Aim: Advances in the techniques and materials used in hypospadias surgery are aimed at improving cosmetic and functional results. In our study, we aimed to compare the impact of using Zaontz urethral stent (ZUS) on patient comfort in children undergoing hypospadias surgery, as opposed to feeding tubes. Materials and Methods: A total of 178 patients who underwent distal hypospadias surgery were included in the study. Snodgrass technique was performed routinely in all cases. In all cases, 6 Fr, 8 Fr and 10 Fr urethral stents were used depending on the surgical situation. Patients were divided into two groups according to the catheterization method. While in Group 1, catheterization was performed with a feeding tube, ZUS was used in Group 2. Demographic data of the patients were analyzed. In the postoperative period, the groups were compared in terms of patient comfort according to the Parents' Postoperative Pain Measurement (PPPM) scale. This scale was evaluated three times a day during the hospitalization period. A score of at least 6 out of 15 was considered clinically significant pain. Results: Age, catheterization size, operation and hospitalization time were similar between the groups (p>0.05). PPPM score in Group 2 was observed as 3.25, significantly lower than Group 1 (p<0.001). Objective: Predisposing factors of Peyronie’s disease remain controversial. We know that obesity has extremely negative effects on erectile tissue. Therefore, in our study, we aimed to examine the relationship between Peyronie’s disease and the body shape index (ABSI), which is a new parameter for the evaluation of visceral adiposity. Materials and Methods: In this study, 55 healthy volunteers (group-1) and 50 Peyronie’s disease patients (group-2). Age, comorbidities, waist circumference (WC), height, body mass index (BMI), testosterone, fasting glucose, high-density lipoprotein (HDL) cholesterol, low-density lipoprotein (LDL) cholesterol, International Index of Erectile Function (IIEF), and ABSI scores of all patients were analyzed. In addition, plaque sizes, duration of symptoms, and curvature degrees of patients in group 2 were calculated. Results: The mean ages of group 1 and group 2 were 57.02±8.34 years and 56.02±10.65 years, respectively (p>0.05). Fasting glucose, WC, BMI, and ABSI values were significantly higher in group 2 (p=0.031, p<0.001, p=0.026 and p<0.001). LDL and HDLvalues were similar between both groups (p>0.05). The IIEF score was observed to be lower in group 2 (p<0.001). In terms of ABSI values, the discrimination power of Peyronie’s disease was strong. The cut-off value for the ABSI score was 0.08. For this cut-off point, classification success was determined as 88.0% sensitivity and 80.0% selectivity. Conclusion: ABSI can be a reliable independent risk factor for Peyronie’s disease and a predictor of visceral adiposity. [ABSTRACT FROM AUTHOR]

Objective: This study aimed to evaluate the sexual function in patients who underwent a transrectal ultrasonography-guided prostate needle biopsy (TRUS-Bx) using 16G and 18G needles. Materials and Methods: Ninety patients underwent TRUS-Bx. Group 1 included patients who underwent biopsy with an 18G needle (n=45), and group 2 included patients who underwent biopsy with a 16G needle (n=45). Sexual function in both groups was prospectively compared. Additionally, the patients' age, prostatespecific antigen (PSA), prostate volume, cancer detection rate, complication rates, visual analog scale (VAS), and International Index of Erectile Function (IIEF) scores were compared. Results: Age, PSA, prostate volume, cancer detection rate, complication rates and VAS were not found statistically different between the groups. No difference was observed between the two groups in terms of the five main items of IIEF before the procedure. The intercourse satisfaction value was found to be significantly lower after the procedure than before the procedure in group 1 (p<0.05). Orgasmic function, intercourse satisfaction, and overall satisfaction were statistically significantly lower after the procedure than before the procedure in group 2 (p<0.001). Orgasmic function and overall satisfaction were lower in group 2 and significantly different between the two groups four weeks after the procedure (p<0.001). Conclusion: Although erectile function, sexual desire, and intercourse satisfaction were not different on using different needle sizes, in patients where 16G needles were used, orgasmic function and overall satisfaction were lower four weeks after the procedure. This study aims to evaluate the protective efficacy of remifentanil against testicular ischemia-reperfusion injury.The study included 24 male rats. The rats were randomized into three groups: Group 1 was the control group. Group 2 was subjected to a testicular torsion/detorsion model. Group 3 underwent similar procedures and additionally received remifentanil (0.6 μg/kg/min) intravenously for the first 20 min of reperfusion. Blood samples were taken for biochemical analyses, and orchiectomy was performed for histopathologic examination.Biochemical analysis of blood samples showed a significant increase in antioxidant enzyme activity, including superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in Group 3 compared to Group 2 (p:0.004 and p:0.002, respectively). There was a dramatic decrease in the levels of proinflammatory cytokines, including interleukin-1 beta (IL-1 Beta), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha) in Group 3 compared to Group 2 (p:0.001, p:0.046, and p:0.004, respectively). Similarly, malondialdehyde (MDA) levels decreased in Group 3 compared to Group 2 (p:0.004). Histopathologic examination of Group 3 rats showed positive changes in inflammation, hemorrhage, edema, and congestion levels compared to Group 2 (p<0.001). Similarly, there was a positive effect on the Johnsen and Cosentino score in Group 3 compared to Group 2 (p:0.001 and p<0.001, respectively).In our study, it has been documented that remifentanil protects against testicular ischemia-reperfusion injury. Background: This experimental study aimed to evaluate the potential protective effects of lidocaine on ischemia-reperfusion injury resulting from testicular torsion/detorsion in rats., Methods: A total of 18 male rats were randomized into three groups. Group 1 served as the control group. Group 2 was designed to evaluate testicular ischemia-reperfusion injury using a torsion/detorsion model. In Group 3, the treatment group, a similar ischemia-reperfusion model was used as in Group 2. Additionally, lidocaine at a dose of 15 mg/kg was administered intraperitoneally five minutes before reperfusion. Blood biochemical analyses and testicular histopathological evaluations were conducted., Results: Blood biochemical analysis showed that malondialdehyde (MDA) and protein carbonyl (PC) levels were significantly higher in Group 2 compared to the other groups (p<0.001 and p=0.008, respectively). Proinflammatory cytokine levels, including interleu-kin-1 (IL-1), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha), were lower in Group 3 than in Group 2 (p<0.001, p=0.007, and p=0.026, respectively). Antioxidant enzyme activities, including glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD), were higher in Group 3 compared to Group 2 (p=0.005 and p=0.025, respectively). Histopathological evaluations revealed significant improvements in all testicular damage scores, including hemorrhage, edema, vasocongestion, and inflammation in Group 3 compared to Group 2 (p=0.015, p=0.035, p=0.015, and p=0.034, respectively). Additionally, there was a notable improvement in the Johnsen score in Group 3 compared to Group 2 (p=0.034)., Conclusion: Lidocaine, an effective local anesthetic, significantly alleviates the effects of testicular ischemia-reperfusion injury.

Objective: Penile torsion is a counterclockwise rotational anomaly of the penile shaft or glans. We aimed to evaluate the results of dorsal dartos flap rotation technique in children with isolated penile torsion., Materials and Methods: 5470 boys who applied to our clinic between 2012 and 2022 for circumcision were evaluated for congenital isolated penile torsion. They were classified according to the degree and direction of torsion and clinical findings were analyzed.  According to the degree of torsion, penile degloving or dorsal dartos flap rotation technique with circumcision was performed. Patients whose torsion corrected after penile degloving were excluded from the study. The results of dorsal dartos flap rotation technique were evaluated.  Results: Congenital isolated penile torsion ≥60° was identified in 1.04 % (n=57) of the children. Eight patients whose torsion corrected after penile degloving were excluded from the study. 49 patients who underwent dorsal dartos flap rotation had a mean age of 4.94 years (1-9) and a mean operation time of 29.9 min (20-40). The mean degree of torsion was 77.6° (60-110). The mean operation times in the < 90° and ≥ 90° dorsal dartos flap groups were statistically significant (p<0.05).  Residual torsion was statistically significant in the < 90° and ≥ 90° dorsal dartos flap groups (p<0.05).  At the postoperative 1st and 6th month follow-ups, torsion less than 10 degrees was observed in 3 patients who underwent dorsal dartos flap rotation technique. No residual torsion was observed in other patients., Conclusion: Isolated penile torsion cases should not be overlooked during circumcision.  It seems possible to obtain successful results with dorsal dartos flap rotation in moderate and severe torsions.

Background: This study aimed to evaluate the histopathological and biochemical effects of ketamine on penile tissues following ischemia-reperfusion injury induced by priapism., Methods: Twenty-four male rats were randomized into three groups. Group 1 served as the control group. Group 2 underwent the priapism model to induce ischemia-reperfusion injury. Group 3, the treatment group, experienced a similar ischemia-reperfusion model as Group 2; additionally, 50 mg/kg of ketamine was administered intraperitoneally just before reperfusion. Blood biochemical analyses and penile histopathological evaluations were performed., Results: In Group 3, significant improvements were observed in all histopathological scores, including desquamation, edema, inflammation, and vasocongestion compared to Group 2 (p<0.001). Blood biochemical analyses showed that the malondialdehyde (MDA) levels were recorded as 10 in Group 2, with a significant decrease in Group 3 (p=0.013). Similarly, proinflammatory cytokine levels, including interleukin-1 beta (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α), were found to be suppressed in Group 3 compared to Group 2 (p=0.003, p=0.022, and p=0.028, respectively). Antioxidant enzyme activities, such as glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD), were higher in Group 3 compared to Group 2 (p=0.016 and p=0.024, respec-tively)., Conclusion: Ketamine is an effective anesthetic agent in alleviating the effects of penile ischemia-reperfusion injury.
