1. Efficient WENO schemes for nonuniform grids

Abstract

A set of arbitrarily high-order WENO schemes for reconstructions on nonuniform grids is presented. These non-linear interpolation methods use simple smoothness indicators with a linear cost with respect to the order, making them easy to implement and computationally efficient. The theoretical analysis to verify the accuracy and the essentially non-oscillatory properties are presented together with some numerical experiments involving algebraic problems in order to validate them. Also, these general schemes are applied for the solution of conservation laws and hyperbolic systems in the context of finite volume methods.

Abstract

In this paper we introduce a new nonlinear reconstruction operator over two dimensional triangularized domains with equilateral triangles. We focus on the local definition of the operator. The ideas behind this definition come from some basic properties of the Harmonic mean of three positive values. We prove some results regarding the approximation properties of the operator and we carry out some numerical tests giving evidence of the avoidance of any Gibbs effects., Comment: 12 pages, 3 figures

14. Short-term exposure to pollen and the risk of allergic and asthmatic manifestations:a systematic review and meta-analysis

Abstract

Background: Several studies have assessed effects of short-term exposure to pollen on allergic and asthmatic manifestations. The evidence is inconclusive, and no meta-analysis has been published. Objective: To synthesise the evidence on the relations between short-term pollen exposure and the risk of allergic and asthmatic manifestations. Methods: We performed a systematic literature search of PubMed and Scopus databases up to the end of August 2018. In addition, we reviewed the reference lists of relevant articles. Two authors independently evaluated the eligible articles and extracted relevant information in a structured form. We calculated summary effect estimates (EE) based on the study-specific ORs and regression coefficients (β) by applying both fixed-effects and random-effects models. Results: 26 studies met the a priori eligibility criteria, and 12 of them provided sufficient information for the meta-analysis. The summary EE related to 10 grains per m³ increase in pollen exposure showed an 1% increase (EE 1.01, 95% CI 1.00 to 1.02) in the risk of lower respiratory symptoms and a 2% increase (EE 1.02, 95% CI 1.01 to 1.03) in the risk of any allergic or asthmatic symptom. Correspondingly, the risk of upper respiratory symptoms and ocular symptoms increased 7% (EE 1.07, 95% CI 1.04 to 1.09) and 11% (EE 1.11, 95% CI 1.05 to 1.17), respectively, in relation to such pollen exposure. Short-term exposure to pollen did not show any significant effect on daily lung function levels. Conclusion: Our results provide new evidence that short-term pollen exposure significantly increases the risks of allergic and asthmatic symptoms.

Abstract

BACKGROUND: The availability of high fidelity electronic health record (EHR) data is a hallmark of the learning health care system. Washington State's Surgical Care Outcomes and Assessment Program (SCOAP) is a network of hospitals participating in quality improvement (QI) registries wherein data are manually abstracted from EHRs. To create the Comparative Effectiveness Research and Translation Network (CERTAIN), we semi-automated SCOAP data abstraction using a centralized federated data model, created a central data repository (CDR), and assessed whether these data could be used as real world evidence for QI and research. OBJECTIVES: Describe the validation processes and complexities involved and lessons learned. METHODS: Investigators installed a commercial CDR to retrieve and store data from disparate EHRs. Manual and automated abstraction systems were conducted in parallel (10/2012-7/2013) and validated in three phases using the EHR as the gold standard: 1) ingestion, 2) standardization, and 3) concordance of automated versus manually abstracted cases. Information retrieval statistics were calculated. RESULTS: Four unaffiliated health systems provided data. Between 6 and 15 percent of data elements were abstracted: 51 to 86 percent from structured data; the remainder using natural language processing (NLP). In phase 1, data ingestion from 12 out of 20 feeds reached 95 percent accuracy. In phase 2, 55 percent of structured data elements performed with 96 to 100 percent accuracy; NLP with 89 to 91 percent accuracy. In phase 3, concordance ranged from 69 to 89 percent. Information retrieval statistics were consistently above 90 percent. CONCLUSIONS: Semi-automated data abstraction may be useful, although raw data collected as a byproduct of health care delivery is not immediately available for use as real world evidence. New approaches to gathering and analyzing extant data are required.

Abstract

Aims: Carotid intima media thickness (CIMT) predicts cardiovascular (CVD) events, but the predictive value of CIMT change is debated. We assessed the relation between CIMT change and events in individuals at high cardiovascular risk. Methods and results: From 31 cohorts with two CIMT scans (total n = 89070) on average 3.6 years apart and clinical follow-up, subcohorts were drawn: (A) individuals with at least 3 cardiovascular risk factors without previous CVD events, (B) individuals with carotid plaques without previous CVD events, and (C) individuals with previous CVD events. Cox regression models were fit to estimate the hazard ratio (HR) of the combined endpoint (myocardial infarction, stroke or vascular death) per standard deviation (SD) of CIMT change, adjusted for CVD risk factors. These HRs were pooled across studies. In groups A, B and C we observed 3483, 2845 and 1165 endpoint events, respectively. Average common CIMT was 0.79mm (SD 0.16mm), and annual common CIMT change was 0.01mm (SD 0.07mm), both in group A. The pooled HR per SD of annual common CIMT change (0.02 to 0.43mm) was 0.99 (95% confidence interval: 0.95–1.02) in group A, 0.98 (0.93–1.04) in group B, and 0.95 (0.89–1.04) in group C. The HR per SD of common CIMT (average of the first and the second CIMT scan, 0.09 to 0.75mm) was 1.15 (1.07–1.23) in group A, 1.13 (1.05–1.22) in group B, and 1.12 (1.05–1.20) in group C. Conclusions: We confirm that common CIMT is associated with future CVD events in individuals at high risk. CIMT change does not relate to future event risk in high-risk individuals.

Abstract

Background: For an individual participant data (IPD) meta-analysis, multiple datasets must be transformed in a consistent format, e.g. using uniform variable names. When large numbers of datasets have to be processed, this can be a time-consuming and error-prone task. Automated or semi-automated identification of variables can help to reduce the workload and improve the data quality. For semi-automation high sensitivity in the recognition of matching variables is particularly important, because it allows creating software which for a target variable presents a choice of source variables, from which a user can choose the matching one, with only low risk of having missed a correct source variable. Methods: For each variable in a set of target variables, a number of simple rules were manually created. With logic regression, an optimal Boolean combination of these rules was searched for every target variable, using a random subset of a large database of epidemiological and clinical cohort data (construction subset). In a second subset of this database (validation subset), this optimal combination rules were validated. Results: In the construction sample, 41 target variables were allocated on average with a positive predictive value (PPV) of 34%, and a negative predictive value (NPV) of 95%. In the validation sample, PPV was 33%, whereas NPV remained at 94%. In the construction sample, PPV was 50% or less in 63% of all variables, in the validation sample in 71% of all variables. Conclusions: We demonstrated that the application of logic regression in a complex data management task in large epidemiological IPD meta-analyses is feasible. However, the performance of the algorithm is poor, which may require backup strategies.

Abstract

Background The clinical use of carotid intima media thickness (cIMT) requires normal values, which may be subject to variation of geographical factors, ethnicity or measurement details. The influence of these factors has rarely been studied. The aim of this study was to determine whether normative cIMT values and their association with event risk are generalizable across populations. Design Meta-analysis of individual participant data. Method From 22 general population cohorts from Europe, North America and Asia we selected subjects free of cardiovascular disease. Percentiles of cIMT and cIMT progression were assessed separately for every cohort. Cox proportional hazards models for vascular events were used to estimate hazard ratios for cIMT in each cohort. The estimates were pooled across Europe, North America and Asia, with random effects meta-analysis. The influence of geography, ethnicity and ultrasound protocols on cIMT values and on the hazard ratios was examined by meta-regression. Results Geographical factors, ethnicity and the ultrasound protocol had influence neither on the percentiles of cIMT and its progression, nor on the hazard ratios of cIMT for vascular events. Heterogeneity for percentiles of cIMT and cIMT progression was too large to create meaningful normative values. Conclusions The distribution of cIMT values is too heterogeneous to define universal or regional population reference values. CIMT values vary widely between different studies regardless of ethnicity, geographic location and ultrasound protocol. Prediction of vascular events with cIMT values was more consistent across all cohorts, ethnicities and regions.

Abstract

© 2014 European Society of Cardiology. Background Large-scale epidemiological evidence on the role of inflammation in early atherosclerosis, assessed by carotid ultrasound, is lacking. We aimed to quantify cross-sectional and longitudinal associations of inflammatory markers with common-carotid-artery intima-media thickness (CCA-IMT) in the general population. Methods Information on high-sensitivity C-reactive protein, fibrinogen, leucocyte count and CCA-IMT was available in 20 prospective cohort studies of the PROG-IMT collaboration involving 49,097 participants free of pre-existing cardiovascular disease. Estimates of associations were calculated within each study and then combined using random-effects meta-analyses. Results Mean baseline CCA-IMT amounted to 0.74 mm (SD = 0.18) and mean CCA-IMT progression over a mean of 3.9 years to 0.011 mm/year (SD = 0.039). Cross-sectional analyses showed positive linear associations between inflammatory markers and baseline CCA-IMT. After adjustment for traditional cardiovascular risk factors, mean differences in baseline CCA-IMT per one-SD higher inflammatory marker were: 0.0082 mm for high-sensitivity C-reactive protein (p < 0.001); 0.0072 mm for fibrinogen (p < 0.001); and 0.0025 mm for leucocyte count (p = 0.033). 'Inflammatory load', defined as the number of elevated inflammatory markers (i.e. in upper two quintiles), showed a positive linear association with baseline CCA-IMT (p < 0.001). Longitudinal associations of baseline inflammatory markers and changes therein with CCA-IMT progression were null or at most weak. Participants with the highest 'inflammatory load' had a greater CCA-IMT progression (p = 0.015). Conclusion Inflammation was independently associated with CCA-IMT cross-sectionally. The lack of clear associations with CCA-IMT progression may be explained by imprecision in its assessment within a limited time period. OBJECTIVE Carotid intima-media thickness (CIMT) is a marker of subclinical organ damage and predicts cardiovascular disease (CVD) events in the general population. It has also been associated with vascular risk in people with diabetes. However, the association of CIMT change in repeated examinations with subsequent CVD events is uncertain, and its use as a surrogate end point in clinical trials is controversial. We aimed at determining the relation of CIMT change to CVD events in people with diabetes. RESEARCH DESIGN AND METHODS In a comprehensive meta-analysis of individual participant data, we collated data from 3,902 adults (age 33-92 years) with type 2 diabetes from 21 populationbased cohorts. We calculated the hazard ratio (HR) per standard deviation (SD) difference inmean common carotid artery intima-media thickness (CCA-IMT) or in CCA-IMT progression, both calculated from two examinations on average 3.6 years apart, for each cohort, and combined the estimates with random-effects meta-analysis. RESULTS Average mean CCA-IMT ranged from 0.72 to 0.97mmacross cohorts in peoplewith diabetes. The HR of CVD events was 1.22 (95% CI 1.12-1.33) per SD difference in mean CCA-IMT, after adjustment for age, sex, and cardiometabolic risk factors. Average mean CCA-IMT progression in people with diabetes ranged between 20.09 and 0.04 mm/year. The HR per SD difference in mean CCA-IMT progression was 0.99 (0.91-1.08). CONCLUSIONS Despite reproducing the association between CIMT level and vascular risk in subjects with diabetes, we did not find an association between CIMT change and vascular risk. These results do not support the use of CIMT progression as a surrogate end point in clinical trials in people with diabetes.

Abstract

The fluid-structure interaction (FSI) problem has received great attention in the last few years, mainly because it is present in many physical systems, industrial applications, and almost every biological system. In the parallel computational field, outstanding advances have been achieved for the individual components of the problem, allowing, for instance, simulations around complex geometries at very high Reynolds numbers or simulations of the contraction of a beating heart. However, it is not an easy task to combine the advances of both fields, given that they have followed development paths in a rather independent way, and also because physical and numerical instabilities arise when dealing with two highly nonlinear partial differential equations. Nonetheless, in the last few years great advances in the coupled FSI field have been achieved, recognizing the most challenging problems to tackle and enabling a new generation of numerical simulations in aerodynamics, biological systems, and complex industrial devices. Keeping in mind that efficient parallel codes for the individual components already exist, this paper presents a framework to build a massively parallel FSI solver in a multicode coupling partitioned approach, with strong focus in the parallel implementation aspects and the parallel performance of the resulting application. The problem is casted in an algebraic form, and the main points of interest are the parallel environment needed to be able to transfer data among the codes, the location of the exchange surface, and the exchange of information among the parallel applications. The proposed framework has been implemented in the HPC multiphysics code Alya, and the multicode coupling is carried out running separated instances of this code. Two coupling algorithms with different acceleration schemes are revised, and three representative cases of different areas of interest showing the reach of the proposed framework are solved. Good agreement with literature and experiments is obtained. In addition to the numerical validation of the FSI solver, an assessment of the parallel performance of the proposed multicode strategy is done. In particular, a special distribution of the fluid code and solid code MPI processes on the supercomputer nodes based on computing cores overloading is investigated. Finally, a strong scalability test, running up to a 30 million elements case using 1280 MPI processes, is done. [ABSTRACT FROM AUTHOR]

Abstract

The objective of this work was to assess how the cow pregnancy depends on the type of service as, race and follicular development in cattle with dual purpose in the Ecuadorian Amazon. The 341 records of the individual services were evaluated from January 2015 to December 2015 that corresponds to cows of different genotypes and cross: Brown Swiss (BS) n = 135, Charolais (Ch) n = 24, Holstein Friesian (HF) n = 104, Jersey (J) n = 12, Normando (N) n = 21, x Brown Swiss Holstein Friesian (HFxBS) n = 35 and Holstein Friesian x Normando (HFxN) n = 10. The work was conducted in the Cantons and Santa Clara which is located in the Pastaza Province (Ecuador). The Two insemination techniques were performed: a estrus detected (natural) 136 cows and a Insemination Time (TAI). The independent variables are "Race" (BS, CH, HF, J, HFxBS and HFxN), the "Technical Service" (N and IATF) and "Follicular Development" (DF). An additive model was adjusted and it was not significant for the interactions between independent variables and the result of the model. Abstract

The economic optimization of a distillation column involves the selection of the number of trays and the feed- and side-streams locations, as well as the operating conditions that minimize the total investment and operation cost. In this paper, we present a superstructure-based optimization algorithm that combines the capabilities of commercial process simulators&sbd;taking advantage of the tailored algorithms designed for distillation and property estimation implemented in these simulators&sbd;and generalized disjunctive programming (GDP). The proposed algorithm iterates between two types of subproblems:  A nonlinear programming (NLP) subproblem, in which the trays are divided into existing and nonexisting (nonexisting trays behave like simple bypasses without mass or heat exchange through the use of Murphree efficiencies), and a specially tailored master mixed integer linear programming (MILP) problem. The NLP subproblems are solved by integrating the process simulator with an external NLP solver. Several examples are presented which show promising results.

Abstract

The inclusion of S0 hydrolysis in a kinetic model of autotrophic denitrification has been recently proposed; however the model has not been calibrated or validated yet. Thus, a new methodology was developed and applied to calibrate and validate this kinetic model for the first time. An inoculum adapted from a poultry wastewater treatment plant at stoichiometric S0/NO3− ratio was used. The model was calibrated with batch data (initial nitrate concentrations of 50 and 6.25 mg NO3−-N/L) at an S0/N ratio = 2.29 mg S/mg N and validated with seven different batch data. The sensitivity analysis showed that the most sensitive parameters are related to S0 hydrolysis. The kinetic model was successfully calibrated with the new methodology and validated, with Theil inequality coefficient values lower than 0.21. Abstract

Presented at the XXXIV Mazurian Lakes Conference on Physics, Piaski, Poland, September 6–13, 2015, During the last years, the ECOS working group has been considering the construction of a new high-intensity accelerator of stable ion beams for the next Long-Range Plan of the nuclear physics community in Europe. The new facility (LINCE) will be a multi-user facility dedicated to ECOS science: fundamental physics, astrophysics, nuclear structure and reaction dynamics. In this paper, we summarize preliminary design studies of the low-energy part of this facility based on the use of a multi-ion supercon- ducting linac., Work partially supported by the Spanish Government under the grant Feder-Interconnecta “Aceltec” ITC-20111070. 47. Neurocognitive Dysfunction After Short (<20 Minutes) Duration Hypothermic Circulatory Arrest.

Abstract

Background: It has long been held that the safe duration of hypothermic circulatory arrest (HCA) is at least 25 to 30 minutes. However, this belief is based primarily on clinical outcomes research and has not been systematically investigated using more sensitive brain imaging and neurocognitive assessments., Methods: This exploratory substudy of the randomized Cognitive Effects of Body Temperature During Hypothermic Circulatory Arrest (GOT ICE) trial, which compared outcomes for deep vs moderate hypothermia during aortic arch surgery, investigated the frequency of neurocognitive and structural and functional magnetic resonance imaging (MRI) deficits with HCA of short (<20 minutes) duration. Neurocognitive deficit was defined as ≥1 SD decline in ≥1 of 5 cognitive domains on neurocognitive testing., Results: Of 228 GOT ICE participants with complete 4-week cognitive data, 74.6% (n = 170 of 228) had HCA durations <20 minutes, including 59 patients randomized to deep hypothermia (<20.0 °C), 55 patients randomized to low-moderate (20.1-24.0 °C) hypothermia, and 56 randomized to high-moderate (24.1-28.0 °C) hypothermia. Of these participants, cognitive deficit was detected 4 weeks postoperatively in ∼40% of patients in all 3 groups (deep hypothermia, 22 of 59 [37.3%]; low-moderate hypothermia, 23 of 55 [41.8%]; and high-moderate hypothermia, 24 of 56 [42.9%]). Furthermore, in a subset of patients with complete MRI data (n = 43), baseline to 4-week postoperative right frontal lobe functional connectivity change was inversely associated with HCA duration (range, 8-17 minutes; P for familywise error rate < .01)., Conclusions: Even short durations of HCA result in cognitive deficits in ∼40% of patients, independent of systemic hypothermia temperature. HCA duration was inversely associated with frontal lobe functional MRI connectivity, a finding suggesting that this brain region may be preferentially sensitive to HCA. 48. A comparison of steroid dose with or without local anesthetic in lumbar interlaminar epidural steroid injections.

Abstract

Introduction: Epidural steroid injections (ESIs) are commonly used as a treatment for lumbar radiculopathy. Currently, most research on comparative efficacy of various steroids in epidural steroid injections is focused on transforaminal ESIs (TFESIs). Through this study, we aimed to compare various steroid doses with or without local anesthetic in interlaminar ESIs (ILESIs)., Methods: We reviewed charts for all adult patients who received ILESIs identified by CPT code 62323 between January 2017 to April 2021. Baseline demographic data including age, sex, BMI, and smoking status were recorded. NRS pain scores before the injection and percentage of pain relief at 1-month follow-up were recorded. We compared percentage of patients reporting pain relief at 1 month follow-up of low-dose dexamethasone alone (5 mg), to low-dose dexamethasone mixed with local anesthetic, and to high-dose dexamethasone (10 mg) mixed with local anesthetic, specifically for ILESIs., Results: Data were available for 311 patients. There was no significant difference in pain relief between the 3 groups at 1 month follow-up. The majority of patients had moderate to significant improvement in pain, supporting the use of ILESIs. Moreover, low-dose steroid with local anesthetic was found to be as efficacious as high-dose steroid alone. Although not statistically significant, the addition of local anesthetic to low-dose or high-dose steroid increased the percentage of patients reporting moderate to significant pain relief., Conclusion: ILESIs with non-particulate steroids provide moderate to significant pain improvement in the short term, with low-dose steroid mixed with local anesthetic being as efficacious as a high-dose steroid., (© 2024 World Institute of Pain.)

49. Relationship of pain relief with catastrophizing following interventional pain procedures for low back pain.

Abstract

Introduction: Catastrophizing is associated with worse pain outcomes after various procedures suggesting its utility in predicting response. However, the stability of pain catastrophizing as a static predictor has been challenged. We assess, among patients undergoing steroid injections for chronic low back pain (cLBP), whether catastrophizing changes with the clinical response to pain interventions., Methods: This prospective study enrolled patients undergoing fluoroscopic-guided injections for cLBP. Patients filled out Brief Pain Inventory (BPI) and Pain Catastrophizing Scale (PCS) at baseline and 1-month follow-up. We assessed the change in PCS scores from pre-injection to post-injection and examined its predictors. We also examined the correlation of various domains of BPI, such as pain severity and effect on Relationships, Enjoyment, and Mood (REM), with PCS scores at baseline and follow-up., Results: 128 patients were enrolled. Mean (SD) PCS and pain severity scores at baseline were 22.38 (±13.58) and 5.56 (±1.82), respectively. Follow-up PCS and pain severity scores were 19.76 (±15.25) and 4.42 (±2.38), respectively. The change in PCS pre-injection to post-injection was not significant (p=0.12). Multiple regression models revealed baseline PCS and REM domain of BPI as the most important predictors of change in PCS after injection. Pain severity, activity-related pain, age, sex, insurance status, depression, prior surgery, opioid use, or prior interventions did not predict change in PCS score. In correlation analysis, change in PCS was moderately correlated with change in pain (r=0.38), but weakly correlated with baseline pain in all pain domains., Conclusions: PCS showed non-significant improvement following steroid injections; the study was not powered for this outcome. Follow-up PCS scores were predicted by the REM domain of BPI, rather than pain severity. 50. Optical Flow-Based Obstacle Detection for Mid-Air Collision Avoidance.

Abstract

The sky may seem big enough for two flying vehicles to collide, but the facts show that mid-air collisions still occur occasionally and are a significant concern. Pilots learn manual tactics to avoid collisions, such as see-and-avoid, but these rules have limitations. Automated solutions have reduced collisions, but these technologies are not mandatory in all countries or airspaces, and they are expensive. These problems have prompted researchers to continue the search for low-cost solutions. One attractive solution is to use computer vision to detect obstacles in the air due to its reduced cost and weight. A well-trained deep learning solution is appealing because object detection is fast in most cases, but it relies entirely on the training data set. The algorithm chosen for this study is optical flow. The optical flow vectors can help us to separate the motion caused by camera motion from the motion caused by incoming objects without relying on training data. This paper describes the development of an optical flow-based airborne obstacle detection algorithm to avoid mid-air collisions. The approach uses the visual information from a monocular camera and detects the obstacles using morphological filters, optical flow, focus of expansion, and a data clustering algorithm. The proposal was evaluated using realistic vision data obtained with a self-developed simulator. The simulator provides different environments, trajectories, and altitudes of flying objects. The results showed that the optical flow-based algorithm detected all incoming obstacles along their trajectories in the experiments. The results showed an F-score greater than 75% and a good balance between precision and recall.
