Exergy is an energy quality measure used to determine the sustainability of a system and its efficiency. The low exergy (Lowex) approach is useful to get sustainability goal in building sector. The energy and exergy flow from primary energy source to the building envelope from the are determined by this approach which is beneficial for improving energy and exergy use at all stages of the heating system in a building. In this study, a comparative evaluation of the energy and exergy performances of the different heating system alternatives for a three storey building, which has the same architectural features but is considered to be in Izmir and Paris, was made by the help of Lowex approach. The considered six heating system options are bio-mass/wood, a standard boiler, an electric boiler, a solar thermal collector, a ground source heat pump, and an air source heat pump. Various indicators such as energy and exergy efficiency, sustainability index and flexibility ratio are used in analyses and evaluations. In terms of total energy and exergy efficiency, it has been seen that the solar collector heating system alternative is the best option for both cities.

Hipertansiyon, dünya genelinde erişkinlerde ölüme neden olan risk faktörleri arasında birinci sırada yer almaktadır. 2007 ve 2017 yıllarında geçerli olan bu durumun 2040 yıllarında da süreceği öngörülmekte olup, yetişkin bireylerde en önemli halk sağlığı sorunu olarak karşımıza çıkmaktadır. Günümüzde hipertansiyonun risk faktörü olarak ön plana çıkmasının ana nedeni;erişkinlerde en sık görülen kronik hastalık risk faktörü olması ve oldukça sessiz bir sağlık sorunu olduğunu söylemek mümkündür. Bu nedenle gerek tespit edilmesinde gerekse tedavisinin sürdürülmesinde “belirsiz oluşu” nedeni ile gecikmeler ve sorunlar yaşanmaktadır.Bu noktada hipertansiyonlu hastaların bakımında hemşirenin rolü hipertansiyon yönetiminin her aşamasında önemli bir yere sahip olmaktadır. Nitekim hemşirelerin hipertansiyon yönetimindeki başlıca rollerini; tanılama, ilaç yönetimi, hasta eğitimi danışmanlık ve beceri geliştirme, bakımın koordinasyonu, toplum sağlığı yönetimi ve performans ölçümü ve kalite iyileştirme olarak sıralamak mümkündür. Ayrıca hipertansiyon hastalara kronik bir hastalığa sahip olma psikolojisini yaşatmakta ve kişinin yaşam şeklini değiştirmesini gerektirdiğinden zorluk yaratmakta olup tüm bu nedenlerle bireyin ve ailesinin yaşam kalitesini de olumsuz etkilemektedir. Dolayısıyla hipertansiyonda hemşirelik yönetimine yönelik kanıt temelli çalışmaların yapılması, klinik hemşirelerinin hipertansiyon hastalarında bakım yönetimindeki rollerini aktif olarak yerine getirmeleri hipertansiyonda hemşirelik bakımı ve uygulamaları açısından önerilmektedir. Derleme türünde olan bu çalışma ile etkili bir hipertansiyon yönetiminde hemşirenin rollerini güncel bilgiler ışığında vurgulamak amaçlanmıştır. Çalışmanın literatüre katkı sağlaması beklenmektedir., Hypertension ranks first among the risk factors that cause death in adults worldwide. This situation, which is the case for 2007 and 2017, is expected to last until 2040 and is the most important public health problem in adult individuals. Today, it is possible to say that the main reason for hypertension to come to the forefront as a risk factor is that it is the most common chronic disease risk factor in adults and it is a rather silent health problem.Therefore, delays and problems are experienced in both its detection and maintenance of treatment due to its “symptom-free” nature. At this point, the role of the nurse in the care of patients with hypertension has an important place at every stage of hypertension management. In fact, the main roles of nurses in the management of hypertension are diagnosis, drug management, patient education, counseling and skill development, coordination of care, community health management and performance measurement, and quality improvement. In addition, hypertension causes patients to experience the psychology of having a chronic disease and creates difficulties because it requires the person to change their lifestyle, and for all these reasons, it negatively affects the quality of life of the individual and his family. Therefore, it is recommended to carry out evidence-based studies on nursing management in hypertension and that clinical nurses actively fulfill their roles in care management in hypertension patients in terms of nursing care and practices in hypertension. The aim of this study, which is a compilation, is to highlight the roles of nurses in effective hypertension management in the light of up-to-date information. The study is expected to contribute to the literature.

Tip 1 diyabet çocukluk çağı kronik hastalıklarının başında gelmekte olup düzenli metabolik kontrol ve takip gerektiren aynı zamanda çocuk, aile ve diyabet ekibi için kontrolü son derece güç olan bir durumdur. Diyabet takibi ve kontrolünü zorlaştıran en önemli neden ise her yaş döneminde ayrı, farklı bakım ve eğitim stratejilerine gereksinim duyulmasıdır. Özellikle çocukluk dönemindeki takip ve kontrolden, erişkin dönemdeki takip ve kontrole geçiş aşamasında bazı tedbirler alınması, takibin devamının sağlanması ve bu sürecin sorunsuz atlatılması ergen, aile ve diyabet ekibi için büyük önem taşımaktadır. Bu çalışma, adölesan diyabetlinin erişkin döneme geçişinde yaşadığı psikososyal sorunlar ve kaygı durumlarını belirlemek üzere Haziran-Temmuz 2016 tarihleri arasında niteliksel olarak yürütülmüştür. Veriler görüşme formu, Beck Depresyon Ölçeği(BDÖ), Durumluluk ve Süreklilik Kaygı Ölçeği ve derinlemesine görüşme yöntemi kullanılarak toplanmıştır. Düzenli kontrollere gelen, daha önce bireysel ve grup eğitimi-ileri düzey karbonhidat sayımı eğitimi almış, insülin pompası (9/16) (%56,2) çoklu insulin rejimi kullanan (7/16(%43,7), 18-24 yaş arası araştırmaya katılmayı kabul eden 16 diyabetli ergen çalışmaya dahil edilmiştir. Çalışma grubuna sorular yöneltilerek, alınan yanıtlar ses kayıt cihazı ile kayıt altına alınmış, elde edilen kayıtların yazılı dökümü çıkarılmıştır. Verilerin analizinde içerik analiz yönteminin yanı sıra frekans ve yüzde dağılımı gibi tanımlayıcı istatistiklerden faydalanılmıştır. Görüşme içerik analiz gruplamasına göre 5 tema, 13 ana kategori, 52 tematik ünite oluşturulmuştur. BDÖ'ye göre çalışmaya katılanların %25,0'i depresif olarak ve STAI FORM TX-I' ye göre %67'si ve STAI FORM TX-II'ye göre %68,7'si kaygılı olarak tespit edilmiştir. İçerik analizi sonuçları normbağımlı değerlendirmelerden elde edilen bulguları destek nitelikte olup, adölesanlar erişkin kliniğine geçişe ilişkin çok çeşitli alan ve konularda kaygılarını belirtmişlerdir. Çalışmadan elde edilen bulgulara göre, iyi yapılandırılmış bir geçiş eğitim programına ihtiyaç olduğu ve ön hazırlıkla bu kaygıların azalacak hatta ortadan kalkacağı düşünülmektedir. Geçiş protokolleri ve rehberlerinin, çocuk ve erişkin kliniklerinde uygulanarak bakımda ortak dilin oluşturulması sağlanmalıdır. Çocukla erişkin arasında geçiş polikliniği oluşturulmasına ve adölesanların bu poliklinikte takip edilmesine ihtiyaç duyulmaktadır. Araştırma bulguları, literatür eşliğinde yorumlanmıştır. Anahtar Kelimeler: Tip 1 diyabet,kronik hastalık,ergenlik dönemi, erişkine geçiş dönemi Type I diabetes mellitus (DM-I)is the most frequently observed chronic disease in childhood, requiring prolonged metabolic monitoring and regular check-ups that can be quite demanding for the caregivers, medical team and, ultimately, the children themselves.The most important factor behind the challenges in monitoring and controlling diabetes is that every age group necessitates separate areas of expertise. It is especially important for adolescent patients, their families and medical team, for instance, to adjust to and take necessary precautions regarding changing caring conditions when proceeding from childcare to adult care.This study focused on identifying psychosocial problems faced by late adolescents moving on to young adulthood, and employed a combination of quantitative as well as qualitative techniques in data gathering during June-July, 2016. Tools for data gathering included interview forms, Beck Depression Inventory (BDI),State-Trait Anxiety Inventory (STAI) and in-depth interviewing. Study sample involved 16 voluntary adolescent patients with DM-I, who underwent regular check-ups, finished advanced carbohydrate counting training in both individual and groupsettings, were using insulin pumps (9 out of 16; 56.2%) or multiple daily insulin regimens (7 out of 16; 43.7%), and were aged between 16 and 24 years old. Researchers directed various questions to the study group and recorded answers in audio; then,these answers were transcribed into print-out forms which were eventually analysed with descriptive statistics, involving content analysis as well as assignment of frequencies and percentage distributions.Content analysis was done by grouping data into 5 themes, 13 main categories and 52 thematic sections. According to the BDI data 25.0% of participants demonstrated identifiable depression profiles, and STAI TX-I and TX-II forms demonstrated identifiable anxiety profiles in 67.0% and 68.7% of participants, respectively. Normative assessment of content analysis findings were in line with the indications of findings, which means thatadolescents experience a number of problems in various areasand topics when proceeding to adult care sections. Findings of this study suggest that well organized education programmes on the transition from child to adult care are essential for late adolescent patients with DM-I; and with adequate preparations the anxiety levels these patients are currently suffering can be ameliorated, even eliminated.Transition protocols and consultants need to be implemented in child and adolescent care to support mutual understanding and dialogue. This means that a transition polyclinic service needs to be formed to fill the gap between child and adult care services, also to conduct medical monitoring of transition patients.Research findings were discussed with respect to current literature on the subject. Girişimcilik kavramı sanayi toplumundan bilgi toplumuna geçişle birlikte, özellikle son yıllarda üzerinde önemle durulan konuların başında gelmektedir. Hemşirelik mesleğinde girişimcilik tam olarak yeni bir kavram olmayıp, 20. yüzyılın başında hemşireler hastalara bakım vermek için bağımsız olarak çalışmışlardır. Problem çözme becerisi her insanın hayatını başarılı bir şekilde devam ettirebilmesi için gerekli özelliklerden biri, hemşirelik uygulamalarının ise odak noktasıdır. Bu çalışma İstanbul'da faaliyet gösteren bir grup hastanesinde çalışan hemşirelerin girişimci kişilik özellikleri, problem çözme beceri düzeyleri ve bu faktörlerin arasındaki ilişkiyi belirlemek amacıyla gerçekleştirildi. Verilerin normal dağılıma uygunluğu Tek Örneklem Kolmogorov Smirnov testi ile test edildi ve anlamlılık değerleri 0,05'den küçük olduğu için ileri düzey çözümlemelerde parametrik olmayan testler kullanıldı. İki bağımsız değişken için Mann-Whitney testi, ikiden fazla bağımsız değişken için Kruskal-Wallis testi, ilişki analizleri için ise Spearman's korelasyon katsayıları kullanıldı.Çalışmada veri toplama aracı olarak anket yöntemi ile 410 hemşireden veri toplandı. Verilerin toplanması için hemşire bilgi formu, Problem Çözme Envanteri, Girişimci Kişilik Özellikleri Ölçeği ve Girişim Fırsatlarını Görme Ölçeği kullanıldı. Katılımcıların yaş ortalamasının 29,50 ± 6,205 olduğu, %81,5'inin kadın olduğu görüldü.Araştırmanın sonucunda hemşirelerin problem çözme becerileri orta düzeyde 93,03 ± 20,794 bulundu. Girişimci Kişilik Özellikleri Ölçeğinde en düşük puanı 2,23 ± 0,655 `Belirsizliğe Karşı Toleranslı Olma` alt boyutu alırken, en yüksek puanı 4,39 ± 0,571 `Başarı İhtiyacı`nın aldığı görüldü. Problem Çözme Envanteri puanları ile Girişimci Kişilik Özellikleri Ölçeği ve Girişim Fırsatlarını Görme Ölçeği puanları arasında negatif yönde çok düşük düzeyde, Girişimci Kişilik Özellikleri ile Girişim Fırsatlarını Görme Ölçeği puanları arasında pozitif yönde düşük düzeyde ilişki olduğu saptandı. The concept of entrepreneurship is one of the important issues that is particularly focused in the recent years with the shift from industrial society to knowledge society. The idea of entrepreneurship in nursing profession is not a new concept, rather it was studied independently at the beginning of the 20th century so that nurses could provide patients with care. Problem solving skills are important properties that each individual needs to continue their life, however, they are at the focal point of nursing profession. The aim of this study was to identify problem solving skill levels, entrepreneurial personality traits of the nurses who work in group company-owned hospitals in Istanbul. The suitability of normal distribution was examined through Kolmogorov-Smirnov test. Non-parametric tests were used in advanced level analyses since the values of significance were less than 0.05. Mann-Whitney test was used for two independent variables, Kruskal-Wallis test for multiple variables, and relation analyses were carried out with Spearman's correlation coefficients.Survey method was used in the study. Data was collected from 410 nurses using nurse information form, Problem Solving Inventory, Entrepreneurial Personality Traits Scale, and Perception of Entrepreneurship Opportunities Scale. The mean age of the participants was 29.50 ± 6.205 and 81.5% of them were female.We found that problem solving skills of nurses is medium-level (93.03 ± 20.794). As the lowest point (2,23 ± 0,655) was related to subdimension of `Being Tolerant For Uncertainty` under Entrepreneurial Personality Traits Scale, the highest point (4.39 ± 0.571) was related to `Need For Success`. The results showed that there was a a little relationship between Problem Solving Inventory points and Entrepreneurial Personality Features Scale points, Problem Solving Inventory points and Perception of Entrepreneurship Opportunities Scale points were negatively related while Entrepreneurial Personality Features Scale points and Perception of Entrepreneurship Opportunities points were positively related. 244

Quality defied as extent of features of any product or service to meet expectations of consumers enjoying such product or service. Although it was a concept fist emerged in manufacturing industry, today, it has become an important competitive factor for service institutions as well. Total Quality Management approach is drawing more and more interest as international borders start to fade out in parallel with globalization and increasing competition. Total Quality Management is a management style that aims to meet customer expectations fully by adopting the prevention focused approach and placing the human element at the heart of all activities to achieve this goal. This study aims to present a compilation of healthcare professionals (n=150) working in Istanbul, Ankara, Diyarbakır, Erzurum and identify factors associated with opinions on the Total Quality Management. A questionnaire was used for measuring perception about the Total Quality Management. Healthcare professionals working in Istanbul and Ankara have a stronger confience that the Total Quality Management will be a helpful management tool in raising quality level of services offered by hospitals and regards quality a responsibility shared by all, compared to those working in Erzurum and Diyarbakır. The study is planned to be further extended to different provinces in order to compare perceptions of healthcare professionals by provinces.

Heating, cooling and lighting appliances in buildings account for more than one third of the world’s primary energy demand. Therefore, main components of the building heating systems play an essential role in terms of energy consumption. In this context, efficient energy and exergy utilization in HVAC-R systems has been very essential, especially in developing energy policies towards increasing efficiencies. The main objective of the present study is to assess the performance of a family house with a volume of 326.7 m3 and a net floor area of 121 m2, located in the city of Izmir, Turkey in terms of energetic, exergetic and sustainability aspects. The indoor and exterior air temperatures are taken as 20°C and 1°C, respectively. In the analysis and assessment, various metrics (indices or indicators) such as exergetic efficiency, exergy flexibility ratio and sustainability index are utilized. Two heating options (Case 1: condensing boiler and Case 2: air heat pump) are considered for comparison purposes. The total heat loss rate of the family house is determined to be 3770.72 W. The overall energy efficiencies of the studied cases are calculated to be 49.4% for Case 1 and 54.7% for Case 2. The overall exergy efficiencies, the flexibility factor and the sustainability index of Cases 1 and 2 are computed to be around 3.3%, 0.17 and 1.034, respectively., {"references":["A. M. öz 1992 yılında kurulan İzmir Yüksek Teknoloji Enstitüsü İzmir'in 3. devlet üniversitesidir. Gelişim içerisindeki kampüste her fakülte kendisine ait fuel-oil yakıtlı ısıtma sistemine sahiptir. Kampus aynı zamanda bir jeotermal kaynağa sahiptir. 2002 yılında 5 gradyen kuyusu açılmış ve bunların birinden 33°C sıcaklığında jeotermal akışkan elde edilmiştir fakat akışkanın gerçek debisi henüz ölçülmemiştir. Tezin ana amacı, bu kaynağın İzmir Yüksek Teknoloji Enstitüsü Kampusu için bölgesel ısıtma sistemi uygulamasında kullanılıp kullanılmayacağının araştırılmasıdır. Başlıca iki tip ısıtma sistemi düşünülmüştür;. Isı pompalı ısıtma sistemi (IPIS) (yenilenebilir bir enerji kaynağı kullanarak- jeotermal enerji).. Kazanlı ısıtma sistemi (KIS) (konvensiyonel bir enerji kaynağı kullanarak-fuel-oil). Mevcut jeotermal akışkan sıcaklığı düşük olduğundan IPIS, HPO tipinde düşünülmüştür. IPIS sadece bölgesel olarak düşünülürken. KIS bölgesel ve bağımsız olarak iki alternatif altında incelenmiştir. Her bir ısıtma sistemi saatlik dış hava sıcaklıkları kullanılarak simule edilmiştir. Simulasyonlarda ana kontrol parametresi binaların iç hava sıcaklığıdır. Matematik modeller, Matlab ve EES programlan kullanılarak oluşturulmuştur. IPIS'nde, farklı kondenser çıkış sıcaklıkları ve jeotermal akışkan debileri için, çeşitli ısıtma rejimi alternatifleri çalışılmıştır. Sonuçta, 35°C kondenser giriş. 45°C kondenser çıkış sıcaklıkları 120 kg/s jeotermal akışkan debisi ile birlikte en iyi seçenek olarak belirlenmiştir. Tezde KISleri de, değişik kazan set sıcaklıkları için simule edilmiştir. Demirdöküm tarafından önerilen kazan set sıcaklıkları, düşük fuel-oil tüketimi ve 20°C'nin etrafında en iyi iç hava sıcaklığı ile en iyi alternatiftir. Isıtma sistemi simulasyonunun yanında, boru hattı simulasyonu da Pipelab programı kullanılarak yapılmıştır. Boru hattı kampus ve jeotermal olmak üzere iki çevrimden oluşmaktadır. Besleme ve dönüş hatlarını içeren her çevrim toprak altında gömülüdür. Sistemin ekonomisi için ısı merkezinin yeri ve birim boru boyundaki basınç kaybı genel dizayn parametreleridir. Bu nedenle çeşitli alternatifler çalışılmış ve endüşük yatırım ve işletme maliyetlerinden dolayı ısı merkezi kampusun ortasında olan 3. alternatif 1 50 Pa/m basınç düşümü hedefi ile en iyi seçenek olarak düşünülmüştür. Ayrıca, her bir ısıtma sistemi alternatifi için yatırım ve işletme maliyetlerine bağlı olarak ekonomik analiz de yapılmıştır. İşletme maliyeti için, kampüsteki binaların ısıtma periyotlara bağlı olarak 3 ısıtma senaryosu düşülmüştür. Ekonomik analizlerin sonuçlarına göre, ısı pompalı bölgesel ısıtma sistemi (IPBIS) 3,040,125 US$ ile en büyük yatırım maliyetine sahipken, en düşük işletme maliyetine de sahiptir. Alternatifler, sistemlerin yatırımın karlılığını gösteren iç karlılık oranı (IKO) metoduna göre değerlendirilmiştir. Ekonomik analiz sonuçlan, IPBIS'nin 20 yıllık periyodun sonunda fuel oil kazanlı bölgesel ısıtma sistemine (FKBIS) göre en az %3.02 oranında karlı olduğunu göstermektedir. Isıtma sistemlerinin işletme periyodunun artması ile kar yükselmektedir. vı ABSTRACT Izmir Institute of Technology (IZTECH), founded in 1992, is the third state university of İzmir. At present IZTECH Campus has individual fuel boiler heating systems for each faculty building and the Campus is still under development. But the Campus has also a geothermal source. In 2002. 5 gradient wells were drilled. Of these, one well has a geothermal fluid of 33°C is obtained but the actual flowrate of the geothermal fluid has not been measured yet. The aim of this Thesis is to investigate this source whether it can be used for district heating application for IZTECH Campus. Mainly two heating system types have been considered;. Heat pump heating system (HPHS) (using a renewable energy source, geothermal energy).. Fuel boiler heating system (FBHS) (using a conventional energy source, fuel-oil). HPHS is considered as HPO type since the existing geothermal fluid temperature is low. While HPHS is considered only as district, FBHS is considered as district and individual. Each heating system is simulated using hourly outdoor temperature data. For these heating simulations, the main control parameter is the indoor temperature of the buildings. Mathematical models are derived using Matlab [16] and EES [17] programs. Various heating regime alternatives have been studied for HPHS for the various condenser outlet temperature and geothermal fluid flowrate. Consequently, the heating regime with 35°C condenser inlet and 45°C condenser outlet temperature with 120 kg/s geothermal fluid flowrate considered to be the best option. FBHSs are also simulated for various boiler set temperatures. Boiler set temperatures have been recommended by Demirdöküm [39], is the best alternative with the least fuel consumption and best indoor temperature around 20°C. Besides heating system simulations, piping network simulation is made using the software Pipelab [1 8]. The piping network of the Campus has been considered with two loops as geothermal and Campus. Each loop contains supply and return main. The location of the heat centre and the pressure loss per unit length are common design parameters' for economy of the system. Therefore, several alternatives have been studied inand because of the lowest investment and operational cost, Alternative 3. where the heat centre is in the middle of the Campus, is considered to be the best option with target pressure loss of 150 Pa/m. For installation type of piping network, underground (buried) pipeline installation is selected. Furthermore, economic analysis has also been done for each heating system alternative depending on investment and operational costs. For operational cost. 3 heating scenarios are considered depending on the heating period of the buildings in the Campus. According to the results of economical analyses, while heat pump district heating system (HPDHS) has the biggest investment cost with 3,040,125 US$. it has minimum operational cost. The alternatives are evaluated according to internal rate of return (IRR) method, which shows the profit of the investment. The results indicate that, the HPDHS has minimum 3.02% profit according to the fuel boiler district heating system (FBDHS) at the end of the 20-year period.
