6. The Variable Insertional Anatomy of the Abductor Pollicis Longus: Functional Relevance and Relationship to Adjacent Thumb Extensors.

- Abstract
Background: The abductor pollicis longus (APL) is classically described as inserting on the base of the first metacarpal. This study analyzed APL insertional anatomy and quantified the size of various elements of the extensor side of the thumb to determine associations with size and function. Methods: Twenty-four formalin-preserved upper limbs were dissected. The insertional anatomy of the APL, extensor pollicis brevis, and extensor pollicis longus were characterized, and the capacity of APL tendon slips to perform palmar abduction of the first digit was quantified based on slip size and insertion. Results: The mean number of APL tendon slips observed was 2.3. Abductor pollicis longus insertion sites included the base of the first metacarpal, trapezium, abductor pollicis brevis, and opponens pollicis. Only 4 specimens had a solitary metacarpal slip, while 83% of specimens had insertions onto at least 1 thenar muscle. A total of 62.5% of APL tendons exhibited some form of branching that we categorized into "Y" and "Z" patterns. In assessing palmar abduction capacity, we found that APL tendon slips inserting into the base of the first metacarpal were larger in cross-sectional area than nonmetacarpal slips and reproduced complete palmar abduction of the digit in the absence of nonmetacarpal slips. The abduction capacity of APL tendon slips was not correlated to the cross-sectional area. Conclusions: There is significant variability in APL tendon slips, branching patterns, and insertional anatomy. - Abstract
Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation. - Abstract
Background Reoperation rates are high after surgery for hip fractures. We investigated the effect of a sliding hip screw versus cancellous screws on the risk of reoperation and other key outcomes. Methods For this international, multicentre, allocation concealed randomised controlled trial, we enrolled patients aged 50 years or older with a low-energy hip fracture requiring fracture fixation from 81 clinical centres in eight countries. Patients were assigned by minimisation with a centralised computer system to receive a single large-diameter screw with a side-plate (sliding hip screw) or the present standard of care, multiple small-diameter cancellous screws. Surgeons and patients were not blinded but the data analyst, while doing the analyses, remained blinded to treatment groups. The primary outcome was hip reoperation within 24 months after initial surgery to promote fracture healing, relieve pain, treat infection, or improve function. 23. Combined Palmer Type 1A and 1B Traumatic Lesions of the Triangular Fibrocartilage Complex.

- Abstract
We present a series of eight patients who underwent wrist arthroscopy for presumed solitary tears of the triangular fibrocartilage (TFC) and were, instead, found to have combined 1A (central tear) and 1B (ulnar avulsion) tears. The Palmer Classification does not currently categorize this combined pattern. All but one patient had a traumatic injury. Each subject had preoperative radiographs and MRI scans. TFC tears were evident on all MRI scans, though only one was suggestive of a combined tear pattern. Surgical management included arthroscopic central tear debridement and ulnar peripheral repair. Average follow-up was 22 months. Grip strength in the affected hand improved from 16% deficit as compared to the unaffected side, to 3.5% deficit postoperatively (p = 0.003), and visual analog scores (VAS) decreased from an average of 7.1/10 preoperatively to 2.3/10 postoperatively (p < 0.001). There was no statistically significant change in wrist range of motion (ROM), however. Arthroscopic debridement of the central perforation (1A lesion) with concomitant repair of the ulnar detachment (1B lesion) resulted in functional and symptomatic improvement. This combined 1A/1B TFC injury is not reliably diagnosed preoperatively and should be considered a new subset in the Palmer classification, as this will raise awareness of its presence and assist in preoperative planning of such lesions. Purpose: In patients having basal joint arthroplasty, rates of concomitant carpal tunnel syndrome may exceed 43%. Excision of the trapezium during basal joint arthroplasty partially releases the transverse carpal ligament. The objective of this study was to determine whether indirect release of the remaining fibers of the ligament at its insertion on the scaphoid tubercle through the same incision would adequately decompress the carpal canal. This would avoid the morbidity of a second incision. Methods: We retrospectively reviewed 10 consecutive patients treated over a 2-year period who had concurrent ligament reconstruction, tendon interposition basal joint arthroplasty of the thumb with complete trapezial resection, and indirect carpal tunnel release through a single incision. Self-reported postoperative pain relief was documented. Preoperative and postoperative evaluations of light touch sensibility, Tinel''s sign, Phalen''s test, median nerve compression test, and Weinstein Enhanced Sensory Test monofilament testing were analyzed. All patients had preoperative electrodiagnostic testing. Results: At mean 12 month follow-up (range, 5–35 months), numbness and paresthesias resolved in all cases: 9 patients had good or excellent pain relief and 1 reported fair pain relief despite transient postoperative reflex sympathetic dystrophy. Tinel''s, Phalen''s, and median nerve compression tests were each positive in 9 patients preoperatively and 0 patients postoperatively. Diminished light touch sensibility was present in 7 preoperatively and 1 postoperatively. WEST monofilament thresholds improved postoperatively in 8 patients. These improvements were statistically significant. One patient had postoperative nerve conduction studies performed to evaluate an ulnar nerve lesion at the elbow; the preoperative median nerve abnormalities had normalized. Conclusions: Carpal tunnel syndrome can be successfully managed with indirect release of the transverse carpal ligament during basal joint arthroplasty, thus avoiding a second incision, reducing operating time, and potentially reducing the possibility of perineural adhesions, recurrent motor branch injury, palmar cutaneous nerve damage, weakness, stiffness, and pillar pain. Background : A major consequence of the COVID-19 pandemic on the U.S. healthcare system has been the rapid transition away from in-person healthcare visits to telehealth. This study analyzed patient and surgeon satisfaction in the utilization of telehealth within the hand surgery division during the COVID-19 pandemic. Methods : All hand surgery patients who completed a telemedicine visit from March 30
th , 2020 through April 30th , 2020 completed a 14-question survey via e-mail. Hand surgeons who participated in telemedicine completed a separate 14-question survey. Survey results were presented descriptively (mean ± standard deviation) and patient factors influencing satisfaction were determined using univariate and multivariate proportional modeling. Results : 89 patients and five surgeons completed the surveys. Patients were very satisfied with their telemedicine visits (4.21/5.00 ± 0.89). Multivariate proportional modeling determined patients who found it "very easy" (5/5) to arrange telemedicine visits had greater satisfaction (OR = 4.928; 95% CI = 0.94 to 25.84) compared to those who found it "difficult" (2/5) (p = 0.059). Patients who believed they could ask/relay questions/concerns "extremely effectively" (5/5) had greater satisfaction (OR = 55.236; CI = 11.39 to 267.80) compared to those who asked/relayed questions only "slightly effective" to "moderately effectively" ( p < 0.001). Surgeons were similarly satisfied with their telemedicine experience (4.00/5.00 ± 0.89) and were confident in their diagnoses (4.20/5.00 ± 0.84). All surgeons responded they will continue using telemedicine. 30.7% of patients would choose telemedicine over an inperson visit. Conclusions : Telemedicine provides a viable platform for healthcare delivery with high patient and surgeon satisfaction. Most patients still prefer in-person visits for the post-pandemic future.- Published

Background: Owing to the COVID-19 pandemic, there has been a large shift in health care toward virtual platforms. This study analyzed patient and physician satisfaction with telehealth during the height of the pandemic within the division of sports medicine. Methods: All sports medicine patients who completed a telemedicine visit from March 30, 2020, through April 30, 2020, were sent a 14-question Likert scale (1-5/5) survey. Sports medicine physicians who used telemedicine were sent a separate 14-question Likert scale (1-5/5) survey at the end of the study period. Factors influencing patient satisfaction were determined using a multivariate linear regression model. Results: A total of 143 patients and 9 sports medicine attendings completed the surveys. Most patients were "satisfied" (4/5) or "very satisfied" (5/5) (88.8%). A multivariate linear regression determined that patients who believed they had a greater ability to adopt new technology and were more effective at communicating questions/concerns to their physicians had greater satisfaction ( p = 0.009 and p = 0.015, respectively). Most physicians were either "satisfied" (4/5) or "very satisfied" (5/5) (75.0%). On average, physicians felt that physical examinations conducted through telemedicine were "moderately effective" (2.75/5.00 ± 1.3), that they were "fairly confident" (3.86/5.00 ± 0.83) in their diagnoses, and that most sports medicine attendings plan to use telemedicine in the future (87.5%). Conclusion: Telehealth emerged as a valuable tool for the delivery of health care to sports medicine patients during the COVID-19 pandemic. Patients and physicians reported high levels of satisfactions with its use, and this study further identifies areas that can improve the patient and physician experience.

- Abstract
Spontaneous ruptures of the flexor carpi radialis tendon (FCR) are rare and associated with systemic inflammatory diseases, localized tendinopathy related to scaphotrapezial-trapezoidal arthritis, or chronic immunosuppression from corticosteroids. While most cases do not require operative intervention, some patients develop weakness, impaired range of motion, and persistent pain. Previously reported surgical options include synovectomy, tendon stump resection, and osteophyte removal. We describe a surgical technique for patients with persistent symptomatology following FCR rupture in which the FCR is transposed end-to-side to the palmaris longus tendon. Three cases using this technique are presented with follow-up of 4-9 months that were collected at Lenox Hill Hospital. All three patients did well regarding specific outcome measures: grip strength, range of motion, and functional activity. FCR transfer to palmaris is an alternative to other surgical options for the spontaneous rupture of the FCR tendon in patients who remain symptomatic despite a course of non-operative therapy.

Background: Review of the recent literature suggests substantial use of the Internet by patients seeking health care information despite questionable accuracy and readability of information presented on some websites. The purpose of our study was to assess the accuracy, quality, and readability of online information regarding trigger finger., Methods: Three terms ("trigger finger", "stenosing tenosynovitis", and "finger locking") were used to query three Internet search engines to evaluate websites regarding information about trigger finger. Three hand surgery fellows independently assessed website accuracy and quality using standardized scoring criteria. The Fleisch-Kincaid reading grade level score was used to assess website readability. Website authorship and commercial association were also noted., Results: Sixty-nine unique websites were assessed. Internet information obtained using the search term "stenosing tenosynovitis" was written at a significantly higher reading level than information found using "finger locking" or "trigger finger". Website quality and accuracy were both significantly better in websites authored by physicians compared to websites authored by non-physicians. However, website accuracy was significantly poorer in websites featuring commercial association. Additionally, websites presenting information written below the 8(th) grade reading level demonstrated poorer accuracy., Conclusions: Search term has a significant effect on the readability of online information regarding trigger finger. Despite the terminology used in searches, most websites are largely inaccurate and may not be easily understandable by the general population. This demonstrates a substantial barrier to accessing accurate health care information and may impact patient outcomes. Hand surgeons should direct patients towards websites presenting accurate information with easily readable content.
