Purpose of Review: After a warm-up period of imaging research, several modalities of positron emission tomography (PET) are under development for evaluating ischemic heart disease. Recent Findings: Several types of well-documented stem/progenitor PET imaging have been utilized for changes in myocardial blood flow and carbohydrate metabolism. Some recent experimental and human studies reported that these data may have beneficial effects on cardiac research. Summary: Although the role of PET in the pathology of ischemic heart disease has not been sufficiently elucidated, many studies attempting imaging research of myocardial metabolism and neural regulation have been reported. Further studies are needed to better evaluate the potential of PET in evaluating ischemic heart disease.

A patient with obvious difficult airway who was received awake tracheal intubation using combination of an Airtraq® optical laryngoscope with smartphone and video flexible endoscope posted for elective surgery of tonsillectomy for OSAHS was reported. Our experiences strongly indicate that fully communication with patients, detailed preoperative assessment of airways and perfect plan, appropriate endotracheal intubation approach and devices play important roles in awake tracheal intubation in difficult airway.

A 56-year-old epileptic patient underwent right hemicolectomy and cholecystectomy surgery under general endotracheal anesthesia. Anesthesia was maintained with sevoflurane, and sufentanil, rocuronium, and dexmedetomidine infusions. After the operation and confirmation of neuromuscular recovery, the patient woke from anesthesia within 15 min and successfully extubated. After the vital signs of patient were stable, the patient was transported to post anesthesia care unit (PACU). 6 h after the surgery, he fell into a stuporous state for lasting 14 h and EEG showed no epileptiform discharges. Stupor did re-occur in 2 days after operation. 36 hours after operation, all signs of the stuporous state resolved spontaneously. Apparent dexmedetomidine-induced stuporous state has not been reported in the human literature.

It is important to realize that characterization of the circadian rhythm patterns of seizure occurrence can implicate in diagnosis and treatment of selected types of epilepsy. Evidence suggests a role for the suprachiasmatic nucleus (SCN) circuits in overall circadian rhythm and seizure susceptibility both in animals and humans. Thus, we conclude that SCN circuits may exert modifying effects on circadian rhythmicity and neuronal excitability during epileptogenesis. SCN circuits will be studied in our brain centre and collaborating centres to explore further the interaction between the circadian rhythm and epileptic seizures. More and thorough research is warranted to provide insight into epileptic seizures with circadian disruption comorbidities such as disorders of cardiovascular parameters and core body temperature circadian rhythms.

Some reports have demonstrated that deep brain stimulation (DBS) is a promising treatment for patients who suffer from intractable anorexia nervosa. However, the nature of DBS may not be viewed as a standard clinical treatment option for anorexia nervosa because of the unpredictable outcome before DBS. Just like DBS in the brain, electroacupuncture at acupoints is also efficient in treating refractory anorexia nervosa. Some neuroimaging studies using functional magnetic resonance imaging, single-photon emission computed tomography (SPECT), and positron emission tomography (PET) had revealed that both DBS and electroacupuncture at acupoints with electrical stimulation are related to the changes in cerebral glucose metabolism. Therefore, we hypothesize that the changes in cerebral glucose metabolism after electroacupuncture might be useful to predict the therapeutic effect of deep brain stimulation for refractory anorexia nervosa.

The factors underlying epilepsy are multifaceted, but recent research suggests that the brain's neural circuits, which play a key role in controlling the balance between epileptic and antiepileptic factors, may lie at the heart of epilepsy. This article provides a comprehensive review of the neural mechanisms and potential treatment of intractable epilepsy from neural inflammatory responses, melanocortin circuits in brain and pedunculopontine tegmental nucleus. Further studies should be undertaken to elucidate the nature of neural circuits so that we may more effectively apply these new preventive and symptomatic therapies to the patient suffering from medically refractory seizures and its complications.

Vestibular nuclei have been identified as a uniform multifunctional structure in order to maintain physiological homeostasis, including the participation of renal sympathetic activity. In this study, the medial vestibular nuclei (MVe) of 10 adult male C57BL/6J strain mice were mapped retrograde using injections of pseudorabies virus (PRV)-614. The virus, injected into the kidney, was specifically transported to the medial vestibular nuclei (MVe). We used a fluorescence immunohistochemistry to characterize the chemical neuroanatomical substrate of MVe innervating the kidney in the mouse. At five days after PRV-614 injection in the kidney, PRV-614 infected neurons were retrogradely labeled in MVeMC and MVePC; PRV-614/tyrosine hydroxylase (TH) double-labeled neurons located predominantly in MVeMC and not in MVePC, whereas PRV-614/tryptophan hydroxylase (TPH) neurons were not localized in MVeMC and MVePC. Our results revealed direct neuroanatomical evidence to identify catecholaminergic projections from the MVeMC to the kidney, suggesting that medial vestibulo-renal pathway may be catecholaminergic.
