Abstract

Measurement of the chemical and isotopic composition of cosmic rays is essential for the precise understanding of their propagation in the galaxy. While the model parameters are mainly determined using the B/C ratio, the study of extended sets of ratios can provide stronger constraints on the propagation models. In this paper, the relative abundances of light-nuclei lithium, beryllium, boron, and carbon are presented. The secondary-to-primary ratios Li/C, Be/C, and B/C have been measured in the kinetic energy range 0.35-45 GeV nucleon-1. The isotopic ratio 7Li/6Li is also determined in the magnetic rigidity interval 2.5-6.3 GV. The secondary-to-secondary ratios Li/Be, Li/B, and Be/B are also reported. These measurements are based on the data collected by the Alpha Magnetic Spectrometer AMS-01 during the STS-91 space shuttle flight in 1998 June. Our experimental results are in substantial agreement with other measurements, where they exist. We describe our light-nuclei data with a diffusive-reacceleration model. A 10%-15% overproduction of Be is found in the model predictions and can be attributed to uncertainties in the production cross-section data. © 2010. The American Astronomical Society.

Abstract

The Alpha Magnetic Spectrometer (AMS) was flown on the space shuttle Discovery during flight STS-91 (June 1998) in a 51.7 deg orbit at altitudes between 320 and 390 km. A search for antihelium nuclei in the rigidity range 1–140 GV was performed. No antihelium nuclei were detected at any rigidity. An upper limit on the flux ratio of antihelium to helium of < 1.1×10^−6 was obtained. The high energy proton, electron, positron, helium, antiproton and deuterium spectra were accurately measured. For each particle and nuclei two distinct spectra were observed: a higher energy spectrum and a substantial second spectrum. Positrons in the second spectrum were found to be much more abundant than electrons. Tracing particles from the second spectra shows that most of them travel for an extended period of time in the geomagnetic field, and that the positive particles (p and e+) and negative ones (e−) originate from two complementary geographic regions. The second helium spectrum flux over the energy range 0.1–1.2 GeV/nucleon was measured to be (6.3 ± 0.9) × 10^−3(m^2 s sr)^−1. Over 90 percent of the helium flux was determined to be 3He at the 90% confidence level

Abstract

The variety of isotopes in cosmic rays allows us to study different aspects of the processes that cosmic rays undergo between the time they are produced and the time of their arrival in the heliosphere. In this paper, we present measurements of the isotopic ratios [superscript 2]H/[superscript 4]He, [superscript 3]He/[superscript 4]He, [superscript 6]Li/[superscript 7]Li, [superscript 7]Be/([superscript 9]Be+[superscript 10]Be), and [superscript 10]B/[superscript 11]B in the range 0.2-1.4 GeV of kinetic energy per nucleon. The measurements are based on the data collected by the Alpha Magnetic Spectrometer, AMS-01, during the STS-91 flight in 1998 June., United States. 4. [Development of a nomogram for predicting the risk of colorectal advanced adenomas].

Abstract

Objective: To explore the risk factors of colorectal advanced adenomas (AA) and construct a nomogram to predict the risk of colorectal AAs. Methods: Clinical data of patients were retrospectively collected who underwent their first colonoscopy from January 2017 to December 2020 in the First Affiliated Hospital of Nanjing Medical University and were pathologically confirmed harboring colorectal polyps. A credible random split-sample method was used to divide data into training and validation cohorts (split ratio=7∶3). Univariate and multivariate logistic regression analysis were used to identify the predictors of colorectal advanced adenomas, and a nomogram was developed based on the above results. The validation cohort was used for internal validation of the nomogram. The discriminatory value of the nomogram was evaluated using the area under the receiver operating characteristic (ROC) curve (AUC). The consistency between actual outcomes and predicted probabilities was evaluated by the calibration curve. The clinical validity of the model was evaluated by the decision analysis curve (DCA). Results: A total of 1 936 patients with colorectal polyps were eligible. Including 1 356 patients in the training cohort (840 males and 516 females), and 580 patients in the validation cohort (379 males and 201 females), with the mean ages of (57.4±9.8) and (57.6±9.7) years, respectively. There were 1 502 (77.6%) patients without AAs and 434 [22.4%,1-9 mm 73(16.8%) cases、>9-<20 mm 271(62.5%) cases、≥20 mm 90(20.7%) cases] patients with AAs. The regression analysis found that age ( OR =1.018, 95% CI: 1.003-1.033), fatty liver ( OR =1.870, 95% CI :1.274-2.744), low-density lipoprotein (LDL) ( OR =1.378, 95% CI :1.159-1.637), fecal occult blood test (FOBT) ( OR =2.597, 95% CI :1.857-3.631), and location of adenomas [proximal ( OR =2.869, 95% CI :1.727-4.764), distal ( OR =2.791, 95% CI :1.721-4.527)] were identified as predictors of colorectal AAs. The AUC of the nomogram was 0.664 (95% CI :0.630-0.698) in the training cohort and 0.640 (95% CI :0.587-0.693) in the validation cohort. The calibration curve showed good consistency between the predicted and actual risk, and the Hosmer-Lemeshow (H-L) test P value was 0.830 and 0.150 in the training cohort and the validation cohort. DCA demonstrated that the nomogram had a better clinical application value. Conclusions: A nomogram with five predictors, including age, fatty liver, LDL, FOBT, and location of adenomas, helped predict the risk of colorectal AAs in patients with polyps and implemented colorectcal cancer stratified screening strategy for colonoscopy in the high-risk population.

Abstract

Background: The long-term functional outcome of discharged patients with coronavirus disease 2019 (COVID-19) remains unresolved. We aimed to describe a 6-month follow-up of functional status of COVID-19 survivors., Methods: We reviewed the data of COVID-19 patients who had been consecutively admitted to the Tumor Center of Union Hospital (Wuhan, China) between 15 February and 14 March 2020. We quantified a 6-month functional outcome reflecting symptoms and disability in COVID-19 survivors using a post-COVID-19 functional status scale ranging from 0 to 4 (PCFS). We examined the risk factors for the incomplete functional status defined as a PCFS > 0 at a 6-month follow-up after discharge., Results: We included a total of 95 COVID-19 survivors with a median age of 62 (IQR 53-69) who had a complete functional status (PCFS grade 0) at baseline in this retrospective observational study. At 6-month follow-up, 67 (70.5%) patients had a complete functional outcome (grade 0), 9 (9.5%) had a negligible limited function (grade 1), 12 (12.6%) had a mild limited function (grade 2), 7 (7.4%) had moderate limited function (grade 3). Univariable logistic regression analysis showed a significant association between the onset symptoms of muscle or joint pain and an increased risk of incomplete function (unadjusted OR 4.06, 95% CI 1.33-12.37). This association remained after adjustment for age and admission delay (adjusted OR 3.39, 95% CI 1.06-10.81, p = 0.039)., Conclusions: A small proportion of discharged COVID-19 patients may have an incomplete functional outcome at a 6-month follow-up; intervention strategies are required., (© 2021. Abstract

The prevalence and outcomes of patients who had re-activation of coronavirus disease 2019 (COVID-19) after discharge remain poorly understood. We included 126 consecutively confirmed cases of COVID-19 with 2-month follow-up data after discharge in this retrospective study. The upper respiratory specimen using a reverse-transcription polymerase chain reaction test of three patients (71 years [60-76]) were positive within 11-20 days after their discharge, with an event rate of 19.8 (95%CI 2.60-42.1) per 1,000,000 patient-days. Moreover, all re-positive patients were asymptomatic. Our findings suggest that few recovered patients may still be virus carriers even after reaching the discharge criteria.

7. [Comparison of clinical efficacy of two different arthroscopic release methods for the treatment of primary frozen shoulder].

Abstract

Objective: To compare clinical effects of inside-out technique and outside-in technique for the treatment of idiopathic frozen shoulder under arthroscopy., Methods: From April 2015 to July 2019, 65 patients with primary frozen shoulder were divided into observation group and control group according to different treatment methods. In observation group, there were 32 cases, including 14 males and 18 females, aged 48 to 64 (54.82±5.35) years old, 18 cases on the right side and 14 cases on the left side. The course of disease was 4 to 10 (7.76±1.19) months. The patients were treated with outside in technique. In control group, there were 33 cases, 16 males and 17 females, aged 45 to 62 (54.64±4.16) years old, 18 cases on the right side and 15 cases on the left side. The course of disease was 5 to 9 (7.65±1.24) months. The patients were treated with inside out technique. The operation time, hospitalization days and treatment cost were compared between the two groups. Constant-Murley function score before and after the operation andthe shoulder joint range of motion one month after operation were compared to evaluate the clinical efficacy., Results: All 65 patients were followed up for 9 to 17 months with an average follow up time of (11.34±2.24) months. Compared with control group, operation time in observation group was shorter[(55.53± 10.23) min vs (85.58±13.39) min], and functional scores of Constant-Murley after surgery were significantly changed in both groups compared with that before surgery( P <0.05). There was no significant difference in functional scores of Constant-Murley between two groups ( P >0.05). There was no significant differences in hospitalization time and treatment cost between two groups ( P >0.05), and there was no significant difference in shoulder abduction, extension flexion and rotation activity between two groups ( P >0.05), but internal rotation of observation group was improved compared with that of control group ( P <0.05)., Conclusion: The two arthroscopic release schemes have achieved satisfactory results for thetreatment of primary frozen shoulder, and the shoulder joint function and pain degree have been effectively improved. Compared with the inside-out technique, the outside in release technique is more direct, the operation is simpler and the operation time is shorter. It has certain advantages in releasing operation for primary frozen shoulder.

Abstract

Aim: To investigate changes in gut microbiota and metabolism during nonalcoholic steatohepatitis (NASH) development in mice fed a methionine-choline-deficient (MCD) diet., Methods: Twenty-four male C57BL/6J mice were equally divided into four groups and fed a methionine-choline-sufficient diet for 2 wk (Control 2w group, n = 6) or 4 wk (Control 4w group, n = 6) or the MCD diet for 2 wk (MCD 2w group, n = 6) or 4 wk (MCD 4w group, n = 6). Liver injury, fibrosis, and intestinal barrier function were evaluated after 2 and 4 wk of feeding. The fecal microbiome and metabolome were studied using 16s rRNA deep sequencing and gas chromatography-mass spectrometry., Results: The mice fed the MCD diet presented with simple hepatic steatosis and slight intestinal barrier deterioration after 2 wk. After 4 wk of feeding with the MCD diet, however, the mice developed prominent NASH with liver fibrosis, and the intestinal barrier was more impaired. Compared with the control diet, the MCD diet induced gradual gut microbiota dysbiosis, as evidenced by a marked decrease in the abundance of Alistipes and the ( Eubacterium ) coprostanoligenes group ( P < 0.001 and P < 0.05, respectively) and a significant increase in Ruminococcaceae UCG 014 abundance ( P < 0.05) after 2 wk. At 4 wk, the MCD diet significantly reduced the promising probiotic Bifidobacterium levels and markedly promoted Bacteroides abundance ( P < 0.05, and P < 0.01, respectively). The fecal metabolomic profile was also substantially altered by the MCD diet: At 2 wk, arachidic acid, hexadecane, palmitic acid, and tetracosane were selected as potential biomarkers that were significantly different in the corresponding control group, and at 4 wk, cholic acid, cholesterol, arachidic acid, tetracosane, and stearic acid were selected., Conclusion: The MCD diet induced persistent alterations in the gut microbiota and metabolome., Competing Interests: Conflict-of-interest statement: There is no conflict of interest in this study.

9. Distribution of Two Subgroups of Human T-Lymphotropic Virus Type 1 (HTLV-1) in Endemic Japan.

Abstract

Endemic areas of human T-lymphotropic virus type 1 (HTLV-1) have been reported in Japan as well as tropical Africa, Central and South America and Melanesia. The existence of two subgroups, i.e., the transcontinental and Japanese subgroups, was reported in Japan. In the present study, we provide data on the ratio of the two subgroups in each endemic area and infection foci and examine the distribution of HTLV-1 in Japan and neighboring areas. A 657 bp fragment of env region of HTLV-1 proviral genome was successfully amplified for 183 HTLV-1 positive DNA samples. The subgroup determination was done by RFLP reactions using endonucleases HpaI and HinfI. The northern part of mainland Kyushu, represented by Hirado and Kumamoto, was monopolized by the Japanese subgroup, while the transcontinental subgroup ranged from 20 to 35% in the Pacific coast areas of Shikoku (Kochi), the Ryukyu Archipelago (Kakeroma and Okinawa) and Taiwan. An interesting finding in the present study is the presence of the transcontinental subgroup in Kochi, suggesting the endemicity of the transcontinental subgroup along the Kuroshio Current.
