3. Meeting food hygiene challenges in older people : mobilising health assets for health promotion

Abstract

Background: Listeriosis is a serious foodborne infection with significant rates of morbidity and mortality in the older population. The majority of food safety research has focused on food behaviour, practices and perceptions of 'risk'; isolating actions from their social and health context and historical significance. The alternative, positive and novel approach presented in this dissertation is to identify the accumulated resources, or health 'assets', that older people draw upon in daily life at home to protect themselves against foodborne illness informing future health promotion interventions. This research will also further the theoretical development of the 'asset model' and its contribution to health promotion theory and practice. Methods: There were three chronological phases to the study. Phase I: Sought to gather contextual information about the older people recruited in Phase II through the collection of socio-demographic, health and summary current food hygiene asset related data collected through a researcher completed questionnaire at i) five AgeUK lunch clubs ii) one County Council-run lunch club and iii) via a 'University of the Third Age' webpage advert across Buckinghamshire and Hertfordshire. Fifty respondents were recruited via self-selection (20 men, 30 women, mean age of 78.98 years (SD 8.82) mean Visual Analogue Scale of subjective health, 6.89 cm/10cm). Quantitative data were analyzed using SPSS. Phase II: Fifteen semi-structured interviews with older people selected via purposive sampling from the questionnaire cohort to seek their accounts of how food is acquired, stored and cooked and the historical events and influences that shaped these practices (7 men and 8 women, mean age 77.87 (SD 8.06) years, mean Visual Analogue Scale of subjective health, 6.62cm/10cm). Phase III: Three semi-structured interviews with sheltered housing staff in Buckinghamshire recruited via snow-ball sampling. Qualitative data were analyzed using a Grounded Theory approach with NVivo software. Results and Discussion: All of the data indicated that older people have a multiplicity of external food hygiene assets through which to acquire 'safe' and 'fresh' food. Differences in the frequency and type of external asset utilisation were identified between men and women and those reporting severe restrictions in activities of daily living recorded by the EQ5D Quality of Life tool from the questionnaire employed in Phase I. The qualitative data from Phases II and III indicated that food hygiene assets were contextdependent, many being accumulated through the life course and fulfilling non-food related purposes. A number of historically formed internal assets were also identified which served to either facilitate or hinder access to the external food hygiene assets. Members of the social network, specifically the family, were identified as being the principal food hygiene asset throughout the life course from all data sources, fulfilling the newly conceptualised role of 'foodcarers' in the lives of (older) people in a context-dependent manner. Conclusions: The cause of foodborne infections in the older population may be influenced by complex historical factors beyond specific food hygiene knowledge and practice. Current competing or allied concerns in daily life may also serve to motivate or demotivate the employment of 'safe' food hygiene practice. Strategies aimed to reduce the incidence of foodborne illness in the older population could be addressed by placing health promotion within the home setting through the mobilisation of the social network. The contextdependent nature of asset mobilisation has called in to question the validity of some sections of the asset model for use in negotiated small-scale health promotion initiatives and whether the model can serve as a coherent whole.

Abstract

The COVID-19 pandemic resulted in changes in all areas of clinical practice, including clinical research and within the intellectual disability population. While there have been some benefits from this rapid adoption of change, those involved in research have had to overcome a number of additional challenges. These adaptive changes, which have included the use of technology, closure of social spaces, working with specific groups who are more vulnerable to COVID-19, and mask use impairing communication, have had both positive and negative impacts on research. As the pandemic and related restrictions evolve, it is important to examine the changes that have occurred. In the future, the adoption of a hybrid model in research is likely to be a common approach, establishing a balance between technology and in-person interaction. [ABSTRACT FROM AUTHOR]

Abstract

Within current mainstream understandings of patient and public involvement (PPI) in health research, a clear distinction is made between what ‘involvement’ in research is: ‘research being carried out ‘with’ or ‘by’ members of the public rather than ‘to’, ‘about’ or ‘for’ them'1 and what it is not: namely ‘engagement with’ and ‘participation in’ research. Research evidence describes problems than can arise when such distinctions are unclear or misunderstood (often by those new or unfamiliar with PPI); or when distinctions are intentionally blurred e.g. by ‘dual roles’ being created within some projects, where research participants also advise on the conduct of projects. What is less widely examined, however, is the blurring of boundaries between the object of enquiry which is the business of PPI for that project, the data which is the object of qualitative collection involving discussion with participants and the purposeful research activities which are best progressed through engagement with stakeholders. This poster draws upon case study findings from two recent, similar National Institute for Health Research (NIHR)-funded evaluations of PPI in health research: RAPPORT (England-wide) and IMPRESS (regional research programme-specific), pertaining to how researchers (from various disciplines, using various research designs) within different case study research projects can blur the boundaries between qualitative research, stakeholder events and PPI. We pose questions pertaining to the consequences of blurred research design boundaries for the success of outcomes of public-research collaborations. We query why, and to what extent, such distinctions matter in co-producing knowledge and in measuring the impact of various investments in collaborative research activities.

Abstract

Eighteen Caucasian (white, Middle East and Asian) children diagnosed by paediatric rheumatologists in the UK and France as having systemic juvenile idiopathic arthritis (sJIA) were enrolled in this open label, single dose trial. All patients had evidence of continued symptoms and disease activity for at least three months while receiving0.2 mg/kg/day of prednisolone, or its equivalent, prior to recruitment. Twelve patients also received methotrexate (or =20 mg/m2/week). The patients were divided into three groups receiving 2, 4 or 8 mg/kg of MRA (tocilizumab) by intravenous infusion. No evidence of dose-limiting toxicity was observed and there were no dose-limiting safety issues. MRA appeared to be dramatically effective, with clinical and laboratory responses observed by 48 h post infusion, and these improvements continued well after serum MRA was undetectable. Eleven patients achieved the JIA definition of improvement (at least 3 of 6 core set criteria with a 30% improvement and no more than one worsened by 30%) and eight achievedor =50% improvement. There were no observable differences with age. Clinical improvement in these children was observed for up to eight weeks, supporting the hypothesis that IL-6 is a key cytokine in the upregulation of genes crucial in the inflammation processes of sJIA, and the possibility of sequestration of MRA in the extra-vascular compartment needs to be considered.
