Abstract

International audience; BACKGROUND:Rituximab plus chemotherapy has been shown to be effective in patients with advanced-stage, previously untreated follicular lymphoma; nevertheless, most patients will have a relapse. Combination immunotherapy with lenalidomide and rituximab is an immunomodulatory regimen that has shown promising activity in patients with indolent B-cell non-Hodgkin's lymphoma.METHODS:We conducted this multicenter, international, phase 3 superiority trial to evaluate rituximab plus lenalidomide, as compared with rituximab plus chemotherapy, in patients with previously untreated follicular lymphoma. Patients were randomly assigned to receive one of the two regimens, followed by maintenance monotherapy with rituximab. Treatment with rituximab plus lenalidomide consisted of 18 cycles of the two drugs, followed by rituximab maintenance therapy every 8 weeks for 12 cycles (six additional doses). Treatment with rituximab plus chemotherapy consisted of the investigator's choice of one of three rituximab-based regimens, followed by maintenance monotherapy with rituximab every 8 weeks for 12 cycles. The primary end points were complete response (confirmed or unconfirmed) at 120 weeks and progression-free survival.RESULTS:A total of 1030 patients were randomly assigned to receive rituximab plus lenalidomide (513 patients) or rituximab plus chemotherapy (517 patients). The rate of confirmed or unconfirmed complete response at 120 weeks was similar in the two groups: 48% (95% confidence interval [CI], 44 to 53) in the rituximab-lenalidomide group and 53% (95% CI, 49 to 57) in the rituximab-chemotherapy group (P=0.13). The interim 3-year rate of progression-free survival was 77% (95% CI, 72 to 80) and 78% (95% CI, 74 to 82), respectively. A higher percentage of patients in the rituximab-chemotherapy group had grade 3 or 4 neutropenia (32% vs. 50%) and febrile neutropenia of any grade (2% vs. 7%), and a higher percentage of patients in the rituximab-lenalidomide group had grade 3 or 4 cutaneous reactions (7% vs. 1%).CONCLUSIONS:Among patients with previously untreated follicular lymphoma, efficacy results were similar with rituximab plus lenalidomide and rituximab plus chemotherapy (with both regimens followed by rituximab maintenance therapy). Abstract

Remote assistive technologies are one of the most promising solutions for an aging society in the future. This paper describes a design of a remote assistive system to guide elderly to find and recognize hidden objects in a kitchen through ubiquitous technologies utilizing sensing and light projection. These intelligent technologies can play a vital role in taking care of the elderly with cognitive impairments when the caregiver’s lives or work far away. The main goal of this research is to provide visual guidance to elderly to overcoming the deficits of initiation, planning, attention and memory deficits while performing kitchen-based activities of daily living such as locating and identifying items for cooking. In a standard kitchen where objects can be placed in open and closed spaces, it is difficult for elderly with cognitive impairment to find and locate objects that are invisible and sometimes hidden behind other objects. In this situation the RFID technology can directly provide the location of the items and projection technology can display the image of the object at the exact location. An initial prototype has been developed and a user study with twelve elderly people has been conducted. The initial results show that the visual guidance makes the task of finding and identifying objects easier and simpler. Additionally, results show promise for further development and system can be used for other kitchen activities.

Abstract

We present an eyeglass-type wearable device that has light emitting diode (LED) indicators on the frame of it. The device produces lighting patterns of 14 RGB LEDs near user’s eyes as guiding information. Since installed LEDs on the frame of glasses are light and saving power, it is feasible to develop it for daily use. On the other hand, it cannot provide rich information such as text or images. In this study, we aim to realize a remote assistive system that provides assistive commands by lighting patterns of the eyeglass-type device from remote sites. Especially, we consider elderlies who are suffering from mild cognitive impairment as users. They would be one of potential user groups since the device does not block their sights by text or images and it can be worn in daily life without the additional sense of restraint. This paper explains our conceptual assistive system structure, a prototype eyeglass-type device with near-eye LED indicators and usability experimentation in simple navigational tasks.

Abstract

We propose a grid-pattern indicating interface to provide instructions remotely from remote site to support independent daily life of senior citizens. Our aim is to realize smooth and easy telecommunication between supported senior citizens at local site and supporting caregivers who are in remote site. Although we have used a monitoring method with video streaming where the remote caregivers indicate work steps as a conventional way, occlusion and depth perception problem was occurred. Our method that provides grid-pattern interface to remote caregivers could be a solution for the problems by indicating the spatial instruction easily on 2D input interface. Abstract

Systematic atomic simulations based on molecular mechanics were conducted to investigate the pull-out behavior of a capped carbon nanotube (CNT) in CNT-reinforced nanocomposites. Two common cases were studied: the pull-out of a complete CNT from a polymer matrix in a CNT/polymer nanocomposite and the pull-out of the broken outer walls of a CNT from the intact inner walls (i.e., the sword-in-sheath mode) in a CNT/alumina nanocomposite. By analyzing the obtained relationship between the energy increment (i.e., the difference in the potential energy between two consecutive pull-out steps) and the pull-out displacement, a set of simple empirical formulas based on the nanotube diameter was developed to predict the corresponding pull-out force. The predictions from these formulas are quite consistent with the experimental results. Moreover, the much higher pull-out force for a capped CNT than that of the corresponding open-ended CNT implies a significant contribution from the CNT cap to the interfacial properties of the CNT-reinforced nanocomposites. Abstract

Richieri‐Costa‐Pereira syndrome is a rare autosomal recessive acrofacial dysostosis that has been mainly described in Brazilian individuals. The cardinal features include Robin sequence, cleft mandible, laryngeal anomalies and limb defects. A biallelic expansion of a complex repeated motif in the 5′ untranslated region of EIF4A3 has been shown to cause this syndrome, commonly with 15 or 16 repeats. The only patient with mild clinical findings harbored a 14‐repeat expansion in 1 allele and a point mutation in the other allele. This proband is described here in more details, as well as is his affected sister, and 5 new individuals with Richieri‐Costa‐Pereira syndrome, including a patient from England, of African ancestry. This study has expanded the phenotype in this syndrome by the observation of microcephaly, better characterization of skeletal abnormalities, less severe phenotype with only mild facial dysmorphisms and limb anomalies, as well as the absence of cleft mandible, which is a hallmark of the syndrome. Although the most frequent mutation in this study was the recurrent 16‐repeat expansion in EIF4A3, there was an overrepresentation of the 14‐repeat expansion, with mild phenotypic expression, thus suggesting that the number of these motifs could play a role in phenotypic delineation.
