3. A Novel Approach to Assessing Differentiation Degree and Lymph Node Metastasis of Extrahepatic Cholangiocarcinoma: Prediction Using a Radiomics-Based Particle Swarm Optimization and Support Vector Machine Model

Abstract

BackgroundRadiomics can improve the accuracy of traditional image diagnosis to evaluate extrahepatic cholangiocarcinoma (ECC); however, this is limited by variations across radiologists, subjective evaluation, and restricted data. A radiomics-based particle swarm optimization and support vector machine (PSO-SVM) model may provide a more accurate auxiliary diagnosis for assessing differentiation degree (DD) and lymph node metastasis (LNM) of ECC. ObjectiveThe objective of our study is to develop a PSO-SVM radiomics model for predicting DD and LNM of ECC. MethodsFor this retrospective study, the magnetic resonance imaging (MRI) data of 110 patients with ECC who were diagnosed from January 2011 to October 2019 were used to construct a radiomics prediction model. Radiomics features were extracted from T1-precontrast weighted imaging (T1WI), T2-weighted imaging (T2WI), and diffusion-weighted imaging (DWI) using MaZda software (version 4.6; Institute of Electronics, Technical University of Lodz). We performed dimension reduction to obtain 30 optimal features of each sequence, respectively. A PSO-SVM radiomics model was developed to predict DD and LNM of ECC by incorporating radiomics features and apparent diffusion coefficient (ADC) values. We randomly divided the 110 cases into a training group (88/110, 80%) and a testing group (22/110, 20%). The performance of the model was evaluated by analyzing the area under the receiver operating characteristic curve (AUC). ResultsA radiomics model based on PSO-SVM was developed by using 110 patients with ECC. This model produced average AUCs of 0.8905 and 0.8461, respectively, for DD in the training and testing groups of patients with ECC. The average AUCs of the LNM in the training and testing groups of patients with ECC were 0.9036 and 0.8889, respectively. For the 110 patients, this model has high predictive performance. The average accuracy values of the training group and testing group for DD of ECC were 82.6% and 80.9%, respectively; the average accuracy values of the training group and testing group for LNM of ECC were 83.6% and 81.2%, respectively. ConclusionsThe MRI-based PSO-SVM radiomics model might be useful for auxiliary clinical diagnosis and decision-making, which has a good potential for clinical application for DD and LNM of ECC.

Abstract

Background and aimsCurrent treatment guidelines recommend ursodeoxycholic acid (UDCA) as the first-line treatment for new-diagnosed primary biliary cholangitis (PBC) patients. However, up to 40% patients are insensitive to UDCA monotherapy, and evaluation of UDCA response at 12 months may result in long period of ineffective treatment. We aimed to develop a new criterion to reliably identify non-response patients much earlier.MethodsFive hundred sixty-nine patients with an average of 59 months (Median: 53; IQR:32-79) follow-up periods were randomly divided into either the training (70%) or the validation cohort (30%). The efficiency of different combinations of total bilirubin (TBIL), alkaline phosphatase (ALP), and aspartate aminotransferase (AST) threshold values to predict outcomes was assessed at 1, 3 or 6 month after the initiation of UDCA therapy. The endpoints were defined as adverse outcomes, including liver-related death, liver transplantation and complications of cirrhosis. Adverse outcome-free survival was compared using various published criteria and a proposed new criterion.ResultsA new criterion of evaluating UDCA responses at 1 month was established as: ALP ≤ 2.5 × upper limit of normal (ULN) and AST ≤ 2 × ULN, and TBIL ≤ 1 × ULN (Xi'an criterion). The 5 year adverse outcome-free survival rate of UDCA responders, defined by Xi'an criterion, was 97%, which was significantly higher than that of those non-responders (64%). An accurate distinguishing high-risk patients' capacity of Xi'an criterion was confirmed in both early and late-stage PBC.ConclusionsXi'an criterion has a similar or even higher ability to distinguish high-risk PBC patients than other published criteria. Xi'an criterion can facilitate early identification of patients requiring new therapeutic approaches.

Abstract

The calculation of a three-dimensional underwater acoustic field has always been a key problem in computational ocean acoustics. Traditionally, this solution is usually obtained by directly solving the acoustic Helmholtz equation using a finite difference or finite element algorithm. Solving the three-dimensional Helmholtz equation directly is computationally expensive. For quasi-three-dimensional problems, the Helmholtz equation can be processed by the integral transformation approach, which can greatly reduce the computational cost. In this paper, a numerical algorithm for a quasi-three-dimensional sound field that combines an integral transformation technique, stepwise coupled modes and a spectral method is designed. The quasi-three-dimensional problem is transformed into a two-dimensional problem using an integral transformation strategy. A stepwise approximation is then used to discretize the range dependence of the two-dimensional problem; this approximation is essentially a physical discretization that further reduces the range-dependent two-dimensional problem to a one-dimensional problem. Finally, the Chebyshev--Tau spectral method is employed to accurately solve the one-dimensional problem. We provide the corresponding numerical program SPEC3D for the proposed algorithm and describe several representative numerical examples. In the numerical experiments, the consistency between SPEC3D and the analytical solution/high-precision finite difference program COACH verifies the reliability and capability of the proposed algorithm. Abstract

A high-precision numerical sound field is the basis of underwater target detection, positioning and communication. A line source in a plane is a common type of sound source in computational ocean acoustics. The exciting waveguide in a range-dependent ocean environment is often structurally complicated; however, traditional algorithms often assume that the waveguide has a simple seabed boundary and that the line source is located at a horizontal range of 0 m, although this ideal situation is rarely encountered in the actual ocean. In this paper, a novel algorithm is designed that can solve for the sound field excited by a line source at any position in a range-dependent ocean environment. The proposed algorithm uses the classic stepwise approximation approach to address the range dependence of the environment and uses the Chebyshev--Tau spectral method to solve for the horizontal wavenumbers and modes of approximately range-independent segments. Once the modal information of these flat segments has been obtained, a global matrix is constructed to solve for the coupling coefficients of all segments, and finally, the complete sound field is synthesized. Numerical experiments using a robust numerical program developed based on this algorithm verify the correctness and usability of our novel algorithm and software. Abstract

The stepwise coupled-mode model is a classic approach for solving range-dependent sound propagation problems. Existing coupled-mode programs have disadvantages such as high computational cost, weak adaptability to complex ocean environments and numerical instability. In this paper, a new algorithm is designed that uses an improved range normalization and global matrix approach to address range dependence in ocean environments. Due to its high accuracy in solving differential equations, the spectral method has recently been applied to range-independent normal modes and has achieved remarkable results. This algorithm uses the Chebyshev--Tau spectral method to solve for the eigenmodes in the range-independent segments. The main steps of the algorithm are parallelized, so OpenMP multithreading technology is also applied for further acceleration. Based on this algorithm, an efficient program is developed, and numerical simulations verify that this algorithm is reliable, accurate and capable. Compared with the existing coupled-mode programs, the newly developed program is more stable and efficient at comparable accuracies and can solve waveguides in more complex and realistic ocean environments., Comment: 49 pages, 12 figures
