11. Eradication of oesophageal varices recurring after portal non-decompressive surgery by injection sclerotherapy

Abstract

Esophageal varices are enlarged tortuous veins at the lower end of the esophagus often caused by increased pressure on the portal vein of the liver. Bleeding esophageal varices can be successfully treated using injection sclerotherapy (injection of a hardening agent) directly into esophageal varices to stop the bleeding. A specialist who is experienced in performing this endoscopic procedure is not always available. In this case, surgical methods are used, such as a porta-caval shunt or a devascularization procedure. The results of sclerotherapy performed for recurrence of esophageal varices following portal vein (non-decompressive) surgery are presented. There were 60 patients with portal hypertension; 34 were treated prophylactically (as a preventive measure), 7 electively, and 19 on an emergency basis. Acute bleeding was controlled for all patients after one sclerotherapy session. During follow-up, averaging 33.1 months, there were no recurrences of bleeding or varices in patients who complied with treatment protocol (three patients were noncompliant). Two patients developed bleeding from other causes, one from gastric ulcer and the other from gastritis. Narrowing of the esophagus occurred in nine patients (15 percent); and two patients (3 percent) developed stomach varices. Twelve patients died, and the overall four-year survival rate was 80 percent. It is concluded that treatment with sclerotherapy is preferable to invasive portal surgery in treating bleeding esophageal varices and for recurrence of this condition. 43. Hemorrhagic Complications following Percutaneous Transluminal Angioplasty for Carotid Stenosis

Abstract

We report 2 cases with hemorrhagic complications following percutaneous transluminal angioplasty (PTA) for carotid stenosis. Computed tomography (CT) scanning of these cases demonstrated diffuse subarachnoid hemorrhage in 1 case, and intracerebral hemorrhage in the other case on the day after PTA. In the latter case, we measured cerebral blood flow velocity and mean transit time with transcranial doppler (TCD) and dynamic CT scan, and TCD and dynamic CT demonstrated a remarkable increase in blood flow velocity and peak height respectively. From these results, postoperative hyperperfusion was supposed to have caused the hemorrhagic complication.
