Abstract

Minimal hepatic encephalopathy (MHE) has a substantial impact on the clinical outcomes and quality of life (QOL) of patients with cirrhosis. However, timely diagnosis and intervention are challenging due to sophisticated diagnostic methods. In this study, 673 healthy controls and 905 patients with cirrhosis were screened, and 660 healthy controls and 757 patients with cirrhosis, divided into the test (292 patients) and validation (465 patients) cohort, were analyzed after screening. A diagnostic model of the Stroop test (Stroop‐CN) was constructed by multivariate linear regression based on the results of healthy controls. The prevalence of MHE and the comparison results with psychometric hepatic encephalopathy score through the Stroop‐CN model were stable in the test and validation cohorts. Moreover, the prevalence of MHE remained significantly higher in patients with worse disease conditions marked as high Child–Pugh grades and the Model for End‐stage Liver Disease and Sodium (MELD‐Na) scores in the test and validation cohort. The EuroQol 5‐D questionnaire revealed that patients with MHE had a worse QOL than those without MHE both in the test and validation cohort. In conclusion, an easy and practical Stroop‐CN model for MHE diagnosis based on the EncephalApp is established. It is found that a considerable number of Chinese patients with cirrhosis experience MHE, which significantly impacts their QOL. [ABSTRACT FROM AUTHOR]

Abstract

Surveys and research on the applications of the hepatic venous pressure gradient (HVPG) are important for understanding the current status and future development of this technology in China. This article aimed to investigate the status of hepatic venous pressure gradient measurement in China in 2022. We investigated the overall status of HVPG technology in China—including hospital distribution, hospital level, annual number of cases, catheters used, average cost, indications, and current challenges by using online questionnaire. By counting the number and percentages of cases of these results, we hope to clarify the current status of HVPG measurements in China. According to the survey, 85 hospitals in China used HVPG technology in 2022 distributed across 29 provinces. A total of 4989 HVPG measurements were performed in all of the surveyed hospitals in 2022, of which 2813 cases (56.4%) were measured alone. The average cost of HVPG measurement was 5646.8 ± 2327.9 CNY. Of the clinical teams who performed the measurements (sometimes multiple per hospital), 94.3% (82/87) used the balloon method, and the majority of the teams (72.4%, 63/87) used embolectomy catheters. This survey clarified the clinical application status of HVPG in China and confirmed that some medical institutions in China have established a foundation for this technology. Abstract

Objective To compare different treatment methods for patients with gastric calculi and provide data support for clinical treatment. Methods A total of 197 patients diagnosed with gastric calculi by gastroscopy at the General Hospital of Ningxia Medical University and Cardio-Cerebrovascular Disease Hospital of General Hospital of Ningxia Medical University from July 2013 to January 2024 were enrolled. The study collected general information and other data of the patients, and divided them into groups based on the selected treatment method using a real-world research approach. The subjects were divided into four groups: drug conservative treatment group, endoscopic homemade snare treatment group, disposable snare treatment group, and stone fragmentation treatment group. Statistical analysis was performed using one-way analysis of variance. Results Gastric calculi were more common in men, with an average age of (55.45±14.21). 85.3% of the patients had a history of eating persimmon, 86.3% had ulcers, and 65.9% were located in the gastric angle. The self-made snare group had the lowest treatment cost, while the stone fragmentation group had the highest. There was no significant difference in the remission time of clinical symptoms among the three endoscopic treatment methods. The self-made snare had the highest patient satisfaction, but the drug combined with carbonated beverage group had the longest remission time of clinical symptoms and the lowest patient satisfaction. The frequency and duration of endoscopic treatment of dark green gastric stones were significantly higher than those of mottled and golden yellow gastric stones. Conclusion When treating patients with gastric stones, it is important to consider the size and color of the stone, as well as the patient's preferences. Patients should be fully informed about their condition and the advantages of different treatments.
