Objective: The aim of this study was to evaluate the relationship between maternal serum PAPP-A (pregnancy-associated plasma protein A) and free ß-hCG (human chorionic gonadotropin) values with fetal birth weight. Material and Methods: 554 patients who applied to Yıldırım Beyazit University Atatürk Training and Research Hospital Obstetrics Clinic between January 2009 and December 2016 for first trimester combined screening tests were included in the study. Antenatal follow-up and pregnancy outcomes were examined. The relationship between adjusted multiples of median (MoM) values of maternal serum PAPP-A and free ß-hCG levels and fetal birth weight (FBW); small for gestational age (SGA--fetal birth weight <10th percentile) and large for gestational age (LGA--fetal birth weight > 90th percentile) were analyzed. Results: Maternal serum PAPP-A level was found to have a relationship with SGA development (p=0.03). Every decrease per unit in the adjusted MoM value of maternal serum PAPP-A level increases the risk of SGA development three times (RR, 1/0.333; 95% CI, 0.1-0.8, p=0.030). Conclusion: Correlation between PAPP-A value and SGA was found to be statistically significant. However, a similar prediction was not eligible between maternal serum PAPP-A level and LGA fetuses. On the other hand, maternal serum free ß-hCG level did not have a statistical significance as a predictor for both SGA and LGA fetuses. [ABSTRACT FROM AUTHOR]

Objectives: Urinary incontinence (UI) is a prevalent health issue affecting women's quality of life and sexual function. This study investigates the impact of different types of UI on sexual function using the Pelvic Organ Prolapse/Urinary Incontinence Sexual Function Questionnaire short form (PISQ-12). Methods: A total of 123 sexually active women with stress, urge, or mixed-type UI, who presented to the Urogynecology clinic of Etlik City Hospital between May 2023 and September 2023, were included in the study. Patients with a history of pelvic floor surgery or pelvic organ prolapse were excluded. The PISQ-12 short form, validated in Turkish, was administered. Statistical analysis was performed using SPSS 21 software. Results: The study population had a mean age of 48.2±10.5 years and a mean BMI of 30.0±4.6 kg/m². The average duration of UI was 5.4±3.8 years. Mixed-type UI was the most common (46.3%), followed by stress UI (36.5%) and urge UI (17.2%). The mean total PISQ-12 score was 27.1±6.7, with the lowest scores observed in the mixed-type UI group (25.1). Coital incontinence was reported in 46% of the patients. Statistically significant differences were observed in coital incontinence and coital incontinence anxiety between groups with poor and better sexual function (p<0.05). However, there were no statistically significant differences in PISQ-12 scores between the types of UI. Additionally, no significant difference in sexual dysfunction was found between premenopausal and postmenopausal groups. Conclusion: Urinary incontinence significantly impacts women's physical, psychosocial, and sexual health, necessitating thorough evaluation and treatment to improve quality of life and sexual function. Addressing UI and its associated sexual dysfunctions in clinical practice is crucial, despite the challenges in diagnosing these conditions. [ABSTRACT FROM AUTHOR]

Objective: Second to fourth digit (2D:4D) ratio is used as an indicator of androgen exposure in fetal life. It is known that the differentiation of urogenital tract and finger length are both controlled by Homeobox genes (HoxA and HoxD). Therefore, factors that are connected with anthropometric characteristics of fingers are expected to be in relation with urogenital anatomy, depending on genetic and hormonal basis. Aim of the study is to evaluate the correlation between 2D:4D ratio and urethrovaginal space measurements. Materials and methods: In this prospective cohort study, digital anthropometric measurements of hands and ultrasonographic measurements of urethrovaginal space were evaluated and the correlation between these parameters were assessed in a sample of women in reproductive period. Results: A significant negative correlation was detected between both right and left hand 2D: 4D ratio and length of urethrovaginal space (p=0.007 and p=0.003, respectively). The thickness of urethrovaginal space at proximal, middle and distal segment was also correlated negatively with 2D:4D ratio for both hands. Conclusion: The findings of the current study have shown a negative correlation between 2D: 4D ratio and urethrovaginal space measurements. The insights gained from these data will serve as a base for future research about the relationship between intrauterine androgen exposure and female genital anatomy. Objective: To evaluate the relationship between entanglement of umbilical cord around the fetal neck and adverse perinatal outcomes. Methods: In this prospective cohort study, perinatal outcomes of 218 pregnancies complicated with nuchal cord (NC) (study group) were compared with 190 uncomplicated pregnancies (control group). Main outcome measures were umbilical cord pH values, APGAR scores and cesarean section (C/S) rates. Fetal distress was stated as an abnormal heart rate pattern on electronic fetal heart monitorization. Results: There were no statistically significant differences in maternal demographic and obstetric features, between groups. There were no statistically significant differences regarding C/S rates between groups, even though fetal distress was significantly the leading indication for cesarean delivery, in the study group (p = 0.021). The number of entanglement was significantly related with fetal distress (p < 0.001). There were no statistically significant differences in umbilical cord gas values, one-minute and five-minute APGAR scores between the groups. Furthermore, there was a significant male dominance in the study group (p = 0.014) and also, amniotic fluid indexes (AFI) were significantly higher in this group (p = 0.002). Conclusion: This study demonstrated that, entanglement of umbilical cord around the fetal neck or NC is not related with adverse perinatal outcomes such as acidosis and low APGAR scores. So that, a targeted care on NC via ultrasound during labor, is not an essential part of the examination. [ABSTRACT FROM AUTHOR]

Objective: To present our experiences with an innovative surgical approach for vaginal agenesis that uses amnion as the allograft to create a functional neovagina., Study Design: This study involved 5 patients with diagnosed vaginal agenesis. One patient had karyotype 46,XY and was diagnosed with complete androgen insensitivity syn- drome, whereas the others had karyotype 46,XX and were diagnosed with Mayer-Rokitansky-Kiister-Hauser syndrome. All patients underwent Mclndoe vagino- plasty using amnion and were followed up at 2 weeks, 3 months, 6 months, and 12 months postsurgery. Functionality of the neovagina was assessed by Rosen Female Sexual Function Index (FSFI) questionnaire., Results: At the 12-month follow-up 1 patient report- ed. an inability to use the vaginal dilator effectively; for the other 4 patients the mean depth of the neovagina was 9.5 cm and the mean width was 3.5 cm. The mean overall FSFI score was 28.3., Conclusion: McIndoe vaginoplasty is a simple, safe, and quick operative method that results in a ftnctional vagina. The amnion is a suitable material to apply to the surface of the neovagina because it is readily avail- able and does not have any associated immune reiection problems or costs.
