Abstract

Microscale needle-like electrode technologies offer in vivoextracellular recording with a high spatiotemporal resolution. Further miniaturization of needles to nanoscale minimizes tissue injuries; however, a reduced electrode area increases electrical impedance that degrades the quality of neuronal signal recording. We overcome this limitation by fabricating a 300 nm tip diameter and 200 μm long needle electrode where the amplitude gain with a high-impedance electrode (>15 MΩ, 1 kHz) was improved from 0.54 (−5.4 dB) to 0.89 (−1.0 dB) by stacking it on an amplifier module of source follower. The nanoelectrode provided the recording of both local field potential (<300 Hz) and action potential (>500 Hz) in the mouse cortex, in contrast to the electrode without the amplifier. These results suggest that microelectrodes can be further minimized by the proposed amplifier configuration for low-invasive recording and electrophysiological studies in submicron areas in tissues, such as dendrites and axons.

Abstract

Pacific water is an important nutrient source for sustaining biological production in the Chukchi Sea, western Arctic Ocean, which is one of the productive regions in the world. Therefore, to understand the impacts of future environmental changes on biological production in the sea, it is crucial to understand the origins, modification processes, and spatiotemporal variations of the water masses from the Bering Sea with changes in nutrient concentrations. To improve water mass analysis in the shelf regions of the Bering and Chukchi Seas, we observed levels of humic-like fluorescent organic matter (FOMH) by using an in situ fluorometer directly connected to a temperature-salinity sensor during a cruise in the early summer of 2013 and evaluated the potential of FOMH as a third parameter of water mass analysis. The levels of FOMH were different among specific water masses in the region, and FOMH seemed to behave semiconservatively in the shelf regions of the Bering and Chukchi Seas during the early summer of 2013. The distributional pattern of FOMH implies that FOMH estimated by the in situ fluorometer has the potential to (1) separate warm water into riverine-affected Alaskan Coastal Water and historically photobleached summer Bering Basin Water; (2) distinguish the Anadyr Water, which has low FOMH levels and high nutrient concentrations, from the Bering Shelf Water; and (3) determine different formation/modification processes of dense shelf water that contains high nutrient concentrations.

A fingertip guiding manipulator was developed as a haptic graphic display for blind people: it helps them create mental images of raised graphics and reliefs with stroke order. Through fingertip kinesthetic sense, while foreseeing the direction of the upcoming stroke by perceiving the knob's direction, users experience the uplifted and fallen strokes by perceiving their fingertips being pulled in the horizontal and vertical directions. Abstract

Background: According to the aging in Japan, surgery of lung cancer with cardiovascular disease is increasing. Among them, the merger of the heart and large blood vessel disease would be expected to be directly linked to the perioperative management., Materials and Methods: We conducted a retrospective analysis of 1,005 patients who had undergone surgical resection of primary lung adenocarcinoma between January 1997 and June 2014 at Hokkaido University Hospital. Among them, 81 patients had more than one cardiovascular disease., Results: The median age was 71 years old. Men are 62 cases(76%). 64 cases (79%)had smoking history. Ischemic heart disease( IHD) was 29 cases(33%), followed by atrial fibrillation(Af), arrhythmia other than Af, valve disease and macrovascular disease. Anticoagulant or antiplatelet therapy were preoperatively done in 41% of IHD cases, 39% of Af cases. Thirty four% of IHD, 50% of vascular disease received the treatment other than medication. Video-assisted thoracic surgery was performed in 67 cases(83%). Lobectomy, sublobectomy were performed for 59 cases(73%), 19 cases(23%), respectively. Postoperative complications were occurred in 32 cases(40%). Two patients(2.5%)died in hospital without discharge. Univariate analysis revealed tumor size, pStage, lymph node dissection, operation time or blood loss were correlated with postoperative complication., Conclusions: For the patients with cardiovascular disease, careful evaluation of surgical indication, perioperative management is required.

Abstract

Background: The i-gel is a rescue device for ventilation or tracheal intubation in patients with a difficult airway. The aim of this study was to evaluate the safety and reliability of fiberoptic-guided intubation through the i-gel in anesthetized patients with no history of difficult intubation undergoing elective surgery., Methods: Patients were enrolled in the study with prior informed consent. After insertion of the i-gel, the larynx was observed by bronchoscopy, and the bronchoscopic view through the i-gel was graded. Tracheal intubation was performed under fiberoptic guidance, and the i-gel was removed. The outcome was evaluated using the success rate of initial intubation as the primary variable, and complications were evaluated as a secondary variable., Results: The first attempt at intubation was successful in all 52 patients evaluated, and there was no problem with i-gel removal. No arterial oxygen desaturation was noted throughout the induction of anesthesia, and no serious complication was observed., Conclusions: Fiberoptic-guided intubation could be performed safely through the i-gel. The i-gel is considered to be potentially useful as an alternative conduit for fiberoptic-guided intubation.

Abstract

We report two cases of long-standing iron-deficiency anemia in a young man and an elderly woman that improved after eradication of Helicobacter pylori. Neither patient demonstrated any bleeding symptoms nor reported an extremely unbalanced diet. However, H. pylori infection was demonstrated with the urea breath test. After successful eradication of H. pylori, the iron-deficiency anemia dramatically improved in both patients, making it unnecessary to administer ferric medicine for about 20 months. These cases suggest that eradication of H. pylori may be useful in treating some patients with long-standing iron-deficiency anemia.

46. [A hairy B cell lymphoproliferative disorder resembling hairy cell leukemia].

Abstract

This case report describes a hairy B cell lymphoproliferative disorder (HBLD) with clinical and hematological features resembling hairy cell leukemia. The patient was a 29-year-old female who demonstrated atypical lymphocytes in her peripheral blood. Physical examination demonstrated splenomegaly, but there were no palpable superficial lymph nodes. Hematological examination showed a leukocyte count of 10.6 x 10(3)/mm3 with 41% atypical lymphocytes. Bone marrow examination showed a normal cellular and an atypical lymphocyte count of 42%. The atypical lymphocytes had microvilli and prominent membranous ruffles on their surfaces. Atypical lymphocytes expressed CD5- CD10- CD11c+ CD19+ CD20+ CD23- CD25- on the surface of the cells on examination by with a fluorescence activated cell sorter. Although these findings were similar to hairy cell leukemia, Japanese variant, the surface marker of the kappa chain and lambda chain was unbiased and studies of immunoglobulin gene rearrangements and expression showed polyclonal proliferation of B cells. Therefore, we diagnosed this patient as having HBLD. Because she did not demonstrate anemia or thrombocytopenia, she is not currently receiving medication. To date, the atypical lymphocyte count has not changed.
