Amaç: Atriyal fibrilasyon (AF) akut miyokard infarktüsü (AMİ) sonrası en sık görülen ritim bozukluğu olup kısa ve uzun dönem prognozu olumsuz yönde etkilemektedir. Koroner arter hastalığı (KAH) AF gelişimi için bağımsız bir risk faktörüdür. SYNTAX (SYNergy between PCI with TAXUS and Cardiac Surgery) skorlama sistemi, revaskülarizasyon öncesi, kantitatif ve tekrarlanabilir temel bir ölçüm olması nedeniyle koroner anatomik ciddiyet ve karmaşıklığı gösterebilir. Bilgimize göre SYNTAX skoru ile AF arasındaki ilişki araştırılmamıştır. Bu nedenle AMİ ‘li hastalarda yeni gelişen AF ile SYNTAX skoru arasındaki ilişkiyi araştırmayı amaçladık. Metod: AMİ tanısıyla Ocak 2015-Haziran 2016 tarihleri arasında kliniğimize yatırılan 350 hasta retrospektif olarak çalışmaya alındı. Hastaların 273’ü (%78) erkekti. Hastaların 23 ‘ü dışlandı. Hastaların demografik ve klinik özellikleri kaydedildi ve hastalar düşük SYNTAX skorlu ve orta-yüksek SYNTAX skorlu olmak üzere iki gruba ayrıldı. SYNTAX skoru ile AF gelişimi arasındaki ilişki değerlendirildi. AF gelişen hastaların bağımsız belirteçleri çoklu regresyon analizine ile belirlendi. Bulgular: Çalışmaya dahil edilen 327 hastanın 255’i (%77,8) düşük SYNTAX skorlu iken 72’si (%22,2) orta-yüksek SYNTAX skorlu idi. Hastaların 37’inde (%10,6) takipte AF gelişti. Düşük ve orta-yüksek SYNTAX skorlu grupta AF gelişme sıklığı benzer bulundu (%9,4’a karşılık %12,5, p=0,442). Çoklu regresyon analizi sonucunda hastanede ADEİ başlanması (Olasılık oranı=0,362, %95 güven aralığı 0,157-0,835, p=0,017), hastaların yaşı (Olasılık oranı=1,041, %95 güven aralığı 1,008-1,075 p=0,014) ve KYBÜ’sinde yatış süresinin (Olasılık oranı=2,911, %95 güven aralığı 1,595-5,315, p=0,001) AF gelişim riskiyle bağımsız ilişkili olduğu saptandı. Sonuç: AMİ hastalarında AF gelişimi ile SYNTAX skoru arasında ilişki saptanmadı. Bu çalışmada yaş, yatış sonrası ADEİ başlanması ve KYBÜ yatış süresi AF gelişimi ile bağımsız ilişkili olduğu gösterildi.

Objective Aortic stiffness is an important cardiovascular risk marker, which can be determined using different noninvasive techniques. Aortic propagation velocity (APV) has recently been established as a novel echocardiographic parameter of aortic stiffness. This study aimed to investigate the association between APV and the classical echocardiography-derived aortic stiffness parameters, aortic distensibility (AD) and aortic strain (AS), in a group of otherwise healthy individuals. Methods In total, 97 consecutive healthy subjects were recruited in this observational study. APV was measured using color M-mode echocardiography from the suprasternal window in the descending aorta. AS and AD were calculated using clinical blood pressure and the M-mode echocardiography-derived aortic diameters. Correlation analyses were performed between cardiovascular risk factors related to increased aortic stiffness (age, obesity, and blood pressure) and measured stiffness parameters (APV, AS, and AD). Correlation analyses were also performed among the measured stiffness parameters. Results Good correlation of age, blood pressure, and BMI with AS and AD was observed. One-on-one correlation of age, blood pressure, and BMI with APV was not observed. No correlation was observed between APV and AS (r=-0.05, p=0.6) or between APV and AD (r=-0.17, p=0.8). Conclusion Although APV has been proposed as a novel and practical echocardiographic parameter of aortic stiffness, especially in patients with coronary artery disease, correlations between classical stiffness parameters (AS and AD) and APV were absent in healthy individuals at low-intermediate risk. The clinical and research applicability of APV should be further evaluated.

The aim of this study was to investigate the effect of systemic hypertension on the cardiac venous system by evaluating the coronary sinus strain (CSS). Methods One-hundred and twelve hypertensive patients without diabetes and 44 healthy individuals (the control group) were evaluated consecutively at the outpatient clinic and enrolled in the study. CSS was evaluated by echocardiography in all the participants before blood pressure evaluations. Twenty-four-hour ambulatory blood pressure monitoring enabled the study population to be divided into two groups: 52 patients with dipper pattern hypertension and 60 with nondipper pattern hypertension. Results Nondipper pattern patients had significantly lower values of CSS compared with the participants in the control group (140.8 +/- 54.2 and 193.9 +/- 48.1, P < 0.001). Similarly, dipper pattern patients had significantly lower values of CSS values compared with the controls (164 +/- 68.4 and 193.9 +/- 48.1, P = 0.036). On comparing the three groups, the CSS values showed a progressive decrease from normal individuals to dipper and nondipper hypertension patients. Correlation analysis indicated a positive correlation between the aortic strain and the CSS (r = 0.247, P = 0.002). There was a weak correlation between left ventricular mass and CSS (r = -164, P = 0.041). Conclusion Our study suggests that systemic hypertension may affect the cardiac venous system as well as the arterial system, which has been reported in many papers. WOS: 000346136500011 PubMed: 25036324 Objective: Restless legs syndrome (RLS) is a common sleep disorder in which patients feel unpleasent leg sensations and urge to move the legs during rest, especially at night, and symptoms are improved by leg movement. Prior studies analyzing the associations between cardiovascular disease and restless legs syndrome has shown controversial results. The goal of the study was to estimate the relationship between restless legs syndrome and slow coronary flow (SCF). Methods: The present study was cross-sectional and observational and consists of 176 individuals who underwent coronary angiography and had angiographically normal coronary arteries of varying coronary flow rates. The study included 86 patients with isolated SCF and 90 control participants with normal coronary flow (NCF). RLS was assessed the day after the coronry flow was evaluated, using a self-administered questionnaire based on the International Restless Legs Study Group criteria. The following question was asked: "Do you have unpleasant leg sensations (like crawling, paraesthesia, or pain) combined with motor restlessness and an urge to move?" The possible responses were as follows: no, less than once/month, 2-4 times/month, 5-14 times/month, and 15 or more times per month. Those who answered that they had these feelings were asked the following two more questions: 1) "Do these symptoms occur only at rest and does moving improve them?" and 2) "Are these symptoms worsen in the evening/at night compared with the morning?" RLS is considered to be probable if the participant has answered "yes" for all three of the above questions, and has a frequency of >= 5 times/month. Introduction: Both high-sensitivity CRP (hs-CRP) and uric acid (UA) levels are known to be increased in heart failure patients and are associated with poorer functional capacity and adverse outcome. The role of these markers in patients with mitral regurgitation (MR) is less clear. The aim of this study was to assess the relationship between hs-CRP, UA and organic MR. We also assessed whether hs-CRP and UA levels are correlated with symptoms of MR, severity of MR, LV remodeling and outcome during follow-up. Methods: A total of 200 consecutive patients (87 men [43.5%]; mean age 61.6±12.5 years) with moderate or severe isolated and organic MR were included in the study. All the patients were assessed clinically and were managed and treated with standard medical therapy according to evidence-based practice guidelines. Patients were categorized according to New York Heart Association (NYHA) functional class. We assessed and graded the severity of MR using a multiparametric approach. hs-CRP was measured with chemiluminescent immunometric assay using an IMMULITE® 1000 autoanalyzer (Siemens, Germany). Grayanotoxin-III type toxin is commonly found in these plants. Clinical effects of this toxin are bradycardia, atrioventricular block, asystole, hypotension and symptoms such as salivation. In this study; different doses of grayanotoxin-III were intravenously administered to rats (i.v) and the effect of grayanotoxin on cardiovascular system was evaluated. Methods: This study was performed on 42 Wistar albino rats and the animals were divided into six groups. Control, Group 1 (5 mu g/kg), Group 2 (10 mu g/kg), Group 3 (20 mu g/kg), Group 4 (50 mu g/kg) and Group 5 (100 mu g/kg) groups were formed. Baseline values for electrocardiography (ECG), arterial blood pressure and respiratory rate were obtained and recorded for 60 minutes after administration of i.v. grayanotoxin-III. Results: Group1 (5 mu g/kg) was not significantly changed in all time periods. In 10 and 20 mu g/kg groups, respiratory rates, peak heart rates decreased significantly and drop in arterial blood pressures was observed after 2nd and 5th minutes. While 25 minutes after administration of i.v. grayanotoxin-III all parameters returned to baseline values again. In 50 and 100 mu g/kg grayanotoxin-III groups all parameters fell sharply from 2 minutes on, and rats died within 5-10 minutes. Conclusion: In conclusion, it was found that the lethal dose of grayanotoxin-III for rats is 50 mu g/kg. In addition the death was developed due to cardiac arrest. In case of the lower level of 5 mu g/kg grayanotoxin-III, any toxic effect was not seen. The effects of toxin apparently lasted nearly 25 minutes.

Objectives The aim of this study was to evaluate the relationship between inflammation and development of atrial fibrillation (AF) in patients with hyperthyroidism. Methods A total of 65 patients with newly diagnosed hyperthyroidism, 35 of whom were in sinus rhythm and 30 of whom in AF. Thirty five age- and gender-matched patients in a control group were included in the study. Factors associated with the development of AF were evaluated by multivariate regression analysis. Results Factors associated with AF in multivariate analysis included high sensitivity C reactive protein (HsCRP) [odds ratio (OR): 11.19; 95% confidence interval (95% CI): 1.80–69.53; P = 0.003], free T4 (OR: 8.76; 95% CI: 2.09–36.7; P = 0.003), and left atrial diameter (OR: 1.25; 95% CI: 1.06–1.47; P = 0.008). Conclusions The results of the present study suggest that HsCRP, an indicator of inflammation, free T4 and left atrial diameter are associated with the development AF in patients with hyperthyroidism. To evaluate image quality of carotid computed tomography angiography (CTA) using a low voltage (80kV) and low amount of iodinated contrast material.A total of 101 patients referred for carotid CTA were randomly assigned to receive a specific protocol. In group A patients received intravenous administration of contrast material at a dose of 1mL/kg and CTA examinations were performed at 100kV. In group B, patients received intravenous administration of contrast material at a dose of 0.5mL/kg and CTA examinations were performed at 80kV. The same nonionic iodinated contrast material containing 370mg of iodine per mL was used in both groups. Attenuation values were measured from the center of specific arterial segments using regions of interest. Attenuation values above 300HU were accepted as significant. Institutional review board approval was obtained.A total of 50 patients were included in group A (38 men, 12 women; mean age, 63.56 years±13.18 [SD]) and 51 patients in group B (33 men, 18 women; mean age, 59.60 years±16.63 [SD]). A total of 1615 arterial segments (1515 common carotid artery-middle cerebral artery and 101 aortic arches) were analyzed. Venous contamination was not observed in either group. The mean attenuation values of all arterial segments in both groups were greater than 300HU. Mean arterial attenuation value in group B (499.22HU±97.25 [SD]) was significantly greater than in group A (374.36HU±73.79 [SD]) (P0.01). Hemodynamically significant stenosis (grade III stenosis or70%) was detected in 2 segments in group A and in 3 segments in group B, while grade IV stenosis (occlusion) was detected in 2 segments in group B. Distal common carotid artery dissection was detected in 1 patient and aortic dissection was detected in 1 patient in group B. Total dose-length product (DLP) value was significantly greater in group A (225.74mGy·cm±21.80 [SD]) than in group B (116.60mGy·cm±21.22 [SD]) (P0.01). The mean tube current was similar in group A (2013.11mAs±195.92 [SD]) and in group B (2096.64 mAs±309.03 [SD]) (P0.05).Carotid and cerebral CTA examinations using 128-section CT can be successfully obtained using an imaging protocol that combines low voltage and 50% reduction in the volume of iodinated contrast material. This provides good image quality with low radiation dose.

Amac: Bu calismada 128 kesitli bilgisayarli tomografinin (BT) anatomik koroner arter varyasyonlarini ve anomalilerini saptamadaki etkinligi arastirildi ve elde edilen bulgular literatur verileri ile karsilastirildi. Ca­lis­ma­ pla­ni:­ Subat 2009 Aralik 2010 tarihleri arasinda klinigimizde 128 kesitli BT ile degerlendirilen, supheli veya bilinen koroner arter hastaligi olan 83 hasta (58 erkek, 25 kadin; ort. yas 53.7±10 yil; aralik 31-80 yil) retrospektif olarak incelendi. Tum goruntuler, kardiyovaskuler radyoloji alaninda uzman bir radyolog tarafindan anatomik varyasyon ve anomali acisindan degerlendirildi. Surekli degiskenler aritmetik ortalama ± standart sapma (SS) ile ifade edilirken, kategorik degiskenler yuzde (%) ile ifade edildi. Tanimlayici istatistiksel testler ile istatistiksel analiz yapildi. Bul gu lar: Sag dominans, sol dominans ve kodominans orani sirasiyla %80.7, %8.4 ve %10.8 idi. Konus arterin %16.86 oraninda sag sinus valsalvadan ve %83.1 oraninda sag koroner arterden (sag KA) koken aldigi gozlemlendi. Background:­In this study, we aimed to investigate the efficacy of a 128-slice computed tomography (CT) device in detecting anatomical variations and anomalies of coronary arteries and to compare the results obtained with literature data. Methods: Totally, 83 patients (58 males, 25 females; mean age 53.7±10 years; range 31 to 80 years) with suspected or known coronary artery disease who underwent evaluation using the 128-slice CT in our clinic between February 2009 and December 2010 were retrospectively analyzed. All images were evaluated in terms of anatomic variation and anomaly by a radiologist with expertise in cardiovascular radiology. Continuous variables were expressed as arithmetical mean ± standard deviation (SD), while categorical variables were expressed as percent (%). Statistical analysis was performed using descriptive statistical tests. Results:­ The rate of right dominance, left dominance and co-dominance was 80.7%, 8.4% and 10.8%, respectively. It was observed that the conus artery originated from the right sinus valsalva at a rate of 16.86% and from the right coronary artery (RCA) at a rate of 83.1%. It was found that sinus node artery (SNA) originated from the RCA at a rate of 77.1%, from the circumflex (Cx) artery at a rate of 18.1%, from the left main coronary artery (LMCA) at a rate of 2.4% and from the right sinus valsalva at a rate of 2.4%. Ramus intermedius was present in 30.12% of the patients. Coronary artery anomaly was observed in 48 patients (57.8%). of them, 1.2% had absence of LMCA, 6% had commissural origin, 1.2% had coronary hypoplasia, 4.8% had anomalous of contralateral sinus origin, and 43.3% had myocardial bridging. Conclusion:­The 128-slice CT device is a non-invasive method which provides reliable, detailed and correct information for detecting variations and anomalies of coronary arteries. We believe that the 128-slice CT is of higher value in diagnosis and surgical therapy of coronary artery anomalies.

Background: The prevalence of obesity is increasing in the developed and developing world. It is an independent risk factor for heart failure. Left ventricular (LV) diastolic dysfunction has been demonstrated to be a strong predictor of heart failure. In the present study we aimed to assess the impact of body weight on LV diastolic function. Methods: The study was conducted on 2,228 participants (1,424 women, 804 men with a mean age of 49). Traditional and tissue Doppler echocardiographic examination were performed in all of the participants. The demographic and echocardiographic data were compared. Multivariate logistic regression analysis was used to assess the independent predictors of association of LV diastolic function. The study sample was divided into four groups: group 1 (body mass index [BMI] < 25.0 kg/m2), group 2 (BMI 25.0–29.9 kg/m2), group 3 (BMI ≥ 30–39.9 kg/m2), and group 4 (BMI ≥ 40 kg/m2). Results: Septal E was significantly lower in groups 2 and 3 compared to group 1 (P = 0.003). Septal A and septal A’ were significantly higher whereas septal E’ and lateral E’ were significantly lower in the groups 2, 3, and 4 compared to the normal weight group (P < 0.001). Lateral A’, deceleration time, and ejection time were significantly higher in obese when compared to the normal weight (P = 0.025, P < 0.001, and P = 0.009, respectively). The E/E’ ratio was significantly higher in groups 2, 3, and 4 compared to the group 1 (P < 0.001). Logistic regression analysis revealed that age, BMI (OR = 1.060 [95% CI = 1.040 and 1.080]; P < 0.001), hypertension, and diabetes mellitus were independent predictors of LV diastolic dysfunction. Aim: The aim of the Melen Study was to investigate the cardiovascular risk profile of Turkish adults by utilizing newest techniques. Besides prevalence and types of endemic goiter will be established which was an important health problem in the Black sea region Method: The study was conducted on 2230 participants (1427 women, 803 men with a mean age of 49). The participants underwent a Doppler Ultrasound examination of carotid intima media thickness, echocardiographic examination, ECG recording, bioempedance meter analysis of body composition, pulmonary function test and various biochemical analysis. Result: Twenty nine percent of the population had hypertension, 12% had diabetes mellitus and 17% were smokers. Thyroid ultrasonography revealed that 29% of the cohort had goiter. Echocardiographic measurements showed that 39% of the participants had diastolic dysfunction. Comparison of males with females showed that men had significantly higher visceral fat, triglyceride, hemoglobin and CIMT whereas women had higher mean diastolic blood pressure, HDL and total cholesterol. Conclusion: According to the histories of patients, hypertension, diabetes mellitus and smoking were very frequent among Turkish adults. Among the objectively measured variables, diastolic dysfunction, visceral adiposity and goiter were strikingly high.

The aim of this study was to investigate the possible protective role of selenium and L-carnitine on oxidative stress induced by 2.45-GHz radiation in heart of rat. For this purpose, 30 male Wistar Albino rats were equally divided into five groups namely controls, sham controls, radiation-exposed rats, radiation-exposed rats treated with intraperitoneal injections of sodium selenite at a dose of 1.5 mg/kg/day, and radiation-exposed rats treated with intraperitoneal injections of L-carnitine at a dose of 1.5 mg/kg/day. Except for the controls and sham controls, the animals were exposed to 2.45-GHz radiation during 60 min/day for 28 days. The lipid peroxidation (LP) levels were higher in the radiation-exposed groups than in the control and sham control groups. The lipid peroxidation level in the irradiated animals treated with selenium and L-carnitine was lower than in those that were only exposed to 2.45-GHz radiation. The concentrations of vitamins A, C, and E were lower in the irradiated-only group relative to control and sham control groups, but their concentrations were increased in the groups treated with selenium- and L-carnitine. The activity of glutathione peroxidase was higher in the selenium-treated group than in the animals that were irradiated but received no treatment. The erythrocyte-reduced glutathione and β-carotene concentrations did not change in any of the groups. In conclusion, 2.45-GHz electromagnetic radiation caused oxidative stress in the heart of rats. There is an apparent protective effect of selenium and L-carnitine by inhibition of free radical formation and support of the antioxidant redox system.

OBJECTIVE Strong evidence has suggested that there is a circadian periodicity of acute coronary event. Beta-blockers, aspirin and angiotensin-converting enzyme inhibitors decrease the rate of acute myocardial infarction (AMI) and blunt the peak incidence in the morning. However, such effect has not been evaluated for statins. Accordingly, the present study aimed to evaluate the influence of statin therapy on circadian variation of AMI. METHODS This retrospective study consisted of 451 consecutive patients with acute ST segment elevation AMI. The patients were divided into two group based on prior statin usage. In statistical analysis t test, Chi-square test and Mann Whitney U test were used for comparison of groups. We used harmonic regression models to evaluate the circadian variation of onset of MI symptoms in patients receiving statin and patients not receiving statin. RESULTS In all study participants, the highest incidence of AMI was between 6.00 and 12.00; the odds ratio was 1.34 (95% CI 1.20 to 1.46, p=0.001). In the non-statin group, the highest incidence of AMI occurred between 0:00 A.M. and 06.00. There was still a peak incidence between 6.00 A.M. and noon in the statin therapy receiving group; the odds ratio was 1.61 (95% CI 1.34 to 1.80, p=0.001). Accordingly, there was no statistical difference between the statin and non-statin groups regarding circadian variation of AMI. Prior usage of statin did not blunt the peak incidence of AMI in the morning. CONCLUSION Prior usage of statin does not seem to play a role in the circadian periodicity of AMI.

BACKGROUND Increased procoagulant activity and/or impaired fibrinolysis contribute to the development and organization of thrombus, subsequently resulting in complete or incomplete obstruction in acute coronary syndromes (ACS). We investigated the relationship between culprit lesion severity and thrombotic and fibrinolytic parameters in patients who had non-ST elevation ACS (NSTE-ACS). METHODS This study included 95 consecutive patients with NSTE-ACS. All patients underwent coronary angiography within the first 72 hours depending on the clinical situation. Stenosis >or=50% in the coronary artery was considered significant. NSTE-ACS patients were then divided into two groups: patients with critical stenosis (n = 53) and patients with noncritical stenosis (n = 35). Plasma levels of D-dimer, fibrinogen, thrombin-antithrombin III complex (TAT), and prothrombin fragment 1 + 2 (PF 1 + 2) were measured. RESULTS D-dimer (338 +/- 192 vs. 190 +/- 170 microg/dL, P < 0.001), TAT (4.4 +/- 2.0 vs. 1.2 +/- 0.7 ng/mL, P < 0.001), and PF 1 + 2 (1.6 +/- 0.6 vs. 0.7 +/- 0.3 nmol/L) levels were significantly higher in the critical stenosis group as compared to the noncritical stenosis group. However, fibrinogen levels were similar in both groups. The levels of TAT (r = 0.76, P < 0.001) and PF 1 + 2 (r = 0.73, P < 0.001) were correlated with the culprit lesion severity, but D-dimer and fibrinogen levels were not. Thrombolysis in myocardial infarction flow grades were also correlated with TAT (r = -0.42, P < 0.001) and PF 1 + 2 (r = -0.40, P < 0.001). CONCLUSION The severity of culprit lesion may be associated with plasma D-dimer, TAT, and PF 1 + 2 levels in NSTE-ACS patients. These prothrombotic factors may have a role in the development of significant stenosis in the NSTE-ACS setting.

The aim of this study was to evaluate the factors associated with the development of atrial fibrillation (AF) in patients with rheumatic mitral stenosis (MS). A total of 146 consecutive patients with rheumatic MS were screened. They were accepted to be in AF group and sinus rhythm group according to their rhythm in the baseline ECG. After screening, 38 patients were excluded due to hyperthyroidism (n = 13), chronic obstructive pulmary disease (n = 22), malignancy (n = 2) and romatoid arthritis (n = 1). Objective Arrhythmias have been reported to occur frequently in symptomatic patients with mitral valve prolapse (MVP). The mechanisms causing atrial arrhythmias in patients with MVP have not been fully investigated. The purpose of this study was to determine the clinical, echocardiographic and heart rate variability parameters, and plasma concentrations of electrolytes and inflammatory markers in predicting atrial arrhythmias in patients with MVP. Methods A total of 58 consecutive patients with MVP were included in this study. We performed electrocardiography, echocardiography, Holter analysis, routine biochemical tests including plasma concentrations of electrolytes and inflammatory markers, and evaluated the clinical characteristics. Atrial arrhythmia was defined as occurrence of any of the following: atrial premature contractions, atrial couplets, supraventricular tachycardia, atrial flutter or fibrillation documented by Holter analysis, continuous monitoring or by electrocardiography. Results Twenty-eight patients (48%) had atrial arrhythmias, and 30 (52%) patients had no atrial arrhythmias. Univariable predictors of atrial arrhythmias included isovolumetric relaxation time, lateral systolic (S'), lateral late diastolic (A'), septal A' Doppler velocities, anterior mitral leaflet thickness, anterior mitral leaflet thickness of > or =5 mm, posterior mitral leaflet thickness and the occurrence of moderate to severe mitral regurgitation. Multivariable logistic regression analysis showed that occurrence of moderate to severe mitral regurgitation was the only independent predictor of atrial arrhythmias (relative risk: 2.4, 95% confidence interval 1.32-4.86, P = 0.005). Conclusions The present study showed that the only independent predictor of atrial arrhythmias in patients with MVP is the occurrence of moderate to severe mitral regurgitation.

Systemic thromboembolism is a serious major complication in patients with mitral stenosis. However, the pathogenesis of thromboembolism in mitral stenosis is not fully understood. Previous studies have demonstrated that platelet activation occurs in serum of patients with rheumatic mitral stenosis (MS). The aim of this study was to assess the mean platelet volume (MPV), an indicator of platelet activation, in patients with MS. We investigated effects of two doses of Tenoxicam, a type 2 cyclooxygenase inhibitor, administration on lipid peroxidation and antioxidant redox system in cortex of the brain in rats. Twenty-two male Wistar rats were randomly divided into three groups. First group was used as control. 10 and 20 mg/kg body weight Tenoxicam were intramuscularly administrated to rats constituting the second and third groups for 10 days, respectively. Both dose of Tenoxicam administration resulted in significant increase in the glutathione peroxidase activity, reduced glutathione and vitamins C and E of cortex of the brain. The lipid peroxidation levels in the cortex of the brain were significantly decreased by the administration. Vitamin A and ß-carotene concentration was not affected by the administration. There was no statistical difference in all values between 10 and 20 mg Tenoxicam administrated groups. In conclusion, treatment of brain with 10 and 20 mg Tenoxicam has protective effects on the oxidative stress by inhibiting free radical and supporting antioxidant redox system. © 2008 Springer Science+Business Media, LLC.

Objective: Acute coronary syndrom (ACS) is a common disease that causes severe morbidity and mortality. The most important aspect of ST-elevation myocardial infarction (STEMI) as a subgroup of ACS treatment is the rapid reperfusion of arteries. Successful results depend not only on the experience of the center but also on the rapidity in which reperfusion is achieved. In our study, the transfer parameters were evaluated in patients who were admitted to our hospital with STEMI. Methods: Two hundred consecutive patients (160 males, 40 females) who underwent primary percutaneous coronary intervention (PCI) for acute STEMI between January 2011 and March 2013 were included in our study. Transfer parameters of symptom-to-reperfusion treatment, clinical characteristics, and laboratory parameters were recorded. Results: Thirty-six patients were admitted to our hospital with ambulances; 70 patients were admitted to centers without PCI capability, with a mean transfer time to our hospital of 73.9+-12.5 min. Median pain-to-first medical contact time was 105 min (range: 5–600 min), and average first medical contact- to-balloon time was 115.5 min (range: 20–414 min). Total pain-to-balloon time in females was significantly higher than males (246 min [range: 70–840 min], 195 min [range: 45–684 min], respectively, p=0.032). Mean pain-to-balloon time was significantly lower in patients delivered to the hospital by ambulance than in patients admitted to emergency departments independently (185 min [range: 45–439 min], 248 min [range: 65–840 min], respectively, p=0.017). Conclusion: In this study, our hospital door-to-balloon time was found compatible with the target specified in the European Society of Cardiology and American College of Cardiology STEMI guidelines; however, first medical contact-to-balloon time was found to be above that advised by the current guidelines.

Hyperuricaemia may provide a negative contribution to impaired MPI in hypertension. Aim: The study was designed to assess the MPI in hypertensive patients with or without hyperuricaemia. Methods: A total of 96 consecutive hypertensive patients were divided into two groups according to levels of serum uric acid (SUA); 49 normouricaemic patients (defined as SUA < 7.0 mg/dL in men and < 6.0 mg/dL in women) and 47 hyperuricaemic patients. SUA levels and other biochemistry parameters were determined by a standard analytical technique. All patients were evaluated by two-dimensional and Doppler echocardiography. Results: The two groups were similar according to age, body mass index, and smoking status. Mean MPI value (0.498 +/- 0.06 vs. 0.410 +/- 0.05, p < 0.001) was significantly higher in the hyperuricaemic group than the normouricaemic individuals and positively correlated with the mean value of SUA levels (r = 0.51, p < 0.001). Conclusions: Our study demonstrated that high SUA levels were significantly associated with impaired MPI in hypertensive patients. SUA may suggest a valuable laboratory finding in assessing the risk of developing subclinical impaired left ventricular global function.

Fifty patients having AF without valvular diseases under treatment of warfarin in whom a transition to dabigatran was planned were consecutively enrolled in this study and followed up prospectively between July 2013 and July 2014. All patients completed Beck Depression Inventory and Hamilton Anxiety Scale (HAS) at the initiation of study and 6 months after initiation of study. Of the patients enrolled in the study, age, gender, smoking status, and comorbidities were questioned. A total of 50 patients (28 women; mean age 74.6 +/- 8.7 years) treated with warfarin in whom a transition to dabigatran was planned were included. Basal mean value of BDS (15.6 +/- 7.8 vs 11.5 +/- 4.8, P < .001) and HAS (16.8 +/- 10.4 vs 12.6 +/- 8.1, P < 0.001) was significantly higher in patients when they used warfarin than when they switched to dabigatran. In categorical analysis, frequency of patients with depression (mild, moderate, and severe) was significantly higher in period of warfarin use than after dabigatran transition (n = 24, 48% vs n = 14, 28%, P = .039). Our study demonstrates that patients with nonvalvular AF under treatment of dabigatran had lower BDS and HAS scores compared to warfarin. These findings suggest that dabigatran may increase quality of life and decrease morbidity and mortality due to reduction in anxiety and depression.

Objective We aimed to investigate the effectiveness and radiation protection capability of latex gloves coated with various contrast agents as an alternative to lead gloves. Methods The following six groups were created to evaluate the permeability of X-ray in this experimental study: lead gloves, two different non-ionic contrast media (iopromide 370/100 mg I/mL and iomeprol 400/100 mg I/mL), 10% povidone-iodine (PV-I), 240/240 g/mL barium sulphate and a mixture of equal amounts of all contrast agents. A radiation dose detector was placed in coated latex gloves for each one. The absorption values of radiation from latex gloves coated with various contrast agents were measured and compared with the absorption of radiation from lead gloves. This study was designed as an 'experimental study'. Results The mean absorption value of X-ray from lead gloves was 3.0±0.08 µG/s. The mean absorption values of X-ray from latex gloves coated with various contrast agents were 3.7±0.09 µG/s (iopromide 370/100 mg I/mL), 3.6±0.09 µG/s (iomeprol 400/100 mg I/mL), 3.7±0.04 µG/s (PV-I), 3.1±0.07 µG/s (barium sulphate) and 3.8±0.05 µG/s (mixture of all contrast agents). Latex gloves coated with barium sulphate provided the best radiation absorption compared with latex gloves coated with other radiodense contrast agents. Conclusion Latex gloves coated with barium sulphate may provide protection equivalent to lead gloves.

One hundred five patients were enrolled in the study (65 HT and 40 control subjects). The mean age of all patients was 50.5 +/- 15 years and 60% were female. Echocardiography was performed on all patients. MPI was measured with the classic Tei method (MPI-Tei index) and the PEP-derived MPI method by using tissue Doppler echocardiography. Although the MPI-Tei index is defined as the ratio of isovolumetric contraction time (IVCT) along with isovolumetric relaxation time (IVRT) to ejection time (ET), PEP-derived MPI is defined as the ratio of PEP and IVRT to ET. We compared echocardiographic data between the HT group and the control group. MPI-Tei index and the PEP-derived MPI values were higher in the HT group compared with controls (0.52 +/- 0.10 vs 0.39 +/- 0.07, P < 0.001, and 0.51 +/- 0.09 vs 0.39 +/- 0.07). PEP-derived MPI was strongly correlated with the MPI-Tei index (r = 0.945, P < 0.001). Our study determined that the PEP-derived MPI might be used in the evaluation of left ventricular function in patients with HT, similar to the classic MPI-Tei index.

Lyme disease is a tick-borne spirochete infection that may affect the heart. Cardiac manifestations include conduction disturbances and other pathologies of the heart. Disturbance of atrioventricular (AV) node conduction is the most common cardiac manifestation of Lyme disease. We report a 61-year old female, who was admitted to the emergency department with the complaints of syncope, dizziness and generalized fatigue. Physical examination revealed normal findings. The patient’s electrocardiogram on admission revealed a newly diagnosed bradycardia due to third degree of AV block with40/min ventricular rate. The patient was admitted to the coronary care unit and was placed a transvenous pacemaker. Lyme serology and Western blot were positive for Borrelia antibodies. The symptoms and AV block resolved completely after doxycycline therapy.

Methods: One hundred seven T2DM subjects with severe CVD diagnosed by angiography were included consecutively in this cross-sectional study. Biochemical and clinical parameters were obtained from patients who were not positive for nephropathy and retinopathy. MTHFR C677T genotypes were analyzed using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods. Normally and abnormally distributed continuous variables were analyzed using student t- and Mann-Whitney U tests. Categorical variables were analyzed using chi-square test. Results: In the study, 31 T2DM subjects had the CC (29.0%), 62 had the CT (57.9%), and 14 had the TT (13.1%) genotypes. There were no significant differences between subjects with wild-type (677CC) and with mutant (677CT+677TT) alleles in terms of diabetes duration, visceral fat area, total cholesterol, triglyceride, fasting plasma glucose, systolic blood pressure, diastolic blood pressure, high-sensitivity C-reactive protein, homocysteine (Hcy), and carotid intima-media thickness values. Conclusion: This study suggests that MTHFR gene polymorphisms can not be used as a marker for the assessment of cardiovascular risk in T2DM patients.

Methods Between June 2012 and November 2012, patients admitted to the Cardiology Department of Duzce University Faculty of Medicine and diagnosed previously with essential hypertension were included in the study. Patients were divided into two groups, nondippers and dippers, using ambulatory blood pressure measurement. All patients were evaluated by two-dimensional and Doppler echocardiography. LV MPI was calculated from tissue Doppler imaging parameters. Results There was no significant difference between the two groups in the proportion of each class of antihypertensive medications. Dippers and nondippers had similar age, BMI, lipid profiles, and smoking status. The MPI value was significantly higher in nondippers than in dippers, and was correlated negatively with the rate of systolic and diastolic blood pressure fall at night (P < 0.001). Conclusion Our study showed that MPI is disturbed in patients with nondipper hypertension. MPI may be used in the diagnosis and follow-up of global LV dysfunction in patients with a nondipper pattern, but further prospective studies are needed. Purpose To evaluate the association between statins, N -acetylcysteine (NAC) and angiotensin converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) and postoperative atrial fibrillation (AF). Methods A total of 590 patients undergoing coronary artery bypass and/or valve surgery were studied. An AF episode lasting >5 min during hospitalization was accepted as endpoint. Results AF rate was 18% ( n =106). Multivariable positive predictors of AF included age ≥65, left atrial diameter ≥45 mm, cross clamp time; negative predictors included use of left internal mammarian artery, NAC, and ACEIs or ARBs. Conclusions ACEIs or ARBs and NAC is associated with low incidence of postoperative AF, however, this association was not found with statin or combined use these agents.

The aim of the present study was to assess the relationship between RLS, which was diagnosed clinically, and night-time BP patterns in a relatively large young cohort who had not been treated before. Patients and methods After applying the exclusion criteria, this cross-sectional study included 230 consecutive patients with never-treated hypertension who presented to our institution for initial evaluation of hypertension. RLS was assessed using a self-administered questionnaire based on the International Restless Legs Study Group criteria. The questions on RLS were completed by 214 patients and ambulatory BP monitoring was carried out for all patients. Results In the study group, 133 patients were diagnosed as hypertensive (53.4% nondippings) and 81 patients as normotensives (54.3% nondippings). RLS was present in 61 patients (28.5%) in the total sample. The prevalence of RLS, overall, was significantly higher in nondippings compared with dippings (34.7 vs. 21.2%, respectively; P = 0.028). Logistic regression analysis showed that the RLS is an independent determinant for both hypertension (odds ratio = 0.43, 95% confidence interval = 0.21-0.83; P = 0.013) and the nondipping BP patterns (odds ratio = 1.96, 95% confidence interval = 1.05-3.67; P = 0.035). Conclusion We have shown that clinically diagnosed RLS was associated with the nondipping pattern, which has been shown to be an independent predictor of cardiovascular risk. Endothelial dysfunction and diffuse atherosclerosis have been proposed for the etiology of SCF. Asymmetric dimethylarginine (ADMA), an endogenous inhibitor of nitric oxide synthesis, levels are associated with an increased risk of endothelial dysfunction and atherosclerosis. ADMA levels may be increased by homocysteine. The aim of this study was to evaluate the relationship between ADMA, nitric oxide and homocysteine in patients with SCF. Methods. The study group consisted of 50 patients with SCF. An age-and gender-matched control group was composed of 30 patients with normal coronary arteries and normal coronary flow on coronary angiography. We measured ADMA, nitric oxide and homocysteine plasma concentrations in all patients. Results. Plasma nitric oxide concentrations were significantly lower in the SCF group than in the control group (11.4 +/- 6, 16.1 +/- 9, p = 0.02). Plasma ADMA concentrations (0.9 +/- 0.3, 0.7 +/- 0.3, p = 0.01) and plasma homocysteine concentrations (12.4 +/- 5, 9.8 +/- 2, p = 0.03) were significantly higher in the SCF group than control group. The mean TIMI frame count (TFC) was significantly correlated with plasma ADMA (r = 0.26, p = 0.02) and homocysteine (r = 0.28, p = 0.02) concentrations, but not with nitric oxide concentrations (r = -0.18, p = 0.13). In linear regression analysis, plasma ADMA concentrations (beta = 4.6, p = 0.005) and homocysteine concentrations (beta = 0.2, p = 0.03) were independently and positively associated with mean TFC. Conclusion. Our results suggest that plasma concentrations of ADMA and homocysteine are increased in SCF and also that these are independent predictors of SCF.

Objective Central obesity is a prerequisite for the diagnosis of metabolic syndrome (MetS). Precise measurement of visceral fat by bioelectrical impedance analysis (BIA) has been validated. The aim of this study was to investigate the diagnostic performance of BIA in MetS and validate the best cutoff in a large adult cohort. Materials and Methods The study was performed on the MELEN Study cohort—a prospectively designed survey on the prevalence of cardiometabolic risk factors in Turkish adults. The final cohort consisted of 2219 participants. Weight and visceral body composition were measured without shoes in light indoor clothes using a bioimpedance analyzer (Omron BF 510; Omron Corp, Kyoto, Japan). Plasma concentrations of cholesterol, insulin, fasting triglycerides, high-density lipoprotein cholesterol, glucose, and other biochemical variables were measured. The diagnostic performance of visceral fat measurement by BIA in patients with MetS was assessed. Results Metabolic syndrome was detected in 751 participants (520 women and 231 men with a mean age of 55 [12] years; 34% of the whole study population). Total body fat and visceral fat levels were higher in subjects with MetS. Correlation analyses showed that there were significant correlations between anthropometric and BIA measurements. Receiver operating curve characteristics of visceral adiposity revealed the best cutoff values as greater than 12% for men and greater than 9% for women. The diagnostic performance was good in both sexes (the sensitivity/specificity and area-under-the-curve values were 76%/75% and 0.83 for men and 83%/67% and 0.81 for women, respectively). Conclusions Visceral fat measured with BIA is an easily applicable and useful method for identifying people with MetS. The best cutoff values were higher than 12% for men and higher than 9% for women.

Herein, we present the impact of tirofiban infusion on an infarct-related artery with high thrombotic burden. The thrombus cleared completely without coronary angioplasty.
