This paper summarized the hospital discharge preparedness and nursing for a stroke patient with carotid artery stenting. In addition to treatment and routine nursing, the hospital discharge preparedness and nursing was carried out. The multidisciplinary team had developed the nursing discharge plan based on the risk assessment, in order to provide continuous nursing service, reduce the unplanned hospital or emergency readmission and reduce the economic and psychological burden of the patient. (本文总结1例脑卒中支架植入术后患者应用出院准备服务的护理经验。在配合治疗和常规护理的同时, 护理人员开展出院准备服务, 在患者入院后即对其进行评估, 根据评估结果, 由多学科医疗团队共同讨论, 为患者和家属提供合适的出院后照护计划, 保证患者得到持续而完整的医疗照护服务, 降低患者非计划性再次入院率及再入急诊率, 减轻患者及家属经济和精神负担。)

This paper summarized the practice of evidence-based nursing for a patient with post-stroke dysphagia. Based on the evidence of nursing for post-stroke dysphagia, early screening and evaluation for dysphagia was conducted. Diet management, rehabilitation training and health education were carried out to improve the prognosis, rehabilitation and quality of life of the patient.
