Abstract

Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung's disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung's disease) from 264 hospitals (89 in high-income countries, 166 in middle-income countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Abstract

Background The Patient-Reported Outcomes Measurement Information System® (PROMIS®) created pediatric self-report scales measuring a variety of health attributes (domains), but their responsiveness to changes in health status has not yet been determined in children with sickle cell disease (SCD). Procedure A convenience cohort of symptomatic SCD children, aged 8–17 years, was asked to complete PROMIS pediatric scales at an initial clinic visit, at the end of a subsequent hospitalization for sickle pain, at a subsequent clinic visit or at home 2–3 weeks after hospitalization, and at a clinic visit 1–2 years after their initial assessment. Results A total of 121 participants (mean age 12.5 ± 3.1 years, 56.2% female) participated in the study. Pain interference and fatigue domain scores were elevated at baseline, increased substantially during hospitalization, and largely returned to baseline by the recovery period, whereas the depressive symptoms, anger, and anxiety domain scores displayed a less pronounced elevation during hospitalizations and a slower return to baseline levels. The two physical functioning scales showed a substantial decline in response to hospitalization, but only modest improvements at the recovery assessment, likely representing incomplete recovery. Conclusions Several PROMIS pediatric measures were responsive to changes in health status associated with occurrence and resolution of acute vaso-occlusive pain requiring hospitalization. The substantial differences in these domains during SCD-related pain exacerbations support their potential usefulness in clinical research or in clinical practice. Further studies to characterize variations in symptom patterns over time may provide insights into strategies for more effective management of sickle pain.

Adolescents (ages 12-17 years) reported significantly higher pain interference and depressive symptoms, and worse lower extremity physical functioning domain scores compared to younger children (ages 8-11 years). Female participants reported significantly higher pain interference, fatigue, and depressive symptoms, and worse lower extremity physical functioning domain scores compared with their male counterparts. Abstract

The UV LED mission demonstrates the precise control of the potential of electrically isolated test masses that is essential for the operation of space accelerometers and drag free sensors. Accelerometers and drag free sensors were and remain at the core of geodesy, aeronomy, and precision navigation missions as well as gravitational science experiments and gravitational wave observatories. Charge management using photoelectrons generated by the 254 nm UV line of Hg was first demonstrated on Gravity Probe B and is presently part of the LISA Pathfinder technology demonstration. The UV LED mission and prior ground testing demonstrates that AlGaN UV LEDs operating at 255 nm are superior to Mercury vapor lamps because of their smaller size, lower draw, higher dynamic range, and higher control authority. We show flight data from a small satellite mission on a Saudi Satellite that demonstrates AC charge control (UV LEDs and bias are AC modulated with adjustable relative phase) between a spherical test mass and its housing. The result of the mission is to bring the UV LED device Technology Readiness Level (TRL) to TRL 9 and the charge management system to TRL 7. We demonstrate the ability to control the test mass potential on an 89 mm diameter spherical test mass over a 20 mm gap in a drag free system configuration. The test mass potential was measured with an ultra high impedance contact probe. Finally, the key electrical and optical characteristics of the UV LEDs showed less than 7.5 percent change in performance after 12 months in orbit., Comment: 22 pages, 21 figures

BACKGROUND: Current scales to measure bleeding in clinical trials are inadequate. The aim of this study was to develop a simple, valid, and reliable measurement tool to categorize the severity of bleeding in patients with chemotherapy-induced thrombocytopenia (CIT). STUDY DESIGN AND METHODS: Measurement theory was used to develop the Bleeding Severity Measurement Scale (BSMS) in four steps: 1) identification of the patient population, 2) item generation and reduction, 3) reviewing the items and formatting the scale, and 4) evaluation of psychometric properties. Feasibility was tested in a pilot study. Content and face validity were assessed by expert review. Psychometric evaluation included determination of intra- and interrater reliability and construct and criterion validity. RESULTS: The final BSMS defined two grades of bleeding: not clinically significant (Grade 1) and clinically significant (Grade 2). Grade 2 bleeds were defined as bleeds resulting in morbidity, requiring interventions, or directly causing death. The BSMS had excellent interrater (intraclass correlation coefficient [ICC], 0.80) and intrarater (ICC, 1.0) reliability and good construct and criterion validity. The BSMS distinguished between patients with different bleeding severities. CONCLUSION: Using rigorous methods, we designed a simple bleeding assessment tool with excellent psychometric properties for patients with CIT. Use of this scale in clinical trials should provide valid and reliable assessments of bleeding.

Abstract

Estimation of fast fading channels with large possible delay spread is considered. The time-varying fading channel taps are explicitly modeled by polynomial basis functions, and the basis coefficients are obtained via a matching pursuit (MP) algorithm. It is shown that, in the case of sparse channels, this approach outperforms the conventional least squares (LS) estimation with polynomial basis functions as well as the conventional adaptive matching pursuit, while still providing significant savings in complexity. Moreover, the MP is used to identify the appropriate polynomial model orders for channel taps that vary at different rates. A new least squares algorithm using matching pursuit (LSMP) is proposed. It is shown that the LSMP performs well for both sparse and non-sparse time-varying channels.

Abstract

The UV-LED mission demonstrates the precise control of the potential of electrically isolated test masses. Test mass charge control is essential for the operation of space accelerometers and drag-free sensors which are at the core of geodesy, aeronomy and precision navigation missions as well as gravitational wave experiments and observatories. Charge management using photoelectrons generated by the 254 nm UV line of Hg was first demonstrated on Gravity Probe B and is presently part of the LISA Pathfinder technology demonstration. We show laboratory data demonstrating the effectiveness and survivability of the UV-LED devices and performance of the charge management system. We also show flight data from a small satellite experiment that was one of the payloads on KACST’s SaudiSat-4 mission that demonstrates ‘AC charge control’ (UV-LEDs and bias are AC modulated with adjustable relative phase) between a spherical test mass and its housing. The result of the mission brings the UV-LED device Technology Readiness Level (TRL) to TRL-9 and the charge management system to TRL-7. We demonstrate the ability to control the test mass potential on an 89 mm diameter spherical test mass over a 20 mm gap in a drag-free system configuration, with potential measured using an ultra-high impedance contact probe. Abstract

Cartilage lesion of the patellofemoral joint is a common and challenging disease of the knee and an important cause of anterior knee pain. There are many naturally occurring variations in the anatomy and congruence of the patella and femoral trochlea. The purpose of this study was to identify the variations in patellofemoral anatomy and congruency that predispose to cartilage lesions. Among patients who underwent knee arthroscopy in our center from January 2005 to December 2006, 111 patients with chronic patellofemoral cartilage lesions and anterior knee pain were selected as the lesion group, while 124 patients with isolated meniscus rupture without anterior knee pain were selected as the control group. Twenty-one parameters measured on magnetic resonance images were used to assess the patellofemoral anatomy and congruence. A binary logistic regression model was used to look for possible associations between each of these parameters and the occurrence of patellofemoral cartilage lesions. The Bonferroni correction with a type I error rate of 0.0024 (0.05/21) was adopted to indicate statistical significance. Based on examination of the patellofemoral anatomy, 4 parameters were significantly associated with patellofemoral cartilage lesions. These were the patella lateral facet width, patella lateral facet ratio, sulcus depth and sulcus relative depth (P for linear trend <.0024). For patellofemoral congruence, 3 parameters were significantly associated with patellofemoral cartilage lesions. These were the lateral patella displacement, patella epicondylar axis angle and congruence angle (P for linear trend <.0024). Among the many kinds of patellofemoral variations, several were found to correlate with the development of patellofemoral cartilage lesions. These problems could be important risk factors for patellofemoral cartilage lesions.
