Abstract

Disorders of consciousness are a heterogeneous mixture of different diseases or injuries. Although some indicators and models have been proposed for prognostication, any single method when used alone carries a high risk of false prediction. This study aimed to develop a multidomain prognostic model that combines resting state functional MRI with three clinical characteristics to predict one year outcomes at the single-subject level. The model discriminated between patients who would later recover consciousness and those who would not with an accuracy of around 90% on three datasets from two medical centers. It was also able to identify the prognostic importance of different predictors, including brain functions and clinical characteristics. To our knowledge, this is the first implementation reported of a multidomain prognostic model based on resting state functional MRI and clinical characteristics in chronic disorders of consciousness. We therefore suggest that this novel prognostic model is accurate, robust, and interpretable., Comment: Although some prognostic indicators and models have been proposed for disorders of consciousness, each single method when used alone carries risks of false prediction. 9. Influencing factors for tracheostomy decannulation in patients with prolonged disorders of consciousness

Abstract

Objective To analyze the influencing factors for tracheostomy decannulation in patients with prolonged disorders of consciousness. Methods Clinical data of 59 cases with prolonged disorders of consciousness and having undergone tracheotomy in Guangdong Provincial Work Injury Rehabilitation Hospital from January 2019 to October 2020 were collected and retrospectively analyzed in this study. The patients were divided into extubation group (n=30) and non-extubation group (n=29). Age, cause of disease, duration of diseases (time from onset to extubation), duration of tracheostomy, total score and scores of functional mobility and language in Coma Recovery Scale-Revised (CRS-R), airway clearance by coughing, amount of expectation (evaluated by bronchofiberscopy), laryngeal edema and tracheal stenosis were analyzed to assess their relationships with tracheostomy decannulation. Results There were no statistical differences in age, gender, cause of disease, duration of diseases and average duration of tracheal intubation between the 2 groups. The extubation group had significantly higher total score of CRS-R, larger proportions of those with the score of functional mobility ≥3 and that of language ≥1, better airway clearance by coughing, milder laryngeal edema, lower proportion of residual expectation grade ≤l, and lower incidence of tracheal stenosis when compared with the non-exubation group (all P < 0.05). Logistic regression analysis showed that tracheal obstruction (P=0.001), abnormal salivary secretion (P=0.001) and CRSR-R score (P=0.036) were significant factors influencing extubation. Conclusion Tracheostomy decannulation is closely related to duration of disease, conscious status, cough reaction, laryngeal edema, amount of phlegm, and airway stenosis

28. A Hybrid Brain–Computer Interface Combining P300 Potentials and Emotion Patterns for Detecting Awareness in Patients With Disorders of Consciousness

Abstract

In this study, a hybrid brain–computer interface (BCI) system combining P300 potential and emotion patterns was proposed to improve the performance of awareness detection. Two video clips were flashed randomly to evoke the P300 potential, while a laughing or crying video clip was used to induce the corresponding emotion pattern. The subjects were asked to concentrate on the laughing or crying video clip cued by the instruction and to count the flashes of the corresponding video clip. Two layers of classification were developed. In the first layer, P300 detection and emotion recognition were performed separately using two support vector machine (SVM) classifiers. Specifically, the activation, spatial, and connection patterns were fused in emotion recognition. In the second layer, the SVM scores of P300 detection and emotion recognition were fed into another SVM classifier to determine which video clip the subjects responded to. Six healthy subjects and eight patients with disorders of consciousness (DOC) were involved in the command-following experiment. The results showed that the accuracy of the hybrid BCI system was better than those of the single-modality systems. Furthermore, three patients were able to perform tasks (66%–72%) using our hybrid BCI, which indicated their residual awareness and emotion-related abilities.

Abstract

Objectives: Disorders of consciousness (DoC) is a dynamic and challenging discipline, presenting intriguing challenges to clinicians and neurorehabilitation specialists for the lack of reliable assessment methods and interventions. Understanding DoC keeps pace with scientific research is urgent to need. We quantitively analyzed publications on DoC over the recent 10 years via bibliometrics analysis, to summarize the intellectual structure, current research hotspots, and future research trends in the field of DoC. Methods: Literature was obtained from the Science Citation Index Expanded of Web of Science Core Collection (WoSCC). To illustrate the knowledge structure of DoC, CiteSpace 5.8.R3 was used to conduct a co-occurrence analysis of countries, institutions, and keywords, and a co-citation analysis of references and journals. Also, Gephi 0.9.2 contributed to the author and co-cited author analysis. We found the most influential journals, authors, and countries and the most talked about keywords in the last decade of research. Results: A total of 1919 publications were collected. Over the past 10 years, the total number of annual publications has continued to increase, with the largest circulation in 2018. We found most DoC research and close cooperation originated from developed countries, e.g., the USA, Canada, and Italy. Academics from Belgium appear to have a strong presence in the field of DoC. The most influential journals were also mainly distributed in the USA and some European countries. Conclusions: This bibliometric study sheds light on the knowledge architecture of DoC research over the past decade, reflecting current hotspots and emerging trends, and providing new insights for clinicians and academics interested in DoC. The hot issues in DoC were diagnosing and differentiating the level of consciousness, and detecting covert awareness in early severe brain-injured patients. New trends focus on exploring the recovery mechanism of DoC and neuromodulation techniques. [ABSTRACT FROM AUTHOR]
