Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). We report nonlinear terahertz (THz) effects in monolayer graphene, giving rise to transmission enhancement of a single-cycle THz pulse when the incident THz peak electric field is increased. This transmission enhancement is attributed to reduced photoconductivity, due to saturation effects in the field-induced current and increased intraband scattering rates arising from transient heating of electrons. We have developed a tight-binding model of the response using the length gauge interaction Hamiltonian that provides good qualitative agreement. The model fully accounts for the nonlinear response arising from the linear dispersion energy spectrum in graphene. The results reveal a strong dependence of the scattering time on the THz field, which is at the heart of the observed nonlinear response.

Objective/background Recently, the indications for thoracic endovascular aortic repair (TEVAR) have been expanding, and the applicability of TEVAR for acute type B aortic dissection (TBAD) is proposed with regard to the high mortality of open surgery for chronic TBAD. TEVAR in the acute phase may lead to remodeling of the false lumen (FL), but it is controversial whether it completely resolves the aortic expansion in the chronic phase. In this study, operative results and the relationship between FL status and the time before surgical intervention were retrospectively analyzed. Methods From January 2008 to September 2013, 234 patients underwent open surgery for chronic TBAD. Most patients were on left heart bypass. By considering Japanese aortic disease treatment guidelines and the smaller physique of Japanese patients, operative indications were aneurysm >50 mm in diameter or rapid aneurysm enlargement of >5 mm in a 6 month period. Results In 180 cases, the FL was patent. The mean interval between onset of TBAD and operation was 61 ± 54 months. There was no significant difference between patients in the patent FL group and those in the thrombosed FL group ( p = .44). Mean ratio of FL diameter to maximum aortic diameter (FL/AD) was 0.64 ± 0.21. There was no correlation between FL and AD before the operation ( r = .12). Descending thoracic aortic replacement (DTAR) was performed in 127 cases and thoracic ascending aortic replacement (TAAR) in 107 cases (Crawford type I, n = 9; Crawford type II, n = 65; Crawford type III and IV, n = 22, respectively; Safi type V, n = 11). The overall operative mortality was 6.8%: 3.9% (5/127) for DTAR and 10.3% (11/107) for TAAR. The three year survival was 86.7, and the freedom from re-intervention rate was 97.0%. Conclusion Enlargement of uncomplicated TBAD in the chronic phase was poorly related to FL status and the results of open repair have improved. However, further prospective study is necessary.

BACKGROUND: Obesity is reported to increase the risk of colorectal cancer. We investigated whether it was associated with mortality or morbidity after colorectal-cancer surgery. METHODS: 70 colorectal-cancer patients who underwent elective surgery were investigated in this study. The intra-abdominal fat (IAF) and subcutaneous fat (SCF) areas were quantified by CT scan preoperatively. We investigated whether body mass index (BMI), IAF, or SCF were associated with postoperative mortality or morbidity. RESULTS: The mortality rate was 0%. 11 (16%) of 70 patients had medical complications (pneumonia, 9; arrhythmia, 2) and 16 (23%) had surgery-related complications (wound infection, 12; anastomotic leakage, 5) postoperatively. There was no significant difference in the postoperative medical or surgery-related morbidity rate between the IAF+ (males, ≥160 cm2; females, ≥120 cm2) and IAF– groups. The surgery-related morbidity rate was significantly higher in the SCF+ group (male, ≥180 cm2; female, ≥250 cm2) than in the SCF– group and multilogistic regression analysis confirmed that diabetes mellitus and SCF were significant for surgery-related morbidity. CONCLUSIONS: Subcutaneous fat accumulation is significantly associated with postoperative surgery-related morbidity after colorectal-cancer surgery.

Two states of flux quantum bit (qubit) with three Josephson junctions are confirmed by a single measurement with a DC-SQUID (superconducting quantum interference device). This qubit is an aluminum superconductor loop surrounded by a DC-SQUID for measurement. The qubit has two states, which have persistent currents flowing in opposite directions. These two states are the ground state of the qubit. The qubit is controlled by changing the external magnetic field. We reduced the external noise to improve the DC-SQUID's sensitivity. The switching current distribution of a DC-SQUID is sufficiently narrow to allow us to distinguish between the two ground states of the qubit. Although quantum coherence stability is required for full qubit operation, the readout values clearly show the possibility of single shot measurement.

1. 目的:血清胆汁酸組成比が大腸癌スクリーニングとして有用であるかを検討した.2. 対象と方法:対象は当院で治癒切除された肝機能障害のない大腸癌症例51例と健常者100例.大腸癌症例は手術の2週間前に静脈血および便を採取し,ELISA法でCholicacid(CA)とDeoxycholic acid(DCA)を測定した.3. 結果:大腸癌症例の血清DCAは健常者に比べ有意に高値を示したが,便中DCAは有意差がなかった.大腸癌症例の血清および便中DCA/CAは健常者に比べ有意に高値を示した.大腸癌同一症例の便中DCA/CAと血清DCA/CAの相関係数は,0.429で相関関係を認めた.便中DCA/CAのカットオフ値を1.1に設定すると感度78%,特異度56%であった.血清DCA/CAのカットオフ値を0.21に設定すると感度69%,特異度75%であった.4. 結論:血清DCA/CAの大腸癌スクリーニングとしての有用性が示唆された.

The magnetotransport properties of epitaxial graphene on SiC substrates play a central role in the development of monolithic technology for high quality graphene-based devices. Magnetotransport investigations in both the small field and the quantum Hall regime give valuable information on the disorder properties of the monolayer film and on the role of bilayer inclusions. In the small field region, we find an unexpected dependence of the intra-valley scattering and electron-electron interactions on carrier density in monolayer devices. In the quantum Hall regime, bilayer stripes shunting the devices are responsible for anomalies in the observed characteristics, which we explain in terms of a Landauer-Buttiker model.

We propose a scheme for entangled state measurement at flux qubits (quantum bits) depending on the sensitivity measurement of a DC-SQUID (Superconducting QUantum Interference Device). The DC-SQUID is used as a flux qubit readout device. The switching current distribution of a DC-SQUID is sufficiently narrow to distinguish between two ground states of three Josephson junction qubits with a large mutual inductance between a qubit and the DC-SQUID. But discrimination between two ground states becomes difficult with the smaller mutual inductance that is preferable as regards of qubit coherence. However, we can employ averaging to increase the effective sensitivity. And we found that Bell-pair measurement can be performed with flux qubits to prove the existence of an entangled state over two qubits., Comment: 6 pages, 6 figures

Objective: This study was performed to determine whether in vitro affinities of currently available antipsychotics toward dopamine or other neuronal receptor systems are associated with their in vivo incidence of central and peripheral adverse drug reactions (ADRs) Methods: For 17 antipsychotic drugs available in Japan, the clinical incidences of 7 different types of drug-induced ADRs (i.e., akathisia, dyskinesia, tremor, rigidity, drowsiness, hypotension and dry mouth) were obtained from both post-marketing ADR databases and the investigational clinical trials of eight pharmaceutical companies. Affinity constants (K i ) of the respective drugs toward dopamine D 1 and D 2 receptors, α 1 -adrenoceptors, histamine H 1 receptors, serotonin 5-HT 2 receptors and muscarinic cholinoceptors, determined using rat brain synaptosomes, were obtained from the literature. Relationships between in vitro receptor-binding properties and in vivo incidences of the respective types of antipsychotic-related ADRs were analyzed using Spearman's rank correlation. Results: Significant (P < 0.05) correlations were observed between the K i values for dopamine D 2 receptor and the clinical incidences of akathisia and dyskinesia (r s =-0.68 and -0.66, respectively). Significant (P < 0.05) correlations were also observed between the K i values for α 1 -adrenoceptor and histamine H 1 receptor and the incidence of drowsiness (r s =-0.65 and -0.55, respectively), and between the K i values for three receptor systems (i.e., dopamine D 1 receptor, α 1 -adrenoceptor and histamine H 1 receptor) and the incidence of dry mouth (r s =-0.50, -0.81 and -0.62, respectively). Conclusion: Preclinical receptor-binding data of antipsychotic drugs toward central dopamine and other ancillary neurotransmitter systems may be useful for predicting not only in vivo antipsychotic potency but also clinical incidence of akathisia and dyskinesia for this class of agents. Newly developed antipsychotic drugs with more potent and selective antagonistic activity against the dopamine D 2 receptor may not necessarily be associated with a lower incidence of extrapyramidal ADRs.

With the help of these representations, one can easily understand the dynamic characteristics of electrical power systems.

Acute lung allograft rejection is believed to be initiated by passenger leukocytes, such as alveolar macrophages (AM), in the donor organ, which release TNF-alpha, and present alloantigens to host lymphocytes, to up-regulated Th1 cellular and humoral immunity. However, the role of donor AM in local TNF-alpha synthesis, and their ability to induce local Th1 cellular and humoral immunity have not been evaluated. By depleting Brown Norway (BN, RT1n) rat lung allografts of AM before transplantation into Lewis rat (LEW, RT1(1)) recipients, the current study determined the role of donor AM in including the production of TNF-alpha, IFN-gamma (Th1 cytokine), IL-4 (Th2 cytokine), IgG subtypes, and rejection pathology in the allograft. The data show that compared with untreated BN allografts, pretransplant depletion of donor lung AM resulted in significantly less TNF-alpha, and IFN-gamma production in allograft bronchoalveolar lavage fluid with variable effects on local IL-4 production. Depletion of AM in the donor lung before transplantation affected the local production of several IgG subclasses. However, pretransplant depletion of donor AM had no effect on the development of the pathology of severe acute rejection. These data show that donor AM have a central role in the local synthesis of TNF-alpha and induce the production of IFN-gamma and IgG subtypes, locally, during acute lung allograft rejection. However, depletion of AM before transplantation does not prevent the development of severe acute rejection in BN rat lungs, transplanted into LEW recipients.

Variable impedance apparatus such as a static VAr compensator (SVC) and a variable series capacitor (VSC) can improve the steady-state stability of a power system if they are located appropriately. The present paper proposes an index for identifying the location of SVC and VSC in large scale power systems for effective damping. The index is called LIED (location index for effective damping) by the authors. Since LIED can be computed quickly for large scale power systems which have more than two hundred generators without conducting a number of digital simulations and eigenvalue analyses, it is valuable for the system planner who needs to identify the effective location of SVC and VSC. The proposed index is applied to the New England test system and its validity is demonstrated through digital simulations. >

Deep vein thrombosis (DVT) is a main cause of pulmonary thromboembolism (PTE), and therefore both diseases are categorized as a serial pathophysiology of venous thromboembolism (VTE). Treatment goals for DVT include stopping clot propagation and preventing the recurrence of thrombus, the occurrence of PTE, and the development of pulmonary hypertension, which can be a complication of multiple recurrent pulmonary emboli. Clinical guidelines stratify the risk of VTE to 4 levels and recommend the treatment options. In high or extremely high risk patients for VTE, the use of low-dose heparin is recommended. The prevention against VTE, such as elastic compression stockings and intermittent sequential pneumatic leg compression( ISPC), is the most important prophylactic treatment against perioperative PTE by reducing thrombotic risk in low or moderate high risk patients for VET. Since there is no clear evidence that screening all or even selected patients for thrombophilias improves long-term outcomes, the physician's clinical judgment, and consultation with appropriate subspecialists should guide management perioperatively. Once PTE is suspected, immediate and accurate diagnosis and appropriate treatment are mandatory.

We report weak antilocalization (WAL) measurements in a nearly symmetric In0.53Ga0.47As/In0.52Al0.48As quantum well. In this system, an enhancement of the spin‐orbit scattering time, equivalently the weakening of the WAL effect, is expected at carrier densities Ns where equal magnitudes between the Rashba and Dresselhaus parameters (α = ±β) are realized. Our magnetotransport measurements indeed exhibited a weakening of the WAL effect as a function of Ns, though the weakening took place at only one carrier density. Our interpretation is that the value of β is so small that the two carrier densities corresponding to α = β and α = −β are very close to each other. It turned out that the deduced values for the bulk Dresselhaus parameter γ in In0.53Ga0.47As/In0.52Al0.48As quantum wells is considerably smaller than the k⋅p value of 27 eVA3.

The paper outlines the present status and future prospects of nuclear power generation in Japan with emphasis on the situations peculiar to Japan. It first describes why among the many kinds of energy resources available, Japan has decided to follow a nuclear energy policy. Next presented is the current status of nuclear power generation with a brief review of nuclear power technology development. All phases of nuclear power generation from procurement of nuclear fuel to the waste disposal are covered. Social, political and technological problems that the Japanese electric power industry has to tackle in order to utilize fully nuclear energy are also discussed, together with the new technologies that will need to be developed to solve the problems effectively. As many quantitative data as possible are presented throughout the paper to provide a precise description.

The authors describe a 10-b, 100-Ms/s ADC (analog-to-digital converter) with a pipelined subranging scheme, a sample-and-hold amplifier with 7.6-ns acquisition time, and a 94-dB, 335-MHz op amp, enabling it to operate with 950-mW power dissipation from a single -5 V power supply. The design consists of sample-and-hold amplifiers, a coarse 6-b flash ADC, a fine 5-b flash ADC, a digital-to-analog converter, an analog subtractor, a register, and a digital adder with an error-correction function. The ADC is fabricated using a 0.8- mu m BiCMOS process featuring a double-layer polysilicon capacitor. The signal-to-noise-plus-distortion ratio as a function of input frequency at a 100-Ms/s conversion rate is shown. >

We report of a 77-year-old woman who was admitted to our hospital in coma by emergency. A computed tomography scan revealed acute aortic dissection (Stanford type A). We established selective antegrade cerebral perfusion within 3 hours of the onset and then performed ascending aortic replacement. In the state of hypothermia (35 degrees C), the patient was weaned from cardiopulmonary bypass. The patient was kept hypothermic until the operation was completed. We kept mild hypothermia (34.5 degrees C) in intensive care unit (ICU) for 40 hours. The patient was extubated at 94 hours after the operation. The patient was discharged from the hospital on foot on postoperative day 21.

Ro 09-1428, a new parenteral cephalosporin with a catechol moiety attached at position 7 of the cephalosporin ring, showed high in vitro activity against Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Proteus vulgaris, and Streptococcus pyogenes, with MICs for 90% of strains tested (MIC90s) of less than or equal to 0.39 micrograms/ml. Morganella morganii, Providencia rettgeri, Citrobacter freundii, Haemophilus influenzae, Staphylococcus aureus, and Streptococcus pneumoniae were inhibited with MIC90s of less than or equal to 3.13 micrograms/ml. Serratia marcescens was less susceptible to Ro 09-1428, with a MIC90 of 25 micrograms/ml. The most distinctive feature of Ro 09-1428 was its potent activity against Pseudomonas aeruginosa and Acinetobacter calcoaceticus, with MIC90s of 0.39 and 6.25 micrograms/ml, respectively. Most of the ceftazidime-resistant strains of P. aeruginosa, E. cloacae, and C. freundii were inhibited by Ro 09-1428, while those of S. marcescens were resistant at a concentration of 12.5 micrograms/ml. Ro 09-1428 was more active than ceftazidime against staphylococci. PBP 3 was the most sensitive target in E. coli and P. aeruginosa. The response to ferric iron in growth medium suggests that Ro 09-1428 may be taken up by transport mechanisms similar to those of other catechol cephalosporins. In accordance with its in vitro activity, Ro 09-1428 activity was equal to or greater than ceftazidime activity in efficacy against experimental septicemias in mice. The results indicate that Ro 09-1428 is a broad-spectrum cephalosporin with advantages over ceftazidime in its activity against P. aeruginosa, staphylococci, and ceftazidime-resistant strains of C. freundii and E. cloacae.

A discussion is presented of the reverse action that occurs when the secondary voltage of a transformer is pulled down and the tap position of an on-load tap changer is raised to increase the secondary voltage. A dynamic model of an induction motor is adopted as a load model simulating this kind of reverse action during voltage collapse. The transient mechanisms of the reverse action are analyzed using P-V curves. It is found that the reverse action caused by on-load tap changers may cause the operating condition to deteriorate in a heavily loaded power system where cascaded voltage collapse may occur. >

The dynamic phenomena of power system voltage collapse are analyzed by the method of dynamic simulation using induction motor models. From the viewpoint of dynamic phenomena, the voltage collapse starts locally at the weakest node and spreads out to the other weak nodes. The main conclusions drawn from this study are as follows. The voltage can be recovered, even after the node voltage is lowered beyond its static critical value. The operating state corresponding to the lower voltage solution is not always unstable. It could be stable depending upon the dynamic characteristics of individual loads, even if they are not of constant impedance type. The voltage collapse is initiated at the weakest node and spreads out to the other weak nodes. These results can be applied to radial voltage collapse phenomena. >

Coronary artery fistulae are relatively rare congenital anomalies. Those associated with saccular coronary artery aneurysms are even rarer. Including the current case, only 65 such cases have been reported in Japan. A 62-year-old female was admitted to our hospital for evaluation of abnormal shadow on the chest X-ray. The enhanced chest computed tomography (CT) scan demonstrated a giant saccular coronary aneurysm on the left side of the pulmonary artery. Multi-detector row CT (MDCT) scan demonstrated the coronary artery aneurysm was connected to the left anterior descending artery. Coronary angiography revealed 2 aneurysms with bilateral coronary artery to pulmonary artery fistulae. The patient underwent aneurysmectomy and ligation of fistulae under cardiopulomonary bypass. The postoperative course was uneventful and postoperative coronary angiography revealed complete resection of the aneurysms and only slight blood flow through the fistulae. She was discharged on the 10th postoperative day.

A 55-year-old man had suffered from chest oppression while asleep for 1 to 2 years. Moderate aortic regurgitation and aneurysm of Valsalva sinuses were revealed by echocardiography. It was diagnosed as chronic localized dissecting aneurysm of Valsalva sinuses during the operation, and modified Bentall's operation was performed. The patient was extubated 2 hours after the operation. ST segment depression in leads I, aV(L) and V3-V6, and elevation in leads III and aV(R) suddenly occurred 5 hours after the operation. However, hemodynamics was very stable, and he complained of no chest pain. Cardiac asystole was detected 7 hours after the operation, and percutaneous cardiopulmonary support as well as intraaortic balloon pumping (IABP) was immediately started. Emergency coronary angiography revealed acute myocardial infarction due to severe coronary spasm in the left coronary artery. The patient expired on the 3rd postoperative day.
