62/78 (80%). The volume of air pockets ranged between 0.01 and 2.1 cm 3 (mean: 0.15 cm 3 ± 0.36 cm 3 ), and the maximum displacement of vaginal mucosa from cylinder surface was between 0.1 and 1.09 cm (mean: 0.34 cm ± 0.2 cm). The dose reduction to the vaginal mucosa generated by the air pockets ranged from 0.5 to 66% (mean: 26.4% ± 13.9%). Conclusions: The presence of air pockets around vaginal cylinder applicators is frequently noticed in post-operative vaginal cuff brachytherapy. The dose to the vaginal mucosa is reduced, as a result of displacement generated by air pockets. The effect on the clinical outcome of this dose reduction is yet to be determined. Aim To retrospectively assess the performance indicators of our radiotherapy department and their temporal trends, using a commercially available electronic-medical-record (EMR) system. Background A recent trend in healthcare quality is to define and evaluate performance indicators of the service provided. Materials and methods Patient and external-beam-radiotherapy-treatments data were retrieved using the Mosaiq EMR system from 1-January-2012 till 31-December-2015. Annual performance indicators were evaluated as: productivity (number of new cases/year and diagnosis distribution); complexity (ratio of Volumetric-Modulated-Arc-Therapy (VMAT) courses, average number of imaging procedures/patient); and quality (average, median and 90th percentile waiting times from admission to first treatment). The temporal trends of all performance indicators were assessed by linear regression. Results Productivity: the number of new cases/year increased with an average rate of 4%. Diagnosis distribution showed that breast is the main pathology treated, followed by gastro-intestinal and head-and-neck. Complexity: the ratio of VMAT courses increased from 13% to 35%, with an average rate of 7% per year. The average number of imaging procedures/patient increased from 8 to 11. Quality: the waiting times from admission to treatment remained stable over time ( R 2 ≤ 0.1), with average, median and 90th percentile values around 20, 15, and 31 days, respectively. Conclusions An EMR system can be used to: monitor the performance indicators of a radiotherapy department, identify workflow processes needing attention and improvement, estimate future demands of resources. Temporal analysis of our data showed an increasing trend in productivity and complexity paired with constant waiting times.

Purpose To investigate the correlation between the dose predicted by the treatment planning system using digitally reconstructed radiographs or three-dimensional (3D)–reconstructed CT images and the dose measured by semiconductor detectors, under clinical conditions of high-dose-rate brachytherapy of the cervix uteri. Patients and methods Thirty-two intracavitary brachytherapy applications were performed for 12 patients with cancer of the cervix uteri. The prescribed dose to Point A was 7 Gy. Dose was calculated for both International Commissioning on Radiation Units and Measurements (ICRU) bladder and rectal points based on digitally reconstructed radiographs and for 3D CT images–based volumetric calculation of the bladder and rectum. In vivo diode dosimetry was performed for the bladder and rectum. Results The ICRU reference point and the volumes of 1, 2, and 5 cm3 received 3.6 ± 0.9, 5.6 ± 2.0, 5.1 ± 1.7, 4.3 ± 1.4 and 5.0 ± 1.2, 5.3 ± 1.3, 4.9 ± 1.1, and 4.2 ± 0.9 Gy for the bladder and rectum, respectively. The ratio of the 1 cm3 and the ICRU reference point dose to the diode dose was 1.8 ± 0.7 and 1.2 ± 0.5 for the bladder and 1.9 ± 0.6 and 1.7 ± 0.5 for the rectum, respectively. Conclusions 3D image–based dose calculation is the most accurate and reliable method to evaluate the dose given to critical organs. In vivo diode dosimetry is an important method of quality assurance, but clinical decisions should be made based on 3D-reconstructed CT image calculations.

AIM: Women in Saudi Arabia develop breast cancer at a young age with high prevalence of poor prognostic features. Because of such features, it is necessary to examine prognostic factors in this population. One such factor is the prognostic role of lymph node ratio (LNR). METHODS: We performed retrospective analyses of patients with invasive non-metastatic breast cancer who underwent axillary lymph node dissection and had one or more positive axillary lymph nodes. RESULTS: Two hundred and seventeen patients were considered eligible for the analysis. The median age was 46 years. At a median follow-up of 39.8 months, the median disease-free survival (DFS) was 67.3 months (95% CI, 50.4 to 84.3 months). Neither the classification of patients based on positive lymph node (pN) staging system, nor the absolute number of pN prognosticated DFS. Conversely, age 0.20 to 0.65) LNR categories were the only variables that were independently associated with adverse DFS. Using these variables in a prognostic model allowed the classification of patients into three distinctive risk strata. The overall survival (OS) in this series was 92.5 months (95% CI, 92.1-92.6). Only ER negative tumor adversely influenced OS. CONCLUSION: Analysis of survival outcome of mostly young patients with early breast cancer identified adverse prognostic variables affecting DFS. If the utility of the derived model including LNR is proven in a larger patient population, it may replace the use of absolute number of positive axillary lymph nodes

The aim is to compare the acute skin toxicity between two different fractionation schedules of adjuvant whole breast radiotherapy, conventional fractionation radiotherapy and hypofractionated radiotherapy. This is a prospective study of breast cancer patients referred for adjuvant radiation therapy. Radiation therapy was given either as conventional fractionation radiotherapy (5000 cGy/ 25 fractions / 5 weeks) or hypofractionated radiotherapy (4005 cGy/15 fractions / 3 weeks). Acute RT toxicity was evaluated using the Radiation Therapy Oncology Group toxicity grading system; the incidence of radiation therapy toxicity was compared. Seventy-eight patients were accrued in the study, 58% had breast conservative surgery and 42% had modified radical mastectomy. 54% were treated with hypofractionated radiotherapy and 46% with conventional fractionation radiotherapy, while 64% had Grade 0-II radiation therapy toxicity and 36% had Grade III-IV toxicity. On univariate analysis, the incidence of radiation therapy toxicity was (52.8%) in conventional fractionation radiotherapy group as compared to 21.4% in hypofractionated radiotherapy group (p = 0.004); other variables were not statistically significant. On multivariate analysis radiotherapy fractionation was the only factor of statistical significance regarding the incidence of radiation reaction (p = 0.03). This study showed fewer incidences of acute radiation reactions in hypofractionated arm as compared to conventional fractionated arm in the adjuvant whole breast radiotherapy.

The aim is to evaluate the pathological complete response and toxicity when using brachytherapy boost as part of the preoperative chemo- radiation therapy in patients with rectal cancer. A retrospective review of all patients with rectal cancer treated at King Abdulaziz University Hospital. Fifty-five patients had neoadjuvat radiotherapy 45 Gy in 25 fractions of external beam radiation as Phase I of radiotherapy followed by external beam boost: 540 cGy/3 fractions in 31 patients or Ir192 HDR brachytherapy boost in 17 patients. The pathological response rate, toxicity profile, overall survival and disease free survival was compared between different radiation therapies modalities. The pathological response rate was complete in 14 (25.5%) patients, partial (PR) in 29 (52.7%) patients, unknown (did not do surgery) in 9 (16.5%) patients and stable disease in 3 (5.4%) patients. In brachytherapy boost, pathological complete response was 47% vs. 18% external beam boost (p = 0.004). Toxicity profile was better in the brachytherapy group. Overall survival was 93% and the disease free survival was 60% which were not influenced by the modality of radiation therapy used. High Dose Rate Brachytherapy boost result in a higher pathological response rate without increase in side effect as compared to the external beam radiation boost in the neoadjuvant radiation therapy for locally advanced rectal cancer.

Purpose Intensity-modulated radiotherapy (IMRT) and volumetric-modulated arc therapy (VMAT) provide highly conformal target radiation doses, but also expose large volumes of healthy tissue to low-dose radiation. With improving survival, more children with medulloblastoma (MB) are at risk of late adverse effects of radiotherapy, including secondary cancers. We evaluated the characteristics of IMRT and VMAT craniospinal irradiation treatment plans in children with standard-risk MB to compare radiation dose delivery to target organs and organs at risk (OAR). Patients and methods Each of 10 children with standard-risk MB underwent both IMRT and VMAT treatment planning. Dose calculations used inverse planning optimization with a craniospinal dose of 23.4 Gy followed by a posterior fossa boost to 55.8 Gy. Clinical and planning target volumes were demarcated on axial computed tomography images. Dose distributions to target organs and OAR for each planning technique were measured and compared with published dose-volume toxicity data for pediatric patients. Results All patients completed treatment planning for both techniques. Analyses and comparisons of dose distributions and dose-volume histograms for the planned target volumes, and dose delivery to the OAR for each technique demonstrated the following: (1) VMAT had a modest, but significantly better, planning target volume-dose coverage and homogeneity compared with IMRT; (2) there were different OAR dose-sparing profiles for IMRT versus VMAT; and (3) neither IMRT nor VMAT demonstrated dose reductions to the published pediatric dose limits for the eyes, the lens, the cochlea, the pituitary, and the brain. Conclusions The use of both IMRT and VMAT provides good target tissue coverage and sparing of the adjacent tissue for MB. Both techniques resulted in OAR dose delivery within published pediatric dose guidelines, except those mentioned above. Pediatric patients with standard-risk MB remain at risk for late endocrinologic, sensory (auditory and visual), and brain functional impairments.

The aim is to study the relationship between radiation induced small bowel toxicity and radiation dose received by the small bowel during pelvic irradiation for patients with rectal cancer treated with pelvic radiotherapy and concomitant 5-Fluorouracil. Thirty-two patients with rectal cancer were referred for either a postoperative or preoperative pelvic irradiation concomitant with 5-Fluorouracil chem- otherapy at the Radiotherapy Unit of King Abdulaziz University Hospi- tal. All patients had computerized 3D treatment planning. Radiation therapy was given in two phases, to a total dose of 5040 cGy / 28 frac- tions / 5.5 weeks. Small bowel loops were contoured on CT cuts and a dose volume histogram was constructed and the mean radiation dose received by the small bowel, the volume of the small bowel irradiated and the mean radiation dose / volume of small bowel involved ratio. Univariate analysis showed a significant association between small bowel toxicity and mean radiation therapy dose received by small bowel (p = 0.001), the volume of small bowel involved (P = 0.043) and the type of surgical intervention with higher incidence for those patients who had abdominoperineal resection (P = 0.003). In conclu- sion, this study confirms the relation between small bowel toxicity and the volume of small bowel receiving radiotherapy.

To report our early experience using the Intrabeam radiotherapy delivery system for intraoperative radiotherapy (IORT) in early breast cancer.This is a prospective phase 2 study carried out at the Department of Surgery and Radiology, King Abdulaziz University Hospital, Jeddah, Kingdom of Saudi Arabia from December 2010 to November 2012. Females eligible for breast-conserving surgery with biopsy-proven invasive duct carcinoma, and with a mass of/=3.5 cm were included in this study. After wide local excision, sentinel lymph node dissection, and surgically positioning of the appropriately sized applicator on the tumor bed, a 20 Gray (Gy) single dose was prescribed using the Intrabeam x-ray generator. External beam radiotherapy (EBRT; 46 Gy/23 fractions/4.5 weeks) was given when the tumor was3 cm, with lymphovascular invasion, multifocal lesion, extensive intraductal carcinoma, and positive nodes. Early and late toxicity were recorded using the Radiation Therapy Oncology Group (RTOG) criteria.Forty-five patients were included with a median age of 54 (range: 27-79 years). Thirty-six cases (80%) had tumor3 cm in diameter, and 36 (67%) have pathologically negative axillary lymph node metastases. None of the patients developed delayed wound healing, postoperative infection requiring intravenous antibiotic, or breast seroma requiring aspiration. Sixteen (36%) received EBRT after IORT. Twelve patients developed radiologically proved fat necrosis.The IORT for early stage breast cancer patients using the Intrabeam delivery system was easily implemented in our center with an acceptable toxicity profile and cosmetic outcome.

BACKGROUND. The objective of this study was to correlate the failure pattern of localized prostate carcinoma after radiotherapy (RT) with pretreatment (preTx) PSA and post-RT nadir PSA, using systematic biopsies and serum PSA in the assessment of outcome. METHODS. From January 1990 to February 1994, 207 patients treated with external beam RT were followed prospectively with systematic transrectal ultrasound-guided biopsies and measurements of serum PSA levels. Three hundred forty-three biopsies were performed, with 4-7 samples taken per session. The distribution of T classification was as follows: 19 patients had T1b, 15 had T1c, 34 had T2a, 79 had T2b/c, 53 had T3, and 7 had T4. Median follow-up was 36 months (range, 12-70 months). Failures were categorized as biochemical (chemF) (PSA > 2.0 ng/mL and >1 ng/ mL over nadir), local (LF) (positive biopsy and PSA > 2), and distant (DF). The Cox proportional hazards model was used for multivariate analysis (MVA). RESULTS. Overall, failures were seen in 68 of 207 patients: 20 LF, 24 DF, 7 LF + DF, and 17 chemF. In univariate analysis, failures correlated significantly with preTx PSA, post-RT nadir PSA, T classification, and Gleason's score (GS). The total failure rate was 12% for T1b, T1c, and T2a; 39% for T2b and T2c; and 60% for T3 and T4 (P 50 ng/ml. (P 2 ng/mL (P < 0.0001). In MVA, nadir PSA (P < 0.0001) and T classification (P < 0.0005) were independent predictors for any failure. LF occurred in 13% of patients (27 of 207). For these 27 patients, the categorization of T classification was: T1b/T1c/T2a, 7%; T2b/T2c, 16%; and T3/T4, 15% (P = not significant). In MVA, only nadir PSA (P = 0.0004) predicted for LF. DF occurred in 15% of patients (31 of 207). In MVA, nadir PSA (P < 0.0001) and T classification (P < 0.0001) predicted for DF, with pretreatment PSA of borderline significance (P < 0.05). To assess preTx predictors of outcome, post-RT nadir PSA was removed from the model. PreTx PSA then became the dominant variable to predict any failure (P < 0.0001), LF (P = 0.05), chemF (P = 0.0001), and DF (P < 0.003), while T classification also predicted for any failure (P = 0.03), chemF (P = 0.05), and DF (P < 0.0001). CONCLUSIONS Systematic prostate biopsies, performed as part of the rigorous followup of prostate carcinoma after RT, define the pattems of failure and confirm the prognostic value of preTx PSA, post-RT nadir PSA, and T classification. Prior to treatment, preTx PSA is the overwhelming independent predictor of failure, but it is surpassed by post-RT nadir PSA when this is added to the model.

BACKGROUND Sequential systematic biopsies after radiotherapy (RT) for prostate carcinoma were evaluated with respect to radiation effect, tumor differentiation, and prostate specific antigen (PSA) staining, and these histopathologic parameters were correlated with local outcome. METHODS Since 1990, transrectal ultrasound-guided biopsies have been used to follow patients with localized prostate carcinoma after radical external RT at the General Hospital division of the Ottawa Regional Cancer Centre. Eighty-nine patients with residual tumor in 1 or more biopsies at 10-78 months of follow-up were selected for review. All patients had undergone biopsy on more than one occasion. The 198 biopsies were stained for PSA, high molecular weight keratin, and proliferative cell nuclear antigen (PCNA). Therapy effect was graded by nuclear and cytoplasmic changes. RESULTS Of these 89 selected patients, 25 progressed to local failure (LF), 38 had delayed tumor clearance, and 26 remained biopsy positive without local or biochemical progression. In the 24- and 36-month biopsies, RT effect correlated with mean nadir serum PSA (Grade 4-6 RT effect: 0.7 ng/mL; Grade 0-3 RT effect: 1.3 ng/mL [P = 0.01]) and with local outcome (Grade 0-2 RT effect: 55% LF; Grade 3-4 RT effect: 30% LF; and Grade 5-6 RT effect: 0% LF [P = 0.003]). Of tumors with minimal RT effect, 61% were PCNA positive, compared with 34% for moderate and 17% for marked RT effect (P < 0.001). Adjacent areas within biopsies showed varied RT effect in 27 patients, of whom 41% had local failure, compared with 18% of patients with uniform RT effect (P = 0.03). Gleason score was assigned only if RT effect was minimal (36 patients/59 biopsies); 74% had the same Gleason score (±1) as the original, whereas 23% were 2 points lower. Only 1 biopsy lost PSA staining completely and 10% stained faintly, whereas 34% stained moderately and 55% strongly. CONCLUSIONS PSA staining is apparently retained in residual tumor after RT, despite obvious RT effect. RT effect correlates with serum PSA nadir, PCNA staining, and local outcome. Varied RT effect is commonly observed in biopsies, and correlates with local failure. There is no convincing evidence for tumor dedifferentiation after RT. Various breast boost irradiation techniques were studied and compared. The most commonly used techniques are external beam radiation therapy (EBRT) (photons or electrons) and high dose rate (HDR) interstitial brachytherapy, but recent studies have also revealed the use of advanced radiotherapy techniques, such as intensity modulated radiation therapy (IMRT), intra-operative radiation therapy (IORT), tomotherapy, and protons. The purpose of this study is to systematically review the literature concerning breast boost radiotherapy techniques, and suggest evidence based guidelines for each. A search for literature was performed in the National Library of Medicine's (PubMed) database for English-language articles published from 1st January 1990 to 5th April 2011. The key words were `breast boost radiotherapy`, `breast boost irradiation`, and `breast boost irradiation AND techniques`. Randomized trials comparing the long-term results of boost irradiation techniques, balancing the local control, and cosmesis against logistic resources, and including cost-benefit analysis are further needed.

To compare the inverse planning optimization based on total dose constraints versus conventional treatment plan (point A planning method) for cervical carcinoma, and evaluate the benefit of CT-based image-guided brachytherapy.We prospectively analyzed data of 10 consecutive patients with cervical cancer treated with external beam radiotherapy to the whole pelvis (45 Gy in 25 fractions) followed by high-dose-rate (HDR) brachytherapy (21 Gy in 3 fractions). For treatment planning of HDR brachytherapy, the basic equations of the linear-quadratic model were used to calculate the physical dose for each brachytherapy fraction needed to achieve a given total iso-effective dose for the whole treatment. Specific dosimetric parameters are evaluated for high risk (HR CTV), intermediate risk (IR CTV) clinical target volumes, and organs at risk (OARs).In conventional plans, the HR CTV was well covered in only 15/31, and the IR CTV in 7/31 of the brachytherapy implants, while dose constraints of OARs bladder and rectum were respected in 28/31 and 14/31 implants. After optimization, the HR CTV and IR CTV dose constraints were respected in all the implants, and the bladder and rectum of cases dose constraints were respected in 25/31 and 17/31 of cases.Point A is a poor surrogate of target dose. Significant differences between point doses and dose volume histogram parameters indicate the need for inverse planning in image-guided brachytherapy of cervical cancer.

To use electronic portal images (EPI) to clinically evaluate inter-fraction variations during tangential breast irradiation, using either a skin marks setup, or a bony anatomy setup, and to determine the required margins for simultaneously integrated boost (SIB) planning target volume (PTV).Ten patients undergoing radiotherapy to the entire breast with tangential fields, after breast conservation surgery were considered for this pilot prospective study in the Radiation Therapy Unit of King Abdulaziz University Hospital between February and September 2009. Patient setup was carried out either using skin marks or bony anatomy landmarks. The EPIs of the medial tangential radiation fields were performed daily; displacement of the EPI with respect to the digital reconstructed radiographs (DDRs) was quantified after manual registration with the corresponding DDRs and recorded in both antero-posterior (AP) and cranio-caudal (CC) directions. The inter-fraction variations were used to calculate required margins for SIB PTV.Considerable geometric uncertainties in patient positioning have been observed for both investigated treatment setup protocols. The margins required for a correct assessment of boost PTV were: 15.6 mm for AP and 15.4 mm for CC directions for the skin marks setup protocol, and 12 mm for AP and 12.2 mm for CC directions for the bony anatomy landmarks setup protocol.Systematic and random errors induced by inter-fraction patient setup variations are significant in tangential breast radiotherapy, and lead to a large PTV margin for SIB. Such large margins indicate the need for image-guided radiotherapy.

Rectal cancer is one of the most common malignancies in the West. The incidence in the developing countries is also increasing. There are various reasons for this escalation, including changes in dietary habits, particularly among urban dwellers, and parasitic infections. The risk of colorectal cancer is known to be increased in patients with long-standing schistosomal colitis. We present two male patients who had low rectal cancer (pT3N3M0, and pT3N2M0) for which they were treated with A-P resection and adjuvant chemoradiation. Histological examination of the specimen revealed moderately differentiated adenocarcinoma with extensive schistosomal colitis. The literature was reviewed. The relation between colorectal cancer and intestinal schistosomiasis should be considered as precancerous.

Background: Locally advanced breast cancer (LABC) is common in developing countries and it frequently affects younger women. Patients do very poorly when treated by locoregional therapy alone; therefore, pre-operative systemic therapy (PST) is commonly used. Materials and methods: Medical records of 64 Saudi patients with LABC treated with PST in a single institution were retrospectively reviewed. Results: At diagnosis, most patients were young (median age 41 years), and had poor clinicopathological characteristics. Following surgery, complete pathologic response (pCR) in the breast was achieved in 13 patients (20%). Of 62 patients with known nodal status, 22 (34%) had negative axillary nodes. Presence of oestrogen receptor (ER) negative tumour was the only dependent variable that predicted pCR in the breast (p = 0.03). At a median follow-up of 42 months, the median progression-free survival (PFS) was 48 months (95% CI, 20–76 months) and the projected five-year overall survival (OS) was 68%. The recently published scoring system (Jeruss et al (2008) J Clin Oncol 26 2 246–52), was the only variable that independently influenced PFS, while ER negative tumours and presence of lymphovascular space invasion were the only factors that adversely affected OS. Conclusions: despite the use of standard multi-modality approach in the management of patients with LABC, prognosis remains guarded.

Purpose: Postradiotherapy (RT) prostate biopsies are prone to problems in interpretation. False negatives due to sampling error, false positives due to delayed tumor regression, and indeterminate biopsies showing radiation effect in residual tumor of uncertain viability are common occurrences. Methods and Materials: A cohort of 498 men treated with conventional RT from 06/87–10/96 were followed prospectively with systematic transrectal ultrasound (TRUS)-guided post-RT prostate biopsies, starting 12–18 months after RT. If there was residual tumor but further decline in serum prostate-specific antigen (PSA), biopsies were repeated every 6–12 months. Patients with negative biopsies were rebiopsied at 36 months. Residual tumor was evaluated for RT effect and proliferation markers. The 498 men had 978 biopsies. Median time of the first biopsy (n = 498) was 13 months, biopsy #2 (n = 342) 28 months, biopsy #3 (n = 110) 36 months, biopsy #4 (n = 28) 44 months, and biopsy #5 (n = 4) 55 months. Median follow-up is 54 months (range 13–131). One hundred seventy-five patients (34%) had prior hormonal therapy for a median of 5 months (range 1–60). Results: Clinical stage distribution was T1b: 46; T1c: 50; T2a: 115; T2b/c: 170; T3: 108; T4: 11; Tx: 1. Distribution by Gleason score was: 28% Gleason score 2–4; 42%: 5–6; 18%: 7; and 12%: 8–10. Seventy-one men have died, 26 of prostate cancer and 45 of other causes. Actuarial failure-free survival by T stage at 5 years is T1b: 78%; T1c: 76%; T2a: 60%; T2b/c: 55%; T3: 30%; and T4: 0%. Actuarial freedom from local failure at 5 years is T1b: 83%; T1c: 88%; T2a: 72%; T2b/c: 66%; T3: 58%; and T4: 0%. The proportion of indeterminate biopsies decreases with time, being 33% for biopsy 1, 24% for biopsy 2, 18% for biopsy 3, and 7% for biopsy 4. Thirty percent of indeterminate biopsies resolved to NED status, regardless of the degree of RT effect, 18% progressed to local failure, and 34% remained as biopsy failures with indeterminate status within the time frame of this report. Positive staining for proliferation markers was associated with both subsequent local failure and also any type of failure. In multivariate analysis, only PSA nadir (p = 0.0002) and biopsy status at 24–36 months (p = 0.0005) were independent predictors of outcome. Conclusions: Post-RT prostate biopsies are not a gold standard of treatment efficacy, but are an independent predictor of outcome. Positive immunohistochemical staining for markers of cellular proliferation is associated with subsequent local failure. Indeterminate biopsies, even when showing marked RT effect, cannot be considered negative.
