Aims/Introduction: This study aimed to evaluate the problems in screening for gestational diabetes mellitus (GDM) by casual blood glucose (CBG) measurements at 24–28 gestational weeks. Materials and Methods: Overall, 763 pregnant women who underwent the 50‐g glucose challenge test (GCT) at 24–28 gestational weeks were enrolled. The preload blood glucose (0‐h BG) level of 50‐g GCT was considered as CBG. Results: A total of 240 women with BG levels at 1‐h after loading (1‐h BG) on 50‐g GCT ≥140 mg/dL underwent the 75‐g oral glucose tolerance test, and 98 (40.8%) were diagnosed with GDM. Of the 99 women with GDM, 71 (71.7%) had 0‐h BG on 50‐g GCT <100 mg/dL. Conclusions: This study, where pregnant women underwent both CBG and 50‐g GCT simultaneously, showed that when CBG at 24–28 gestational weeks ≥100 mg/dL alone was used for screening GDM, many pregnant women with GDM were overlooked. - Abstract
Reproductive interference (RI), an interspecific mating interaction that reduces the fitness of at least one of the species involved, can lead to exclusive distributions in closely related species. A hypothesis previously proposed is that RI in plants may occur by ovule usurpation, in which pistils lack interspecific incompatibility and mistakenly accept heterospecific pollen, thereby losing an opportunity for conspecific pollen fertilization. However, few comparative studies have evaluated the consistency of the inferred mechanism within and among individuals and populations. We conducted hand-pollination experiments in six populations of three native Taraxacum species that suffered from different levels of RI from an alien congener, T. officinale, and compared pollen–pistil interactions among populations. We also investigated the interactions for eight individual T. japonicum plants whose response to heterospecific pollen deposition had been previously measured. Our results revealed that pollen tubes often penetrated native ovaries following heterospecific pollination in populations suffering from strong RI, whereas they seldom did in populations suffering from marginal RI. However, the relative frequency of the pollen tube penetration was not significantly related to the strength of alien RI. Not all pistils on an individual plant showed the same pollen receptivity following heterospecific pollination; rather, some accepted and some refused the pollen tubes. The relationship between pollen tube penetration following heterospecific pollination and the strength of the alien RI was also not significant among individuals. Our present results generally support the ovule usurpation hypothesis, but suggest that other factors, such as competition for pollinator services, variation in the effects of heterospecific pollen donors, and condition of the native inflorescences, might also affect the observed RI strength.

Among continuous glucose monitoring (CGM) devices, which continuously measure glucose concentration in subcutaneous interstitial fluid for comprehensive monitoring of blood glucose profile, only FreeStyle Libre Pro(®) (Abbott Diabetes Care) is currently available in Japan as a professional system. FreeStyle Libre Pro(®) is easy to use because it does not require calibration by self-monitoring of blood glucose (SMBG), but information on its accuracy has been insufficient. To evaluate the measurement accuracy of FreeStyle Libre Pro(®), we have now compared blood glucose levels determined by this device with those measured by SMBG in 40 individuals with type 2 diabetes mellitus. The mean absolute relative difference (MARD) for FreeStyle Libre Pro(®) measurements compared with SMBG measurements was calculated as an index of CGM accuracy. Overall blood glucose values measured by SMBG were 167.0 ± 60.1 mg/dL, and those determined by FreeStyle Libre Pro(®) were 155.0 ± 60.7 mg/dL, with this difference being statistically significant. The MARD for FreeStyle Libre Pro(®) relative to SMBG was 12.7 ± 9.3%. It was substantially higher in 2 of the 40 patients, at 49.2% and 47.5%, than in the other 38 individuals. 22. Toxicity of House Cricket (Acheta domesticus) in Mice

- Abstract
There is an urgent need to address the shortage of animal protein due to food shortages caused by the global population growth. Crickets contain an abundance of proteins in their exoskeleton and muscles and have attracted attention as a new protein source; however, their safety as a food source has not been confirmed. We evaluated the toxicity of the House cricket (Acheta domesticus), on cells and mammals. In genotoxicity in vitro, cricket powder was added to Chinese hamster lung CHL-IU cells at concentrations of 5,000 µg/mL, and the rate of chromosomal aberrations was assessed. In genotoxicity in vivo, mice were orally administered up to 2,000 mg/kg of cricket powder for 2 days. In both tests, cricket powder did not show any toxic effect. A repeated oral toxicity study was performed administering up to 3,000 mg/kg of cricket powder or control (saline) for 14 or 90 consecutive days and measuring body weight changes, blood biochemistry, blood properties, and organ weights. In each time course, there were no differences in there parameters between the control and cricket powder treated groups. These results suggest that House crickets (≤3,000 mg/kg) are not toxic to cells and organisms.

Introduction: We herein examined changes in expression levels of the glucocorticoid receptor subtypes GRα and GRβ in very low birth weight (VLBW) and term infants to clarify time-dependent changes in glucocorticoid sensitivity after birth. Methods: Whole blood samples were collected at birth and on postnatal days 4–7, and the mRNA expression levels of GRα and GRβ were measured using RT-qPCR. The relative gene expression levels of GRα and GRβ as the target genes normalized to actin beta as the endogenous control were calculated by the comparative cycle threshold method. Results: The GRα/GRβ expression ratio at birth was significantly lower in 32 VLBW cesarean section (CS) infants than in term planned CS infants (median [IQR], 1.5 [1.1–1.8]- and 1.1 [0.7–1.6]-fold change, p < 0.05). Furthermore, the GRα/GRβ expression ratio increased from day 0 to days 4–7 (1.0 [0.6–1.4]- and 1.7 [0.6–1.4]-fold change, p < 0.01) in 43 VLBW infants. Discussion/Conclusion: The present results suggest that glucocorticoid sensitivity in VLBW infants increases after birth and this rapid change may play a role in surviving critical postnatal events. - Abstract
Objective: We examined whether elevated blood pressure (BP) (≥120/80 mmHg) was associated with several anthropometric, metabolic, and clinical variables, including the family history of type 2 diabetes (FHD) and low birth weight, in young normal weight Japanese women. Methods: BP, body composition, and fasting glucose, insulin, lipids, lipoproteins, apolipoproteins, and adipokines were measured in 332 young Japanese women. They received a questionnaire on birth weight and FHD. Results: The prevalence of low birth weight was 2.4% and that of positive FHD was 22.9%. Homeostasis model assessment-insulin resistance averaged <1.5 and did not differ cross-sectionally between 32 women with elevated BP and 300 women with normal BP although mean body mass index was higher in the former than in the latter (21.7 ± 2.9 kg/m
2 vs. 20.8 ± 2.2 kg/m2 , P = 0.02). Women with elevated BP had higher fat mass index (P = 0.02) and trunk fat percentage (P = 0.04). They had lower high-density lipoprotein (HDL) cholesterol and apolipoprotein A1 (both P = 0.01) while fasting triglycerides and apolipoprotein B did not differ. In addition, they had higher plasminogen activator inhibitor-1 (PAI-1) (P = 0.001). Furthermore, the prevalence of low birth weight (9.4% vs. 1.7%, P = 0.03) and positive FHD (40.6% vs. 20.0%, P = 0.01) was higher in women with elevated BP. Multivariable logistic regression analyses revealed that elevated BP was independently associated with PAI-1 [odds ratio (OR); 1.05, 95% confidence interval (CI): 1.02–1.08, P = 0.001], low birth weight (OR: 12.3, 95% CI: 2.3–67.3, P = 0.04), and FHD (OR: 3.0, 95% CI: 1.3–7.9, P = 0.01). Conclusion: Elevated BP was associated with positive FHD, low birth weight, and elevated serum PAI-1 in young normal weight Japanese women. [ABSTRACT FROM AUTHOR]- Published

Background The effects of empagliflozin in patients with chronic kidney disease who are at risk for disease progression are not well understood. The EMPA-KIDNEY trial was designed to assess the effects of treatment with empagliflozin in a broad range of such patients. Methods We enrolled patients with chronic kidney disease who had an estimated glomerular filtration rate (eGFR) of at least 20 but less than 45 ml per minute per 1.73 m2 of body-surface area, or who had an eGFR of at least 45 but less than 90 ml per minute per 1.73 m2 with a urinary albumin-to-creatinine ratio (with albumin measured in milligrams and creatinine measured in grams) of at least 200. Patients were randomly assigned to receive empagliflozin (10 mg once daily) or matching placebo. The primary outcome was a composite of progression of kidney disease (defined as end-stage kidney disease, a sustained decrease in eGFR to Results A total of 6609 patients underwent randomization. During a median of 2.0 years of follow-up, progression of kidney disease or death from cardiovascular causes occurred in 432 of 3304 patients (13.1%) in the empagliflozin group and in 558 of 3305 patients (16.9%) in the placebo group (hazard ratio, 0.72; 95% confidence interval [CI], 0.64 to 0.82; PConclusions Among a wide range of patients with chronic kidney disease who were at risk for disease progression, empagliflozin therapy led to a lower risk of progression of kidney disease or death from cardiovascular causes than placebo. 46. Foster parent support and needs of public health nurses, pediatric nurses, and childcare workers: A survey of three municipalities with high foster parent placement rates in Japan.

- Abstract
Objective This study aimed to elucidate foster parent support afforded by and the related needs of public health nurses, pediatric nurses, and childcare workers in Japan.Methods We conducted a quantitative study involving 95 public health nurses, 450 pediatric nurses, and 360 childcare workers in three municipalities with foster parent placement rates higher than the national average in Japan. The participants were questioned regarding their experiences in supporting foster parents at work; participation in foster parent support training; awareness and perceptions of foster parents; knowledge about foster parents, foster children, and the foster care system; and confidence in supporting foster parents. Descriptive statistics were used to analyze quantitative data. Fisher's exact test was used for statistical analysis.Results Questionnaires were collected from 179 participants (recovery rate: 19.8%). The breakdown of respondents was as follows: 19 public health nurses (10.6%), 77 pediatric nurses (43.0%), and 83 childcare workers (46.4%). Twenty-four participants (13.4%) replied that they had participated in foster parent support training, and 46 (25.7%) replied that they had experience supporting foster parents at work. Only 20.8% of the respondents answered that they were confident in supporting foster parents. There were differences in the experience of supporting foster parents at work and participation in foster parent support training depending on the participants' job type. The proportion of public health nurses was high in both categories. Experience supporting foster parents at work and participation in foster parent support training were significantly related to confidence.Conclusion We found that the limited opportunities for public health nurses, pediatric nurses, and childcare workers to engage in supporting foster parents and participate in foster parent support training contribute to their low confidence levels in these areas. Our findings underscore the necessity of disseminating knowledge about foster care systems and fostering an understanding of foster families among pediatric nurses and childcare workers. Integrating training related to social care and foster care systems into basic nursing education is necessary. Moreover, actively listening to the perspectives and experiences of foster parents can provide valuable insights for professionals in these fields.

47. Prediction of insulin requirements by explainable machine learning for individuals with type 1 diabetes.

- Abstract
Objective: Adverse events related to insulin therapy remain common in individuals with type 1 diabetes. Optimization of insulin dose can reduce the frequency of these events and help to prevent macrovascular and microvascular complications. The aim of the present study was to develop machine learning models to predict the total daily dose (TDD) of insulin on the basis of data available in routine clinical practice, to evaluate the performance of the models, and to interpret the relation between its predictions and features., Materials and Methods: This retrospective observational study conducted at a single center recruited individuals diagnosed with type 1 diabetes who visited Kobe University Hospital and used continuous glucose monitoring in combination with continuous subcutaneous insulin infusion between 1 April 2010 and 29 February 2024. We developed TDD prediction models based on machine learning and evaluated its performance on the basis of the mean absolute percentage error (MAPE). Explainable artificial intelligence frameworks were applied to the machine learning model to facilitate interpretability., Results: A total of 110 individuals with type 1 diabetes was included in the study. The best-performing model, the Random Forest, achieved a MAPE of 19.8%. The most important feature of the model for prediction of the TDD of insulin was body weight, followed by waist circumference and carbohydrate intake., Conclusions: We here developed machine learning models that predict the TDD of insulin from clinical information. - Abstract
Among continuous glucose monitoring (CGM) devices, which continuously measure glucose concentration in subcutaneous interstitial fluid for comprehensive monitoring of blood glucose profile, only FreeStyle Libre Pro® (Abbott Diabetes Care) is currently available in Japan as a professional system. FreeStyle Libre Pro® is easy to use because it does not require calibration by self-monitoring of blood glucose (SMBG), but information on its accuracy has been insufficient. To evaluate the measurement accuracy of FreeStyle Libre Pro®, we have now compared blood glucose levels determined by this device with those measured by SMBG in 40 individuals with type 2 diabetes mellitus. The mean absolute relative difference (MARD) for FreeStyle Libre Pro® measurements compared with SMBG measurements was calculated as an index of CGM accuracy. Overall blood glucose values measured by SMBG were 167.0 ± 60.1 mg/dL, and those determined by FreeStyle Libre Pro® were 155.0 ± 60.7 mg/dL, with this difference being statistically significant. The MARD for FreeStyle Libre Pro® relative to SMBG was 12.7 ± 9.3%. It was substantially higher in 2 of the 40 patients, at 49.2% and 47.5%, than in the other 38 individuals. MARD values did not differ significantly between before and 2 h after meals. However, the MARD was significantly higher for SMBG values of <100 mg/dL than for those of ≥250 mg/dL. Our results thus indicate that the measurement accuracy of FreeStyle Libre Pro® is relatively good, but that some cases in which values determined by the device deviate from SMBG values require caution in interpretation.

- Abstract
The specification of anterior neuroectoderm is controlled by a highly conserved molecular mechanism in bilaterians. A forkhead family gene, foxQ2, is known to be one of the pivotal regulators, which is zygotically expressed in this region during embryogenesis of a broad range of bilaterians. However, what controls the expression of this essential factor has remained unclear to date. To reveal the regulatory mechanism of foxQ2, we performed cis-regulatory analysis of two foxQ2 genes, foxQ2a and foxQ2b, in a sea urchin Hemicentrotus pulcherrimus. In sea urchin embryos, foxQ2 is initially expressed in the entire animal hemisphere and subsequently shows narrower expression restricted to the anterior pole region. In this study, as a first step to understand the foxQ2 regulation, we focused on the later restricted expression and analyzed the upstream cis-regulatory sequences of foxQ2a and foxQ2b by using the constructs fused to short half-life green fluorescent protein. Based on deletion and mutation analyses of both foxQ2, we identified each of the five regulatory sequences, which were 4-9 bp long. Neither of the regulatory sequences contains any motifs for ectopic activation or spatial repression, suggesting that later mRNA localization is regulated in situ. We also suggest that the three amino acid loop extension-class homeobox gene Meis is involved in the maintenance of foxQ2b, the expression of which is dominant during embryogenesis., (© 2019 Wiley Periodicals, Inc.)
