Aim: To compare the demographic, clinical, surgical, histopathological, and oncological outcomes of vNOTES and conventional laparoscopy (CL)for early‐stage endometrial cancer. Methods: A retrospective study was carried out in the Gynecologic Clinic of a tertiary hospital from January 2019 to November 2020. Patient demographic characteristics, surgical outcomes, histopathological characteristics, visual analog scale (VAS) pain scores at postoperative 6th, 12th, and 24th, intra‐ and postoperative complications, and follow‐up results were noted. Results: A total of 45 patients enrolled, of which 16 underwent CL and 29 were vNOTES. The operative time and decrease in hemoglobin levels were similar for both groups (p = 0.202, p = 0.699). Postoperative hospital stay did not differ between the vNOTES group and the CL group (p = 0.549). VAS pain scores at postoperative 6th, 12th, and 24th h were significantly lower in vNOTES group than in the CL group (p < 0.001). The requirement for additional opioid/narcotic analgesic was lower in the vNOTES group than in the CL group (p = 0.037). Conclusion: vNOTES may be a safe and feasible option in early‐stage endometrial cancer, having less postoperative pain and less requirement of opioid/narcotic analgesic compared with laparoscopy. Background and Objectives: There is a lack of studies examining balance problems and Achilles tendon thickness in prediabetes despite their common occurrence in diabetes mellitus. The aim of this study was to evaluate Achilles tendon size and static and dynamic balance, as well as the role of the Achilles tendon in balance, in prediabetic patients. Materials and Methods: A total of 96 participants were divided into three groups: (1) the control group, consisting of participants without diabetes mellitus; (2) the prediabetes group; and (3) the diabetes mellitus group. Ultrasonographic measurements of Achilles tendon sizes (thickness, width and area) were performed. Dynamic balance was assessed using the Berg Balance Scale, and static balance (the Fall and Stability Indices) was assessed using a Tetrax device. The Self-Leeds Assessment of Neuropathic Symptoms and Signs was utilized to identify neuropathic pain. Results: In the prediabetes group, the median dynamic balance scores [54.0 (51.0–56.0)] were lower than those of the control group [55.0 (54.0–56.0)] but higher than those of the patients with diabetes mellitus [52.50 (49.0–54.25)]; however, this difference did not reach statistical significance. The ultrasonographic measurements of the Achilles tendon size were similar among the three groups. On the other hand, in the prediabetes group, a positive correlation was observed between the bilateral Achilles tendon anterior–posterior thickness and Fall Index score (p = 0.045), while a negative correlation was found between the left Achilles tendon anterior–posterior thickness and the Berg Balance Score (p = 0.045). Conclusions: In prediabetes, neither Achilles tendon size nor static or dynamic balance appears to be significantly affected. However, in prediabetic patients, increased Achilles tendon thickness appears to be associated with increased risk of falls and decreased balance. [ABSTRACT FROM AUTHOR]

This study was conducted to determine the changes in the quality of rainbow trout (Oncorhynchus mykiss) meatballs cooked with the sous-vide technique. Vacuum-packaged fishball were cooked using the sous-vide method under two different temperature (60 °C and 80ºC) and two different times (15 and 20 minute) and stored at 4±1ºC for 25 days. The results showed that sous vide cooking technique at 80 °C for 20 min. effectively limited bacterial growth. It was determined that sous vide cooking technology significantly reduced the levels of total volatile base nitrogen (TVB-N) and thiobarbituric acid reactive substances (TBARS) at the end of storage. As the cooking temperature increased, the pH values of the meatball samples also increased. According to the sensory evaluation results, the meatballs cooked with the sous vide technique received higher scores than the control group, and the most preferred cooking temperature and time was determined to be 60ºC for 20 minutes. Objective: To describe the characteristics of pregnant women diagnosed with adnexal masses and review our experience with diagnosis, management and treatment modalities in this area for which there are no definitive treatment guidelines. Methods: All women with adnexal masses during pregnancy whose management and follow-up was completed at a large tertiary referral center between January 2018 and September 2023 were retrospectively evaluated. After applying the exclusion criteria, a total of 19 women who underwent antepartum laparoscopic adnexal surgery (group 1) and 34 women who underwent expectant management (group 2) were included. Results: Gestational age at birth, cesarean section rates, birthweight and APGAR-5 scores were similar, while the largest mass diameter and CA-125 levels were significantly higher in group 1 (p<0.001 and p=0.014, respectively). Of the patients in group 1, 15 (78.9%) had unilateral complex cysts and 4 (21.1%) had unilateral simple cysts, while there were no bilateral masses. Emergency laparoscopy was performed in 6 (31.6%) of patients for adnexal torsion and in 7 (36.8%) for cyst rupture, while elective laparoscopy was performed in 6 (31.6%) asymptomatic patients with suspected malignancy. While functional cyst was the most common histopathologic finding, malignancy was found in 2 cases (granulosa cell tumor and endometrioid adenocarcinoma). Preterm births occurred in 8 (41.1%) women, of whom 5 (26.3%) gave birth before 34 weeks' gestation and 3 (15.7%) between 34-36 weeks and 6 days. Conclusion: Parallel to the increase in ultrasound examinations in early pregnancy, both the overdiagnosis and early detection of adnexal masses during pregnancy have increased. Given the high rate of spontaneous regression in most lesions and the fact that bleeding in ruptured cases is usually self-limiting, an expectant approach would be appropriate in cases without hemodynamic instability, acute abdomen and/or suspected malignancy. Objective: The study aimed to investigate the relationship between mid-trimester cervical length and pre-term delivery and maternal characteristics. Methods: Cervical length measurement was carried out in 98 pregnant women who presented to antenatal outpatient clinic between 20 and 24th weeks' pregnancy. Age, obstetric history, gravida, parity, the number of abortion, and body mass index (BMI) were also recorded. Births before 37th week formed preterm birth group, and those after 37th week formed term birth group. Results: Of pregnant women included in the study, 77 (78.6%) gave birth after 37th week of pregnancy, while 21(21.4%) gave birth before 37th week of pregnancy. Fourteen cases (14.3%) with cervical length under 25 mm, while 84 (85.7%) cases with cervical length over 25 mm. Twelve (12.24%) of the study group had a history of preterm birth. Of these cases, 11 (91.7%) had a recurrent pre-term birth. The rate of pre-term birth was higher in cases with a cervical length less than 25 mm compared to other cases (p<0.05). There was a signifi- cant relation between cervical length and age and BMI levels (p<0.05). No significant relation was found between parity and term and pre-term births (p>0.05). There was a significant relation between BMI and term and pre-term birth rates (p<0.01). The rate of pre-term birth was significantly higher in cases with previous history of pre-term birth (p<0.05). Conclusion: Mid-trimester short cervical length associated with presence of funneling, history of preterm birth, and obesity. Both a history of pre-term birth and a short cervix were associated with preterm birth. Therefore, in this patient group, routine cervical length measurement at 20--24 pregnancy weeks is recommended. Purpose: To assess the fracture resistance of monolithic zirconia crowns cemented with different types of cement on cement‐retained implant abutments. Materials and methods: Forty implant analogs were positioned in acrylic resin blocks, and cement‐retained straight implant abutments were fastened to the analogs. Crowns were designed with/without occlusal vent holes and produced from monolithic zirconia blocks by the CAD‐CAM technique. The two crown types were divided into two groups and cemented with resin and zinc‐polycarboxylate cement under 5 kg weight. A universal testing machine applied compressive forces to the crowns until fracture. Fracture resistance values were analyzed using two‐way ANOVA and the independent samples t‐test with statistical significance set at p < 0.05. Results: According to the two‐way ANOVA results, although the crown design did not have a significant effect on fracture resistance (1417.65 ± 337.39 N, 1565.16 ± 517.12 N; crowns with and without vent holes, respectively), the main effect of the cement variable on the fracture resistance was significant. Zinc‐polycarboxylate cement (1680.1 ± 375.23 N) showed higher fracture resistance than resin cement (1302.71 ± 420.64 N) in the crowns designed with vent holes (p < 0.005). Conclusion: The use of cement‐retained implant‐supported monolithic zirconia crowns with an occlusal vent hole is safe, and zinc‐polycarboxylate cement use may be an appropriate choice for cementation of these crowns. Objective: To compare the effect of oral and vaginal micronized progesterone in terms of regularity of menstrual cycle, blood hemoglobin and lipid levels, and endometrial thickness in patients with abnormal uterine bleeding (AUB). Methods: A total of 80 patients with AUB were randomized into two groups: the oral micronized progesterone group (n=40) and the vaginal micronized progesterone group (n=40). After 3 months of treatment, patients were inquired about compliance to treatment, treatment satisfaction (satisfied-unsatisfied), whether a regular cycle was present, and side effects. Pre- and posttreatment parameters were compared in terms of ultrasonographic endometrial thickness measurement, and hematologic and biochemical parameters. Results: Overall 66 patients, the first group (oral progesterone) and the second group (vaginal progesterone) were evaluated. There was no statistically significant difference between the two groups in terms of age, parity, and body mass index. In both groups, hemoglobin levels were found to be significantly increased after treatment (p=0.0001). No statistically significant intragroup or intergroup difference was found in either group in terms of lipid values before and after treatment. There was a significant decrease in endometrial thickness after treatment in both groups (p=0.0001). After treatment, in the oral progesterone group, the menstrual cycle became regular in 26 (81.25%) patients while in the vaginal progesterone group, it became regular in 30 (88.23%) patients, with no significant difference between groups. In the first group (n=25/32), 78.12% of patients were satisfied with the treatment, whereas in the second group (n=28/34) 82.35% were satisfied with the treatment, with no significant difference between groups. Conclusion: Considering its efficacy and safety, easy toleration, and few side effects, vaginal micronized progesterone may be a good alternative to oral preparations in the treatment of AUB.
