Aim: In previous studies it has been shown that scapular morphology is associated with supraspinatus tears. However, the relationship between subscapularis tears and scapular morphology is controversial. The aim of this study is to investigate whether or not subscapularis tear accompying supraspinatus tear is related to scapular morphology. Materials and Method: Records and imaging of 679 shoulder arthroscopy surgeries performed for rotator cuff tear in our orthopedics and traumatology clinic between 2016 and 2020 were retrospectively analyzed. 162 patients with isolated full-thickness supraspinatus tear and 83 patients with subscapularis tear accompanying supraspinatus tear were included in the study. The critical shoulder angle (CSA), glenoid version (GV), glenoid inclination (GI) and critical coracoid process angle (CCPA) of the patients were measured. Results: In the patient group with isolated supraspinatus tear and in the group with subscapularis tear accompanying supraspinatus tear; The mean age was 53.8±8.9 and 61.1±8.4 respectively, and there was a significant difference (P<0.001); the mean CSA values were 36.9°±4.25° and 35.6°±5.1°respectively, (P=0.32); GI mean values were 7.72°±5.4° and 8.4°±5.3°respectively, (P=0.51); GV mean values were 2.1°±4,5° and 2.5°±4.7° respectively, (P = 0.85); mean CCPA values were 21.4°±4.5° and 22.3°±4.9°respectively, (P=0.73). There was no significant difference between the groups in terms of gender (P=0.59). In the logistic regression analysis performed by using criteria of the measured angles, age and gender, it was observed that advanced age had an effect on the subscapularis tear accompanying the supraspinatus tear, independent of the other parameters (P < 0.001 OR:1.09). Conclusion: There appears not to be a significant difference between isolated supraspinatus tears and subscapularis tear accompanying supraspinatus tear, in terms of radiological parameters regarding scapular morphology. Age seems to be the most important factor in subscapularis tear accompanying supraspinatus tears. In patients with supraspinatus tear, an increase in age by one unit (in years) increases the odds of subscapularistear by1.09 times. [ABSTRACT FROM AUTHOR]

It is estimated that 5–30% of traumatic anterior shoulder dislocations are accompanied by greater tuberosity fracture (GTF), and the pathomechanism of these fractures is not yet clear. Our hypothesis is to examine the relationship between the scapula morphology and anterior shoulder dislocation (ASD) accompanying GTF. The patients were divided into two groups according to the accompanying GTF. 40 patients with isolated traumatic ASD and 31 patients with accompanying GTF were included in the study. Critical shoulder angle (CSA), glenoid inclination (GI), acromial index (AI) and greater tuberosity angle (GTA) values were measured in two sessions by two independent observers in the standard antero-posterior radiographs of the patients in both groups. The mean CSA was 40.82°±3.19° and 35.49°±2.19° in accompanying GTF group and the isolated ASD group, respectively. The mean CSA was significantly higher in accompanying GTF group than isolated ASD group(P <0.001). The GI was significantly higher in the isolated ASD than in accompanying GTF group (P = 0.001). The mean GI was 18.7°±6.85° and 10.45°±4.87° in accompanying GTF group and the isolated ASD, respectively. Cut-off value of CSA and GI was 38° (88.2% sensitivity,88.9% specificity) and 14.5° (70.6% sensitivity and 72.2% specificity), respectively. There was no significant difference regarding the mean GTA and AI values between GTF group and the isolated ASD group (P = 0.98, P = 0.63). Objective: This study aimed to compare the clinical and functional outcomes of patients with large posterosuperior rotator cuff tears between those with intact rotator cables (crescent-shaped tears) and those with repaired rotator cables (U-, V-, or L-shaped tears)., Methods: Eighty-two patients with a mean age of 64.05 ± 9.06 years who underwent arthroscopic repair due to large posterosuperior rotator cuff tears were evaluated with a follow-up period of 32 ± 5.9 months. Forty-two patients with an intact rotator cable and 40 patients with a repaired rotator cable (rotator cable restored with tendon-tendon sutures) were evaluated regarding preoperative and postoperative pain and functional outcomes., Results: There was no significant difference in the constant score between the rotator cable repaired and the intact group (P=.22). However, when the sub-dimensions of the Constant score were compared separately, the forward flexion was significantly higher in the group with the intact rotator cable (P=.002). When the postoperative visual analog scale (VAS) scores were compared, lower scores were observed in the group with the repaired rotator cable (P < .001)., Conclusion: In large posterosuperior rotator cuff tears, patients with a repaired rotator cables experienced more significant pain relief compared to those with intact rotator cables, although their forward flexion was lower. Therefore, a detailed analysis of the tear type and the rotator cable condition in large posterosuperior rotator cuff tears may help predict postoperative pain and functional outcomes., Level of Evidence: Level III case-control study.

Anterior cruciate ligament rupture (ACLR), tibial eminence fracture (TEF), and Osgood-Schlatter disease (OSD) have been linked to a steep posterior tibial slope (PTS) in children and adolescents. This study aims to examine and compare these associations. 418 patients aged 8-18, 237 males and 181 females with knee pain between 2010 and 2019 were included and the patients belonged to the Turkish population. They consisted of ACLR (n = 47), TEF (n = 22), OSD (n = 51), and a control group (n = 298). The study and control groups were compared for PTS, sex, and age, with ACLR, TEF, OSD, retrospectively. The ACLR, OSD, and TEF groups had higher PTS values than the control group ( P < 0.001). There was no significant difference in ACLR-OSD, ACLR-TEF, and OSD-TEF comparisons ( P = 0.22, P = 0.99, and P = 0.99, respectively). PTS increase was linked to ACLR, TEF, and OSD in the multinomial regression analysis, ( P < 0.001). Increase in PTS was associated with ACLR, TEF, and OSD for both males and females (independently of other factors; ACLR: P < 0.001, P < 0.001, OR: 1.59, OR: 2.63, TEF: P = 0.001, P < 0.001, OR: 1.98, OR: 1.44; OSD: P = 0.001, P < 0.001, OR: 1.49, OR: 1.28 for males and females respectively). ACLR, TEF, and OSD are associated with increased PTS in the pediatric and adolescent age groups, and there are differences between these pathologies in terms of the amount of PTS increase, age, and sex. Consideration of age and gender in pediatric and adolescent patients with increased PTS may be useful in predicting pathologies associated with increased PTS. Objectives: This study aims to examine the relationship between foot angles and the presence and localization of osteochondral lesions of the talus (OLTs)., Patients and Methods: Between January 2014 and January 2019, a total of 152 patients with a diagnosis of medial OTLs (95 males, 57 females; mean age: 28.8±6.4 years; range, 18 to 40 years), 51 patients with a diagnosis of lateral OTLs (36 males, 15 females; mean age: 27.1±6.2 years; range, 18 to 39 years), and 114 patients without known foot-ankle trauma as the control group (56 males, 58 females; mean age: 29.0±6.1 years; range, 18 to 40 years) were included. Magnetic resonance imaging and radiographs of each group were analyzed retrospectively. Lateral talocalcaneal angle (LTCA), calcaneal inclination angle (CIA), Böhler angle (BA), and Gissane angle (GA) were measured on the images and the values were compared among the groups., Results: The CIA had a significant relationship with the localization (p<0.001). It was higher in patients with OLTs and had an effect on localization. The mean CIA was 26.6º±3.9º in the medial OLTs group, 23.0º±3.5º in the lateral OLTs group, and 18.5º±3.6º in the control group. There was a significant difference in the LTCA between the control and OLTs groups (p<0.001). The LTCA was higher in patients with OLTs, but had no effect on localization. The mean LTCA was 41.1º±4.2º for medial OLTs, 41.3º±4.2º for lateral OLTs, and 35.7º±6.8º for the controls. No significant relationship was found for BA and GA among the three groups., Conclusion: Factors affecting the localization of OLTs are still not fully understood. However, foot morphology seems to play a role in determining medial or lateral localization. The LTCA is not related to the localization of OLTs; however, an increased LTCA may be related to the occurrence of OLTS. Increased CIA may be related to both OLTS localization and OLTs occurrence.

Background: The epidemiology of pediatric fractures has been changing timely, in a multifactorial fashion. The aim of this study was to put forward a recent 5-year epidemiological analysis of pediatric fractures, according to the current AO/OTA fracture classification, in the current decade of action for road safety., Methods: A total of 3261 pediatric patients who were diagnosed with at least one fracture related with orthopedics and traumatology in a level-one trauma center were included in this retrospective and epidemiological descriptive study. The patients were grouped according to their ages as follows; <2, 2-5.9, 6-9.9, and 10-17.9. The fractures were examined according to the AO/OTA classification., Results: A total of 3396 fractures were present in 3261 patients. The mean age of the patients was 9.8±4.6 (1-17). The number of patients according to the age groups was as follows; 28 (0.008%), 735 (22.53%), 863 (26.47%), and 1635 (50.99%), respectively. The most frequent three fractures according to the AO/OTA fracture classification were; 23 (radius/ulna distal 22.9%), 13 (humerus distal, 13.3%), and 7 (hand/carpal, 12%). About 68.8% and 31.2% of the patients were treated non-surgically and surgically, respectively. Overall mortality rate was 0.1%., Conclusion: To the best of our knowledge, this study represents the first analysis of pediatric fractures according to the AO/OTA classification, over a 5-year period. As a future prospect, further multicentric epidemiological studies are warranted to constitute a sustainable action plan for the prevention of major traumas.

The aim of this study was to put forward a recent 5-year epidemiological analysis of adult fractures, according to the current AO/OTA fracture classification, in the current decade of action for road safety., Methods: 5324 adult patients who were diagnosed with at least one fracture related with orthopedics and traumatology in a level-one trauma center were included in this retrospective, epidemiological descriptive study. The patients were grouped according to their ages as; 18-35, 36-55, 56-69, and ≥70. The fractures were examined according to the AO/OTA classification., Results: 5865 fractures were present in 5324 patients. The mean age of the patients was 48.6±21.5. The number of patients according to the age groups was as follows; 1947 (36.6%), 1636 (30.7%), 881 (16.5%), and 860 (16.2%), respectively. The most frequent three fractures according to the AO/OTA fracture classification were; 7 (hand 19.6%), 23 (distal forearm, 12.1%), and 8 (foot, 11.8%). About 54.4% and 45.4% of the patients were treated non-surgically and surgically, respectively. About 0.2% of the patients preferred an alternative treatment. Overall mortality rate was 0.4%., Conclusion: To the best of our knowledge, this study represents the first analysis of adult fractures according to the AO/OTA classification, over a 5-year period. As a future prospect, further multi-centric epidemiological studies are warranted to constitute a sustainable action plan for the prevention of major traumas.
