Abstract

OBJECTIVE: Flexible fiberoptic bronchoscopy (FOB) is an often-employed invasive method in diagnosing, staging, and treating lung diseases. Conventional sedative agents facilitate this process. Dexmedetomidine (DM) has low side effects and is easy to administer for trans-mucosal absorption. This study aimed to investigate trans-mucosal DM used with local anesthesia during the FOB procedure. PATIENTS AND METHODS: Fifty-nine cases were retrospectively analyzed who had undergone diagnostic flexible fiberoptic bronchoscopy (FOB) in our clinic between September 2016 and September 2019. The two methods (Group 1: Sublingual, and Group 2: Intranasal) employed during the FOB procedure for the local anesthesia were compared. RESULTS: Fifty-nine patients were included in the study, wherein forty-six were males (77.9%), and thirteen (22.1%) females had a mean age of 58.02±8.7 years (range: 39-72 years). Thirty-three patients were in Group 1 (Sublingual) and 26 in Group 2 (Intranasal). No significant differences were there between groups regarding age, gender, body mass index, or ASA physical status. Modified Aldrete Score >9 was significant to reach with time as a correlation between operator and patient satisfaction. Sedation scores for groups at 1st, 9th, 12th, and 15th min were similar. Excessive coughing was observed in two (7.7%) patients of Group 2 but in none of Group 1 (p=0.105). Patients in both groups had no complaints of swallowing, excessive body movement, or lower oxygen saturation during examination (p>0.05). There were no complications (hypotension, bradycardia, respiratory depression, allergy, permanent amnesia, nausea, and vomiting) observed in patients. CONCLUSIONS: Our study results revealed that easily administered trans-mucosal dexmedetomidine sedation is safely applied during flexible fiberoptic bronchoscopy for adequate sedation, high satisfaction, and low complication rates with no significant difference in sublingual or intranasal administration. Abstract

Silver nanoparticle (Ag NP) is one of the most widely used nanoparticles in consumer products due to its antimicrobial properties. The wide usage of Ag NPs has led to their increased release into the environment and it may affect plants physiological and biochemical functions. Therefore, this study has been performed to reveal the dose-dependent (0, 0.5, 1, 5, 10 and 20 ppm) effects of 10-nm-sized Ag NPs on wheat (Triticum aestivum) roots after 15 d, correlating with the appearance of various biochemical and physiological stress responses. The effects of Ag NPs were examined using different experiment such as root elongation, uptake of Ag NPs, TEM analysis, lignin accumulation, callose deposition, H2O2 content, lipid peroxidation, activities of non-enzymatic and enzymatic antioxidants. The result showed that 10 nm-sized Ag NPs caused the inhibition of root elongation over 5 ppm, dose-dependent accumulation of Ag+, ultrastructural changes, lignin accumulation, callose deposition, increase of H2O2 content and lipid peroxidation and SOD, CAT, GPOX, APOX and GR enzymes activities altered as compared to control. As a result, 0.5 and 1 ppm Ag NPs demonstrated hormetic response, but over 5 ppm reduced plant root growth inducing the plant antioxidant defence systems., Annali di Botanica, Vol 9 (2019)

20. The effects of tenoxicam and Ringer's lactate on post-operative adhesion reformation

Abstract

OBJECTIVE: To determine the effects of tenoxicam, non-steroidal antiinflammatory agent, and Ringer's lactate on adhesion reformation in rat model. STUDY DESIGN: After creating a standard lesion over the right uterine horn of 40 adult female rats, relaparotomy and adhesiolysis were performed intraperitoneally; Group II: tenoxicam, Group III: Ringer's lactate, Group IV: tenoxicam + Ringer's lactate, Group I: Control group. Two weeks later third laparotomy was performed to evaluate the adhesions. RESULTS: The average adhesion scores of group II, III and IV were significantly lower than the score of the control group (p0.05), although Group III had a lower score than group II and IV. CONCLUSION: Tenoxicam, when used alone or together with Ringer's lactate solution, significantly decreases adhesion reformation, however, in this study it doesn't reveal any superiority to the classical Ringer's lactate solution.

Abstract

The article discusses the rare malignancy in women called metastatic breast carcinoma. It states that the primary breast carcinoma, lymphoma-leukemia, melanoma and sarcomas can spread to the breast area. It adds that ovarian cancer and other gynecologic cancers rarely spread towards the breast, however, it can occur as a result of prolonged cancer treatments in other organs. It suggests not to rule out the possibility that ovarian cancer patients can develop breast metastasis.

35. Are amniotic fluid C-reactive protein and glucose levels, and white blood cell counts at the time of genetic amniocentesis related with preterm delivery?

Abstract

OBJECTIVE: To compare women with spontaneous preterm delivery before 37 weeks and women who delivered at term with respect to amniotic fluid C-reactive protein (CRP), glucose levels, and white blood cell counts at the time of genetic amniocentesis. STUDY DESIGN: The study was conducted on 216 pregnant women who underwent genetic amniocentesis between the 15th and 18th weeks of gestation at Baskent University Obstetrics and Gynecology Department. All patients were followed until delivery for the occurrence of pregnancy complication. Indications for amniocentesis included abnormal triple test results showing increased risk for Down's syndrome, advanced maternal age and sonographic findings indicative for chromosomal abnormalities. The samples were carried immediately to the laboratory for cytogenetic and biochemical examination. Women with spontaneous preterm delivery before 37 weeks (n = 20) and those who delivered at term (n = 196) were compared with respect to some maternal and infant characteristics, amniotic fluid C-reactive protein, glucose levels, and amniotic fluid white blood cell counts. RESULTS: During the study period 244 patients underwent amniocentesis. A chromosomal abnormality was present in 11 patients. 1 patient had a spontaneous pregnancy loss within 3 weeks after the procedure and 16 patients were delivered for fetal or maternal indications (preeclampsia, fetal growth restriction, placenta previa). The remaining 216 women were included in the study and investigated for the risk of preterm delivery. The prevalence of spontaneous preterm delivery before 37 weeks was 9.3% (20/216). There were no significant differences between the preterm delivery and the term delivery groups with respect to C-reactive protein levels and white blood cell counts. Mean amniotic glucose levels were significantly lower in the preterm delivery group (P<0.05). Amniotic fluid glucose levels of < or = 46 mg/dL had a sensitivity of 100% and NPV of 100%. CONCLUSION: Amniotic fluid glucose levels at the time of genetic amniocentesis are lower in women with spontaneous preterm delivery before 37 weeks compared to those who delivered at term. Amniotic fluid glucose levels of < or = 46 mg/dL at the time of genetic amniocentesis may be more sensitive, cheaper and have higher negative predictive value than C-reactive protein levels and white blood cell counts for the prediction of patients in spontaneous preterm labor. The greatest benefit of amniotic fluid glucose testing might be when the physician judges the patient to be at low risk for preterm delivery. [ABSTRACT FROM AUTHOR]

Abstract

The object of this study was to compare the effects of oral conjugated estrogen (CEE) alone, CEE plus medroxyprogesterone acetate (MPA) and tibolone on lipid profiles, and investigate whether these effects change in time. Plasma lipid levels were studied for CEE (n = 49), CEE + MPA (n = 50) and tibolone (n = 51). Mean per cent changes at certain intervals were compared with their previous intervals for each therapy. Paired t-test was used for statistical analysis. CEE alone had raised high-density lipoprotein (HDL) and triglyceride levels and lowered total cholesterol and low-density lipoprotein (LDL) levels at the end of the 2-year study period. Addition of MPA to the CEE regimen weakened the effect on HDL and triglyceride, augmented the decrease in total cholesterol and did not affect LIDL. The tibolone group revealed similar but more prominent effects in total cholesterol and LDL levels. HDL and triglyceride levels were significantly below baseline in the first 6 months, but HDL changes vanished and triglyceride levels remained decreased at the end of 2 years. These data did not support a correlation between lipid levels and the biphasic incidence of cardiac events that were observed in the Heart and Estrogen/progestin Replacement Study (HERS), but revealed period-dependent changes in the tibolone group. 46. Helicobacter pyloriSeropositivity in Patients With Hyperemesis Gravidarum

Abstract

In theory, increased fluid accumulation resulting from elevated steroid hormones in pregnant women could lead to a pH shift in the gastrointestinal tract, with subclinical Helicobacter pyloriinfection becoming manifest as a consequence. Hyperemesis gravidarum is relatively frequent in primigravidas with high serum estradiol. This prospective study used an H. pylori–specific fluorescent serum antibody test to find any association between H. pyloriinfection and hyperemesis gravidarum. Subjects included 95 pregnant women with hyperemesis gravidarum and 116 unaffected women matched for gestational age. H. pyloriinfection was detected in 91.5 percent of women with hyperemesis gravidarum and in 45 percent of control subjects. The two groups did not differ significantly in age, gravidity, or parity, and there were no substantial differences in preterm births, small-for-gestational-age infants, perinatal deaths, or miscarriages. The hospital stay averaged 5.5 days for women with hyperemesis gravidarum and 1.5 days for control subjects.

49. Sleep quality, perceived stress and associated factors in women undergoing IVF treatment: short-term longitudinal study.

Abstract

Background: Despite limited studies in the literature, it is hypothesised that lifestyle factors are modifiable habits that have an impact on couples' fertility health. However, the relationship between sleep quality and infertility is an area that needs to be clarified., Objective: To determine sleep quality and associated factors in women receiving in vitro fertilisation treatment and to evaluate the changes in sleep over the treatment cycle., Methods: This is a longitudinal descriptive study conducted on 158 women in two infertility centres in Izmir, Turkey. Data were collected using the Personal Introduction Form, Pittsburgh Sleep Quality Index, Visual Analogue Stress Scale, and Physical Symptoms List. Data were collected during three different treatment periods: beginning of the in vitro fertilisation trial, oocyte pick-up day and the postembryo transfer period., Results: The sleep quality score of women progressively decreased as treatment progressed ( p  < .001). A strong positive correlation was determined between stress and sleep quality ( p  < .001). Additionally, advanced age, low education level, shift work, prolonged treatment and economic distress were associated with poorer sleep quality ( p  < .001). In addition, breast fullness, abdominal distension, and pain ( p  < .001) observed until the oocyte pick-up day, as well as nausea and fatigue during the postembryo transfer period ( p  < .001), were identified as physical symptoms affecting sleep quality., Conclusion: This article provides information on sleep quality during in vitro fertilisation and guides healthcare professionals to encourage and improve women's sleep quality.

50. Outcomes of Non-intubated Versus Intubated Thoracoscopic Surgery for Primary Spontaneous Pneumothorax.

Abstract

Background: This study aimed to compare the outcomes of non-intubated video-assisted thoracic surgery (N-VATS) and intubated video-assisted thoracic surgery (I-VATS) for primary spontaneous pneumothorax (PSP)., Materials and Methods: We retrospectively analyzed 120 consecutive patients who underwent VATS for PSP. The patients were divided into N-VATS and I-VATS groups. Demographics, clinical characteristics, postoperative results, pain scores, follow-up results, and management were evaluated and compared between the groups. Local anesthesia and deep sedation (ketamine 2 mg/kg IV and propofol 2 mg/kg IV slow infusion) were administered under spontaneous ventilation in the N-VATS group., Results: The groups did not differ significantly in terms of age, sex, American Society of Anesthesiology score, pneumothorax side, or smoking history ( P >0.05). The mean operation time, anesthesia time, oral intake opening time, and mobilization time were significantly shorter in the N-VATS group (26.04±4.61 vs. 48.26±7.82 min, 42.14±6.40 vs. 98.16±12.4 min, 2.1±0.4 vs. 8.4±1.2 h, and 4.2±0.9 vs. 2.6±1.4 between N-VATS and I-VATS, respectively; P <0.05). The surgical outcomes did not differ in terms of minor complications (12%-13%) and recurrence rates (5.1%-6.4%) during a mean follow-up period of 88.4±10.2 mo. No cases of conversion to open surgery or mortality were observed. General anesthesia and intubation were not required for any patient in the N-VATS group., Conclusions: Our results revealed no differences in minor complications or recurrence rates between groups. However, the N-VATS group had significantly shorter operation, anesthesia, oral intake opening, and mobilization times. The most important advantage of N-VATS for PSP is its fast recovery while avoiding the risks of general anesthesia and intubation.
