Abstract

Given that esophageal cancer is highly malignant, the discovery of novel prognostic markers is eagerly awaited. We performed serological identification of antigens by recombinant cDNA expression cloning (SEREX) and identified SKI proto‐oncogene protein and transmembrane p24 trafficking protein 5 (TMED5) as antigens recognized by serum IgG antibodies in patients with esophageal carcinoma. SKI and TMED5 proteins were expressed in Escherichia coli, purified by affinity chromatography, and used as antigens. The serum anti‐SKI antibody (s‐SKI‐Ab) and anti‐TMED5 antibody (s‐TMED5‐Ab) levels were significantly higher in 192 patients with esophageal carcinoma than in 96 healthy donors. The presence of s‐SKI‐Abs and s‐TMED5‐Abs in the patients' sera was confirmed by western blotting. Immunohistochemical staining showed that the TMED5 protein was highly expressed in the cytoplasm and nuclear compartments of the esophageal squamous cell carcinoma tissues, whereas the SKI protein was localized predominantly in the nuclei. Regarding the overall survival in 91 patients who underwent radical surgery, the s‐SKI‐Ab‐positive and s‐TMED5‐Ab‐negative statuses were significantly associated with a favorable prognosis. Additionally, the combination of s‐SKI‐Ab‐positive and s‐TMED5‐Ab‐negative cases showed an even clearer difference in overall survival as compared with that of s‐SKI‐Ab‐negative and s‐TMED5‐Ab‐positive cases. The s‐SKI‐Ab and s‐TMED5‐Ab biomarkers are useful for diagnosing esophageal carcinoma and distinguishing between favorable and poor prognoses. [ABSTRACT FROM AUTHOR]

Abstract

The relationship between energy production and cancer is attracting attention. This study aimed to investigate the clinicopathological significance of fumarate hydratase (FH), a tricarboxylic acid cycle enzyme, in gastric cancer using autoantibodies as biomarkers. The study analyzed 116 patients who underwent gastric cancer surgery and 96 healthy controls. Preoperative serum FH autoantibody (s-FH-Ab) titers were analyzed using an immunosorbent assay with an amplified luminescent proximity homogeneous assay. Receiver operating characteristic analysis was used to determine the cutoff s-FH-Ab titer. Clinicopathological factors and prognosis were compared between the high and low s-FH-Ab groups. The s-FH-Ab levels were significantly higher in the gastric cancer group than in the control group (p = 0.01). Levels were elevated even in patients with stage I gastric cancer compared with healthy controls (p = 0.02). A low s-FH-Ab level was significantly associated with distant metastasis (p = 0.01), peritoneal dissemination (p < 0.05), and poor overall survival (p < 0.01). Multivariate analysis revealed that low s-FH-Ab levels were an independent risk factor for poor prognosis (p < 0.01). Abstract

Inflammation is closely associated with cerebrovascular diseases, cardiovascular diseases, diabetes, and cancers, and it is accompanied by the development of autoantibodies in the early stage of inflammation-related diseases. Hence, it is meaningful to discover novel antibody biomarkers targeting inflammation-related diseases. In this study, Jumonji C-domain-containing 6 (JMJD6) was identified by the serological identification of antigens through recombinant cDNA expression cloning. In particular, JMJD6 is an antigen recognized in serum IgG from patients with unstable angina pectoris (a cardiovascular disease). Then, the serum antibody levels were examined using an amplified luminescent proximity homogeneous assay-linked immunosorbent assay and a purified recombinant JMJD6 protein as an antigen. We observed elevated levels of serum anti-JMJD6 antibodies (s-JMJD6-Abs) in patients with inflammation-related diseases such as ischemic stroke, acute myocardial infarction (AMI), diabetes mellitus (DM), and cancers (including esophageal cancer, EC; gastric cancer; lung cancer; and mammary cancer), compared with the levels in healthy donors. The s-JMJD6-Ab levels were closely associated with some inflammation indicators, such as C-reactive protein and intima–media thickness (an atherosclerosis index). A better postoperative survival status of patients with EC was observed in the JMJD6-Ab-positive group than in the negative group. An immunohistochemical analysis showed that JMJD6 was highly expressed in the inflamed mucosa of esophageal tissues, esophageal carcinoma tissues, and atherosclerotic plaques. Hence, JMJD6 autoantibodies may reflect inflammation, thereby serving as a potential biomarker for diagnosing specific inflammation-related diseases, including stroke, AMI, DM, and cancers, and for prediction of the prognosis in patients with EC. Abstract

Original Article, Introduction: Immune-related adverse events (irAEs) have been reported to be a favorable prognostic factor in patients treated with immune checkpoint inhibitors. To identify the convenient biomarkers to predict irAEs, we analyzed the relationship between pretreatment laboratory data and irAEs in gastric cancer patients treated with nivolumab. Methods: A retrospective analysis was performed on 38 patients treated with nivolumab for unresectable or recurrent gastric cancer between September 2017 and December 2019. We evaluated the relationships between pretreatment laboratory data, adverse events, and prognosis. The significance of irAE predictors was evaluated via univariate and multivariate analyses. The prognostic significance of irAE predictors was also evaluated. This study was approved by the relevant ethics committee of Toho University Omori Medical Center (No. M21048, 20108, M20300). Results: In total, 14 irAEs were recorded in 13 of 38 patients (37%). The irAE (+) group showed favorable treatment response (P = 0.038) and prognosis (P = 0.093) in comparison with the irAE (-) group. The irAE (+) group showed low urea nitrogen, high sodium, and low white blood cell count (less than 5100/μl). Although low urea nitrogen and high sodium were identified to be not associated with good overall survival, the group with low white blood cell count was significantly associated with good overall survival (P = 0.022). Conclusions: A low white blood cell count at pretreatment may be a predictor of irAEs and favorable overall survival in gastric cancer patients treated with nivolumab. Gastric cancer patients with a low white blood cell count may be a good candidate for nivolumab treatment.

Abstract

Original Article, Introduction: The effect of proton pump inhibitors on capecitabine- and S-1-based treatment for human epidermal growth factor receptor 2 (HER2)-positive gastric cancer is unknown. Therefore, we compared the effect of proton pump inhibitors between capecitabine- and S-1-based treatments for HER2-positive gastric cancer. Methods: In a multi-institutional study, we retrospectively analyzed the effect of proton pump inhibitors on 155 HER2-positive advanced or recurrent gastric cancer patients who were treated with oral 5-fluorouracil (capecitabine or S-1) and trastuzumab. Capecitabine- and S-1-based treatments were compared in terms of the response rate and time to treatment failure, and the negative effects of proton pump inhibitors on the treatment response were evaluated. In this study, the primary endpoint was the response rate, and the secondary endpoint was time to treatment failure. Results: There was no significant difference in the response rate between the capecitabine- and S-1-based treatments. However, in cases without proton pump inhibitor intake, the response rate significantly improved with capecitabine-based treatment rather than with S-1-based treatment (P = 0.046). Compared with the S-1-based treatment, the capecitabine-based treatment significantly prolonged the time to treatment failure (P = 0.044), including for patients with differentiated adenocarcinoma (P = 0.035). Conclusions: For HER2-positive and differentiated gastric cancer, capecitabine-based treatment may be better than S-1-based treatment. Proton pump inhibitors might decrease capecitabine's effects and should be avoided to increase the response rate to capecitabine-based treatment.

Abstract

Case Report, A 58-year-old Japanese man was diagnosed with T4b (descending aorta) N2M1, stage IVB lower thoracic Barrett's esophageal adenocarcinoma. He was initially treated with definitive chemotherapy with cisplatin and 5-fluorouracil (FP therapy). After one course of FP therapy, he developed renal dysfunction complications. Computed tomography showed no remarkable change in the main tumor and metastatic lesion. Therefore, radical esophagectomy with resection of the invaded descending aorta and removal of the solitary metastatic lesion was performed. Superior mediastinal dissection, esophageal transection, and left adrenal gland removal were performed simultaneously. The patient underwent curative resection but died of multiple hematogenous metastases 116 days postoperatively. As aortic infiltrates were infected, artificial blood vessel replacement was selected. Although he underwent radical esophagectomy, hematogenous metastases have occurred in the early postoperative period.

Abstract

Although the clinicopathological significance of serum p53 antibodies (s-p53-Abs) in esophageal cancer have been evaluated previously, previous reports only analyzed around 100–200 patients. This study was a multi-institutional study promoted by the Japan Esophageal Society to evaluate the clinical significance of preoperative s-p53-Ab status and antibody titers in 1487 esophageal cancer patients without neoadjuvant therapy. A total of 1487 patients with esophageal squamous cell carcinoma surgically treated between 2008 and 2016 in 15 hospitals in Japan were enrolled. The cut-off value to classify the patients into s-p53-Ab positive and negative groups was 1.30 U/ml. A receiver operating characteristic curve was constructed to assess the s-p53-Abs cut-off levels to differentiate poor prognosis among the s-p53-Ab positive group. Univariate and multivariate analyses were used to evaluate the clinicopathological and prognostic significance of s-p53-Ab status and titers. Although s-p53-Ab status was significantly associated with tumor depth (P = 0.002), nodal status (P = 0.027), and pathological stage (P = 0.002). The s-p53-Ab positive status was not significantly associated with poor overall survival (P = 0.699). Using 9.82 U/ml as a cut-off, the high s-p53-Ab titer group showed a significantly worse overall survival than the low s-p53-Ab titer group (P = 0.038). However, the difference was not significant in the multivariate analysis. The presence of s-p53-Abs was associated with tumor progression. Although high s-p53-Ab titers more than 9.82 U/ml, might be associated with poor prognosis for patients with esophageal squamous cell carcinoma, it was not an independent risk factor.

Original Article, Introduction: The initial high systemic inflammatory index (SII), which is calculated as platelets × (neutrophils / lymphocytes), indicates a poor prognosis in patients with various malignant tumors, but its value after treatment in esophageal cancer is not well described. Methods: The prognostic value of the perioperative SII was evaluated in 103 esophageal cancer patients treated by radical esophagectomy. Fifty-nine of the patients received neoadjuvant chemotherapy (NAC); 44 received surgery only. The prognostic value of the SII was evaluated at each stage of the treatment. The impact of clinicopathological factors on the SII and prognosis was also evaluated after stratifying patients by treatment with surgery and NAC or surgery only. Results: Pre-treatment SII was not associated with prognosis, but pre-operative SII (p = 0.11), post-operative SII (p = 0.07), and post-NAC SII (p < 0.05) were associated with prognosis. A decrease in the SII during treatment was also associated with a good prognosis. A high SII before NAC was significantly associated with a decrease of SII during treatment (p < 0.05). Conclusions: A high or non-decreased SII after treatment of esophageal cancer was a poor prognostic indicator. The prognosis of patients with high SSIs might be improved by NAC.
