OBJECTIVE Neuroprotection during aortic arch surgery involves selective antegrade cerebral perfusion. The parameters of cerebral perfusion, e.g. flow rate, are inconsistent across centers and are subject of debate. The aim of this study was to determine the cerebral perfusion flow rate during hypothermic circulatory arrest required to meet preoperative awake baseline regional cerebral oxygen saturation (rSO2). METHODS Patients scheduled for aortic arch surgery with hypothermic circulatory arrest were enrolled in this prospective observational study. After initiation of hypothermic circulatory arrest, bilateral selective antegrade cerebral perfusion was established and cerebral flow rate was continuously increased. The primary endpoint was the difference of cerebral saturation from baseline during cerebral perfusion flow rates of 6 ml/kg/min, 8 ml/kg/min, and 10 ml/kg/min. RESULTS A total of 40 patients were included. During antegrade cerebral perfusion rSO2 was significantly lower than the baseline at 6ml/kg/min (-7.3, 95%-CI: -1.7,-12.9; p=0.0015). In contrast flow rates of 8 and 10 ml/kg/min resulted in rSO2 that did not significantly differ from the baseline (-2; 95%-CI: -4.3,8.3; p>0.99 and 1.8; (95%-CI: -8.5%, 4.8%; p>0.99). Cerebral saturation was significantly more likely to meet baseline values during selective antegrade cerebral perfusion with 8ml/kg/min than at 6ml/kg/min (44.1%; 95%-CI: 27.4%,60.8% vs 11.8%; 95% CI: 0.9%,22.6%; p = 0.0001). CONCLUSION At 8 ml/kg/min cerebral flow rate during selective antegrade cerebral perfusion regional cerebral oximetry baseline values are significantly more likely to be achieved than at 6 ml/kg/min. Further increasing the cerebral flow rate to 10 ml/kg/min does not significantly improve rSO2. CLINICALTRIALS.GOV IDENTIFIER NCT03484104.

Türkiye Cumhuriyeti’nin ilk yılları, bir yandan toplumsal dönüşümleri esas alan inkılapların uygulandığı ve öte yandan kurumsal dönüşümleri esas alan idari düzenlemelerin yapıldığı bir süreç olmuştur. Atatürk, merkezden uygulanan devlet politikalarının ne denli gerçekleştirilebildiğini yerinde görmek ve vatandaşları dinlemek üzere 1930-1931 yıllarında kapsamlı bir yurt gezisine çıkmıştır. Bu çalışma, Atatürk’ün yurt gezisi sırasında aldığı notlar gereği Dâhiliye Vekâletinin hazırladığı raporu konu edinmektedir. Dâhiliye Vekâletinin hazırladığı rapor, Türkiye’nin idari taksimatının yeniden ele alınması, valilerin salahiyetleri, Türkiye’de topak reformu, göçebelerin yerleşik hayata geçirilmelerine ilişkin sayısal verileri, yerel yönetimlerin işleyişi ve umumi müfettişlikler gibi birçok konuyu kuşatan çok önemli bilgileri içermektedir. Bu çalışmanın amacı; dönemin en yetkili makamlarının hazırlayarak Başbakanlığa sunduğu bu raporda verilen bilgilerden yola çıkarak dönemin idari yapısı, demografik özellikleri, idari anlamda yaşanan sorunlar ve sunulan çözüm önerilerini ortaya koyarak literatüre katkı sunmaktır.

Amaç: Uzun süreli evde NIV kullanımı, kompanse ılımlı hiperkapnik KOAH olan hastalar için tartışmalı bir tedavi şeklidir. Çalışmamızın amacı kliniğimizden taburcu edilen kompanse fakat ılımlı hiperkapnik hastalardan evde non-invaziv mekanik ventilatörü olmayan hasta grubunun taburculuk sonrası hastaneye başvuru sayıları, hastaneye yatış sayıları ve orta vadeli (6-8 ay) mortalite durumlarını incelemektir.Gereç ve Yöntem: Çalışma grubunu oluşturan hastalar 01.01.2019 ve 31.12.2020 tarihleri arasında Göğüs Hastalıkları Yoğun Bakım Kliniğinde yatarak tetkik edilip taburcu edilmiş kompanse hiperkapnik olgulardan oluşmaktadır. Araştırmamız retrospektif gözleme dayalı kohort çalışma olarak planlanmıştır.Bulgular: Çalışmaya 01.01.2019 ve 31.12.2020 tarihleri arasında yüksek PaCO2 düzeyiyle taburcu edilmiş toplam 245 hasta dahil edildi. Olguların %58’i erkek (n=142) cinsiyette idi. Hastaların yaş ortalaması 71,89±12,63 idi. Her iki grup arasında cinsiyet, yatışları sırasında NIV kullanımı, yatışları sırasında NIV kullanım gün sayısı, evde USOT veya Oksijen tüpü kullanımı ve GYBÜ yatışı öncesinde entübasyon öyküsü bulunması açılarından istatistiksel olarak anlamlı farklılık bulunmamaktadır (p>0,05). Mortalite grubunda konjestif kalp yetmezliği, koroner arter hastalığı ve atriyal fibrilasyon bulunma oranları istatistiksel olarak anlamlı düzeyde daha yüksektir (sırasıyla p=0,017, p=0,032 ve p=0,002). Her iki grup arasında yatış, taburcu ve kontrol sonuçları açısından istatistiksel olarak anlamlı farklı izlenmemiştir. Yüksek PaCO2 düzeyiyle taburcu olmuş olguların 1, 3, 6, 8, 12 aylık ve takip süresinin tamamındaki mortalite oranlarına karşılık taburculuk PaCO2 düzeyleri ROC analizine tabi tutulmuştur. ROC analizi sonucunda, taburculuk PaCO2 düzeyleri için 8. ay mortaliteyi belirlemede cut-off değeri 50,25 mmHg sınır değer kabul edildiğinde, duyarlılık %78,6 özgüllük %43 olarak hesaplanmıştır.Sonuç: Taburculukta ılımlı hiperkapnisi (PaCO2>50 mmHg) olan, kalp ilişkili hastalığı bulunan hasta grubunda evde NIV tedavisinin dikkatle değerlendirilmesi gerektiğini düşünüyoruz., Aim: Long-term domiciliary use of a non-invasive mechanical ventilator (NIV) is a controversial form of therapy for patients with chronic obstructive pulmonary disease (COPD) and moderate hypercapnia. The present research attempted to examine hospital admissions, hospitalizations, and medium-term (6-8 months) mortality in a group of patients with compensated but moderate hypercapnia who were discharged from our clinic without a domiciliary NIV.Material and Method: The sample of this retrospective, observation-based cohort study consisted of compensated hypercapnic cases that were hospitalized in our Pulmonology Intensive Care Unit (ICU) between 01.01.2019 and 12.31.2020.Results: A total of 245 patients discharged with high partial pressure of carbon dioxide (PaCO2) levels between 01.01.2019 and 12.31.2020 were included in the study. While 58% of the cases were males (n=142), we found the mean age of the patients to be 71.89±12.63 years. The findings yielded no significant differences between the groups by sex, use of NIV during hospitalization, number of days of NIV use during hospitalization, use of LTOT or Oxygen tube at home, and intubation history before ICU admission (p>0.05). Similarly, we could not conclude significant differences between the groups by hospitalization, discharge, and follow-up arterial blood gas (ABG) parameters. Yet, the rates of congestive heart failure, coronary artery disease, and atrial fibrillation were significantly higher in the mortality group (p=0.017, p=0.032, and p=0.002, respectively). Besides, high PaCO2 levels versus mortality rates at 1, 3, 6, 8, and 12 months and in the entire follow-up period were subjected to the ROC analysis. Accordingly, when accepting 50.25 mmHg as the cut-off value for determining the 8-month mortality for discharge PaCO2 levels, we calculated the sensitivity to be 78.6% and the specificity to be 43%.Conclusion: Overall, it is highly convenient to consider the possible positive effects of NIV therapy on mortality among patients with heart-related diseases and with moderate hypercapnia (PaCO2>50 mmHg) at discharge.

Giriş: COVID-19 pandemisi nedeniyle sağlık personelinde el hijyeni, alkol bazlı dezenfektan kullanımı, eldiven, maske ve diğer bariyer önlemlerin kullanımında farkındalık artışı gözlenmektedir. Çalışmamızın amacı, 2019 ve 2020 yıllarının ilk 6 aylarında COVID-19 tanısı dışlanmış, yoğun bakım izlemi gereken hastalarda oluşan enfeksiyonların oranlarında bir farklılık olup olmadığını karşılaştırmaktır.Gereç ve Yöntemler: 01/01/2019-30/06/2019 arasındaki 6 ay süreyle (kontrol grubu) ve 01/01/2020-30/06/2020 arasındaki 6 ay süreyle (vaka grubu) ikinci basamak Göğüs Hastalıkları Yoğun Bakım Ünitesine (GYBÜ) kabul edilen tüm hastalar çalışmaya dahil edildi. Çalışmamızda hariç bırakma kriteri yoktu. COVID-19 PCR pozitifliği tespit edilen veya COVID-19 PCR testi negatif olsa da klinik ve radyolojik kuşkuyla COVID-19 hastalığı düşünülen, ve/veya spesifik tedavi başlanmış/devam eden hiçbir hasta bu dönemde bu yoğun bakımımıza alınmadığı için çalışmamıza da dahil edilmediler.Bulgular: Çalışmanın kontrol grubu (n=327) ve vaka grubu (n=260) demografik bulgular ve genel özellikleri bakımından benzerdi. Gruplar arasında yaş, cinsiyet, hastanın GYBÜ’ne nakil geldiği yer, hastanın taburcu edildiği yer, GYBÜ öncesi üçüncü basamak yoğun bakımda entübasyon öyküsü, evde USOT ve/veya NIMV cihazlarının bulunması, eve raporlanan cihaz tipleri ve taburculuk sonrası takipte mortalite durumları, GYBÜ’de NIMV ihtiyacının olması ve NIMV gün sayısı, üçüncü basamak YBÜ’de yatış gün sayısı ve NIMV uygulama gün sayısı açısından gruplar arasında istatistiksel olarak anlamlı ilişki bulunmadı. Kontrol grubu ile vaka grubu arasında hastalardan alınan tüm kültürlerin üreme durumu ve kültürlerde üreyen mikroorganizmalar açısından istatistiksel olarak anlamlı farklılık izlenmedi (sırasıyla; p=0.775 ve p=0.137).Sonuç: Çalışmamızda GYBÜ’de çalışan sağlık personelinin el hijyeni ve bariyer önlemler hakkında farkındalığında artış olmasına rağmen gerek yüksek riskli dirençli bakteriyel üremelerde dönemler arasında farklılık izlenmemesi gerekse de tanı konulan enfeksiyonlarda dönemler arasında farklılık olmaması enfeksiyon kontrol önlemlerinin içselleştirildiğini ve uygulandığını göstermektedir., Aim: Due to the COVID-19 pandemic, an increase in awareness is observed in the use of hand hygiene, alcohol-based disinfectant use, gloves, masks, and other barrier measures in healthcare personnel. Our study aims to compare whether there is a difference in the rates of infections in patients who need intensive care follow-up in the non-COVID-19 patient group in the first 6 months of 2019 and 2020.Material and Methods: All patients admitted to the secondary level Pulmonary Intensive Care Unit (PICU) for 6 months between 01/01/2019-30/06/2019 (control group) and 6 months between 01/01/2020-30/06/2020 (case group) were included in the study. There were no exclusion criteria in our study. Patients who were found to be positive for COVID-19 PCR test or whose COVID-19 PCR test was negative, but was thought to have COVID-19 disease with clinical and radiological suspicion, and/or for whom specific treatment was initiated/continued, were not included in our study because they were not taken to our intensive care unit during this period.Results: The control group (n=327) and case group (n=260) of the study were similar in terms of demographic findings and general characteristics. Among the groups, there was no statistically significant relationship between the groups in terms of age, gender, where the patient was transferred to the ICU, where the patient was discharged, intubation history in the tertiary intensive care unit before PICU, presence of LTOT, and/or NIV devices at home, device types reported to the home at discharge and mortality status at post-discharge follow-up, the need for NIV in the hospital and the number of NIV days, the number of days of hospitalization in the tertiary ICU and the number of days of NIV application. There was no statistically significant difference between the control group and the case group in terms of the growth status of all cultures taken from the patients and the microorganisms grown in the cultures (p=0.775 and p=0.137, respectively).Conclusion: In our study, although there was an increase in the awareness of healthcare personnel working in the PICU about hand hygiene and barrier measures, there was no difference between periods in high-risk resistant bacterial growths and there was no difference between periods in diagnosed infections, which shows that infection control measures were internalized and applied.

This dissertation examines the making of modernity in the late Ottoman Empire by tracing the connections between sports, the body, male subject formation, nation building, and communal and imperial identity. It focuses on the development of a shared Ottoman physical culture amongst upper and middle-class Muslim, Christian, and Jewish men of Istanbul from the 1870s until World War I. My research draws from a diverse array of archives and primary sources written in Ottoman-Turkish, Armenian, Armeno-Turkish, French, English, German, and Greek, such as government reports and documents, school and association records, private correspondence, periodicals, books, and pamphlets, as well as vernacular photographs, in order to present an alternative understanding of cultural transformations and the historical linkages between different ethno-religious communities of the late Ottoman Empire. The central argument of this dissertation is that Muslims, Christians, and Jews of Istanbul engaged sports as a shared civic activity that offered benefits for the individual, community, and empire. This study investigates how Ottoman physical culture was underpinned by novel understandings of the body and implicated in larger debates and processes concerning the self, gender, ethno-religious communal identity, and the nation by pursuing three principle areas of inquiry. The first area of focus is the institutionalization of Ottoman physical culture in schools, voluntary associations, and government ministries. The study begins by exploring the development of athleticism as an educational ideology in a government lycée, Mekteb-i Sultani, and a foreign missionary school, Robert College. It demonstrates that many of the leading physical culture enthusiasts first encountered discussions about the educational significance of exercise and corporeal development as students in these two schools, and went on to establish voluntary athletic associations, as private spaces, in which young men formed, negotiated, and performed novel male subjectivities and identities. Ottoman government officials in the Ministry of Public Education recognized the success of these institutions in popularizing physical culture and sought to harness its potential in order to create strong and healthy Ottoman young men. In order to achieve this, the Ottoman government created the Physical Training Inspectorate and the position of Inspector to teach physical training to Ottoman teachers and oversee the integration of physical training classes in all state schools throughout the imperial domains. The second area of inquiry is Istanbul’s multilingual physical culture press, which consisted of illustrated magazines, daily newspapers, and sports periodicals. These experimental publications constituted a public forum that offered an aspiring middle-class textual and visual instructions on how to cultivate a modern male subject and community on the soccer field, in the gymnasium, and at the sports club. The final area of inquiry is the public display of sports in newly constructed urban spaces. Two spaces in particular, theatres and stadiums, served as the venues in which schools, athletic associations, and the state exhibited physical dexterity, celebrated sports as civilized activities, and redefined communal divisions separating Muslims, Christians, and Jews during the period.

Çalışmada Boyabat şehrinin kuruluşundan günümüze dek geçirmiş olduğu gelişim süreci, gelişimde etkili olan faktörler, gelişim sonucu ortaya çıkan sorunlar incelenmiştir. Boyabat İlçe Merkezi’nde 1970’li yıllardan sonra sanayi ve hizmet sektöründeki gelişmelere bağlı olarak nüfus artmaya başlamış ve yerleşme hızlı bir büyüme sürecine girmiştir. Şehirsel fonksiyonların belirginleşmeye başlamasıyla İlçe Merkezi şehir özelliği kazanmış ve yerleşim alanları genişlemiştir. Bu yıllarda doğu-batı yönünde uzanan Adnan Menderes Bulvarı’nın açılmasıyla büyüme bu bulvar çevresinde yoğunlaşmıştır. Topoğrafik yapının uygun olmasına bağlı olarak yerleşim alanları doğuya doğru genişlemiş bu yöndeki Saraydüzü (D030) ve Durağan (D785) yolları büyüme üzerinde etkili olmuştur. İlgili kurumlardan temin edilen veriler ve Google Maps üzerinden elde edilen görüntüler ArcGıs 10.8 programına işlenerek yerleşmenin gelişimi için dijital veri tabanı oluşturulmuş, elde edilen bulgular ışığında harita, tablo ve şekillerle büyümenin yatay ve dikey gelişimi analiz edilmiştir. Yatay ve dikey yöndeki büyümeye bağlı olarak başta tarım alanlarında betonlaşma olmak üzere hava kirliliği, otopark ve ulaşım sorunları ortaya çıkmıştır., In the study, the development process of the city of Boyabat from its foundation to the present, the factors that were effective in the development, the problems that emerged as a result of the development were examined. In Boyabat District Center, the population started to increase after the 1970s, depending on the developments in the industry and service sector, and the settlement entered a rapid growth process. As the urban functions began to become evident, the District Center gained the character of a city and the residential areas expanded. During these years, with the opening of Adnan Menderes Boulevard, which extends in the east-west direction, growth has been contentrated around this boulevard. Depending on the suitability of the topographical structure, the residential areas have expanded to the east, and the Saraydüzü (D030) and Durağan (D785) roads in this direction have had an impact on the growth. The data obtained from the relevant institutions and the images obtained from Google Maps were processed into the horizontal and vertical development of growth was analyzed in the light of the findings obtained. Due to the horizontal and vertical growth, air pollution, parking and transportation problems have arisen, especially concreting in agricultural areas.
