Background The family-centered and community-based community home-based elderly care has gradually become the main trend in the context of increased population aging in China. As an important component of the primary care service system, community health services play a cornerstone role in community home-based elderly care. Understanding the experience of home-dwelling old adults on community health services will help to improve the healthcare quality and promote the development of combined medical and nursing care. Objective To understand the reasons for the home-dwelling old adults to prefer community home-based elderly care and their experience, analyze the existing problems, so as to provide reference for developing needs-oriented community health services. Methods From February to June in 2022, the home-dwelling old adults aged ≥65 years old were selected from 10 community health centers in Chengdu as the study subjects by using purposive sampling method. 10 focus groups were set up to conduct face-to-face, semi-structured interviews. Data collection was performed by audio recording and text transcription. NVivo 12 software was used for the coding and categories extracting of interview content and the thematic framework analysis method was used to organize and analyze the interview data to extract the themes. Results A total of 40 home-dwelling old adults were enrolled, including 20 males and 20 females aged 65-86 years with an average age of (74.4±5.6) years, 31 cases from urban areas and 9 cases from suburban areas. The interview data were analyzed and 3 themes were extracted: Comprehensive factor of the old adults is the primary factor affecting their choice of pension modes and health needs; Community health service is an important way to provide health services for home-dwelling old adults, which cannot completely meet the health needs of the old adults; Optimizing service modes, enriching service programs and increasing external support can increase the sense of contentment and promote the health of old adults. Conclusion Home-dwelling old adults are basically satisfied with the health services provided in the community, but the personal factors of old adults, the modes and programs of community health services have influenced the experience of home-dwelling old adults with community health services, inducing their growing health needs unmet. Providing professional, diversified and precise community health services, improving the quality of medical services and the mode of medical services combining the health situation of home-dwelling old adults, are of great significance to improve the community health service system and and promote the health of older adults.

Abstract Disruption of gamma‐amino butyric acid type A receptors (GABAARs) synaptic clustering and a decrease in the number of GABAARs in the plasma membrane are thought to contribute to alteration of the balance between excitatory and inhibitory neurotransmission, which promotes seizure induction and propagation. The multipass transmembrane protein cleft lip and palate transmembrane protein 1 (Clptm1) controls the forward trafficking of GABAAR, thus decaying miniature inhibitory postsynaptic current (mIPSC) of inhibitory synapses. In this study, using a pentylenetetrazol (PTZ)‐induced epilepsy rat model, we found that Clptm1 regulates epileptic seizures by modulating GABAAR‐mediated inhibitory synaptic transmission. First, we showed that Clptm1 expression was elevated in the PTZ‐induced epileptic rats. Subsequently, we found that downregulation of Clptm1 expression protected against PTZ‐induced seizures, which was attributed to an increase in the number of GABAARγ2s in the plasma membrane and the amplitude of mIPSC. Taken together, our findings identify a new anti‐seizure target that provides a theoretical basis for the development of novel strategies for the prevention and treatment of epilepsy.

The purpose of this systematic review and meta-analysis of the literature was to analyze and evaluate the impact of prematurity and accelerated weight gain on the risk of childhood and adolescent obesity. CINAHL, Embase, PubMed, and Web of Science databases were searched until December 2019 which yielded 19 studies with a total of 169,439 children enrolled were systematically reviewed. The results revealed that preterm infants had a greater likelihood of childhood obesity (defined as BMI ≥95th percentile for age-sex), than term infants (OR = 1.19, 95% CI [1.13, 1.26]). However, no difference of childhood obesity was found between small for gestational age(SGA) and appropriate for gestational age(AGA) among preterms. Accelerated weight gain (defined as weight gain velocity during first two years after birth) significantly increased the likelihood of subsequent childhood obesity among preterms (aOR = 1.87, 95% CI [1.57, 2.231]). In conclusion, accelerated weight gain at infancy among preterm children may be a critical contributor to obesity in later life. Establishing optimal growth trajectories and timely referral to health care providers may be of clinical importance.

Secreted frizzled-related protein 5 (SFRP5) plays a pivotal role in regulating the development of many tissues and organs, however, as an inhibitor of Wnt signaling, the role of SFRP5 in vitiligo remains unknown. Hence, we speculated that SFRP5 might be associated with melanogenesis in melanocytes by regulating Wnt signaling in vitiligo. In this study, we found that SFRP5 was overexpressed in the skin lesions of patients with vitiligo. Compared with that in normal epidermal melanocytes (PIG1), the expression of SFRP5 was increased in vitiligo melanocytes (PIG3V). To investigate the effect of SFRP5 on melanin synthesis, PIG1 cells were infected with recombinant SFRP5 adenovirus (AdSFRP5), and PIG3V cells were infected with recombinant siSFRP5 adenovirus (AdsiSFRP5). The results showed that SFRP5 overexpression inhibited melanin synthesis in PIG1 cells through downregulation of microphthalmia-associated transcription factor (MITF) and its target proteins via suppression of the Wnt/β-catenin signaling pathway. Accordingly, SFRP5 silencing increased melanin synthesis and activated the Wnt signaling pathway in PIG3V cells. Moreover, SFRP5 overexpression also downregulated the transcriptional activity of T cell factor/lymphoid enhancer factor (TCF/LEF) in PIG1 cells. Furthermore, this inhibitory effect of SFRP5 on melanin synthesis was reversed by treatment with the β-catenin agonist, SKL2001. The inhibitory action of SFRP5 in pigmentation was further confirmed in vivo using a nude mouse model. Hence, our results indicate that SFRP5 can inhibit melanogenesis in melanocytes. Additionally, our findings showed that SFRP5 plays a vital role in the development of vitiligo, and thus may serve as a potential therapeutic target for vitiligo.

Background: Subdural empyema is one of the more serious complications of bacterial meningitis and therapeutic challenges to clinicians. We aimed to evaluate the clinical characteristics, treatment, and outcome of subdural empyema in neonates with bacterial meningitis. Methods: A retrospective cohort study was conducted in two medical centers in Taiwan that enrolled all cases of neonates with subdural empyema after bacterial meningitis between 2003 and 2020. Results: Subdural empyema was diagnosed in 27 of 153 (17.6%) neonates with acute bacterial meningitis compared with cases of meningitis without subdural empyema. The demographics and pathogen distributions were comparable between the study group and the controls, but neonates with subdural empyema were significantly more likely to have clinical manifestations of fever (85.2%) and seizure (81.5%) (both p values < 0.05). The cerebrospinal fluid results of neonates with subdural empyema showed significantly higher white blood cell counts, lower glucose levels and higher protein levels (p = 0.011, 0.003 and 0.006, respectively). Neonates with subdural empyema had a significantly higher rate of neurological complications, especially subdural effusions and periventricular leukomalacia. Although the final mortality rate was not increased in neonates with subdural empyema when compared with the controls, they were often treated much longer and had a high rate of long-term neurological sequelae. Conclusions: Subdural empyema is not uncommon in neonates with acute bacterial meningitis and was associated with a high risk of neurological complications, although it does not significantly increase the final mortality rate. Close monitoring of the occurrence of subdural empyema is required, and appropriate long-term antibiotic treatment after surgical intervention may lead to optimized outcomes. [ABSTRACT FROM AUTHOR]

This document is a correction notice for an article titled "Accelerated weight gain, prematurity, and the risk of childhood obesity: A meta-analysis and systematic review." The authors identified errors in Figure 3 and the use of the term "impact" throughout the article, which have been corrected. Some concerns were raised about possible biases in the statistical analyses, and the authors provide additional information about the covariates/confounders that were adjusted and reported in each original study selected. The authors address concerns about the lack of sensitivity analyses and provide additional information on the results. They caution readers to interpret the conclusion with caution due to the influence of one study in the meta-analysis comparing preterm and term infants. The authors also clarify that the meta-analysis on preterm small for gestational age (SGA) infants and childhood obesity remained robust even after excluding a specific study. They explain the criteria used for childhood obesity in the meta-analysis and state that the corrections and clarifications support the original findings and conclusions of the study. Objective To investigate the expression changes of G-protein-coupled receptor kinase-interacting protein 1 (GIT1) in lithium-pilocarpine-induced epileptic rat model and explore its role in the genesis and development of epilepsy. Methods The lithium-pilocarpine-induced model of status epilepticus (SE) was established in 42 specific pathogen free (SPF) male adult Wistar rats, and those rats were randomly divided into control group and 6 epilepsy groups (1, 3, 7, 14, 30, 60 d after SE). The expression of GIT1 mRNA was detected by fluroescent quantitative polymerase chain reaction (PCR), while the expression of GIT1 protein was examined by Western blotting and immunohistochemistry was applied to test the expression of CA1 region, dentate gyrus and parahippocampal cortex in rat hippocampus at different time points. Results GIT1 mRNA level rised in acute phase on 1st and 3rd day after SE (P = 0.012, 0.002), then increased continously in latency on 7th and 14th day (P = 0.003, 0.001), and reached the peak in chronic phase on 30th and 60th day (P = 0.000, for all). GIT1 protein expression rised in acute phase and increased continously in chronic phase, but there was no significant difference compared with control group (P > 0.05, for all). Then, it reached the peak in chronic phase (P = 0.000, for all). Until the 30th day, the GIT1 expression of CA1 region, dentate gyrus and parahippocampal cortex in the hippocampus of rats in 6 epilepsy groups was significantly higher than that of control group (P = 0.000, for all). Conclusions The up-regulated expression of GIT1 in the hippocampus of epileptic rat was probably involved in the formation process of chronic epilepsy by regulating cytoskeleton dynamic regrouping to influence excitatory neural networks.
