Poor integration and landmark views make opposing claims regarding the relationship between post-traumatic stress symptoms and trauma memory integration. This study tested these approaches using an event cluster paradigm. In total, 126 participants (Nptsd = 61; Nnon-ptsd = 65) remembered memories from the same story as trauma, positive and neutral memories and reported whether each memory was directly retrieved or generated. Moreover, the retrieval time (RT) was recorded. Finally, the participants completed the Centrality of Event Scale (CES) and Post-traumatic Stress Disorder Symptom Scale-Self Report (PSS-SR). The results demonstrated that participants with post-traumatic stress disorder (PTSD) recalled their clusters of memories more slowly and less directly than those without PTSD. However, the CES predicted PTSD severity more strongly than RT and retrieval strategy. These results suggest that traumatic memories are more disorganised but perceived as more central in PTSD. Objectives: The clinical course of COVID-19 ranges from mild to severe. The predictability of clinical outcomes gains importance in managing the disease. In this retrospective cohort study, we aimed to investigate the relationship between biomarker levels and the clinical severity of COVID-19. Methods: COVID-19 patients (n=618) admitted to a tertiary care hospital in Istanbul, Turkey were classified according to their clinical status using a scoring system designed by WHO. Laboratory parameters such as D-dimer, ferritin, and lymphocyte count levels were evaluated. In order to find out the relation between laboratory biomarkers and the severity of COVID-19, univariable and multivariable logistic regression analyses were used. Results: A positive correlation was found when WHO Score was compared with D-dimer levels (r=.508, p<0.01), and ferritin levels (r=.391, p<0.01), whereas a negative correlation was observed between WHO Score and lymphocyte count levels (r=-.381, p<0.01). The cut-off values for D-dimer and ferritin were found as 0.86 ng/mL (70.9% sensitivity and 87.1% specificity) and 92.74 ng/mL (78.5% sensitivity and 52.2% specificity) respectively. According to the multivariable logistic regression model, lymphocyte count (ß=- 0.305, p<0.001) and D-dimer levels (β=1.326, p<0.001) are the statistically significant regressors for hospitalization need. Conclusion: Patients with higher D-dimer and ferritin levels were likely to have more severe disease, whereas patients with higher lymphocyte counts overcame the disease mildly. Unlike other studies, evaluating D-dimer and lymphocyte count will likely give more detailed information on COVID-19 clinical outcomes. [ABSTRACT FROM AUTHOR]

Amaç: SARS-CoV-2 enfeksiyonuna karşı aşılama çalışmaları immünosüpresif tedavi (İS) kullanan grupta Mart 2021'de başlamıştır. Bu çalışmada İS kullanan hastalarda COVID-19 aşısına karşı gelişen antikor düzeyi yanıtının, aşılanan sağlıklı popülasyonla ve enfeksiyonu geçirerek antikor yanıtı oluşmuş hasta grubu ile karşılaştırılması amaçlanmıştır. Yöntem: Çalışmaya, Romatoloji polikliniğinden takip edilmekte olan ve İS tedavi alan, COVID-19 enfeksiyonu geçirmemiş, 2 doz aşı olmuş 67 hasta dahil edilmiştir. Kontrol grubu olarak İS tedavi almayan ve 2 doz aşı olmuş 129 hasta dahil edilmiştir. Enfeksiyon ile oluşan antikor yanıtını değerlendirmek amacıyla enfeksiyonu geçirmiş, henüz aşı olmamış, PCR pozitifliği üzerinden 1-3 ay geçmiş 229 hasta alınmıştır. SARS-CoV-2 anti-Spike antikor analizi Abbott-Architect-ci8200 otoanalizöründe kemilüminesan mikropartikül immunassay (CMIA) yöntemi ile yapılmıştır. Bulgular: İS kullanan grup ile İS kullanmayan grup arasında aşı sonrası antikor düzeyleri arasında anlamlı fark bulunmamıştır (p=0,131, medyan: 543,4 ve 332,3 sırasıyla). İS kullanan aşılı grupta anti-Spike antikor düzeyleri, enfeksiyon ile bağışıklık kazanmış gruba göre yüksek bulunmuştur (p=0,000, medyan: 543,4 ve 58,8 sırasıyla). İnaktif virüs aşısı ile mRNA aşısı karşılaştırıldığında mRNA aşısı ile oluşan antikor düzeyleri daha yüksek bulunmuştur (p=0,000, medyan: 312,4 ve 5751,7, sırasıyla). Romatoloji hastalarınının ilaç kullanımlarına göre anti-Spike antikor düzeyleri değerlendirmesinde, metotreksat, leflunomid, steroid, anti-TNF, tofacitinib kullanan hastalarda kullanmayanlara göre fark saptanmamıştır. Rituksimab kullanma öyküsü olan hastalarda antikor düzeyleri anlamlı olarak düşük bulunmuştur (p=0,000, medyan: 7,1 ve 492,9, sırasıyla). Rituksimab (1-6 kür) kullanmış ve son ilaç dozu üzerinden 8-24 ay geçmiş olan hastaların tümünde antikor yanıtı yetersiz bulunmuştur. Sonuç: İS kullanan romatoloji hastalarında rituksimab kullanımı dışında aşı sonrası oluşan antikor düzeyleri, İS kullanmayan grup ile benzer bulunmuştur. Aşı ile oluşan antikor düzeyleri, enfeksiyon ile oluşanlara göre yüksek bulunmuştur. mRNA aşısı ile oluşan antikor yanıtı, inaktif virüs aşısına göre anlamlı yüksek bulunmuştur. Extrapyramidal side effects (EPS), which are induced by antipsychotic agents, are among the major factors that disturb the patient's adaptation to treatment. Especially side effects, like acute dystonia during the early days of the cure or akathisia during the later phases, cause patients and their families to break down the treatment. Recent research, which points out that new antipsychotic agents cause less EPS than typical ones do, leads newer agents to be the first choice. However, as clinical use of new antipsychotic agents become widespread, it is reported that they were not that innocent in terms of side effects. It is known that aripiprazole, a new-generation atypical antipsychotic drug that is a dopamine system stabilizer, has a low propensity to cause EPS. We are going to present a case with distinct symptoms of acute dystonia and then akathisia caused by aripiprazole in a patient. [ABSTRACT FROM AUTHOR]
