Amaç: Lomber disk hernisi nedeni ile opere olan hastalarda disk hernisinin görüldüğü seviye ile pelvik ve lomber vertebra insidansları arasındaki ilişkinin değerlendirilmesi amaçlandıMateryal ve Metot: Retrospektif olarak planlanan çalışmaya dosya kayıtları incelenen hastalar dâhil edildi. Tek seviye lomber disk hernisi tespit edilen ve tek taraflı mikrodiskektomi yöntemi ile opere olan 59 hastanın demografik verileri ve klinik bulguları kayıt edildi. Değişkenlerin analizinde SPSS 25.0 (IBM Corparation, Armonk, New York, United States) programı kullanıldı Bulgular: Yirmi sekizi erkek 31’i kadın olan hastaların yaş ortalaması 43,31±4,27 (38-54) yıl olarak bulundu. Yerleşim yerlerine bakıldığında 28 (%47,5) hastanın sağ, 31 (%52,5) hastanın ise sol yerleşimli disk hernilerinin olduğu saptandı. Seviyeye göre bakıldığında L5-S1seviyesinde disk hernisi olan hastaların pelvik indeks ve L4 indeks ortalamaları L4-5 ve L3-4 seviyelerinde disk hernisi olan hastaların ortalamalarına göre anlamlı derecede düşük bulunurken (p=0,002, p=0,001), L4-5 seviyesinde disk hernisi olan hastaların L5 insidans ortalamaları anlamlı derecede yüksek bulundu (p=0,001).Sonuç: Bu çalışmada, L5-S1 seviyesinde disk hernisi olan hastalarda pelvik indeks, L4 ve L5 indeksler anlamlı derecede düşük bulunmuştur. Gelecekte, uzun süreli takipli ve prospektif, çok merkezli çalışmaların yapılmasının gerekli olduğunu düşünmekteyiz., Aim: The aim of this study is to assess the correlation between the level of disc herniation and pelvic and lumbar vertebral incidences in patients operated for lumbar disc herniation. Material and Method: This retrospective study included those patients whose file records were analyzed. The demographic data and clinical findings of 59 patients diagnosed with single level lumbar disc herniation and underwent unilateral microdiscectomy were recorded. In the analysis of the variables, SPSS 25.0 (IBM Corparation, Armonk, New York, United States) was used.Results: The patients involved 28 men and 31 women whose median age was found as 43,31±4,27 (38-54) years. With regard to location, it was determined that in 28 (47,5%) patients, the herniated discs were located on the right and in 31 patients (52,5%), they were located on the left. In terms of levels, in patients with herniated discs at level L5-S1, the median pelvic index and L4 index were found to be significantly lower than those of patients with herniated discs at levels L4-5 and L3-4 (p=0,002, p=0,001), whereas in patients with herniated discs at level L4-5, the median L5 incidence was found to be significantly high (p=0,001).Conclusion: In this study, the pelvic index and L4 and L5 indexes were found to be significantly low in patients with herniated discs at level L5-S1. It is considered that future prospective and multi-centered studies with long-term follow-up are necessary.

Aim: To evaluate and compare the mRNA expression of antioxidant genes between the cervical and lumbar degenerated disc tissues. Material and Methods: Obtained degenerated Nucleus Pulposus (DNP) materials were divided into two groups, which were cervical DNP (Group I) (n=20) and lumbar DNP (Group II) (n=20). There are 6 men and 14 women in group I with a mean age of 43.8 years and 13 men and 7 women in group II with the mean age of 40.3 years (28 to 54). All cervical DNP materials were obtained between C5 and C6 and all lumbar DNP materials were obtained between L4 and L5 levels. The TGF?1, bFGF, IGF-1, NGF and Matrix metalloprotease III (MMP III) gene expressions of DNP materials were determined by polymerase chain reaction, Real-time polymerase chain reaction and western blotting techniques.Results: When the results of the two groups were compared by polymerase chain reaction, the expressions of TGF?1, bFGF, IGF-1, NGF and MMP III was found lower in lumbar DNP group. Also the TGF?1, bFGF, IGF-1, NGF, and Matrix metalloprotease III genes showed a decrease in gene expression levels when they were analyzed by Real-time polymerase chain reaction. Conclusion: These data showed that; decrease of the TGF?1, bFGF, IGF-1, NGF and MMP III genes in the degenerated lumbar disc tissues, may related with the possibility of molecular background of the disease pathogenesis.

AMAÇ: Uyku Apne Sendromu (UAS), toplumda yaygın olarak gö- rülen bir hastalıktır. UAS’lu hastalarda, depresyon ve anksiyete birlikteliğini değerlendirmek farklı ölçeklerin kullanıldığı çalışmalar olmakla birlikte bu konuyla alakalı çelişkili sonuçlar bulunmaktadır. Bu çalışmamızda amaç UAS ile anksiyete ve depresyon korelasyonunu araştırmaktır. YÖNTEM: Çalışmaya polisomnografik inceleme yapılan 134 olgu alındı. Apne-Hipopne İndeksi (AHİ) 0,05). SONUÇ: Çalışmamızda; HAD ölçeği kullanılarak gündüz aşırı uykululuğu, tanıklı apnesi ve horlaması olan hastalarda anksiyete ve depresyon birlikteliğini değerlendirmeyi ve bunun yanı sıra bu hastalarda apne-hipopne endeksi değerleri ile HAD puanlarının arasında korelasyonun olup olmadığını belirlemeyi amaçladık., Sleep apnea syndrome (SAS) is commonly seen disorder in the population. There are many studies using different questionnaires to evaluate the patients who are diagnosed wit SAS and also suffering from depression and anxiety disorder; as there are many different questionnaires to evaluate these patients, the results of these studies have many discrepancies. We aim to research correlation of anxiety and depression with this study. METHODS: 134 cases were recruited for the polysomnographic evaluation and these cases are used as the subjects of this study. The participating patients are divided into two main groups: 51 cases with AHI5 are named as the patient group. Later, these groups are subdivided into 3 more classes; the first one was the mild SAS patients consisting of 27 cases with AHI: 5–14.9, second one was the middle SAS patients consisting of 24 cases with AHI: 15–29.9 and finally the third one was the heavy SAS class which was consisting of 32 patients with AHI≥30. Hospital Anxiety Depression Test (HADT) was applied to all cases. RESULTS: 56.7% of the patients participating to the study were male and the remaining 43.3% of the patients were female. The mean age was 48.54±10.59. Control group the mean body mass index (BMI) was 30.11±4.84, patient group the mean BMI was 31.97±5.10. There was no statistically significant correlation between the depression and anxiety scores and AHI scores of the control and patient groups. CONCLUSION: We used the HAD scale to evaluate excessive daytime sleepiness and the concurrence with depression and also to determine whether a correlation was present between the apnea-hypopnea index values and HAS scores in these patients in this study.

Amaç: Vücuttaki birçok organda lenfatik drenaj, tanımlı lenfatik kanallardan bölgesel lenf nodlarına kadar olan uzanım boyunca çözünmüş maddeleri, antijenleri, antijen sunucu hücreleri ve yıkılmış doku parçalarını içerir. Santral Sinir Sistemi'nin (SSS) ise, konvansiyonel lenfatik sisteme ve lenfatik drenaja sahip olmadığı, immünolojik açıdan özel bir konuma sahip olduğu düşünülür. Beynin içinde konvansiyonel lenfatik yolaklar olmamasına karşın, fizyolojik çalışmalar insanda beyinden servikal lenf nodlarına kadar zengin ve immünolojik açıdan önemli bir lenfatik drenajın varlığını ortaya koymuştur. Metod: Beyin parankiminden sıvıların ve yıkılan parçaların (anormal-artık proteinler) perivasküler drenajındaki bozukluklar Protein Eliminasyon Yetmezliği Arteriyopatileri (PEYA) olarak isimlendirilmektedir. Bu tip bozukluklarda meydana gelen protein birikimleri, Serebral Amiloid Anjiyopati (SAA), Alzheimer Hastalığı (AH), Crutzfeld-Jacobs Hastalığı (CJH), Parkinson Hastalığı (PH), Kronik Travmatik Ensefalomiyopati (KTE), Frontotemporal Demans (FTD), Sporadik Amiyotrofik Lateral Skleroz (SALS) gibi proteinopatileri kapsayan hastalıkların patofizyolojileriyle ilişkilidirler. Bulgular: Bizim düşüncemize gore, PEYA toksik, dejenere olmuş, olasılıkla kullanılmış, artık (anormal) proteinlerin ekstrasellüler ortamdan eliminasyonundaki yetersizlik ile karakterize bir hastalıktır. Bu proteinlerin anormal bir şekilde katlanmış olan çeşitlerinin nasıl toksik hale gelebildikleri ve hastalığa yol açtıkları ile ilgili bu yeni görüşler, hastalığın patogenezi ile ilgili yeni anlayışlar getirmekte ve ayrıca yeni bazı tedavi olanakları için de imkân sunmaktadır. Sonuç: Bu derlemede biz, beyin interstisyel sıvısının (İSS) beyinde öncelikli yolaklar boyunca akışı, beyin omurilik sıvısı (BOS) ile ilişkisi ve beyin parankiminden lenfatik drenajın blokajının sonuçlarıyla ilgili bulguları gözden geçirmekteyiz., Aim: In most organs of the body, lymphatic drainage involves the drainage of solutes, antigens, antigen presenting cells and tissue debris along the defined lymphatic channels to the regional lymph nodes. The Central Nervous System (CNS) is considered to be an immunologically privileged organ without conventional lymphatic system and lymphatic drainage. Although there are no conventional lymphatic pathways in the brain, physiological studies revealed a substantial and immunologically significant lymphatic drainage from brain to cervical lymph nodes in humans. Methods: Disturbance of the perivascular drainage of fluids and solutes (abnormal- garbage proteins) from the brain parenchyma is called as Protein Elimination Failure Arteriopathies (PEFA). Those protein deposits are associated with the pathophysiology of these diseases and include Cerebral Amyloid Angiopathy (CAA), Alzheimer's Disease (AD), Cruzfeld-Jacobs Disease (CJD), Parkinson's Disease, Chronic Traumatic Encephalomyopathy (CTE), Results: According to our opinion PEFA is a disease characterized by the failure of elimination of toxic, degenerated and possibly used garbage (abnormal) proteins from the extracellular milieu. In lights of this new idea about how abnormally folded versions of these proteins may be toxic and lead to disease, our findings are yielding insights into the disease pathogenesis but may also provide some new therapeutic insights. Conclusion: In this review we survey the evidences for the flow of brain interstitial fluid (ISF) via the preferential pathways through the brain, its relation to cerebrospinal fluid (CSF) and the results of the blockage of lymphatic drainage form the brain parenchyma.
