2. Rare Breast Emergency: A Case of Necrotizing Fasciitis of the Breast in a Lactating Patient.

Necrotizing fasciitis is a rare but potentially lethal infection of the skin and soft tissue, commonly seen in the perianal and gluteal regions. Concomitant diabetes is a predisposing factor. Primary necrotizing fasciitis of the breast is rare in healthy women. In this article, we present a very rare case of breast necrotizing fasciitis in the context of the literature. We report the case of a 35-year-old female patient who had given birth two months prior to admission and developed necrotizing fasciitis of the breast during lactation. The patient presented to the emergency department with sepsis. Examination revealed widespread erythema, dark discoloration, edema, and necrotic areas indicative of wet gangrene and crepitation in the left breast. Necrotizing fasciitis is a rapid and aggressive disease that can be fatal, and delayed diagnosis may unfortunately result in death. Therefore, careful evaluation of all suspected cases, especially for patients with risk factors, is crucial for early diagnosis and timely treatment. This case highlights the importance of recognizing necrotizing fasciitis of the breast in lactating women to ensure prompt and appropriate management, potentially saving lives. [ABSTRACT FROM AUTHOR]

The article titled "Investigation of the expression of immune checkpoint and fatigue-related molecules in cytotoxic T cells in the lymph node near the tumor in breast cancer" explores the expression of immune checkpoint and fatigue-related molecules in cytotoxic T cells in lymph nodes near breast cancer tumors. The study found that cytotoxic T cells in the lymph nodes of breast cancer patients exhibited high expression of PD-1 and CXCR5, indicating a follicular helper T cell-like, exhaustion-related immunophenotype. The study suggests that regional lymph nodes in breast cancer patients contain depleted cytotoxic T lymphocytes with high expression of checkpoint molecules, which may have implications for treatment approaches. This document is a research article from the Turkish Journal of Surgery. The first part of the article discusses the use of incisional negative pressure wound therapy (iNPWT) in breast cancer patients at high risk for wound complications. The study compares healing times and wound site complications between patients treated with iNPWT and those treated with standard wound dressings. The results show that iNPWT significantly reduces postoperative drainage, seroma, flap ischemia, and flap detachment compared to conventional dressings. The second part of the article focuses on the molecular investigation of factors affecting the resolution of obesity and obesity-related metabolic complaints after sleeve gastrectomy in rats. The study examines the association between high expression levels of certain long noncoding RNAs (LncRNAs) and the resolution of non-alcoholic fatty liver disease (NAFLD) and NASH after sleeve gastrectomy. This article discusses the use of 3D printing to create parathyroid tissue scaffolds for the treatment of hypoparathyroidism. The article discusses the potential use of phenotypic analysis of suppressor cells of myeloid origin as a complementary marker in the treatment decision for thyroid nodules. The study aims to increase the efficacy of the Bethesda scoring system, which is used to categorize cytologic findings of thyroid nodules, by correlating the percentage of specific cell subtypes with the scoring system. The researchers analyzed the subpopulations of myeloid-derived suppressor cells (MDSC) in fresh tissue from thyroid nodules and peripheral blood of patients who underwent thyroidectomy. The results showed that MDSC levels were significantly increased in patients with malignant post-operative pathology results, particularly in the atypia of undetermined significance and follicular lesions of undetermined significance group. The study found that immunomodulatory genes increased in both cancer and trauma, but the suppressive properties of low-density neutrophils differed between the two conditions. This article discusses the association between the number of macrophages infiltrating the tissue and the disease stage in gastric cancer patients. The study examines the amount of CD206+ macrophages in peripheral blood, non-tumor normal tissue, and tumor tissue of gastric cancer patients in comparison with the disease stage. The results show that CD206+ macrophages are found in a high percentage in the tumor tissue of gastric cancer patients and that the percentage of macrophages is significantly correlated with the stage of the disease. 23. Evaluation of Treatment Results of Patients Treated with Splenectomy due to Hematological Cancer

Purpose: Hematological disorders are the most common indications for performing splenectomy after trauma in adults. The aim of this study is to investigate the treatment outcomes of splenectomy in hematological malignancy patients who were operated between the years 1998 and 2009. Patients and Methods: We analyzed the patients' clinical characteristics, type of surgical treatment, preoperative and postoperative medical treatments, complications and thrombocyte responses, retrospectively. Results: Seventeen male and 13 female patients, with a mean age of 50.7 (17-79), underwent splenectomy due to following indications: Hodgkin lymphoma (n=10), Non Hodgkin lymphoma (n=12), Chronic Lymphocytic lymphoma (n=5), MALT lymphoma (n=2) and Hairy Cell Leukemia (n=1). The indications for surgery was splenomegaly and its complications (n=20), gastric obstruction (n= 5), diagnosis splenic mass (n=3), major bleeding (n=1) and abscess (n=1). Complications related to surgery occurred in 10 patients: pneumonia (n=6), bleeding (n=2), abscess (n=1) and sepsis (n=1). Early 30-day mortality was observed in two cases. During the follow-up period, platelet value revealed response in 19 (63.3 %), partial response in 8 (26.7 %) and unresponsive in 3 (10 %) patients due to splenectomy. Conclusion: In conclusion; for hematological diseases, thrombocytopenia, massive splenomegaly and its complications, splenectomy is a safe method with acceptable complications rate and improves the patients' survival outcomes., Amaç: Hematolojik hastalıklar, travma sonrası splenektomi geçiren hastalarda en sık görülen endikasyondur. Bu çalışmada, hematolojik malignite nedeniyle splenektomi yapılan hastaların tedavi sonuçlarının değerlendirilmesi amaçlanmaktadır. Hastalar ve Yöntem: 1998-2009 yılları arasında hematolojik malignite tanısıyla splenektomi yapılan hastalar retrospektif olarak değerlendirildi. Hastaların yaşı, cinsiyeti, cerrahi öncesi ve sonrası uygulanan medikal tedavileri, uygulanan cerrahi tedavi ve gelişen komplikasyonlar ve hastaların ameliyat sonrası dönemdeki trombosit yanıtları incelendi. Bulgular: Çalışmaya ortalama yaşı 50,7 (17-79) olan 17 erkek ve 13 kadın hasta alındı. Hastaların 10'unda (% 33,3) Hodgkin lenfoma, 12'sinde (% 40) Non Hodgkin lenfoma, beşinde (% 16,6) Kronik Lenfositik Lösemi, iki (% 6,6) tanesinde MALT lenfoma ve bir (% 3,3) hastada ise Hairy Cell Lösemi tanısı mevcuttu. Yirmi hasta splenomegali ve komplikasyonları nedeni ile opere edilirken, beş hasta mide obstrüksiyon bulguları nedeni ile, üç hasta tanı amaçlı, bir hasta apse ve bir hasta kanama nedeni ile ameliyat edildi. Postoperatif komplikasyon olarak hastalardan 6'sında pnömoni görülürken iki hastada kanama, bir hastada apse ve bir hastada sepsis gözlendi. İki hasta postoperatif erken dönemde kaybedildi. Trombosit yanıtları takip edilen hastaların 19'u (% 63,3) tam yanıt, 8'i (% 26,7) parsiyel ve 3 (%10) tanesi ise yanıtsız olarak değerlendirildi. Sonuç: Splenektomi, hematolojik maligniteli hastalarda hipersplenizme bağlı sitopenilerin ve komplikasyonların geliştiği durumlarda ve tanı amaçlı güvenle uygulanabilen bir cerrahi girişimdir

26. Spontaneous perforation of pyometra: A rare cause of acute abdomen and sepsis.

Pyometra, the accumulation of purulent material in the uterine cavity, is a rare gynecological condition whose etiology is impaired drainage of the uterine cavity. It is uncommon in premenopausal age and occurs mainly in older and postmenopausal women. Clinical signs of pyometra are vaginal discharge, postmenopausal bleeding, and lower abdominal pain. An 87-year-old woman was admitted to our emergency department with abdominal pain, fever, and vomiting. The results of physical examination revealed rebound tenderness and muscular rigidity in the lower abdomen. Ultrasonography demonstrated free fluid in the abdomen, and percutaneous aspiration revealed that this fluid was purulent. A computed tomography scan showed a large amount of free fluid in the abdominal cavity and a uterine myoma. The patient underwent emergency laparotomy due to acute abdomen. During the laparotomy, a 2x1 cm perforation was seen at the fundus of the uterus. The patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy. A culture of the pus grew Escherichia coli. Histopathological examination revealed degenerative uterine myoma with no evidence of malignancy. Pathological results indicated myometrial suppurative inflammation along with neutrophilia and necrosis. Pyometra is a rare event; however, it must be considered when investigating acute abdomen etiology. Because pyometra involves abscess formation, drainage and evacuation of the uterine cavity while leaving a drain in the cavity and dilating the cervical canal is the main treatment protocol. In cases of perforation, hysterectomy is the treatment choice; however, with concomitant diseases in older patients, immediate surgery is related to important morbidity and mortality risks. Objectives: Cutaneous metastases of colorectal cancer are rarely detected in clinical practice. This study aimed to evaluate the patients with cutaneous metastases of colorectal cancer in order to define their clinical presentation, clinical course, and prognosis. Patients and Methods: Patients (7 females, 1 male; mean age 49 years; range 28 to 72 years) treated for colorectal cancer and who developed cutaneous metastases during follow-up were evaluated. Patients' demographic characteristics, location of the primary tumor and cutaneous metastases, histopathologic properties and stage of the primary tumor, location of accompanying metastases, and treatment methods were recorded. Results: All patients had stage III colorectal cancer and were treated with adjuvant chemotherapy. Cutaneous metastases appeared in a mean duration of 25 months after surgery and were located in the anterior abdominal wall and presacral region. Following cutaneous metastases, seven patients died whereas one patient is alive after 63 months. Mean survival time after cutaneous metastases was 17 months. Four patients survived more than four years from the time of surgery, one of which had only cutaneous metastases. Conclusion: In colorectal cancer patients, suspicious looking skin lesions should be excised as early as possible to increase the detection rate of patients with isolated cutaneous metastases. Post-laparotomy wound dehiscence, evantration and evisceration are important complications leading to an increase in both morbidity and mortality. Incisional hernias are frequently observed following abdominal surgeries and their occurrence is related to various local and systemic factors. This study aims to analyze the factors affecting wound healing by investigating the parameters that may cause wound dehiscence, incisional hernia, sinus formation and chronic incisional pain.The records of 265 patients who underwent major abdominal surgery were analyzed. The data on patient characteristics, medication, surgical procedure type, type of suture and surgical instruments used and complications were recorded. The patients were followed up with respect to sinus formation, incisional hernia occurrence and presence of chronic incision pain. Statistical analysis was performed using SPSS 10.00 program. The groups were compared via chi-square tests. Significance was determined as p0.05. Multi-variate analysis was done by forward logistic regression analysis.115 (43.4%) patients were female and 150 (56.6%) were male. Ninety-four (35.5%) patients were under 50 years old and 171 (64.5%) were older than 50 years. The median follow-up period was 28 months (0-48). Factors affecting wound dehiscence were found to be; creation of an ostomy (p=0.002), postoperative pulmonary problems (p=0.001) and wound infection (p=0.001). Factors leading to incisional hernia were; incision type (p=0.002), formation of an ostomy (p=0.002), postoperative bowel obstruction (p=0.027), postoperative pulmonary problems (p=0.017) and wound infection (p=0.011).Awareness of the factors causing wound dehiscence and incisional hernia in abdominal surgery, means of intervention to the risk factors and taking relevant measures may prevent complications. Surgical complications that occur in the postoperative period are especially related to wound healing problems.

- Abstract
Objective: Colorectal cancer is still one of the most common causes of cancer related deaths in the world despite improvements in diagnosis and treatment modalities, and application of community-based screening methods. Symptoms of colorectal cancer are non-specific and usually manifest following local progression. A number of patients with advanced stage colorectal cancer present to emergency departments with obstruction as the first sign of disease without any previous symptoms. This presentation is an indication for emergency surgery that has a high rate of morbidity and mortality. In this study, we aimed to determine the factors associated with early diagnosis and survival by comparing postoperative results of colorectal cancer patients who underwent surgery under emergency or elective situation. Material and Methods: Files of colorectal patients treated between 2009-2013 were retrospectively analyzed. Data on patient age, gender, operation type, intraoperative results, length of hospital stay, co-morbidities, postoperative complications and pathological results were evaluated and compared. Results: There was no statistical difference between groups in terms of age, gender, and pathology results (p>0.05). The difference between groups in terms of postoperative length of hospital stay, presence of co-morbid diseases, pathological stage, and postoperative complications was statistically significant (p<0.05). Length of hospital stay, advanced stage on admission, complications such as surgical site infection, evisceration, and anastomosis leakage rates were higher in patients in the emergency surgery group. Conclusion: Risk groups should be determined in order to diagnose colorectal cancer patients at an early stage while they are still asymptomatic, and this information should be incorporated into effective screening programs. This approach will be beneficial to treatment outcomes, complication rates, length of hospital stay, and survival and treatment results. [ABSTRACT FROM AUTHOR]

Purpose: To identify the factors that affect the unfavorable clinic outcomes of emergent incarcerated groin hernias in adults. Materials and Methods: Emergent repairs for incarcerated groin hernias were compared with elective repairs (n=109 versus 858) regarding age, gender, type of the hernia, recurrence, surgical repair, resections, type of the anesthesia, coexisting diseases, ASA scores, postoperative complications, duration of operation, duration of hospital stay and mortality. In order to analyze the factors associated with strangulation, omentum-bowel resection, postoperative local and systemic complications in emergent hernias, the gender, age, type of the hernia, recurrence rate, duration of symptoms, coexisting diseases, ASA scores and type of the anesthesia were analysed. Results: When compared with elective hernia repairs, emergent hernias were found to be significantly associated with older age, female gender, omentum-bowel resection, higher ASA scores, coexisting diseases especially hypertension, longer duration of the operation, longer hospital stay, systemic postoperative complications and wound infections (p<0,05). In emergency admissions, recurrent hernia, coexisting diseases and high ASA scores were significant factors for strangulation, delayed admission was associated with omental-bowel resection, older age, recurrent hernias, high ASA scores were associated with systemic postoperative complications, advanced age, recurrent hernias, coexisting diseases and high ASA scores were associated with longer hospital stay (p<0,05). Conclusion: Older age, recurrence, delayed admission, coexisting diseases and high ASA scores are unfavorable factors for emergent hernias. We think that it would be appropriate to give priority to these patient in elective hernia repairs. If the hernia is recurrent or the admission is delayed in emergent cases, surgeon should be careful regarding strangulation, resection and postoperative systemic complications. [ABSTRACT FROM AUTHOR]

Adenoid cystic carcinoma is rarely encountered among breast cancer patients as a histologic subtype. For this reason, clinicians have less experience on this subtype and confront difficulties in treatment planning. In this study, patients with adenoid cystic carcinoma were reviewed to understand their clinical course. Four patients with a mean age of 53 (47-68) were treated with modified radical mastectomy. Patients had triple negative breast cancer with a mean tumor size of 5 cm (2.5-8 cm). Two patients received chemotherapy whereas one patient also had radiotherapy. All patients were followed for a mean duration of 35 months (29-48 months) without any evidence of local recurrence and distant metastases. [ABSTRACT FROM AUTHOR]

Objective: Drains and catheters are used for both prophylactic and therapeutic reasons in clinical practice. This study aimed to investigate the factors that affect safety of drains, catheters, nasogastric tube and central venous line in patients who underwent surgery. Material and Methods: Two hundred and four consecutive patients who were operated at the general surgery clinics under general anesthesia were included in the study. Factors that affect the safety of drains and catheter were followed and recorded prospectively. Results: During follow-up period, 12 (5.8%) patients have experienced problems regarding safety of drains/catheters. The mean age of patients who were followed-up in terms of security problems was 63.1 (39-86) years. Eight (66.7%) patients had been operated emergently, and four (33.3%) patients electively. Three (25%) patients had psychiatric/neurological co-morbidities and 3 (25%) patients were confused due to anesthesia/intensive care unit treatment when the drain safety was broken. Eight (66.7%) patients withdrew the drains or catheters by themselves, in 2 (16.7%) patients the drains spontaneously came out and in 2 (16.7%) patients the wrong drain was withdrawn. One patient had dementia, one patient had Alzheimer's disease and one patient was being followed-up with a diagnosis of schizophrenia. In three (25%) patients the abdominal drain, in four (33.3%) patients nasogastric tube, in one (8.3%) patient intubation tube, in one (8.3%) patient central venous catheter, and in three (25%) patients multiple drains were removed. Conclusion: The inaccurate use of drains or re-intervention for an unintentionally removed drain causes problems regarding patient safety. Close monitoring of surgical patients in terms of security, and submission of additional measures for patients with confusion and neurological/psychiatric disorders are of great importance. Purpose: The aim of this study was to analyse the clinical and radiological findings of five patients treated with the diagnosis of hydatid disease of the spleen and discuss their treatment in the view of current literature. The most commonly affected organs are the liver (70%) and the lung (15-20%). But almost every organ of the body can be affected. Among these splenic involvement varies between 0.9 and 8% and isolated splenic involvement is very uncommon. Patients and Methods: Five patients (four female and one male) with hydatid disease of spleen have been enrolled to this study. Results: The average age of the patients was 48.8 (39-60). The most frequent complaint was the pain or mild discomfort in the left upper quadrant. Splenomegaly was found at physical examination of two patients. Three patients were presented with isolated splenic involvement. One of these patients presented with the past medical history of surgery for liver involvement. The cysts of three patients were localised at the splenic hilum and the other two cysts were within paranchima. All patients were operated with splenectomy and intraoperative blood transfusion was needed for one patient. The treatment is indicated due to the risk of infection and rupture. Average operating time was 140 (65-200) minutes and average hospital stay was 11.8 (10-13) days. Conclusion: The preservation of the splenic tissue is especially important for children and should be tried if possible. [ABSTRACT FROM AUTHOR]

Background: There are various surgical and invasive treatment systems such as conservative skin grafts and autologous epider-mal grafting (AEG) for diabetic foot ulcers. This study aims to evaluate the feasibility of using a novel epidermal graft harvesting system in diabetic foot ulcer emergencies., Methods: A retrospective clinical study was conducted with 15 diabetic foot ulcer patients, and after written and signed consent forms were taken, AEG system was applied to all patients. All of the clinical data of the patients such as their American Society of Anesthesiologists (ASA) Physical Status Classification System scores, size of pre-application wound area (cm2), time to complete re-epithelization of the wound, pain scores using the visual analog scale (VAS) for both donor and recipient sites, changes in size of wound, complete dermal response time, and patients' demographics, comorbidities were recorded. The age, gender, pre-post appli-cation wound area (cm2), time of healing, ASA, and VAS variables were compared each other and analyzed statistically. P<0.05 was considered as statistically significant., Results: The mean of time for complete wound healing was 5.9 (range 4-8) weeks. There was no statistically difference between recipient wound size and patient's age; size of both types of wounds (cm2) and time (weeks) for complete reduction for both types of wounds; and time to complete both types of wound healing and gender (p=0.509, 0.788, and 0.233, respectively). ASA scores did not impact the time required for complete healing of the wound (p=0.749)., Conclusion: The current study has tried to evaluate the efficacy of the AEG system in a homogenous population with diabetic foot ulcers. An epidermal harvesting system may be used effectively and safely in patients with diabetic foot ulcer emergencies.

Background: The aim of the present study was to investigate the role of body mass index (BMI) and trauma severity score systems on the mortality rates of patients undergoing surgery due to falls from height., Methods: This retrospective study included 45 consecutive adult patients with multitrauma who were admitted and operated on due to a fall from height between January 2008 and December 2016. Patients were divided into mortality and recovery groups (n=10 and n=35, respectively). The groups were compared in terms of age, gender, weight, cause of the fall, height of the fall, location of the fall, BMI, trauma scores (Injury Severity Score [ISS], New Injury Severity Score [NISS], Glasgow Coma Scale [GCS], Abbreviated Injury Score [AIS], Revised Trauma Score [RTS]), hospital stay, and postoperative complications., Results: There was no statistically significant difference between the two groups in terms of age, gender, and height or cause of the fall (p>0.05), whereas there was a statistically significant difference with respect to weight and BMI values (p<0.01). A statistically significant difference was determined between the groups in terms of hospital stay and postoperative complications (p<0.01) and ISS, NISS, GCS, AIS, and RTS scores (p<0.05)., Conclusion: The effect of weight and BMI on the mortality rate of patients presenting with trauma as a result of a fall from height was objectively demonstrated in the present study.

A number of patients with advanced stage colorectal cancer present to emergency departments with obstruction as the first sign of disease without any previous symptoms. This presentation is an indication for emergency surgery that has a high rate of morbidity and mortality. In this study, we aimed to determine the factors associated with early diagnosis and survival by comparing postoperative results of colorectal cancer patients who underwent surgery under emergency or elective situation., Material and Methods: Files of colorectal patients treated between 2009-2013 were retrospectively analyzed. Data on patient age, gender, operation type, intraoperative results, length of hospital stay, co-morbidities, postoperative complications and pathological results were evaluated and compared., Results: There was no statistical difference between groups in terms of age, gender, and pathology results (p>0.05). The difference between groups in terms of postoperative length of hospital stay, presence of co-morbid diseases, pathological stage, and postoperative complications was statistically significant (p<0.05). Length of hospital stay, advanced stage on admission, complications such as surgical site infection, evisceration, and anastomosis leakage rates were higher in patients in the emergency surgery group., Conclusion: Risk groups should be determined in order to diagnose colorectal cancer patients at an early stage while they are still asymptomatic, and this information should be incorporated into effective screening programs. This approach will be beneficial to treatment outcomes, complication rates, length of hospital stay, and survival and treatment results.

- Abstract
Background: The purpose of this study was to evaluate the effect of clinical, laboratory and radiological results on treatment decision and surgical results in patients with blunt abdominal trauma, who were admitted to the emergency department due to traffic accident., Methods: Two hundred and twenty-two patients with blunt abdominal trauma were included into this retrospective study. Pearson chi square, Mann-Whitney U test and logistic regression methods were used for statistical analysis., Results: All patients were analyzed by complete blood count and biochemistry and abdominal sonography. Eighty-two patients were also evaluated by CT. Twenty-three patients underwent surgery. Positive findings on physical examination, sonography and CT, increased white blood cell count and liver function tests, decreased hemoglobin were associated with the need for surgery., Discussion: For the surgical evaluation of patients with blunt abdominal trauma, a reliable physical examination is not possible when the patients have concomitant injuries causing disturbing pain, or when the patients are unconscious. Laboratory tests should be interpreted with the clinical and radiologic analysis. Radiologic procedures play an important role in the management of patients with blunt abdominal trauma, especially for intubated patients.

43. Diagnosis of acute pancreatitis by diffusion-weighted magnetic resonance imaging.

- Abstract
Diffusion-weighted magnetic resonance imaging has emerged as a successful technique in the early diagnosis of acute pancreatitis. An 82-year-old male patient suspected of acute pancreatitis refused to undergo intravenous contrast-enhanced abdominal computed tomography due to a history of previous allergic reactions to contrast medium. He was imaged with diffusion-weighted magnetic resonance imaging without the use of oral or intravenous contrast material. Diffuse hyperintensity in the pancreas with a relevant apparent diffusion coefficient map showing diffuse hypointensity was demonstrated. The findings were interpreted as restricted diffusion and were diagnostic for acute pancreatitis. Diffusion-weighted magnetic resonance imaging, an imaging modality that does not involve ionizing radiation and does not require the use of contrast material, can successfully demonstrate the manifestations of acute pancreatitis.

44. A prospective evaluation of the risk factors for development of wound dehiscence and incisional hernia.

- Abstract
Objective: Post-laparotomy wound dehiscence, evantration and evisceration are important complications leading to an increase in both morbidity and mortality. Incisional hernias are frequently observed following abdominal surgeries and their occurrence is related to various local and systemic factors. This study aims to analyze the factors affecting wound healing by investigating the parameters that may cause wound dehiscence, incisional hernia, sinus formation and chronic incisional pain., Material and Methods: The records of 265 patients who underwent major abdominal surgery were analyzed. The data on patient characteristics, medication, surgical procedure type, type of suture and surgical instruments used and complications were recorded. The patients were followed up with respect to sinus formation, incisional hernia occurrence and presence of chronic incision pain. Statistical analysis was performed using SPSS 10.00 program. The groups were compared via chi-square tests. Significance was determined as p<0.05. Multi-variate analysis was done by forward logistic regression analysis., Results: 115 (43.4%) patients were female and 150 (56.6%) were male. Ninety-four (35.5%) patients were under 50 years old and 171 (64.5%) were older than 50 years. The median follow-up period was 28 months (0-48). Factors affecting wound dehiscence were found to be; creation of an ostomy (p=0.002), postoperative pulmonary problems (p=0.001) and wound infection (p=0.001). Factors leading to incisional hernia were; incision type (p=0.002), formation of an ostomy (p=0.002), postoperative bowel obstruction (p=0.027), postoperative pulmonary problems (p=0.017) and wound infection (p=0.011)., Conclusion: Awareness of the factors causing wound dehiscence and incisional hernia in abdominal surgery, means of intervention to the risk factors and taking relevant measures may prevent complications. Surgical complications that occur in the postoperative period are especially related to wound healing problems.

- Abstract
Injury due to go-karting accidents is a new kind of surgical emergency in Turkey and may show variations between patients. There are special details as relate to the design of the vehicles and patient characteristics. We report two seriously injured patients as a result of two different go-kart accidents. Severe intraabdominal hemorrhage due to liver laceration was seen in one patient and mesenteric intestinal avulsion in the other. Both patients were treated surgically and discharged uneventfully.
