1. The influence of waiting times and sociopolitical variables on public trust in healthcare: A cross-sectional study of the NHS in England

Abstract: Objectives: This study aims to assess factors influencing public trust in the National Health Service (NHS) in England, focusing on the impact of waiting times in Accident & Emergency (A&E) departments and for GP-to-specialist cancer referrals. Study design: A cross-sectional survey-based research design was employed, covering the period from July 2022 to July 2023. Methods: Data were collected through YouGov surveys, yielding 7415 responses. Our analysis is based on 6952 of these responses which we were able to aggregate to 42 NHS Integrated Care Boards (ICBs) for A&E waiting times and 106 ICB sub-units for cancer referral times. Multiple regression analysis was conducted, with the dependent variable being trust in the NHS. Results: Waiting times for A&E and cancer referrals did not significantly affect trust in the NHS. However, other sociopolitical factors displayed significant influence. Specifically, being a member of an ethnic minority group, or having voted Conservative in the 2019 general election were associated with lower trust scores. Other variables such as age and local unemployment rate were also significant predictors. Conclusions: Our findings suggest that waiting times for healthcare services have no effect on public trust in the NHS. Instead, trust appears to be largely shaped by sociopolitical factors. Policymakers should therefore look beyond operational efficiency when seeking to bolster trust in the healthcare system.

Abstract: Background: The programmed cell death protein 1 (PD-1)/programmed cell death-ligand 1 (PD-L1)/programmed cell death-ligand 2 (PD-L2) axis is responsible for cancer immune escape, which facilitates disease progression. However, the role of PD-L1 and PD-L2 and tumor-infiltrating lymphocytes (TILs) in metastatic colorectal cancer (mCRC) has not been studied. Materials and methods: We conducted a post-hoc analysis of the Nationwide Cancer Genome Screening Project GI-SCREEN in mCRC. PD-L1 (22C3) and PD-L2 (MEB123.3G2.038) expression in formalin-fixed paraffin-embedded tumor samples was centrally assessed by immunohistochemical assays. TILs were morphologically evaluated using hematoxylin and eosin staining. Clinical information was extracted from the GI-SCREEN database. Inclusion of patients with BRAF V600E mutation was prioritized. Results: Two hundred patients with mCRC (median age 65 years and 116 males) were included in the study. Genomic testing identified RAS mutations in 87 (44%) patients, BRAF V600E mutations in 27 (14%), and microsatellite instability-high status in 8 (4%). Positivity of PD-L1 and PD-L2 was 11% and 47% on tumor cells (TC) and 0% and 64% on immune cells, respectively, and that was associated with the presence of TILs (P = 0.011 for PD-L1, 0.024 for PD-L2). PD-L1+ TC was significantly more frequent in BRAF V600E-mutated tumors (P = 0.03). Even in microsatellite stable tumors, BRAF V600E-mutated tumors were significantly associated with higher expression of PD-L1 on TC than BRAF wild-type (25% versus 8%, P = 0.02). Conclusions: Our study showed a distinct pattern of PD-L1 expression on TC of patients with BRAF V600E-mutated mCRC, which could be a potential therapeutic target for PD-1 blockade.

Abstract: The Cherenkov Telescope Array (CTA) is a next-generation ground-based observatory for gamma-ray astronomy at very high energies. The Large-Sized Telescope prototype (LST-1) is located at the CTA-North site, on the Canary Island of La Palma. LSTs are designed to provide optimal performance in the lowest part of the energy range covered by CTA, down to ≃20 GeV. LST-1 started performing astronomical observations in 2019 November, during its commissioning phase, and it has been taking data ever since. We present the first LST-1 observations of the Crab Nebula, the standard candle of very-high-energy gamma-ray astronomy, and use them, together with simulations, to assess the performance of the telescope. LST-1 has reached the expected performance during its commissioning period—only a minor adjustment of the preexisting simulations was needed to match the telescope’s behavior. The energy threshold at trigger level is around 20 GeV, rising to ≃30 GeV after data analysis. Performance parameters depend strongly on energy, and on the strength of the gamma-ray selection cuts in the analysis: angular resolution ranges from 0.°12–0.°40, and energy resolution from 15%–50%. Flux sensitivity is around 1.1% of the Crab Nebula flux above 250 GeV for a 50 hr observation (12% for 30 minutes). The spectral energy distribution (in the 0.03–30 TeV range) and the light curve obtained for the Crab Nebula agree with previous measurements, considering statistical and systematic uncertainties. A clear periodic signal is also detected from the pulsar at the center of the Nebula.

Abstract: Since the last IAEA conference, the scientific programme of JET has focused on the qualification of the integrated operating scenarios for ITER and on physics issues essential for the consolidation of design choices and the efficient exploitation of ITER. Particular attention has been given to the characterization of the edge plasma, pedestal energy and edge localized modes (ELMs), and their impact on plasma facing components (PFCs). 7. Earthquake From the Perspectives of Amputee Children and Their Parents/Caregivers: A Phenomenological Study.

Abstract: Background: Earthquakes cause significant psychological and physical trauma in children, especially when leading to amputations, as they disrupt physical, emotional, and social well-being., Aim: This study was conducted phenomenologically to explore the experiences of children amputated in the Kahramanmaraş earthquake and their parents/caregivers., Method: This study was conducted as a phenomenological study with children who were amputated in the Kahramanmaraş earthquake and their parents/caregivers between August and October 2023. The sample of the study consisted of seven children and their parents/caregivers who met the inclusion criteria and volunteered to participate in the study through purposive sampling method. Data were collected through in-depth interviews using an introductory information form and a semistructured interview form developed by the researcher., Results: It was found that 57.2% of the children who participated in our study were male and their mean age was 11.25 ± 4.02 years. It was found that all the children's houses were destroyed in the earthquake, they were trapped under the debris, and they experienced losses in their family members and relatives. As a result of data analysis, nine themes were identified as apocalypse, pain, fear, and hopelessness for children and apocalypse, helplessness, pain, anger, and hopelessness for parents/caregivers., Conclusions: This study found that earthquake-affected amputee children perceived the earthquake as an apocalypse, experienced prolonged pain during hospitalization, continued to fear the earthquake, and felt hopeless about the future. Abstract: Objective: To investigate the effects of facilitated tucking and white noise on stress and sleep in neonates receiving nasal continuous positive airway pressure (CPAP)., Method: This study was conducted as a randomised controlled experimental study of neonates receiving nasal CPAP in neonatal intensive care units. The study sample consisted of 108 newborns (facilitated tucking (n = 36), white noise (n = 36) and control (n = 36)) receiving nasal CPAP support in the NICU. The neonates' sleep parameters were recorded by actigraphy for 24 h. Data were collected using the Neonatal Descriptive Information Form, the Sleep Tracking Form and the Neonatal Stress Scale. Percentage, mean, chi-squared and one-way ANOVA were used for data analysis., Results: It was found that 50.9% of the newborns were female, their mean gestational age was 33.54 ± 3.38, their mean height was 43.56 ± 5.12, and their mean weight was 2139.23 ± 827.82. The total sleep time of the neonates in the facilitated tucking and white noise group increased by 3 h, their sleep efficiency increased by 20% and their mean stress scores decreased (p < 0.05)., Conclusion: Facilitated tucking and white noise each showed a similar improvement in sleep duration and sleep efficiency and a reduction in stress scores in neonates receiving nasal CPAP. 9. The Effect of Circular and Oscillating Breast Massage on the Amount of Breast Milk Produced: An Innovative Method.

Abstract: Background: In addition to suffering stress and anxiety owing to being separated from their babies, mothers whose babies are hospitalized in neonatal intensive care units suffer breastfeeding problems. Moreover, their breast milk production may decrease over time. Objective: Examining the effect of circular and oscillating breast massage on the amount of breast milk produced. Materials and Methods: This randomized controlled experimental study used a pretest-posttest model. The participants included 120 mothers (experimental group: 60, control group: 60) who went to a breast milk pumping unit of a hospital between December 1, 2017, and July 31, 2019. A circular and oscillating breast massage device was used to massage both breasts for 10 to 15 minutes, three times a day, for 3 days, and milk production was measured daily. The sociodemographic characteristics, perceptions regarding breast massage, and daily breast milk amounts of the mothers were collected. Results: The average age of the mothers that participated in the study was 30.21 ± 5.58 years; the average birth week of their babies was 32.31 ± 4.10 weeks. There were no significant differences in the mothers' amounts of milk production before the procedure in the experimental and control groups. In the study group, milk production increased by 7.92%, 5.25%, and 5.02% on days 1, 2, and 3 of the procedure, respectively. The mothers' total amount of milk production before and after the procedure (3 days) increased by 22.28%. Significant differences were found between the study groups on days 1, 2, and 3 and the total amount of milk produced after the procedure. Conclusion: Our study found circular and oscillating breast massage increased the amount of breast milk produced and decreased breast pain and swelling. Similar studies on the clinical use of this treatment should be conducted in line with the results of this study.

10. The relationship between the care burden and quality of life of parents who have children with hematological problems and their perceived social support.

Abstract: Purpose: The purpose of this study was to examine the interrelationship of the caregiver burden, perceived social support, and quality of life of parents who have children with hematological problems., Design and Methods: The design and methods included a descriptive study with 141 parents of children with hematological problems. The introductory information form, the Zarit Burden Interview (ZBI), multidimensional scale of perceived social support (MSPSS), and the EUROHIS-QOL.8 (WHOQOL-8) scale were used as data collection tools. The data obtained were analyzed using descriptive statistics and structural equation models., Results: As a result of the conducted correlation analysis, it was determined that there was a positive correlation between parents' perceptions of social support and their quality of life, and a negative correlation between their perceptions of social support and the caregiver burden (p < 0.05). As a result of the structural equation model, it was determined that the social support received by the parents had a significant effect on their caregiver burden (β = -0.40; p < 0.05), and their quality of life (β = 0.42; p < 0.05)., Conclusions: The conclusion formed was that it can be said that parents' perceptions of social support affect their caregiver burden and quality of life. Abstract: Purpose: This study was conducted to determine the effect of a new therapeutic model (a therapeutic play/play therapy (TP/PT) program) on alleviating pain in children receiving inpatient treatment in paediatric surgery units and parents' anxiety levels in the post-operative period., Design and Methods: The study was designed as a semi-experimental model using the TP/PT program. Four measurements (pre-test, two interim tests and post-test) for children and two measurements (pre-test and post-test) for parents were performed. The data were collected using the Child and Parent Information Form, the Wong-Baker Faces Pain (WBFP) Rating Scale and the State-Trait Anxiety Inventory (STAI)., Results: The mean pain score for children after TP/PT decreased in each measurement, and a significant difference was found between these measurements. The mean state anxiety score for parents significantly decreased after the TP/PT program, and a statistically significant difference was found between these measurements (p = 0.000)., Conclusions: This study determined that the post-operative TP/PT program was an effective method to reduce acute post-operative pain in children. Abstract: Pentamethylcyclopentadienyl iridium (Cp*Ir) complexes with bidentate ligands consisting of a pyridine ring and an electron-rich diazole ring were prepared. Their catalytic activity toward CO 2 hydrogenation in 2.0 m KHCO 3 aqueous solutions (pH 8.5) at 50 °C, under 1.0 MPa CO 2 /H 2 (1:1) have been reported as an alternative to photo- and electrochemical CO 2 reduction. Bidentate ligands incorporating an electron-rich diazole ring improved the catalytic performance of the Ir complexes compared to the bipyridine ligand. Complexes 2, 4, and 6, possessing both a hydroxy group and an uncoordinated NH group, which are proton-responsive and capable of generating pendent bases in basic media, recorded high initial turnover frequency values of 1300, 1550, and 2000 h -1 , respectively. 14. CO2 Hydrogenation Catalyzed by Iridium Complexes with a Proton-Responsive Ligand.

Abstract: The catalytic cycle for the production of formic acid by CO2 hydrogenation and the reverse reaction have received renewed attention because they are viewed as offering a viable scheme for hydrogen storage and release. In this Forum Article, CO2 hydrogenation catalyzed by iridium complexes bearing sophisticated N^N-bidentate ligands is reported. We describe how a ligand containing hydroxy groups as proton-responsive substituents enhances the catalytic performance by an electronic effect of the oxyanions and a pendent-base effect through secondary coordination sphere interactions. In particular, [(Cp*IrCl)2(TH2BPM)]Cl2 (Cp* = pentamethylcyclopentadienyl; TH2BPM = 4,4',6,6'-tetrahydroxy-2,2'-bipyrimidine) enormously promotes the catalytic hydrogenation of CO2 in basic water by these synergistic effects under atmospheric pressure and at room temperature. Additionally, newly designed complexes with azole-type ligands were applied to CO2 hydrogenation. The catalytic efficiencies of the azole-type complexes were much higher than that of the unsubstituted bipyridine complex [Cp*Ir(bpy)(OH2)]SO4. Furthermore, the introduction of one or more hydroxy groups into ligands such as 2-pyrazolyl-6-hydroxypyridine, 2-pyrazolyl-4,6-dihydroxypyrimidine, and 4-pyrazolyl-2,6-dihydroxypyrimidine enhanced the catalytic activity. It is clear that the incorporation of additional electron-donating functionalities into proton-responsive azole-type ligands is effective for promoting further enhanced hydrogenation of CO2.

15. Formic acid dehydrogenation with bioinspired iridium complexes: a kinetic isotope effect study and mechanistic insight.

Abstract: Highly efficient hydrogen generation from dehydrogenation of formic acid is achieved by using bioinspired iridium complexes that have hydroxyl groups at the ortho positions of the bipyridine or bipyrimidine ligand (i.e., OH in the second coordination sphere of the metal center). In particular, [Ir(Cp*)(TH4BPM)(H2 O)]SO4 (TH4BPM: 2,2',6,6'-tetrahydroxyl-4,4'-bipyrimidine; Cp*: pentamethylcyclopentadienyl) has a high turnover frequency of 39 500 h(-1) at 80 °C in a 1 M aqueous solution of HCO2 H/HCO2 Na and produces hydrogen and carbon dioxide without carbon monoxide contamination. The deuterium kinetic isotope effect study clearly indicates a different rate-determining step for complexes with hydroxyl groups at different positions of the ligands. The rate-limiting step is β-hydrogen elimination from the iridium-formate intermediate for complexes with hydroxyl groups at ortho positions, owing to a proton relay (i.e., pendent-base effect), which lowers the energy barrier of hydrogen generation. 16. o-Phenylene octamers as surface modifiers for homeotropic columnar ordering of discotic liquid crystals.

Abstract: Large-area homeotropic columnar ordering of π-conjugated discotic liquid crystals (LCs) is crucial for certain device applications but generally hard to achieve. Here we report polymeric o-phenylene octamer poly-1 and its monomer 1 as the first surface modifiers for homeotropic columnar order of a variety of discotic LCs up to a macroscopic length scale. Their octameric o-phenylene parts are known to fold helically into a cylinder that is reminiscent of a π-stacked column of discotic LCs. Through-view X-ray diffraction patterns of 1 suggested that this molecule adheres to the glass substrate and directs its cylindrical axis perpendicular to the glass surface. This "face-on" orientation likely nucleates the homeotropic columnar order of discotic LC materials.

Abstract: A series of highly functionalized cyclopentadienyl rhodium(III) complexes, [Cp'Rh(bpy)Br](ClO4) (Cp' = substituted cyclopentadienyl), was synthesized from various multi-substituted cyclopentadienes (Cp'H). [Rh(cod)Cl]2 and Cp'H were firstly converted to [Cp'Rh(cod)] complexes, which were then treated with Br2 to give the rhodium(III) dibromides [Cp'RhBr2]2. The novel complexes [Cp'Rh(bpy)Br](ClO4) were obtained readily by the reaction of 2,2'-bipyridine with [Cp'RhBr2]2. These rhodium complexes [Cp'Rh(bpy)Br](ClO4) were fully characterized and utilized in the hydrogenation of cyclohexanone and acetophenone with generally high yields, but they did not exhibit the same reactivity trends for the two substrate ketones. The different activity of these complexes for the different substrates may be due to the influence of the substituents on the Cp' rings.

19. Photoconductive coaxial nanotubes of molecularly connected electron donor and acceptor layers.

Abstract: Controlled self-assembly of a trinitrofluorenone-appended gemini-shaped amphiphilic hexabenzocoronene selectively formed nanotubes or microfibers with different photochemical properties. In these nanotubes, which are 16 nanometers in diameter and several micrometers long, a molecular layer of electron-accepting trinitrofluorenone laminates an electron-donating graphitic layer of pi-stacked hexabenzocoronene. The coaxial nanotubular structure allows photochemical generation of spatially separated charge carriers and a quick photoconductive response with a large on/off ratio greater than 10(4). In sharp contrast, the microfibers consist of a charge-transfer complex between the hexabenzocoronene and trinitrofluorenone parts and exhibit almost no photocurrent generation.

20. Cooperativity in chiroptical sensing with dendritic zinc porphyrins.

Abstract: Zinc porphyrin-appended dendrimers, 12PZn, 18PZn, 24PZn, and 36PZn, containing 12, 18, 24, and 36 zinc porphyrin units, respectively, were synthesized using zinc porphyrin dyad (2PZn) and triad (3PZn) as precursors. Although these dye-functionalized dendrimers all serve as chiroptical sensors for an asymmetric bipyridine (RR- and SS-Py2), the sensing capability is highly dependent on the structure of the dendritic scaffold. 2PZn, which is chiroptically silent toward Py2, turns cooperative and displays a large ICD (induced circular dichroism) response in the visible region when incorporated into 12PZn. Judging from the extents of contribution of each zinc porphyrin unit to the CD amplitudes ([Deltaepsilonmax]), the cooperativity in 24PZn (112 M-1 cm-1) is lower than that in 12PZn (196 M-1 cm-1) and much lower in dendron 4PZn (59 M-1 cm-1). In contrast, 3PZn, which is ICD-active toward Py2, hardly shows such an enhanced cooperativity when incorporated into 18PZn and 36PZn and dendron 6PZn, as well. Absorption spectroscopy suggests some unique conformational characteristics of the zinc porphyrin units in highly cooperative 12PZn.

21. Successive identification of biodegradation rates for multiple sequentially reactive contaminants in groundwater.

Abstract: At the field scale, the biodegradation rate is usually estimated from analytical solutions to single species transport with first-order reactions, using measured data as input. Because many contaminants, e.g., chlorinated solvents, are degraded in a sequential pattern, with degradation products further reacting to produce new species, it is of great interest to quantify the transformation rate of every reaction. The conventional inverse solutions for identifying the transformation rates are limited to single species problems. In the present study, we propose a successive optimization approach to identify the biodegradation rate for each species by using a previously developed analytical solution to multi-species first-order reactive transport using data obtained at the field scale. By specifying a link between analytical solutions to sequentially reactive transport problems and optimization methods and assuming constant transport parameters (velocity, dispersivities, and retardation factors), the first-order transformation rates are optimized successively from parent species to its daughter species.
