Methotrexate is a familiar chemotherapeutic preferred in a wide range of clinical fields such as leukemia, psoriasis, rheumatoid arthritis, neoplastic and autoimmune disorders. However, methotrexate therapy has limitations as it causes severe side effects from which liver damage is the most important one. Several antioxidant compounds have been studied against methotrexate related liver toxicity, but dexpanthenol has not been experienced. Vitamin B5-derived dexpanthenol is a usual therapeutic having a potent anti-inflammatory and antioxidant effect. In this study, we aimed to evaluate the ameliorating effect of dexpanthenol against methotrexate-induced hepatotoxicity. We performed our experiments on Wistar albino rats divided randomly into four groups involving control, dexphantenol, dexpanthenol + methotrexate and methotrexate applied animals. After this experimental work on rats, for the first time, we showed dexpanthenol improvement effect on ROS-caused hepatotoxicity initiated by methotrexate administration in terms of liver tissue antioxidant/oxidant enzymes, liver function tests, and histological changes. We suggest that dexpanthenol might be applied during methotrexate treatment in order to reduce the liver toxicity. However, further studies are needed to find out the optimal dose regimen and to understand the mechanism of action. [ABSTRACT FROM AUTHOR]

Objective: With the diagnosis of chronic illness in children, a stressful period is likely to begin for both the affected child and their families. The aim of this study was to investigate the factors affecting chronic disease management by the parents of children diagnosed with type 1 diabetes mellitus (T1DM). Methods: The sample consisted of 110 children, aged between 4-17 years and their mothers. The patients had been diagnosed with T1DM for at least one year, and had attended pediatric endocrinology outpatients or were hospitalized in a single center. First, sociodemographic information about the child with T1DM were obtained. Then, the “Family Management Measure” (FaMM) was applied. The FaMM is constructed to measure family functioning and management in families who have a child with a chronic illness. Results: Paternal years of education (p=0.036), family income (p=0.008), insulin pump use (p=0.011), and time elapsed after diagnosis (p=0.048) positively affected both the management of T1DM and the child’s daily life. However, presence of chronic diseases in addition to T1DM (p=0.004) negatively affected diabetes management. Higher maternal education year (p=0.013) and family income level (p=0.001) increased parental mutuality scores. However, as the time after diagnosis increased, parental mutuality scores decreased. Conclusion: It is important to evaluate the child with chronic disease with a biopsychosocial approach. This approach aims to evaluate the problems of the child and his/her family who experience the disease with a holistic approach. [ABSTRACT FROM AUTHOR]

Numerous variables can affect performance before the competition. Nevertheless, it could be a subject of curiosity to find out whether a warm-up strategy has a more positive effect on the power performance of kickboxers. The purpose of this study is to evaluate the effect of different specific warm-up protocols and time of day on optimum power performance in kickboxers. Ten volunteer males performed three different warm-up routines in random order. The warm-up protocols consisted of only 20 min for each group. The groups with low-intensity aerobic running (NSWU) and specific warm-ups (SWU-20m) performed 20-min protocols. The other group followed 10 minutes of jogging and 10 minutes of specific warm-up (SWU-10m). All groups followed the warm-up randomly at two different periods of the day (i.e. morning: 09:00-10:00 and evening: 16:00-17:00) on non-consecutive days. A statistically significant difference was found between the groups in terms of Mean Propulsion (W) (F=3.518; p2=0.036). There was a statistically significant difference in Mean propulsive Power (W) between NSWU and SWU-10m (p3= 0.0007) in addition there was also a statistically significant difference in SWU-10m and SWU-20m (p3= 0.0106) and, NSWU and SWU-20m (p3= 0.415) protocols. In conclusion, mean propulsion (W) performance showed diurnal variation, and performances of the kickboxers' can be affected more positively in the evening hours, especially after SWU protocols. Objective: The primary aim of this study was to use metagenomic next-generation sequencing (mNGS) data to identify coronavirus 2019 (COVID-19)-related biomarker genes and to construct a machine learning model that could successfully differentiate patients with COVID-19 from healthy controls. Materials and Methods: The mNGS dataset used in the study demonstrated expression of 15,979 genes in the upper airway in 234 patients who were COVID-19 negative and COVID-19 positive. The Boruta method was used to select qualitative biomarker genes associated with COVID-19. Random forest (RF), gradient boosting tree (GBT), and multi-layer perceptron (MLP) models were used to predict COVID-19 based on the selected biomarker genes. Results: The MLP (0.936) model outperformed the GBT (0.851), and RF (0.809) models in predicting COVID-19. The three most important biomarker candidate genes associated with COVID-19 were IFI27, TPTI, and FAM83A. Conclusion: The proposed model (MLP) was able to predict COVID-19 successfully. The results showed that the generated model and selected biomarker candidate genes can be used as diagnostic models for clinical testing or potential therapeutic targets and vaccine design. Background: Fournier gangrene (FG) is a necrotizing fasciitis involving perianal and abdominal regions. It progresses quickly and requires urgent intervention. With the use of vacuum-assisted closure (VAC) treatment applied during clinical follow-up and the use of superomedial thigh flap in the region, the treatment is completed with an effective, functional, and rapid approach. This study examines the clinical details of this method for reconstruction., Methods: The study included 15 patients who underwent superomedial thigh flap in VAC treatment reconstruction for tissue defect after FG debridement from 2016 to 2020. The patients were examined in the form of clinical evaluation with hospital admission and surgical evaluation in the postop process., Results: In patients with wound maturation and sufficient granulation, superomedial thigh flap application followed by VAC treatment soon after shortened the operation time, shortened the postop drain time, and provided effective treatment of dead space. An aesthetic and functional result was obtained with the proximity of the flap to the area. In addition, due to the sensory branches present in the flap, a sensory result was obtained according to the ratio of flap size., Conclusions: Superomedial thigh flap provides a practical, effective, and fast solution to the tissue defect that occurs after FG debridement.
