Le traitement des fractures instables des métacarpes et des phalanges reste un objet de controverse. Peu de séries ont été rapportées dans la littérature, rendant leur analyse difficile. Nous rapportons une étude rétrospective comportant 12 patients, opérés par cette technique, ayant eu des fractures déplacées des métacarpes ou des phalanges, sur une période de deux ans. Les résultats globaux ont été bons dans 75% des cas, moyenne dans 16,5% des cas et mauvais dans 8,5% des cas. La stabilité du montage par mini plaques des fractures instables des métacarpiens et des phalanges ont permis une mobilisation précoce des articulations de la main, évitant ainsi la raideur.

L'ostéome ostéoïde est une tumeur osseuse bénigne, mais douloureuse et dont le traitement consiste en l'exérèse chirurgicale totale. Nous rapportons, dans ce travail le cas d'une jeune patiente présentant un ostéome ostéoïde de l'extrémité inférieur du radius.

La luxation traumatique invétérée de la hanche est une affection rare et grave avec une prise en charge difficile et non encore codifiée. L'arthroplastie totale de la hanche reste le traitement chirurgical de choix. Nous rapportons un cas de luxation iliaque invétérée de la hanche chez un jeune patient de 19 ans, traitée par une arthroplastie totale de la hanche. Après un recul de 2 ans, le résultat fonctionnel est satisfaisant, avec une marche indolore sans aide. Le Score de Harris est de 96 et le score de Merle d'aubigné-Postel est de 17.

L'offset fémoral est l'élément pronostic le plus important dans l'arthroplastie de la hanche. Sa restauration est devenue de plus en plus une obligation. Toute modification de la valeur native de l'offset retenti sur la qualité du fonctionnement clinique de la hanche. Le but de notre travail était d'évaluer le fonctionnement clinique de la hanche en fonction de la valeur de l'offset fémoral après une arthroplastie. C'est une étude rétrospective effectuée entre 2010 et 2013, comportant 27 patients ayant bénéficiés d'une arthroplastie totale de la hanche. Les patients ayant eu un geste chirurgicale antérieur sur la même hanche ou sur la hanche controlatérale ont été exclus. Les mesures ont été effectuées sur des radiographies standards de la hanche en rotation interne d'environ 15, avec un agrandissement à 100%. Les résultats cliniques étaient évalués au moyen du score de WOMAC qui était en moyenne de 15,2 points, et le score de Merle d'Aubigné-Postel avec une moyenne de 15 points. Deux autres tests ont été évalués dans notre étude qui sont le step et le hop test. Les meilleurs résultats fonctionnels ont été obtenus chez les patients ayant eu une latéralisation de la tige fémoral avec un offset augmenté.

Benign fibrous histiocytoma (BFH) is a rare mesenchymal tumor, with its deep variant (DBFH) accounting for only 1%-2% of cases. DBFH typically arises in the limbs, head, and neck, and usually measures under 12 cm. MRI can suggest the diagnosis by detailing the tumor's location and internal features. However, histopathological examination remains essential for definitive diagnosis, supported by immunohistochemical staining. Pigmented villonodular synovitis is a rare proliferative disorder of the synovial joints, characterized by synovial thickening and hemosiderin deposition. It predominantly affects the knee joint, with bilateral involvement being exceedingly rare. We present a case of diffuse bilateral pigmented villonodular synovitis in a 20-year-old female, initially presenting with left knee pain and swelling, diagnosed via MRI. Following an open total synovectomy, the patient developed postoperative stiffness, requiring arthroscopic arthrolysis. Subsequently, similar symptoms emerged in the right knee, leading to a second synovectomy and subsequent arthrolysis. Despite the challenges associated with managing this condition, including a high recurrence rate and postoperative stiffness, the patient showed significant improvement 1 year postoperatively. When faced with clinical and radiological findings suggestive of villonodular synovitis, tuberculosis is not often considered a differential diagnosis, especially when the patient is not a known tuberculosis carrier. In this paper, we present an exceptional case of a patient who had a tumefaction (measuring 17 cm in length) in the anterointernal region of her left knee, with a clinical and radiological picture in favor of villonodular synovitis. However, after tumor resection, the anatomopathological study of the surgical specimen came back in favor of a tuberculous lesion. Towards the beginning of the twentieth century, inflammatory myofibroblastic tumors were described as neoplasms characterized by myofibroblastic spindle cells with an infiltrate of inflammatory cells. These rare tumors occur mainly in children and young adults with a preferential pulmonary and abdominal location. They have intermediate biological potency with a tendency to local recurrence and rarely metastasize. We describe the case of an adolescent who presented with an inflammatory myofibroblastic tumor of the thigh, a very rare entity that has only been reported in the literature as case reports, and its clinical, radiological, echographic and MRI appearance. The patient underwent complete surgical resection of the mass with healthy margins and did not receive any additional treatment. This case report details the management of a 60-year-old male who sustained a complex proximal humerus fracture with an axillary artery injury following a fall. Initial assessment revealed a fractured humeral head and complete occlusion of the axillary artery, which was repaired using a saphenous vein graft. Subsequently, the patient underwent reverse shoulder arthroplasty (RTSA) after the limb stabilized. The staged approach, prioritizing urgent vascular repair followed by delayed orthopedic intervention, proved effective. The patient achieved satisfactory functional recovery with an improved range of motion and no pain. Shoulder dislocations are common, with luxatio erecta humeri (LEH) being a rare variant. This report discusses a case of isolated transitory radial nerve palsy following LEH, which occurred after a high-energy motorcycle accident. Neurological examination revealed radial nerve involvement. Following reduction of the dislocation and appropriate physiotherapy, the patient experienced full functional recovery. Tumor calcinosis is a rare condition. It is characterized by the presence of calcified masses in the juxta-articular regions without joint involvement. It particularly affects young adults and adolescents. Its exact pathogenesis remains poorly defined. The diagnosis is suspected clinically and radiologically but confirmed by histological examination. The treatment is mainly surgical, and the prognosis is often good. Osteoid osteoma is a benign bone tumor that frequently affects young adults. The clinical presentation is variable, and the course can lead to spontaneous regression or persistence, necessitating medical or surgical treatment. We report the case of a young patient with an exceptional localization of an osteoid osteoma at the lesser trochanter, exhibiting atypical clinical and radiological features. Iatrogenic bladder rupture is a rare yet serious complication associated with orthopedic surgical procedures, particularly those involving the modified Stoppa (MS) approach for acetabular fractures. We present a case of a 65-year-old patient who experienced iatrogenic bladder rupture during surgery for acetabular fracture fixation using the MS approach. Despite the challenges posed by this complication, prompt diagnosis and repair during the same surgical intervention led to favorable outcomes. Our case underscores the importance of perioperative vigilance in detecting and managing such injuries to mitigate the risk of urinary tract complications and late infections. Dermatofibrosarcoma protuberans (DFS) is a mesenchymal-origin skin tumor with intermediate malignancy. Though rare, it's not exceptional, comprising about 0.1% of malignant skin tumors. The authors discuss clinical, radiological, histopathological studies, and various therapeutic modalities for this tumor. Our 82-year-old patient presented with a 3 cm swelling on the right arm, initially undergoing biopsy followed by surgical excision of the mass. Adjuvant treatment with radiotherapy or chemotherapy is unnecessary unless recurrence or malignant transformation occurs. Histological analysis is crucial for diagnosis. The preferred treatment method is wide surgical excision. Prognosis primarily depends on malignancy, especially at the local level, with a high risk of recurrence. In this study, we discuss the clinical, radiological, and histopathological characteristics of intramuscular myxomas (IMM), a rare form of benign soft tissue tumors. We report the case of a 47-year-old female patient presenting with a painless, non-inflammatory mass in the right thigh, which was mobile relative to both superficial and deep planes. Imaging, biopsy, and subsequent histopathological study established the diagnosis of intramuscular myxoma. The patient underwent surgical excision of the mass, with a straightforward postoperative course. It is important to distinguish IMM from malignant tumors, such as soft tissue sarcomas, through comprehensive examinations including imaging and biopsy. Introduction: Patella fractures account for 1 % of all skeletal fractures, and can interrupt the knee's extensor apparatus. They can also be serious if associated with a cutaneous opening. We report the case of a fracture of the tip of the patella with a large loss of cutaneous substance., Case Report: A 22-year-old man was admitted for the management of a left knee trauma following a motorcycle accident. Clinical examination revealed a large loss of skin substance on the anteromedial aspect of the knee, with exposed bone, pain and lack of extension of the left knee. Standard X-ray of the left knee showed an avulsion fracture of the tip of the patella. The patient underwent patellar tendon reconstruction using transosseous stitches, reinforced by a quadricipital tendon reversal plasty, and protected by steel-wire patellotibial cerclage. A medial gastrocnemius flap was used to cover the loss of skin substance. After a functional rehabilitation protocol, the functional results were satisfactory, with a full return to activity after six months., Conclusion: The stability of osteosynthesis using quadricipital tendon reversal plasty and patellotibial cerclage allows mobilization and immediate weight-bearing.
