Chronic obstructive pulmonary disease (COPD) is one of the major chronic airway diseases, with the highest prevalence among individuals over 60 years old, significantly impacting the health of elderly patients. Both domestic and international COPD prevention and treatment guidelines recommend pulmonary rehabilitation as a complementary and effective adjunct to pharmacological treatment. Pulmonary rehabilitation can significantly improve patients' respiratory function and physical fitness, alleviate symptoms, and reduce hospitalization and mortality rates. However, the specific contents of pulmonary rehabilitation for elderly COPD patients differ depending on the stage of the disease. This expert consensus elaborates on the assessment and treatment contents of pulmonary rehabilitation for elderly COPD patients in the acute exacerbation period, acute exacerbation remission period, and outpatient stable period, aiming to provide guidance for pulmonary rehabilitation treatment at different stages of the disease. This consensus identifies common functional impairments in elderly COPD patients, including respiratory dysfunction, exercise tolerance impairment, psychological issues, swallowing dysfunction and cognitive impairment. It recommends a comprehensive assessment of elderly COPD patients' multifaceted impairments based on the International Classification of Functioning, Disability, and Health (ICF) framework, covering four aspects：body structure, body function (respiratory function, cardiac function, exercise function, psychological and cognitive function, swallowing function, and nutritional factors), activity and participation, and environmental factors. Pulmonary rehabilitation is suitable for all patients exhibiting relevant symptoms and/or those at high risk of acute exacerbations (high-level evidence). As the disease progresses, the rehabilitation team, treatment plan, and treatment location should be adjusted in a timely manner, forming a continuous rehabilitation cycle. Smoking cessation is a crucial intervention to slow down the decline in lung function and the progression of COPD (high-level evidence). Through adequate nutritional support, elderly COPD patients can improve their nutritional status, enhance physical strength and quality of life, better manage the disease, and promote rehabilitation (low to moderate-level evidence). Oxygen therapy is recommended for patients with advanced COPD who remain hypoxemic at rest (low to moderate-level evidence). Some emerging rehabilitation treatments (such as neuromuscular electrical stimulation, acupuncture, and certain traditional Chinese medicines) may have some value for elderly COPD patients (low to moderate-level evidence. This expert consensus divides rehabilitation treatment into three stages：Phase I Pulmonary Rehabilitation, Phase II Pulmonary Rehabilitation, and Phase III Pulmonary Rehabilitation, based on the different stages of elderly COPD patients. It further details the target population, implementation location, rehabilitation content, assessment content, participating disciplines, and rehabilitation goals for each stage, emphasizing the need for clinical respiratory specialists, primary care physicians, rehabilitation physicians/therapists, nurses, nutritionists, psychologists, and social workers to actively coordinate and collaborate throughout the full-cycle rehabilitation process for elderly COPD patients.

Elderly lung cancer is the leading cause of cancer-related deaths worldwide, posing significant challenges to public health. This consensus systematically reviews relevant clinical guidelines and literature both domestically and internationally, integrating recent clinical experiences and research findings. It summarizes new clinical rehabilitation concepts from experts in rehabilitation medicine, respiratory medicine, and nursing regarding elderly lung cancer. The consensus provides detailed discussions on the definition, risk factors, classification, staging, and clinical characteristics of elderly lung cancer, with a focus on functional impairments and their rehabilitation assessment and treatment strategies. Functional impairments encompass multiple aspects, including pulmonary function, cardiac function, exercise capacity, pain, psychological issues, cognition, speech, swallowing, sensory functions, and bowel and bladder dysfunction. The consensus offers comprehensive academic guidance and clinical practice recommendations for rehabilitation professionals across various healthcare settings. It emphasizes that the full-cycle rehabilitation of elderly lung cancer requires multidisciplinary teamwork to ensure seamless integration of clinical care, rehabilitation, and nursing, maximizing patient functional recovery and quality of life. By conducting a comprehensive assessment of patients' overall conditions and formulating personalized rehabilitation treatment plans, effective improvements in functional impairments, symptom relief, and even prolongation of survival can be achieved. Additionally, the consensus outlines the service process for full-cycle rehabilitation of lung cancer, highlighting the interconnectedness of tertiary hospitals, secondary hospitals, community hospitals, and families, as well as the importance of adjusting rehabilitation models based on local circumstances. The promotion and implementation of this consensus aim to provide standardized guidance for the assessment and treatment of full-cycle rehabilitation in elderly lung cancer patients, with the expectation of improving rehabilitation outcomes and quality of life, thereby contributing to the development of elderly lung cancer rehabilitation in China.

Objective To investigate the clinical effect of transanal tube drainage and anal decompression after endoscopic submucosal dissection (ESD) of giant colorectal polyp for postoperative recovery and the prevention of complication. Methods Prospective clinical randomized control study was done. Enrolled patients with giant colorectal polyps (diameter ≥3 cm) were divided into 100 cases in endoscopic treatment group(endoscopic group) and 92 cases in endoscopic treatment with prophylactic anal tube implantation group (endoscopic anal tube group). Postoperative recovery and complication between 2 groups were compared. Results Delay bleeding from anal of postoperative complications was present 5(5.0%) cases in endoscopic group and 2(2.2%) cases in endoscopic anal tube group. There were electrocoagulation syndrome of complications 7 (7.0%) cases in endoscopic group and 1(1.1%) case in endoscopic anal tube group. The difference in postoperative complication rate was significant statistically(P0.05) between two groups. This study aimed to explore the longitudinal associations between healthy eating habits, resilience, insomnia, and Internet addiction by using a cross-lagged panel analysis of Chinese college students. Overall, 807 Chinese college students completed questionnaires on healthy eating habits, resilience, insomnia, and Internet addiction from August 2020 (time 1, T1) to November 2020 (time 2, T2), and were selected for the data analyses. Healthy eating habits (T1) had significant effects on resilience (T2; β = 0.064, p < 0.05) and insomnia (T2; β = −0.064, p < 0.05), but not Internet addiction (T2; β = −0.028, p > 0.05). Insomnia (T1) negatively predicted resilience (T2; β = −0.098, p < 0.01). Insomnia was bidirectionally associated with Internet addiction (Internet addiction at T1 to insomnia at T2: β = 0.085, p < 0.01; insomnia at T1 to Internet addiction at T2: β = 0.070, p < 0.05). Additionally, Internet addiction (T1) significantly predicted resilience (T2; β = −0.075, p < 0.05). This study further expanded the understanding of the longitudinal associations between healthy eating habits, resilience, insomnia, and Internet addiction, which provided higher-level evidence and important implications for the interventions for reducing college students' Internet addiction, developing healthy eating habits, and improving resilience and sleep health. Background. Cerebral ischemia–reperfusion injury is a common complication of ischemic stroke that affects the prognosis of patients with ischemic stroke. The lipid-soluble diterpene Tanshinone IIA, which was isolated from Salvia miltiorrhiza, has been indicated to reduce cerebral ischemic injury. In this study, we investigated the molecular mechanism of Tanshinone IIA in alleviating reperfusion-induced brain injury. Methods. Middle cerebral artery occlusion animal models were established, and neurological scores, tetrazolium chloride staining, brain volume quantification, wet and dry brain water content measurement, Nissl staining, enzyme-linked immunosorbent assay, flow cytometry, western blotting, and reverse transcription–quantitative polymerase chain reaction were performed. The viability of cells was measured by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide assays, while cell damage was measured by lactate dehydrogenase release in the in vitro oxygen glucose deprivation model. In addition, enzyme-linked immunosorbent assay, flow cytometry, western blotting, and reverse transcription–quantitative polymerase chain reaction were used to evaluate the therapeutic effect of Tanshinone IIA on ischemia/reperfusion (I/R) induced brain injury, as well as its effects on the inflammatory response and neuronal apoptosis, in vivo and in vitro. Furthermore, this study validated the targeting relationship between miR-124-5p and FoxO1 using a dual luciferase assay. Finally, we examined the role of Tanshinone IIA in brain injury from a molecular perspective by inhibiting miR-124-5p or increasing FoxO1 levels. Results. After treatment with Tanshinone IIA in middle cerebral artery occlusion–reperfusion (MCAO/R) rats, the volume of cerebral infarction was reduced, the water content of the brain was decreased, the nerve function of the rats was significantly improved, and the cell damage was significantly reduced. In addition, Tanshinone IIA effectively inhibited the I/R-induced inflammatory response and neuronal apoptosis, that is, it inhibited the expression of inflammatory cytokines IL-1β, IL-6, TNF-α, decreased the expression of apoptotic protein Bax and Cleaved-caspase-3, and promoted the expression of antiapoptotic protein Bcl-2. In vitro oxygen-glucose deprivation/reoxygenation (OGD/R) cell model, Tanshinone IIA also inhibited the expression of inflammatory factors in neuronal cells and inhibited the occurrence of neuronal apoptosis. In addition, Tanshinone IIA promoted the expression of miR-124-5p. Transfection of miR-124-5p mimic has the same therapeutic effect as Tanshinone IIA and positive therapeutic effect on OGD cells, while transfection of miR-124-5p inhibitor has the opposite effect. The targeting of miR-124-5p negatively regulates FoxO1 expression. Inhibition of miR-124-5p or overexpression of FoxO1 can weaken the inhibitory effect of Tanshinone IIA on brain injury induced by I/R, while inhibition of miR-124-5p and overexpression of FoxO1 can further weaken the effect of Tanshinone IIA. Conclusion. Tanshinone IIA alleviates ischemic–reperfusion brain injury by inhibiting neuroinflammation through the miR-124-5p/FoxO1 axis. Purpose: This study aimed to evaluate the associations between unhealthy eating habits and insomnia symptoms with Internet addiction (IA) among Chinese left-behind children (LBC) based on potential gender difference.Methods: This cross-sectional study was conducted in junior high schools from an economically disadvantaged area of China in May 2020. Participants were asked to complete the information and scales of socio-demographic characteristics, eating habits (the frequency of eating carbohydrates, fruits, vegetables, protein, and breakfast), insomnia severity and IA. A total of 3156 LBCs (43.9% males and 56.1% females, aged 14.2 years in average) were used for final analyses. Associations between eating habits and insomnia symptoms with IA were analyzed by generalized linear model with binary logistic regression.Results: The study found that left-behind children (LBCs) had unhealthy eating habits, particularly among females. Females reported more insomnia symptoms than males. The prevalence of IA was similar across genders. Consuming fewer fruits and vegetables and skipping breakfast were linked to increased IA risk, especially in females. Insomnia was significantly associated with higher IA risk in both genders.Conclusion: In LBCs, unhealthy eating and sleep were notably linked to IA, especially in females. BACKGROUND: Post-stroke cognitive impairment (PSCI) has a negative effect on activities of daily living. OBJECTIVE: Although a number of studies have been published on PSCI, no quantitative studies have yet been conducted. METHOD: CiteSpace and VOSviewer were used to quantitatively analyze PSCI to illustrate the research hotspots and trends in PSCI. All relevant publications were extracted from the Science Citation Index Expanded (SCI-E) of the Web of Science (WoS). RESULTS: A total of 6536 articles were included in this study. From 349 in 2010 to 942 in 2020, the number of publications increased dramatically. The USA maintained the top position worldwide and provided a vital influence. Harvard University was considered the leader in research collaboration among all institutions. Stroke was the most popular journal in this sector and Vincent Mok published the most articles in this area. We analyzed the keywords and identified five research hotspot clusters. By summarizing the literature on PSCI, we considered the publication information regarding different countries, institutions, authors and journals. CONCLUSION: The mechanism of PSCI is an active hotspot. Cerebral vascular disease, especially white matter lesions, also received more attention. [ABSTRACT FROM AUTHOR]
