The safety and tolerability of benralizumab over 1 year were assessed by the incidence of adverse events (AEs), serious AEs, adverse drug reactions (ADRs), and serious ADRs. Patient background characteristics indicating a more frequent onset of ADRs with benralizumab were explored. The main effectiveness assessment was the change in Asthma Control Questionnaire-5 (ACQ-5) score from baseline. Patients with baseline ACQ-5 scores ≥ 1.5 were defined as having severe uncontrolled asthma.Results: In total, 632 patients were evaluated for safety and 274 for effectiveness; 139 patients were included in the severe uncontrolled asthma subgroup. ADRs were reported in 12.7% and serious AEs in 13.0% of patients. Serious infections occurred in 3.8%, serious hypersensitivity in 0.3%, and malignancy in 0.3% of patients. No helminthic infections occurred. In the effectiveness population, benralizumab improved the mean (standard deviation [95% confidence interval]) ACQ-5 score by − 1.16 (1.40 [− 1.36, − 0.96]) from baseline; forced expiratory volume in 1 second by 0.151 (0.440 [0.09, 0.21]) L; and Mini-Asthma Quality of Life questionnaire score by 1.16 (1.29 [0.94, 1.38]) at the last observation. The annual asthma exacerbation rate was 0.42. A greater ACQ-5 score improvement was observed among patients with eosinophilic asthma characteristics.Conclusion: No new safety concerns were raised, and patients experienced benefits consistent with previous studies of benralizumab, thus supporting the use of benralizumab for the add-on maintenance treatment of patients with eosinophilic severe uncontrolled asthma.Keywords: benralizumab, severe uncontrolled asthma, anti-interleukin-5 receptor α monoclonal antibody, eosinophils, exacerbation, asthma control, quality of life

Abstract

An experimental program is currently underway at the National Ignition Facility (NIF) to compress deuterium and tritium (DT) fuel to densities and temperatures sufficient to achieve fusion and energy gain. The primary approach being investigated is indirect drive inertial confinement fusion (ICF), where a high-Z radiation cavity (a hohlraum) is heated by lasers, converting the incident energy into x-ray radiation which in turn drives the DT fuel filled capsule causing it to implode. Previous experiments reported DT fuel gain exceeding unity [O.A. Hurricane et al., Nature 506, 343 (2014)] and then exceeding the kinetic energy of the imploding fuel [S. Le Pape et al., Phys. Rev. Lett. 120, 245003 (2018)]. We report on recent experiments that have achieved record fusion neutron yields on NIF, greater than 100 kJ with momentary fusion powers exceeding 1PW, and have for the first time entered the burning plasma regime where fusion alpha-heating of the fuel exceeds the energy delivered to the fuel via compression. This was accomplished by increasing the size of the high-density carbon (HDC) capsule, increasing energy coupling, while controlling symmetry and implosion design parameters. Two tactics were successful in controlling the radiation flux symmetry and therefore the implosion symmetry: transferring energy between laser cones via plasma waves, and changing the shape of the hohlraum. In conducting these experiments, we controlled for known sources of degradation. Herein we show how these experiments were performed to produce record performance, and demonstrate the data fidelity leading us to conclude that these shots have entered the burning plasma regime.

To elucidate the progression of COPD, we performed a cluster analysis using the following DPM parameters: normal (DPMNormal), gas-trapping (DPMGasTrap), and emphysematous lesions (DPMEmph). Our findings revealed the characteristics of each cluster and the 3-year disease progression using imaging parameters.Patients and Methods: Inspiratory and expiratory chest computed tomography (CT) images of 131 patients with COPD were examined, of which 84 were followed up for 3 years. The percentage of low attenuation volume (LAV%) and the square root of the wall area of a hypothetical airway with an internal perimeter of 10 mm (√Aaw at Pi10) were quantitatively measured using inspiratory chest CT. A hierarchical cluster analysis was performed using the DPM parameters at baseline. Five clusters were named according to the dominant DPM parameters: normal (NL), normal-GasTrap (NL-GT), GasTrap (GT), GasTrap-Emphysema (GT-EM), and Emphysema (EM).Results: Women were predominantly diagnosed with GT. Forced expiratory volume in 1 s gradually decreased in the following order: NL, NL-GT, GT, GT-EM, and EM. DPMEmph correlated well with LAV%. Four clusters other than NL showed significantly higher values of √Aaw at Pi10 than NL; however, no significant differences were observed among them. In all clusters, DPMEmph increased after 3 years. DPMNormal only increased in the GT cluster.Conclusion: Clusters using DPM parameters may reflect the characteristics of COPD and help understand the pathophysiology of the disease.Keywords: cluster analysis, computed tomography, COPD, DPM, gas-trapping, emphysema

Some patients are unable to complete cCRT and cannot receive immunotherapy due to poor performance status based on adverse events after cCRT. Immunotherapy plays an important role in anti-programmed cell death ligand-1 (PD-L1)-positive advanced NSCLC and is replacing chemotherapy. In addition, radiotherapy and immunotherapy have been reported to have a synergistic effect. This Phase II, multicenter study (DOLPHIN, WJOG11619L, JapicCTI-194840) is designed to assess the efficacy and safety of durvalumab plus concurrent curative radiation therapy for PD-L1-positive unresectable LA-NSCLC without chemotherapy. Unresectable LA stage III NSCLC patients aged 20 years or older with a World Health Organization/Eastern Cooperative Oncology Group (ECOG) performance status (PS) of 0 or 1 and PD-L1 positivity are enrolled. The patients will receive curative radiation therapy (60 Gy) plus durvalumab 10 mg/kg every 2 weeks (q2w) for up to 12 months until there is evidence of disease progression (PD) or unacceptable toxicity. The primary endpoint is the 12-month progression-free survival rate as assessed by an independent central review. The secondary endpoints are progression-free survival, overall survival, objective response rate, treatment completion rate, and safety. Abstract

We analyse the performance of a gas time projection chamber (TPC) as a high-performance gamma-ray telescope and polarimeter in the e$^+$e$^-$ pair creation regime. We use data collected at a gamma-ray beam of known polarisation. The TPC provides two orthogonal projections $(x,z)$ and $(y,z)$ of the tracks induced by each conversion in the gas volume. We use a simple vertex finder in which vertices and pseudo-tracks exiting from them are identified. We study the various contributions to the single-photon angular resolution using Monte Carlo simulations and compare them with the experimental data and find that they are in excellent agreement. The distribution of the azimutal angle of pair conversions shows a bias due to the non-cylindrical-symmetric structure of the detector. This bias would average out for a long duration exposure on a space mission, but for this pencil-beam characterisation we have ensured its accurate simulation by a double systematics control scheme, data taking with the detector rotated at several angles with respect to the beam polarisation direction and systematics control with a non-polarised beam. We measure, for the first time, the polarisation asymmetry of a linearly polarised gamma-ray beam in the low energy pair creation regime. This sub-GeV energy range is critical for cosmic sources as their spectra are power laws which fall quickly as a function of increasing energy. Abstract

Group V phospholipase A2 (Pla2g5) is a lipid-generating enzyme necessary for macrophage effector functions in pulmonary inflammation. However, the lipid mediators involved and their cellular targets have not been identified. Mice lacking Pla2g5 showed markedly reduced lung ILC2 activation and eosinophilia following repetitive Alternaria Alternata inhalation. While Pla2g5-null mice had Wt levels of immediate IL-33 release after one Alternaria dose, they failed to upregulate IL-33 in macrophages following repeated Alternaria administration. Unexpectedly, while adoptive transfer of bone marrow-derived (BM)-macrophages restored ILC2 activation and eosinophilia in Alternaria-exposed Pla2g5-null mice, exogenous IL-33 did not. Conversely, transfers of Pla2g5-null BM-macrophages reduced inflammation in Alternaria-exposed Wt mice. Mass spectrometry analysis of free fatty acids (FFAs) demonstrated significantly reduced FFAs (including linoleic acid (LA) and oleic acid (OA)) in lung and BM-macrophages lacking Pla2g5. Exogenous administration of LA or LA+OA to Wt mice sharply potentiated IL-33-induced lung eosinophilia and ILC2 expansion in vitro and in vivo. In contrast, OA potentiated IL-33-induced inflammation and ILC2 expansion in Pla2g5-null mice, but LA was inactive both in vivo and in vitro. Notably, Pla2g5-null ILC2s showed significantly reduced expression of the FFA-receptor-1 compared to Wt ILC2s. Thus, macrophage-associated Pla2g5 contributes significantly to type-2 immunity through regulation of IL-33 induction and FFA-driven ILC2 activation.

Abstract

Current $\gamma$-ray telescopes suffer from a gap in sensitivity in the energy range between 100keV and 100MeV, and no polarisation measurement has ever been done on cosmic sources above 1MeV. Past and present e$^+$e$^-$ pair telescopes are limited at lower energies by the multiple scattering of electrons in passive tungsten converter plates. This results in low angular resolution, and, consequently, a drop in sensitivity to point sources below 1GeV. The polarisation information, which is carried by the azimuthal angle of the conversion plane, is lost for the same reasons. HARPO (Hermetic ARgon POlarimeter) is an R\&D program to characterise the operation of a gaseous detector (a Time Projection Chamber or TPC) as a high angular-resolution and sensitivity telescope and polarimeter for $\gamma$ rays from cosmic sources. It represents a first step towards a future space instrument in the MeV-GeV range. We built and characterised a 30cm cubic demonstrator [SPIE 91441M], and put it in a polarised $\gamma$-ray beam at the NewSUBARU accelerator in Japan. Data were taken at photon energies from 1.74MeV to 74MeV, and with different polarisation configurations. We describe the experimental setup in beam. We then describe the software we developed to reconstruct the photon conversion events, with special focus on low energies. We also describe the thorough simulation of the detector used to compare results. Finally we will present the performance of the detector as extracted from this analysis and preliminary measurements of the polarisation asymmetry. This beam-test qualification of a gas TPC prototype in a $\gamma$-ray beam could open the way to high-performance $\gamma$-ray astronomy and polarimetry in the MeV-GeV energy range in the near future., Comment: SPIE Astronomical telescopes and instrumentation conference 2016

Abstract

Current {\gamma}-ray telescopes based on photon conversions to electron-positron pairs, such as Fermi, use tungsten converters. They suffer of limited angular resolution at low energies, and their sensitivity drops below 1 GeV. However, current data regarding responses to anti–PD-1 therapy in older patients with NSCLC or those with poor performance status (PS) are limited. Therefore, we examined the therapeutic effect of anti PD-1 antibody in these patients.Patients: We retrospectively examined consecutive patients treated with anti–PD-1 monotherapy (pembrolizumab or nivolumab) from January 2016 to September 2018.Results: We enrolled 125 patients (median age, 60 years), 80.8% of whom were men. Patients aged ≥ 75 years were considered older patients (n = 15), and those with PS 2– 3 were regarded as having poor PS (n = 11). The objective response and disease control rates were 15.4% and 46.2%, respectively, in older patients and 9.1% and 27.3%, respectively, in those with poor PS. Progression-free survival (PFS) and overall survival (OS) did not significantly differ between older and younger patients. However, poor PS was significantly associated with poor survival. High programmed death ligand 1 (PD-L1) expression in tumor specimens (≥ 50%) was associated with favorable survival in the entire cohort as well as patients with poor PS. Safety analyses demonstrated no significant difference in the occurrence of any adverse event, including grade ≥ 3 adverse events, between patients with poor PS or older age and the remaining patients.Conclusion: Anti–PD-1 therapy had similar efficacy in older and younger patients with NSCLC, whereas survival was significantly worse in patients with poor PS. 23. Atomistic study on the cross-slip process of a screw <a> dislocation in magnesium

Abstract

The cross-slip process of a screw $<$a$>$ dislocation from the basal to the prismatic plane in magnesium was studied using the density functional theory and the molecular dynamics calculations. An atomistic method for calculating the total Peierls energy map has been devised to track the transition path of a dissociated and/or constricted screw $<$a$>$ dislocation in the cross-slip process. The barrier of a screw $<$a$>$ dislocation from the basal to the prismatic plane is estimated by the density functional theory for the first time to be $61.4\pm 2.0$ meV per Burgers vector length. The activation enthalpy for the cross slip is calculated using a line tension model based on the density functional theory to be $1.4$ to $1.7$ eV, which is in reasonable agreement with experiments. On the basis of the results, the effect of temperature on the cross-slip process of the dissociated screw $<$a$>$ dislocation on the basal plane is studied in detail using the molecular dynamics method with the embedded-atom-method (EAM) interatomic potential, in which the critical resolved shear stress for the cross slip is evaluated. It is confirmed that the bowed-out dislocation line on the prismatic plane consists of slightly dissociated rectilinear segments with connecting jogs at low temperatures and, as the temperature rises, the curved dislocation line becomes smooth with many segments. The motion of an $<$a$>$ dislocation on the prismatic plane is jerky in the low temperature region, while it is retarded by the formation of the largely dissociated plateau segment above the room temperature. To the best of our knowledge, only four studies have examined the factor structure of the HADS for assessing pregnant women, with conflicting results. This study aimed to assess the factor structure and measurement invariance of the HADS for use with pregnant Japanese women.Participants and Methods: A total of 936 pregnant Japanese women completed the HADS questionnaire at three time points: the first and third trimester of pregnancy, and postpartum. We examined the factor structure of the HADS in Group 1 (n = 466) using exploratory factor analysis (EFA). We then compared the models identified in Group 1 with those from previous studies using confirmatory factor analysis (CFA) in Group 2 (n = 470). We performed multiple-group CFA for Group 2 to test the measurement invariance of the best-fit model across the three time points.Results: The EFA for the Group 1 data at the three time points revealed a two-factor model. In the CFA, the two-factor model from Group 1 showed the best fit with the data at the three time points. However, few studies have focused on the relationships among these factors and mother–infant bonding. This cross-sectional study explored the associations between depression, anxiety and parity, and mother–infant bonding.Materials and Methods: Japanese mothers, both primiparas and multiparas, completed the Mother-to-Infant Bonding Scale (MIBS) and the Hospital Anxiety and Depression Scale (HADS) one month after childbirth. We performed a stepwise multiple regression analysis with the forward selection method to assess the effects of HADS anxiety and depression scores and parity as independent variables on mother–infant bonding as the dependent variable.Results: A total of 2379 Japanese mothers (1116 primiparas and 1263 multiparas) took part in the study. MIBS score (2.89 ± 2.68 vs 1.60 ± 2.11; p < 0.0001) was significantly higher in primiparas than in multiparas. HADS anxiety (6.55 ± 4.06 vs 4.63 ± 3.41; p < 0.0001) and depression (6.56 ± 3.43 vs 5.98 ± 3.20; p < 0.0001) scores were also significantly higher in primiparas than in multiparas. A stepwise multiple regression analysis with the forward selection method revealed that HADS depression and anxiety scores and parity were significantly associated with MIBS score (p = 0.003, 0.015 and 0.023).Conclusion: Depression, anxiety and primiparity were negatively associated with mother–infant bonding one month after childbirth.Keywords: HADS, Japanese, MIBS, parity, postpartum

She was treated with crizotinib 750 mg/day for 4.5 years, with partial tumor response. However, the patient subsequently presented with right hip pain and difficulty in walking. She underwent magnetic resonance imaging (MRI), which detected T2 prolongation in the right femoral bone head, synovial fluid retention, and bone joint fissure narrowing. The patient was diagnosed with rapidly destructive coxarthrosis (RDC) and received a total hip arthroplasty. This represents a rare case of RDC as a potential side effect of crizotinib in a patient with ROS1-positive lung adenocarcinoma. MRI should therefore be recommended in patients receiving crizotinib who experience continuing severe hip pain and difficulty in walking. We aimed to develop FOT-based phenotypes and determine whether the phenotypes and their imaging characteristics could facilitate the understanding of COPD pathophysiology.Patients and methods: FOT and spirometry were examined in 164 COPD patients and 22 non-COPD smokers. COPD patients were divided into four FOT-based phenotypes (NL, normal group; RD, resistance-dominant group; XD, reactance-dominant group; and MIX, mixed group) based on the 3rd quartile values of R5 (Rrs at 5Hz) and X5 (Xrs at 5Hz) in the non-COPD group. The emphysematous lesions and the airway lesions were quantitatively assessed in CT images by low attenuation volume and the square root of the wall area of a hypothetical airway with an internal perimeter of 10 mm (√Aaw at Pi10), respectively. DPM imaging analysis was also performed in 131 COPD patients. We investigated the differences in COPD parameters between the FOT-based phenotypes.Results: √Aaw at Pi10 were significantly higher in the RD, XD, and MIX groups than in the NL group. The XD group showed lower pulmonary function and higher dyspnea scores than the RD group. No significant changes in DPM values were observed between the RD and the NL groups. The gas-trapping area was significantly higher in the XD group than in the NL group. Abstract

We introduce a new model applying to the core-nucleus and two-neutron system. The Faddeev equations of 6He-n-n and 8He-n-n systems for 8He and 10He are solved, respectively. The potential of the subsystem in the model has been determined to make a coupling both of the ground state and the excited one inside the core nucleus. By a similar mechanism the three-nucleon system is solved with the three-body force originating from an isobar excitation of the nucleon. Inputting only the information of subsystem energy levels and widths we get the coupling constants of rank 1 Yamaguchi potential between the core nucleus and neutron. We calculate the Faddeev three-cluster equations to obtain the low-lying energy levels of 8He and 10He. Abstract

We investigate the effect of hydrogen on the mobility of a screw dislocation in body-centered cubic (bcc) iron using first-principles calculations, and show that an increase of screw dislocation velocity is expected for a limited temperature range. The interaction energy between a screw dislocation and hydrogen atoms is calculated for various hydrogen positions and dislocation configurations with careful estimations of the finite size effects, and the strongest binding energy of a hydrogen atom to the stable screw dislocation configuration is estimated to be $256\pm32$ meV. These results are incorporated into a line tension model of a curved dislocation line to elucidate the effect of hydrogen on the dislocation migration process. Both the softening and hardening effect of hydrogen, caused by the reduction of kink nucleation enthalpy and kink trapping, respectively, are evaluated.
