Abstract Esophageal and gastric varices (EGV) bleeding is a dangerous side effect of liver cirrhosis. Ascites may affect the effectiveness of carvedilol in preventing EGV rebleeding. A retrospective analysis was done on patients with EGV bleeding who visited our gastroenterology department between January 1, 2015, and October 29, 2020, and were given carvedilol therapy again. Patients were classified based on whether they had ascites. The primary outcome was EGV rebleeding. A total of 286 patients were included, with a median follow‐up of 24.0 (19.0–42.0) months, comprising those without ascites (N = 155) and those with ascites (N = 131). The mean age of the patients was 55.15 ± 12.44 years, and 177 (61.9%) of them were men. There were 162 (56.6%) Child‐Pugh A grades. Effective emergency logistics facility site selection is vital for ensuring prompt and fair food supply during crises. This study tackles the intricate task of choosing optimal sites for emergency food logistics facilities by considering varying urgency levels of needs, uncertain demands, and potential facility interruptions. A novel weighted Mahalanobis distance–gray relational analysis–TOPSIS method is devised to evaluate demand urgency and guide site selection decisions. The proposed location model aims to minimize total cost and unmet demand while integrating discrete scenario strategies to address interruption events. Leveraging the Social Network Search (SNS) algorithm, the model is solved, and its effectiveness is validated through a case study analysis. The results highlight the accuracy of the urgency level determination method in capturing demand characteristics and the model’s provision of an objective and practical framework for formulating rational facility location strategies. This approach holds significant promise for enhancing the promptness and fairness of food supply assurance during emergencies.

The variability and intermittency inherent in renewable energy sources poses significant challenges to balancing power supply and demand, often leading to wind and solar energy curtailment. To address these challenges, this paper focuses on enhancing Time of Use (TOU) electricity pricing strategies. We propose a novel method based on equivalent load, which leverages typical power grid load and incorporates a responsibility weight for renewable energy consumption. The responsibility weight acts as an equivalent coefficient that accurately reflects renewable energy output, which facilitates the division of time periods and the development of a demand response model. Subsequently, we formulate an optimized TOU electricity pricing model to increase the utilization rate of renewable energy and reduce the peak–valley load difference of the power grid. To solve the TOU pricing optimization model, we employ the Social Network Search (SNS) algorithm, a metaheuristic algorithm simulating users’ social network interactions to gain popularity. By incorporating the users’ mood when expressing opinions, this algorithm efficiently identifies optimal pricing solutions. Our results demonstrate that the equivalent load-based method not only encourages renewable energy consumption but also reduces power generation costs, stabilizes the power grid load, and benefits power generators, suppliers, and consumers without increasing end users’ electricity charges.

Aim: This is a Chinese population-based study aimed to determine the causes and clinical features of drug-induced liver injury (DILI) from traditional Chinese medicines (TCMs) and current Western medicines (WMs) and identify the risk factors of drug-induced liver failure (DILF) and chronic DILI for early recognition and better management. Methods: The medical records of patients who were diagnosed with DILI for at least six-month follow-up between January 2018 to December 2020 were reviewed and investigated. The risk factors of DILF and chronic DILI were identified by univariate and multivariate logistic regression analysis. Results: TCMs (47.5%) including herbal medicine (83.0% in TCM-induced DILI) and some Chinese patent drugs were the leading cause of DILI in the present study. Cholestatic type was more associated with severe and chronic DILI. Pre-existing gallbladder disease, initial total bilirubin (TBIL), initial prothrombin time (PT), initial antinuclear antibodies (ANA), and clinical classification are independent risk factors for DILF. Prolonged T0.5AST and T0.5GGT were independent risk factors for chronic DILI [area under the curve (AUC) = 0.812, 95% confidence interval (CI): 0.748–0.876, P < 0.001] with cut-off values of 8.5 days and 29.5 days, respectively. Conclusions: TCMs especially herbal medicine were the leading causes of DILI, and the risk of developing severe DILI was associated with pre-existing gallbladder disease, clinical classification, initial TBIL, PT, and ANA. T0.5AST and T0.5GGT might serve as indicators for chronicity.

Diatomic molecules phosphorus monoxide (PO) and phosphorus mononitride (PN) are the main reservoirs of gas-phase phosphorus in interstellar and circumstellar environments, indicating the possibility of forming new phosphorus-bearing molecules through reactions with other interstellar species. To explore the astrochemistry of PO and PN, new simple phosphorus-bearing molecules [O, C, N, P] and [2O, C, N, P] were generated in the gas phase and isolated in cryogenic matrices for characterization with matrix-isolation IR and UV/vis spectroscopy in combination with calculations at the CCSD(T)-F12a/VTZ-F12 level of theory. In an inert argon matrix, OPCN isomerizes to OPNC upon UV-light irradiation at 365 nm, followed by successive isomerizations to PNCO and POCN with concomitant dissociation to diatomic PN and CO under further irradiation at 193 nm. By analogy, the isomerization of O _2 PCN to O _2 PNC and OPNCO followed by fragmentation to OPN/CO and PN/CO _2 occurs in the matrix upon irradiation at 193 nm. In a chemically active CO ice, the photolytic reaction of OPCN with CO yields CO _2 and PCN, and O _2 PCN reacts with CO by forming OPCN and CO _2 , in which the photochemical networks for these P-bearing species linking the astrochemically important PN and PO have been proposed. The experimental identification of these phosphorus-bearing molecules is supported by quantum chemical calculations, and the spectroscopic data may aid in their detection in the interstellar and circumstellar medium.

Abstract Gastric cancer ranks fourth for mortality globally among various malignant tumours, and invasion and metastasis are the major reason leading to its poor prognosis. Recently, accumulating studies revealed the role of reticulon proteins in cell growth and transmigration. However, the expression and biological function of reticulon proteins in human gastric cancer remain largely unclear. Herein, we explored the potential role of reticulon 2 (RTN2) in the progression of gastric cancer. Tissue microarray was used to determine the expression levels of RTN2 in 267 gastric cancer patients by immunohistochemistry. Gastric cancer cell lines were utilised to examine the influences of RTN2 on cellular migration and invasion abilities, epithelial-to-mesenchymal transition (EMT) and signalling pathway. In vivo studies were also performed to detect the effect of RTN2 on tumour metastasis. We found that RTN2 expression was notably upregulated in tumour tissues compared to pericarcinomatous tissues. High RTN2 expression was positively correlated with patients’ age, vessel invasion, tumour invasion depth, lymph node metastasis and TNM stage. Besides, high RTN2 staining intensity was associated with adverse survival which was further identified as an independent prognostic factor for gastric cancer patients by multivariate analysis. And the predictive accuracy was also improved when incorporated RTN2 into the TNM-staging system. RTN2 could promote the proliferation, migration and invasion of gastric cancer cells in vitro and lung metastasis in vivo. Mechanistically, RTN2 interacted with IP3R, and activated ERK signalling pathway via facilitating Ca2+ release from the endoplasmic reticulum, and subsequently drove EMT in gastric cancer cells. Chronic hepatitis B virus (HBV) infection continues to be a major health problem worldwide and remains hard to be cured. Therapy with interferon (IFN) α is an important method for the clinical treatment of chronic hepatitis B. IFNα exhibits direct antiviral effects as well as immunomodulatory activities, which can induce sustained antiviral responses in part of the treated chronic hepatitis B patients. Numerous IFNα subtypes with high sequence identity between 76-96% exist which are characterized by diverse, non-redundant biological activities. Our previous studies have demonstrated that the clinically approved IFNα2 is not the most effective subtype for the anti-HBV treatment among all IFNα subtypes. So far very little is known about the IFNα subtype expression pattern during early HBV infection and the IFNα subtype-specific susceptibility during persistent HBV infection as well as its related cellular mechanism. Here we determined the Ifna subtype mRNA expression during acute and chronic HBV infection by using the well-established hydrodynamic injection (HDI) mouse model and we revealed a transient but strong expression of a panel of Ifna subtypes in the spleen of HBV persistent replication mice compared to HDI controls. Immunotherapy with distinct IFNα subtypes controlled chronic HBV infection. IFNα subtype-mediated antiviral response and immune activation were comprehensively analyzed in an AAV-HBV persistent infection murine model and murine IFNα2 was identified as the most effective subtype in suppression of HBV replication. Further analysis of the immune response revealed a strong immunomodulatory activity of murine IFNα2 on splenic and intrahepatic NK and T cell activation during persistent HBV infection. Taken together, our data provide IFNα subtype-specific differences in the antiviral and immunomodulatory effector responses and a strong expression of all IFNα subtypes in the spleen during persistent HBV infection in mice. This knowledge will support the development of novel immunotherapeutic strategies for chronic hepatitis B infection.

Abstract In this study, an energy‐efficient speed trajectory planner is proposed for high‐speed trains traveling on tracks with steep gradients and speed limits, especially for situations in which the speed limit has been reached, which causes adjoint variable discontinuity during calculation. New optimal switching rules at points where the speed limit is reached on steep tracks are derived by analysing the jump condition of state‐constrained Pontryagin's maximum principle. Accordingly, a novel two‐step algorithm for high‐speed trains, including an optimal‐cruise minimum‐time algorithm and search‐substitution algorithm, is designed to solve dynamic train models considering time‐energy and space‐energy conversions, respectively. Practical case studies demonstrates that the proposed method can save energy by approximately 3% and 10% in comparison to the approximate‐optimal time‐satisfied and minimum running time strategies, respectively. Moreover, the proposed method approximately consumes 0.98% and 1.62% of the computation time taken by discrete dynamic programming and reinforcement learning, respectively.

Background Gastric cancer is the third leading cause of cancer-related mortality worldwide. Most tripartite motif (TRIM) family proteins are known as E3 ubiquitin ligases and considerable previous research has revealed the involvement of TRIM proteins in carcinogenesis. TRIM15 is a protein from the TRIM family and the aim of this study is to investigate the role of TRIM15 in gastric cancer. Methods We conducted immunohistochemical staining to examine TRIM15 expression using samples from Zhongshan Hospital of Fudan University. We also conducted transwell assay as well as western blot by using gastric cancer cells. Results The expression of TRIM15 in gastric cancer tissues was higher than normal tissues. Abstract Background Drug addiction is difficult to overcome. The relapse rate is high, and the negative impact on individuals, families and society is severe, therefore exploring social psychological mechanisms to reduce relapse has very important theoretical and practical value. However, the underlying mechanism by which the interaction between family and individual factors influences the tendency to relapse remain unclear. Thus, the purpose of this paper is to discuss the relationship between family intimacy and relapse tendency of people who use drugs, as well as the mediating effect of psychological capital and the role of self-efficacy in it. Methods A total of 817 male who use drugs were investigated via the Family Intimacy and Adaptability Scale, General Self-Efficacy Scale, Positive Psychological Capital Questionnaire and Relapse Tendency Questionnaire. Using Hayes’s process macro carried out moderated mediation analysis. Results (1) The average family intimacy score of people who use drugs was low. (2) Family intimacy negatively predicted relapse tendency in people who use drugs. (3) Psychological capital mediated the relationship between family intimacy and relapse tendency. (4) The first half of the indirect effect of family intimacy on relapse tendency was regulated by self-efficacy, compared with the low level of self-efficacy, the psychological capital level with high self-efficacy is higher. Conclusion The results of this study suggest that the intimacy between the people who use drugs and their family members should be improved, and the rehabilitation center should take various measures to enhance the psychological capital level and the level of self-efficacy of the people who use drugs, which will be helpful to reduce their relapse tendency.

Safe and effective vaccines against SARS-CoV-2 for children are urgently needed. Here we aimed to assess the safety and immunogenicity of an inactivated COVID-19 vaccine candidate, WIBP-CorV, in participants aged 3-17 years. A randomized, double-blind, placebo-controlled, phase 1/2 clinical trial was conducted in Henan Province, China, in healthy children aged 3-17 years. 240 participants in phase 1 trial and 576 participants in phase 2 trial were randomly assigned to vaccine or control with an age de-escalation in three cohorts (3-5, 6-12 and 13-17 years) and dose-escalation in three groups (2.5, 5.0 and 10.0μg/dose), and received 3 intramuscular injections at day 0, 28, and 56. WIBP-CorV showed a promising safety profile with approximately 17% adverse reactions within 30 days after injection and no grade 3 or worse adverse events. The most common adverse reaction was injection site pain, followed by fever, which were mild and self-limiting. The geometric mean titers of neutralizing antibody ranged from 102.2 to 1065.5 in vaccinated participants at 28 days after the third vaccination, and maintained at a range of 14.3 to 218.2 at day 180 after the third vaccination. WIBP-CorV elicited significantly higher titers of neutralizing antibody in the cohort aged 3-5 years than the other two cohorts. There were no detectable antibody responses in all alum-only groups. Taken together, our data demonstrate that WIBP-CorV is safe and well tolerated at all tested doses in participants aged 3-17 years, and elicited robust humoral responses against SARS-CoV-2 lasted for at least 6 months after the third vaccination. Chronic hepatitis B virus (HBV) infection continues to be a global public health burden. B cells play a pivotal role in mediating HBV clearance and can participate in the development of anti-HBV adaptive immune responses through multiple mechanisms, such as antibody production, antigen presentation, and immune regulation. However, B cell phenotypic and functional disorders are frequently observed during chronic HBV infection, suggesting the necessity of targeting the disordered anti-HBV B cell responses to design and test new immune therapeutic strategies for the treatment of chronic HBV infection. In this review, we provide a comprehensive summary of the multiple roles of B cells in mediating HBV clearance and pathogenesis as well as the latest developments in understanding the immune dysfunction of B cells in chronic HBV infection. Additionally, we discuss novel immune therapeutic strategies that aim to enhance anti-HBV B cell responses for curing chronic HBV infection.

Summary: Background: The efficacy and mechanisms of acupuncture for Crohn's disease (CD) are not well understood. We investigated its effects on symptoms, intestinal microbiota, and circulating inflammatory markers in CD patients. Methods: This 48-week, randomized, sham controlled, parallel-group clinical trial was performed at a tertiary outpatient clinic in China. From April 2015 to November 2019, 66 patients (mean age 40·4, 62·1% were male, all were Han Chinese) with mild to moderate active CD and unresponsive to drug treatment were enrolled and randomly assigned equally to an acupuncture group or a sham group. The treatment group received 3 sessions of acupuncture plus moxibustion per week for 12 weeks and a follow-up of 36 weeks. Clinicaltrials.gov: NCT02559037. Findings: At week 12, the clinical remission rate (the primary outcome) and clinical response rate of acupuncture group were significantly higher than that of sham group, with a difference of 42·4% (95% CI: 20·1%-64·0%) and 45·5% (95% CI: 24·0%-66·9%), respectively, both of which maintained at week 48. The acupuncture group had significantly lower CD activity index and C-reactive protein level at week 12, which maintained at 36-week follow-up. The CD endoscopic index of severity, histopathological score, and recurrence rate at week 48 were significantly lower in acupuncture group. The number of operational taxonomic unit of intestinal microbiota and relative abundance of Faecalibacterium prausnitzii and Roseburia faecis were increased. Plasma diamine oxidase, lipopolysaccharide, and Th1/Th17 related cytokines were decreased in 12-week after acupuncture. Interpretation: Acupuncture was effective in inducing and maintaining remission in patients with active CD, which was associated with increased abundance of intestinal anti-inflammatory bacteria, enhanced intestinal barrier, and regulation of circulating Th1/Th17-related cytokines. Background: Moxibustion is increasingly used for treatment of irritable bowel syndrome (IBS). This study investigated the long-term effects of moxibustion for IBS with diarrhea (IBS-D). Methods: Patients with IBS-D were assigned to receive moxibustion or sham moxibustion (52 each, 3× per week, 6 weeks) and were followed up to 24 weeks. The acupoints were bilateral ST25 and ST36, body surface temperatures at acupoints were 43°C ± 1°C and 37°C ± 1°C for the moxibustion and sham groups, respectively. Primary outcome was changes in IBS Adequate Relief (IBS-AR) from baseline to 6 weeks. Secondary outcomes included the following: IBS symptom severity scale (IBS-SSS), Bristol stool form scale (BSS), IBS quality of life (IBS-QOL), and Hospital Anxiety and Depression Scale (HADS). Nanopore detection technology has now developed into an indispensable tool for single molecule experiments, in which useful information on molecular properties can be obtained from the ion current flow induced by individual molecules, but the relatively high background noise affects the data analysis. Therefore, a nanopore signal noise reduction method based on wavelet transform and Kalman filter is proposed, which can achieve multi-scale decomposition and optimal estimation of the nanopore signal. The real measurement shows that the Kalman filter based on the wavelet mode maxima method reduces the root mean square (RMS) of the background noise by 17.8%, and the noise reduction effect is better than the traditional Kalman filter method.

Abstract Objectives Hepatocellular carcinoma (HCC) associated with major vascular invasion is an advanced stage disease with an extremely poor prognosis and low survival rate. Our study evaluated the survival benefit of radiotherapy (RT) in HCC patients with major vascular invasion through Surveillance, Epidemiology, and End Results database. Methods We analyzed 3181 HCC patients with major vascular invasion cases diagnosed from 2004 to 2013. Patients (N = 308) who underwent RT and patients (N = 2873) who did not receive RT were compared. We successfully analyzed patients using propensity score matching (PSM). Kaplan‐Meier and Cox‐regression analyses were applied to assess prognosis. Parking volume forecast is an indispensable part of the parking guidance and information system (PGIS), which is an important component of the intelligent transportation system (ITS). The parking volume forecast of railway stations’ garages will provide information support for garages’ management and will also be a great convenience for car passengers. Parking garages of railway stations serve passengers to arrive or depart stations by car, and their arrival or departure behaviours definitely affect parking volumes. The study results showed that different parking behaviours have different characteristics of the parking duration category. Therefore, passenger behaviour analysis based on parking duration category analysis and time series similarity measures was introduced into the forecast model in this research. Also, a novel parking volume forecast model based on the long short-term memory (LSTM) is proposed. In this paper, the parking volume data of public parking garages of Hongqiao Railway Station in Shanghai of China is used to verify the model, and the proposed model makes it possible for the accurate and real-time prediction of parking volumes which are divided into different parking duration categories. Compared with the ungrouped data model and the conventional forecast model, the proposed parking volume forecast model based on passenger behaviours with the LSTM network achieves a better performance and provides more accurate prediction.

ObjectiveBased on the theory of perceived organizational support (POS), conservation of resource (COR) and job demands-resources (JD-R) model, this study establishes a moderated mediation model to test the role of job satisfaction in mediating the relationship between POS and job burnout, as well as the role of regulatory emotional self-efficacy (RES) in moderating the above mediating process.MethodsA total of 784 police officers were surveyed with the POS Scale, the Job Burnout Questionnaire, the RES Scale, and the Minnesota Job Satisfaction Questionnaire.Results(1) After controlling for gender, seniority, age, police classification, education, and marital status, regression analysis showed a significant negative correlation between POS and burnout (r = −0.42, p < 0.01), and the former had a significant negative predictive effect on job burnout (β = −0.42, p < 0.001). (2) The mediating effect test shows that job satisfaction plays a partial role in mediating the relationship between POS and job burnout. (3) Through the analysis of the moderated mediation model test, RES moderates the first half of the path of “POS → job satisfaction → job burnout.”ConclusionPOS not only directly affects police job burnout but also indirectly affects police job burnout through job satisfaction. RES enhances the influence of organizational support on job satisfaction. This study indicates the combined effect of POS, job satisfaction, and RES on job burnout and has certain guiding significance for alleviating police job burnout.

BackgroundThe work engagement of police officers pertains to social stability and security, as well as to the orderly operation of the political-economic environment. Although there are many studies on work engagement at present, few studies focus on the influencing factors of police officers’ work engagement. According to the job demands-resources model and the conservation of resources theory, job resources (e.g., perceived organizational support) and personal resources (e.g., regulatory emotional self-efficacy) are important factors influencing work engagement. We assume that a moderated mediation model, in which job satisfaction plays a mediating role in the relationship between perceived organizational support and work engagement, regulatory emotional self-efficacy moderates not only the relationship between perceived organizational support and job satisfaction but also the relationship between job satisfaction and work engagement.Objective and MethodThis study explores the drivers of work engagement through perceived organizational support and regulatory emotional self-efficacy among Chinese police officers using a convenient sampling method to administer a questionnaire to 744 Chinese police officers. A mediated model is proposed to investigate the mediating role of job satisfaction and the regulating role of regulatory emotional self-efficacy.ResultsJob satisfaction mediated a positive relationship between organizational support and work engagement, and the perceived organizational support-job satisfaction and the job satisfaction-work engagement relationships were positively moderated by regulatory emotional self-efficacy, such that these relationships were stronger at higher levels of regulatory emotional self-efficacy. These findings have a practical significance for Chinese police officers’ work engagement advancement.

Background/AimsGastroesophageal variceal hemorrhage is a common complication of portal hypertension. Endoscopic therapy is currently recommended for preventing gastroesophageal variceal rebleed. However, the rate of variceal rebleed and its associated mortality remain concerning. This study is aimed at differentiating patient response to endoscopic therapy based on endoscopic ultrasound (EUS) findings.Methods : One-hundred seventy patients previously treated with repeat endoscopic therapy for secondary prophylaxis were enrolled and classified into two groups based on treatment response. Prior to consolidation therapy, all patients received an EUS examination to observe for extraluminal phenomena. All available follow-up endoscopic examination records were retrieved to validate study results.Results : Of the 170 subjects, 106 were poor responders, while 64 were good responders. Protein is an important component of all the cells and tissues of the human body and is the material basis of life. Its content, sequence, and spatial structure have a great impact on proteomics and human biology. It can reflect the important information of normal or pathophysiological processes and promote the development of new diagnoses and treatment methods. However, the current techniques of proteomics for protein analysis are limited by chemical modifications, large sample sizes, or cumbersome operations. Solving this problem requires overcoming huge challenges. Nanopore single molecule detection technology overcomes this shortcoming. As a new sensing technology, it has the advantages of no labeling, high sensitivity, fast detection speed, real-time monitoring, and simple operation. It is widely used in gene sequencing, detection of peptides and proteins, markers and microorganisms, and other biomolecules and metal ions. Therefore, based on the advantages of novel nanopore single-molecule detection technology, its application to protein sequence detection and structure recognition has also been proposed and developed. In this paper, the application of nanopore single-molecule detection technology in protein detection in recent years is reviewed, and its development prospect is investigated.

The results of existing studies examining associations among work engagement, work-family conflict, and life satisfaction have been inconsistent.Objective: We explored the mechanism of action of police work engagement on life satisfaction, and analyzed the relationships among work engagement, work-family conflict, psychological detachment, and police life satisfaction from the angle of family and work relationships.Methods: A total of 760 police officers completed the Utrecht Work Engagement Scale, Satisfaction with Life Scale, Work-Family Conflict Scale, and Psychological Detachment Scale; 714 questionnaires were valid.Results: Work engagement both directly affected police life satisfaction (β = 0.58, p < 0.001), and indirectly influenced police life satisfaction through work-family conflict (β = -0.07, p < 0.05). Different levels of psychological detachment moderated both the relationship between work engagement and work-family conflict (β = 0.17, p < 0.001), and the relationship between work-family conflict and life satisfaction (β = 0.07, p < 0.05).Conclusion: A moderated mediation model was established. Work-family conflict partially mediates the relationship between work engagement and police life satisfaction. Psychological detachment moderates the first and second half of the mediating process by which work engagement affects police life satisfaction through work-family conflict.

Synthesis of azido(trimethylsilyl)acetylene (6) was performed by treating the iodonium salt 5 with highly soluble hexadecyltributylphosphonium azide (QN3) at −40 °C. Although this product is very unstable, it can nevertheless be trapped by the click reaction with cyclooctyne to give the corresponding 1,2,3-triazole, and also directly characterized by 1H- and 13C-NMR data as well as IR-spectra, which were measured in solution at low temperature and in the gas phase. The thermal or photochemical decay of azide 6 leads to cyano(trimethylsilyl)carbene. This is demonstrated not only by quantum chemical calculations, but also by the trapping reactions with the help of isobutene.

Background. Gastric varices (GV) are associated with high morbidity and mortality in patients with portal hypertension. Endoscopic cyanoacrylate injection is the first-line recommended therapy for GV obliteration. This study aims to explore the reason behind related adverse events and better prevent its occurrence. Methods. A retrospective case series study was conducted from January 1, 2013, to December 31, 2016, to identify patients who experienced severe adverse events secondary to endoscopic cyanoacrylate injection. A literature review of similar cases was performed on two medical databases, Medline and Embase. Results. A total of 652 patients underwent cyanoacrylate injection at our center within the study duration. Five cases of severe adverse events related to the use of tissue adhesives were identified. Detailed clinical presentation, patient treatment, and outcomes were reviewed and analyzed. Twenty-seven similar cases were identified based on the literature review providing further insight into the study. Conclusion. Although rare in incidence, systemic embolism associated with cyanoacrylate injection is often fatal or debilitating. This report may raise awareness in treatment protocol, including the necessity of preoperative angiographic studies, to avoid similar adverse events in clinical practice.

Acupuncture and moxibustion have been shown to be effective in treating Crohn’s disease (CD), but their therapeutic mechanisms remain unclear. Here we compared brain responses to either electro-acupuncture or moxibustion treatment in CD patients experiencing remission. A total of 65 patients were randomly divided into an electro-acupuncture group (n = 32) or a moxibustion group (n = 33), and treated for 12 weeks. Eighteen patients in the electro-acupuncture group and 20 patients in the moxibustion group underwent resting-state functional magnetic resonance imaging at baseline and after treatment. Seed-based analysis was used to compare the resting-state functional connectivity (rsFC) between bilateral hippocampus and other brain regions before and after the treatments, as well as between the two groups. The CD activity index (CDAI) and inflammatory bowel disease questionnaire (IBDQ) were used to evaluate disease severity and patient quality of life. Electro-acupuncture and moxibustion both significantly reduced CDAI values and increased IBDQ scores. In the electro-acupuncture group, the rsFC values between bilateral hippocampus and anterior middle cingulate cortex (MCC) and insula were significantly increased, and the changes were negatively correlated with the CDAI scores. In the moxibustion group, the rsFC values between bilateral hippocampus and precuneus as well as inferior parietal lobe (IPC) were significantly elevated, and the changes were negatively correlated with the CDAI scores. We conclude that the therapeutic effects of electro-acupuncture and moxibustion on CD may involve the differently modulating brain homeostatic afferent processing network and default mode network (DMN), respectively.

Background and aims: Statins are reported to have a beneficial effect on portal hypertension (PTH); however, the exact mechanism remains unknown. Hepatic stellate cells (HSCs) can be activated by transforming growth factor beta (TGFβ) and play an important role in angiogenesis leading to PTH. Statins potently stimulate the transcription factor, Kruppel-like factor 2 (KLF2), which can negatively regulate angiogenesis. Our present study aimed to investigate the anti-angiogenic potential of statins in HSCs through the KLF2 pathway. Method: TGFβ-induced human HSCs were exposed to simvastatin. Cell viability and proliferation were determined by MTT and BrdU-proliferation assays, respectively. Cell migration was investigated using a transwell and wound-healing assays. Gene quantification was measured by real-time polymerase chain reaction. Protein expression was detected by western blot analysis and immunohistochemistry. Inflammatory factors were measured using enzyme-linked immunosorbent assays. Result: Simvastatin was found to reduced cell migration and proliferation and inhibit expression of alpha smooth muscle actin in TGFβ-induced HSCs. Furthermore, simvastatin promoted already increased mRNA and protein levels of KLF2 in TGFβ-induced HSCs. In accordance with KLF2 overexpression, simvastatin increased production of endothelial nitric oxide synthesis (eNOS) and downregulated expression of some proangiogenic proteins, such as vascular endothelial growth factor, hypoxia inducible factor-1a and nuclear factor-kappa B in TGFβ-induced HSCs. At the same time, secretion of interferon-gamma increased in TGFβ induced HSCs, which was decreased by simultaneous addition of simvastatin. Conclusion: Simvastatin suppressed the proangiogenic environment of HSCs activated by TGFβ, and KLF2 pathway is involved in the course.

Chloro- and dichloro-methylsulfonyl nitrenes, CH2ClS(O)2N and CHCl2S(O)2N, have been generated from UV laser photolysis (193 and 266 nm) of the corresponding sulfonyl azides CH2ClS(O)2N3 and CHCl2S(O)2N3, respectively. Both nitrenes have been characterized with matrix-isolation IR and EPR spectroscopy in solid N2 (10 K) and glassy toluene (5 K) matrices. Triplet ground-state multiplicity of CH2ClS(O)2N (|D/hc| = 1.57 cm−1 and |E/hc| = 0.0026 cm−1) and CHCl2S(O)2N (|D/hc| = 1.56 cm−1 and |E/hc| = 0.0042 cm−1) has been confirmed. In addition, dichloromethylnitrene CHCl2N (|D/hc| = 1.57 cm−1 and |E/hc| = 0 cm−1), formed from SO2-elimination in CHCl2S(O)2N, has also been identified for the first time. Upon UV light irradiation (365 nm), the two sulfonyl nitrenes R⁻S(O)2N (R = CH2Cl and CHCl2) undergo concomitant 1,2-R shift to N-sulfonlyamines R⁻NSO2 and 1,2-oxygen shift to S-nitroso compounds R⁻S(O)NO, respectively. The identification of these new species with IR spectroscopy is supported by 15N labeling experiments and quantum chemical calculations at the B3LYP/6-311++G(3df,3pd) level. In contrast, the thermally-generated sulfonyl nitrenes CH2ClS(O)2N (600 K) and CHCl2S(O)2N (700 K) dissociate completely in the gas phase, and in both cases, HCN, SO2, HCl, HNSO, and CO form. Additionally, ClCN, OCCl2, HNSO2, •NSO2, and the atmospherically relevant radical •CHCl2 are also identified among the fragmentation products of CHCl2S(O)2N. The underlying mechanisms for the rearrangement and decomposition of CH2ClS(O)2N and CHCl2S(O)2N are discussed based on the experimentally-observed products and the calculated potential energy profile.

Background. Esophagogastric variceal hemorrhage leads to challenging situation in chronic kidney disease patients on maintenance hemodialysis. Aims. To determine the safety and efficacy of endoscopic approaches to patients with hemodialysis-dependent concomitant with esophagogastric varices. Methods. Medical records were reviewed from January 1, 2004, to December 31, 2015, in our hospital. Five consecutive hemodialysis-dependent patients with variceal hemorrhage who underwent endoscopic treatments were retrospectively studied. Results. The median age of the patients was 54 years (range 34–67 years) and the median follow-up period was 21.3 months (range 7–134 months). All the patients received a total of three times heparin-free hemodialysis 24 hours before and no more than 24 hours and 72 hours after endoscopic treatment. They successfully had endoscopic variceal ligation, endoscopic injection sclerotherapy, and/or N-butyl cyanoacrylate injection. The short-term efficacy is satisfying and long-term follow-up showed episodes of rebleeding. Conclusions. Endoscopic approaches are the alternative options in the treatment of upper gastroenterology variceal hemorrhage in hemodialysis-dependent patients without severe complications.

The relapse of methamphetamine (meth) is associated with decision-making dysfunction. The present study aims to investigate the impact of different emotions on the decision-making behavior of meth users. We used 2 (gender: male, female) x 3 (emotion: positive, negative, neutral) x 5 (block: 1, 2, 3, 4, 5) mixed experiment design. The study involved 168 meth users who were divided into three groups: positive emotion, negative emotion and neutral emotion group, and tested by the emotional Iowa Gambling Task (IGT). The IGT performance of male users exhibited a decreasing trend from Block 1 to Block 3. Female meth users in positive emotion had the best performance in IGT than females in the other two groups. In positive emotion, the IGT performance of female meth users was significantly better than that of men. Female meth users in positive emotion had better decision-making than those in negative or neutral emotion. Female meth users in positive emotion had better decision-making performance than males in positive emotion. In negative and neutral emotions, there was no significant gender difference in decision-making. [ABSTRACT FROM AUTHOR]
