Objective: Hydatiform mole (HM) is a non-malignant form of gestational trophoblastic disease (GTD) characterized by failure of normal fetal development and overgrowth of trophoblasts. With this retrospective cohort study, we planned to determine the incidence of PHM, etiology and progression rates to Gestational trophoblastic neoplasia (GTN) during the COVID-19 epidemic. Materials and Method: This retrospective cohort study was conducted in Ankara Etlik Zübeyde Hanım Women’s Health Training and Research Hospital Early Pregnancy Assessment Unit between March 2016 and February 2022. Patients who underwent therapeutic curettage (T/C) with the diagnosis of missed abortion, intrauterine exitus (IUEX), molar pregnancy or incomplete abortion after single spontaneous pregnancy were included in the study. The study group consisted of 138 patients who were diagnosed with partial hydatiform mole as a result of pathological examination in this process. There were 135 patients in the control group. Results: The number of patients who applied during the pandemic period and were diagnosed with PHM was 62 (44.92%). The mean age of the patients in the study group was 31.97±8.26 years. The mean body mass index of these patients was 26.38±5.13 m2/kg. The mean paternal age in the study group was 34.95±8.32 years, and it was higher than the paternal age of the patients in the control group (p=0.028). While the incidence of PHM was 1.22/1000 births in our hospital in 2019, this figure was calculated as 3.79/1000 births in 2020. The incidence of progression to GTN at 4 years before the pandemic was 0.02/1000 births; In 2020-2021, this rate was calculated as 0.25/1000 births. Conclusion: During the pandemic period, along with the increase in the incidence of PHM compared to the pre-pandemic period, an increase in the incidence of progression to GTN disease was detected.

Aim: The aim of the study was to evaluate the incidence of asymptomatic COVID-19 disease in pregnant women withrectal and oropharyngeal/nasopharyngeal swab to compare the efficacy of two samples.Material and Method: This prospective cohort study included 234 asymptomatic pregnant women who had undergoneplanned cesarean section between May 7 and September 24, 2020 in obstetrics unit of a tertiary care center, in Ankara,Turkey. The oropharyngeal/nasopharyngeal swab, rectal swab, placental, amniotic fluid, and cord blood samples wereobtained from all participants. The placental, amniotic fluid, and cord blood samples were tested when any of theoropharyngeal/nasopharyngeal or rectal samples were positive. The real-time reverse transcriptase-polymerase chainreaction (RT-PCR) test was performed to detect SARS-CoV-2 virus in the samples.Results: The incidence of the asymptomatic COVID-19 disease was 0.42% (1/234) in the study population. One of 234oropharyngeal/nasopharyngeal swabs was positive, while none of the rectal swabs including the one positive withoropharyngeal/nasopharyngeal swab were positive for SARS-CoV-2 virus. The RT-PCR test results of the placental,amniotic fluid, and cord blood samples of the COVID-positive case were negative.Conclusion: The incidence of asymptomatic COVID-19 disease in pregnant women who had undergone elective cesareandelivery was low, in Ankara, Turkey. In asymptomatic pregnant women, oropharyngeal/nasopharyngeal swab was foundto be more useful in detecting COVID-19 disease compared to rectal swab. No evidence was found about the intrauterinetransmission of asymptomatic disease., Amaç: Çalışmanın amacı, rektal ve orofaringeal / nazofaringeal sürüntü ile gebelerde asemptomatik COVID-19 hastalıkinsidansını değerlendirerek iki yöntemin etkinliğini karşılaştırmaktı.Gereç ve Yöntem: Bu prospektif kohort çalışmasına, Ankara'da üçüncü basamak bir merkezin obstetri ünitesinde 7Mayıs-24 Eylül 2020 tarihleri arasında planlı sezaryen yapılan 234 asemptomatik gebe dahil edildi. Tüm katılımcılardanorofaringeal / nazofaringeal sürüntü, rektal sürüntü, plasental, amniyotik sıvı ve kordon kanı örnekleri alındı. Orofaringeal/ nazofaringeal veya rektal örneklerden herhangi biri pozitif olduğunda plasental, amniyotik sıvı ve kordon kanı örnekleritest edildi. Örneklerdeki SARS-CoV-2 virüsünü tespit etmek için gerçek zamanlı ters transkriptaz-polimeraz zincirreaksiyonu (RT-PCR) testi yapıldı.Bulgular: Asemptomatik COVID-19 hastalık insidansı çalışma popülasyonunda % 0,42 (1/234) idi. 234 orofaringeal /nazofaringeal sürüntüden biri pozitif iken, orofaringeal / nazofaringeal sürüntü ile pozitif olan dahil rektal sürüntülerinhiçbiri SARS-CoV-2 virüsü için pozitif değildi. COVID pozitif olgunun plasental, amniyotik sıvı ve kordon kanı örneklerininRT-PCR testi sonuçları negatifti.Sonuç: Ankara'da elektif sezaryen ile doğum yapmış gebelerde asemptomatik COVID-19 hastalığı insidansı düşüktü.Asemptomatik hamile kadınlarda, orofaringeal / nazofaringeal sürüntü rektal sürüntü ile karşılaştırıldığında COVID-19hastalığını tespit etmede daha yararlı bulundu. Asemptomatik hastalığın intrauterin geçişine dair bir kanıt bulunamadı.

Amaç: Abortus imminens 20.gestasyonel haftadan önce servikal değişiklikler olmaksızın vajinal kanama görülmesi olarak tanımlanır ve gebelik sonuçlarını etkileyen bir klinik durumdur. Çalışmamızın amacı abortus imminensin (Aİ) gebeliğin birinci veya ikinci trimesterinde izlenmesinin obstetrik sonuçlara etkisini araştırmaktır. Gereç ve Yöntem: Bu retrospektif çalışmaya Ocak 2017 ve Aralık 2019 yılları arasında erken gebelik servisine Aİ nedeniyle yatışı olan 831 tekil gebelik dahil edildi. Aİ tanısını aldığı gestasyonel haftaya göre olgular 1. ve 2. trimester (Grup 1 ve Grup 2) olmak üzere ikiye ayrılarak obstetrik sonuçlar karşılaştırıldı. Bulgular: Toplam 831 olgunun 622’si (%75,8) grup 1’e ve 209’u (%25,2) grup 2’ye ait idi. Grup 1’dekilerin %51,1’inde ve grup 2’dekilerin %44,2’sinde hematom mevcuttu (p=0,075). Objective: The aim of this study was to evaluate the obstetric and perinatal outcomes of fetuses diagnosed with bradycardia in the first trimester by comparing them with the control group.Material and Method: In this study, which was designed as a retrospective cohort study, 58 pregnant women who were followed up and treated in our hospital and diagnosed with bradycardia in the first trimester were included as the study group. 276 normal pregnant women were included in the study as a control group. For each patient, fetal heart rate, age, parity, number of abortions, gestational week, body mass index (BMI), whether assisted reproductive techniques were used, NT value measured between 11-14 weeks, type of abortion, week of delivery, type of delivery, neonatal intensive care unit admission, number of hospital days, and fetal sex were noted, and the perinatal outcomes of these two groups were compared.Results: The mean age was 27.8 ± 6.1 years in the bradycardic group and 28.2 ± 6.0 years in the control group. The mean value of NT in the first trimester was 1.56 ± 0.61 in the bradycardic group and 1.26 ± 0.34 in the control group, and the NT of the bradycardic group was statistically significantly higher than that of the control group (p < 0.001). Also, when comparing NT values by week, the number of patients with NT above the 95th percentile was 13.8% (n=8) in the bradycardic group and was statistically significantly higher than 1.8% (n=5) in the control group (p < 0.001). In the bradycardic group, 81% (n=47) had an intrauterine exitus before 20 weeks, 12.1% (n=7) were hospitalized with an abortion, and 6.9% (n=4) resulted in a live birth. 2.4% (n=5) of the control group had an intrauterine exitus before the 20th week, 1.4% (n=3) were hospitalized with an abortion, 94.7% (n=197) resulted in a live birth, and 1.4% (n=3) had an intrauterine exit after the 20th week.Conclusion: Worse pregnancy outcomes were found such as abortion, intrauterine exitus, and high NT scores in fetuses who were bradycardic in the first trimester than in normal pregnant women., Amaç: İlk trimesterde bradikardi saptanan fetüslerin kontrol grubu ile kıyaslanarak obstetrik ve perinatal sonuçlarının değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Retrospektif kohort dizayn edilen bu çalışmayla hastanemizde takip ve tedavisi yapılmış ilk trimesterde bradikardi tespit edilen 58 gebe ve 276 normal gebe kontrol grubu olarak alınarak çalışmaya dahil edildi. Postoperative adhesions can cause serious complications, including intestinal obstruction, chronic abdominopelvic pain, and infertility in women. Here we investigate the effects of disulfiram on the postoperative adhesion model. Female Wistar rats were used (n = 72). The animals were separated into six groups (12 rats per group): group 1 (control), group 2 (300 mg/kg disulfiram administered for 3 days preoperatively), group 3 (50 mg/kg disulfiram administered for 3 days preoperatively and 14 days postoperatively), group 4 (300 mg/kg disulfiram administered for 3 days preoperatively and 14 days postoperatively), group 5 (50 mg/kg disulfiram administered 14 days postoperatively only), and group 6 (300 mg/kg disulfiram administered 14 days postoperatively only). A histopathologic examination was performed. Immunohistochemical stainings for matrix metalloproteinase-2 and 9 (MMP-2, and MMP-9) and vascular endothelial growth factor (VEGF) were evaluated. The macroscopic adhesion scores were significantly lower in the disulfiram groups (groups 3, 4, and 6) compared to the control group (p < 0.05). Inflammation scores were lower in all groups receiving disulfiram, but only reached statistical significance in group 4 (p < 0.05). In the immunohistochemical evaluation of the groups, MMP-9 was significantly lower in group 5 than group 4 (p < 0.05). There was no significant difference between the groups for MMP-2 and VEGF. We found that disulfiram reduced postoperative adhesion formation. Disulfiram becomes more effective (by directly reducing inflammation) when initiated during the preoperative period at high doses.

Tum myomlarin yaklasik %5’ini olusturan submukoz myomlar, endometriumun hemen altindaki kas tabakasindan kaynaklanirlar ve buyudukce uterin kaviteyi distorte ederler. Kavite icinde olan sapli submukoz myomlar ise, uterin kontraksiyonlarla bazen serviksten gecerek vajene kadar ilerleyebilirler. Vajene dogmus myom diye adlandirilan bu myomlarin en sik tedavi sekli vajinal yolla yapilan myomektomidir. Ancak nadiren, myomun kavite icindeki bolumunu de cok buyuk boyutlara ulasirsa abdominal yaklasim gerekli olabilir. Bu yazida vajeni tamamen dolduran, kavite icinde de dev buyuklugune ulasan ve preoperatif donemde yapilan manyetik rezonans goruntulemede leiomyosarkom suphesi olan bir olgu sunulmustur. Yaklasik 2 aydir vajinal kanama sikayeti olan 45 yasindaki multipar hastanin, yapilan spekulum muayenesinde vajeni tamamiyle dolduran 8 cm capinda kotu kokulu nekrotik bir kitlesi saptanmistir. Uterusun 3 aylik cesamette olmasi 12 cm buyuklugune ulasan submukoz myom izlenmesi uzerine hastaya abdominal histerektomi yapilmistir. Ancak myomun vajendeki bolumu abdominal olarak cikarilamamistir. Myom vajinayi tamamiyle doldurmus oldugundan bu kisim vajinal yolla eksize edilmistir.

Objective To investigate the likelihood of the detection of the necessity of staging preoperatively with the use of clinical parameters and frozen/section (FS). Material and methods 219 patients were included who were operated on between 1996 and 2010 with a diagnosis of grade 1 endometrioid adenocarcinoma in probe curettage. Results Among the clinical characteristics, only age and body mass index (BMI) predicted staging preoperatively. The probability of staging increased as age increased and BMI decreased. The concordance between preoperative diagnosis and FS was 89.5%. The diagnosis was upgraded at postoperative evaluation for 13 patients (5.9%), and downgraded for 2 patients (0.9%) compared with FS. The wrong diagnosis regarding grade, the depth of myometrial invasion DMI, tumour type and cervical invasion in FS was clinically significant and affected the decision of staging in 10 patients. In conclusion, only 7 patients (3.2%) who acquired staging surgery were missed in FS. Conclusion It was shown that preoperative clinical parameters could not effectively predict the patients who should be staged. FS predicted the lymphatic involvement with high accuracy. The patient with a preoperative diagnosis of grade 1 endometrium cancer should be operated upon in centres where FS is utilised and oncologic staging surgery can be performed.
