Abstract

Background: Optic neuritis, myelitis, and neuromyelitis optica spectrum disorder (NMOSD) have been associated with antibodies against myelin oligodendrocyte glycoprotein-immunoglobulin G (anti-MOG-IgG). Furthermore, patients with radiological and demographic features atypical for multiple sclerosis (MS) with optic neuritis and myelitis also demonstrate antibodies against aquaporin-4 and anti-MOG-IgG. However, data on the diagnosis, treatment, follow-up, and prognosis in patients with anti-MOG-IgG are limited. Aims: To evaluate the clinical, radiological, and demographic characteristics of patients with anti-MOG-IgG. Study Design: Multicenter, retrospective, observational study. Methods: Patients with blood samples demonstrating anti-MOG-IgG that had been evaluated at the Neuroimmunology laboratory at Ondokuz Mayıs University’s Faculty of Medicine were included in the study. Results: Of the 104 patients with anti-MOG-IgG, 56.7% were women and 43.3% were men. Approximately 2.4% of the patients were diagnosed with MS, 15.8% with acute disseminated encephalomyelitis (ADEM), 39.4% with NMOSD, 31.3% with isolated optic neuritis, and 11.1% with isolated myelitis. Approximately 53.1% of patients with spinal involvement at clinical onset demonstrated a clinical course of NMOSD. Thereafter, 8.8% of these patients demonstrated a clinical course similar to MS and ADEM, and 28.1% demonstrated a clinical course of isolated myelitis. The response to acute attack treatment was lower and the disability was higher in patients aged > 40 years than patients aged < 40 years at clinical onset. Oligoclonal band was detected in 15.5% of the patients. Conclusion: For patients with NMOSD and without anti-NMO antibodies, the diagnosis is supported by the presence of anti-MOG-IgG. Furthermore, advanced age at clinical onset, Expanded Disability Status Scale (EDSS) score at clinical onset, spinal cord involvement, and number of attacks may be negative prognostic factors in patients with anti-MOG-IgG.

Abstract

Objective: this study was conducted to examine whether lopinavir/ritonavir (Lop/r), an HIV protease inhibitor, can improve disc physiology and slow down intervertebral disc (IVD) degeneration through in vitro experimental methods, as well as whether it can suppress inflammation with interleukin-1 beta (IL-1?) and sex-determining region Y (SRY) protein-related high-mobility group box genes-9 (SOX9) through hypoxia-inducible factor 1-alpha (HIF-1?) and the nuclear factor kappa B (NF-?B) signaling pathway. The aim was to investigate whether Lop/r application is toxic to IVD cells and the microenvironment simultaneously. Patients and methods: human primary cell cultures were prepared using herniated IVD tissues obtained from patients with lumbar disc hernia who were unresponsive to conservative and medical treatment, and thereby, were operated on. The untreated culture samples served as control group, and the samples treated with Lop/r served as study group. Microscopic evaluations were performed simultaneously using fluorescent and supravital dyes in all groups. In addition to cell viability, toxicity, and proliferation analysis through a commercial kit, IL-1?, SOX9, HIF-1?, and NF-?B protein expressions were evaluated using Western blotting. In the statistical comparison of the obtained data, an alpha value less than 0.05 was considered significant. Results: cell proliferation decreased in the Lop/r group, but no cell death was observed (p < 0.05). Moreover, at the end of 72 hours after Lop/r application, IL-1? and NF-kB protein expressions decreased by 40% and 52%, respectively, while HIF-1? and SOX9 protein expressions increased by 4% and 59%, respectively (p< 0.05). Conclusion: although these data were obtained from an in vitro experimental study, it is believed that these findings could make significant contributions to the pharmaco-regenerative treatment modalities of IVD degeneration. BACKGROUND/OBJECTIVES: To investigate if the visual and anatomic response to the first dexamethasone implant (DEX) predicts the 12-month clinical outcome after shifting to fluocinolone acetonide (FAc) implant in patients with diabetic macular oedema (DMO). METHODS: Retrospective cohort study including pseudophakic patients with previously treated DMO, undergone one or more DEX injections before FAc. Functional and morphologic response to DEX was defined based on the best-corrected visual acuity (BCVA) and central macular thickness (CMT) changes after the first DEX, respectively. Steroid-response was defined as intraocular pressure (IOP) elevation ≥5 mmHg or IOP > 21 mmHg after any previous DEX exposure. Pairwise comparisons for BCVA, CMT, and IOP after FAc were performed with linear mixed models and a repeated-measure design. RESULTS: Forty-four eyes of 33 patients were included. 42. Can intraoperative endoscopy prevent esophagojejunal anastomotic leakage after total gastrectomy?

Abstract

Background: Esophagojejunostomy (EJ) is frequently performed after total gastrectomy for proximal gastric tumors. Despite evolving surgical techniques and improving perioperative care, the EJ leak is one of the most severe life-threatening complications. This study investigated the preventability of postoperative anastomotic complications by performing intraoperative endoscopy. Methods: We included 86 patients who underwent total gastrectomy and Roux-en-Y esophagojejunostomy anastomosis in the study. Patients were divided into two groups and analyzed retrospectively. Group 1 consisted of 43 patients who did not undergo intraoperative endoscopy between 2017 and 2019, and Group 2 included 43 patients who underwent intraoperative endoscopy between 2019 and 2020. Results: Esophagojejunostomy anastomotic leak (EAL) was observed in 2.3 % of patients in Group 1 but not in Group 2. Anastomosis-related abnormal findings (anastomotic defect, bleeding, air leak, mucosal separation) were recorded in seven patients of Group 2 during endoscopy. When such findings were observed, additional full-thickness sutures were placed on the anastomosis line and strengthened. Complication related to anastomosis was not observed in the postoperative period in Group 2. Discussion: After a total gastrectomy, the most severe complication affecting mortality, morbidity, and consequently the cost of the disease is esophagojejunal anastomotic leakage. Most of these complications are induced by technical errors not noticed during surgery. The crucial advantage of performing intraoperative endoscopy is the technically detailed evaluation of anastomosis. Conclusion: Intraoperative endoscopy is a safe method to evaluate the strength of anastomosis. This procedure provides detailed information regarding anastomotic integrity. Only eyes with a good morphologic response to DEX had a significant CMT reduction after FAc (p < 0.001), while no significant relationship was found between BCVA improvement after DEX and after FAc. IOP elevation occurred in 9 eyes (20%) following DEX implant. These eyes carried a 20-fold increased risk of having an IOP rise after FAc (p < 0.001), with a non-linear relationship between the IOP increase after DEX and the one after FAc. Conclusion: The response to previous DEX may anticipate the morphologic response to subsequent FAc. Eyes with steroid-induced IOP elevation after DEX are at a high risk of IOP increase after FAc. The visual response after FAc was not associated with the visual response to previous steroids, indicating that FAc may have a role also in patients refractory to DEX implant. © 2021, The Author(s), under exclusive licence to The Royal College of Ophthalmologists.

CFD software Fluent and the grid generation tool Gambit are utilized. Two dimensional compressible time dependent Reynolds Averaged Navier- Stokes equations with k-? turbulence model are solved. Solutions are obtained for a cavity of a length to depth ratio (L/D) of 1,3, 5, 7.6 and 10, and for a Mach number of 1.5, analyses are performed for a two dimensional cavity geometry and grid independence tests are performed. The results of two dimensional computations are compared to each other and L/D=5 simulation results are compared with the experimental results in literature. Sound Pressure Levels (SPL) and mean pressure coefficients are used for comparison. The effects of different cavity geometries on the oscillation mechanism are examined.

45. Control of supersonic cavity flow

Abstract

In this study, laser energy deposition is used as a flow control technique. The effects of laser energy deposition over an open rectangular supersonic cavity flow to realize the capability of the energy deposition for reduction of the pressure fluctuations and relative sound pressure levels are examined numerically. The simulations are performed for the cases both with and without energy deposition for unsteady flow over a two dimensional cavity at a free stream Mach number of 1.5. The length to depth ratio of the cavity (L/D) is 5.07 which represents an open cavity configuration. The Reynolds-averaged compressible time dependent Navier Stokes equations in two dimensions are solved using the flow solver ANSYS Fluent. The k-w model is used as a turbulence model. The application of the laser deposition is modeled by using the Gaussian temperature profile assuming the density within the spot is initially uniform. The results obtained from the simulation performed for 5mJ of energy show that laser energy deposition provides small reduction for SPL values. Also, the impacts of frequency, location, and amount of laser energy deposition will be investigated and the results will be presented in the final paper.

The simulations are performed for the cases both with and without energy deposition for unsteady flow over a two dimensional cavity at a free stream Mach number of 1.5. The length to depth ratio of the cavity (L/D) is 5.07 which represents an open cavity configuration. The Reynolds-averaged compressible time dependent Navier Stokes equations in two dimensions are solved using the flow solver ANSYS Fluent. The k-w model is used as a turbulence model. The application of the laser deposition is modeled by using the Gaussian temperature profile assuming the density within the spot is initially uniform. The results obtained from the simulation performed for 5mJ of energy show that laser energy deposition provides small reduction for SPL values. Also, the impacts of frequency, location, and amount of laser energy deposition will be investigated and the results will be presented in the final paper.
