5. Phased Progressive Learning with Coupling-Regulation-Imbalance Loss for Imbalanced Data Classification

Abstract

Deep convolutional neural networks often perform poorly when faced with datasets that suffer from quantity imbalances and classification difficulties. Despite advances in the field, existing two-stage approaches still exhibit dataset bias or domain shift. To counter this, a phased progressive learning schedule has been proposed that gradually shifts the emphasis from representation learning to training the upper classifier. This approach is particularly beneficial for datasets with larger imbalances or fewer samples. Another new method a coupling-regulation-imbalance loss function is proposed, which combines three parts: a correction term, Focal loss, and LDAM loss. This loss is effective in addressing quantity imbalances and outliers, while regulating the focus of attention on samples with varying classification difficulties. These approaches have yielded satisfactory results on several benchmark datasets, including Imbalanced CIFAR10, Imbalanced CIFAR100, ImageNet-LT, and iNaturalist 2018, and can be easily generalized to other imbalanced classification models.

Few tools and indicators are available to assess TAO, restricting personalized diagnosis and treatment.Aim: To identify an aptamer targeting thyroid-stimulating hormone receptor (TSHR) and utilize this aptamer to evaluate clinical activity in patients with TAO.Methods: An aptamer targeting TSHR was developed by exponential enrichment and systematic evaluation of TSHR ligands. After truncation and optimization, the affinity, equilibrium dissociation constant, and serum stability of this aptamer were evaluated. The affinity of the TSHR-targeting aptamer to isolated fibrocytes was assessed, as was aptamer internalization by fibrocytes. The mechanism of binding was determined by molecular docking. The correlation between disease manifestations and the percentage of TSHR-positive cells was assessed by correlation analysis.Results: The aptamer TSHR-21-42 was developed to bind to TSHR, with the equilibrium dissociation constant being 71.46 Kd. Isolated fibrocytes were shown to bind TSHR-21-42 through TSHR, with its affinity maintained at various temperatures and ion concentrations. TSHR-21-42 could compete with anti-TSHR antibody, both for binding site to TSHR and uptake by cells after binding. In addition, TSHR-21-42 could bind to leukocytes in peripheral blood, with this binding differing in patients with TAO and healthy control subjects. The percentage of TSHR-positive monocytes, as determined by binding of TSHR-21-42, correlated positively with clinical activity score in patients with TAO, indicating that TSHR-21-42 binding could assess the severity of TAO.Conclusion: This aptamer targeting TSHR may be used to objectively assess disease activity i

Abstract

Objectives Nucleotide-binding oligomerization domain-like receptor family, pyrin domain containing 3-associated autoinflammatory disease (NLRP3 -AID) is a rare autosomal dominant systemic autoinflammatory disease. We aimed to summarize the phenotypic and genotypic features of Chinese adult NLRP3 -AID patients with hearing loss. Methods A retrospective cohort study of twenty-one adult patients with NLRP3 -AID was conducted in Peking Union Medical College Hospital between July 2015 and March 2023. All patients underwent whole exome sequencing and otorhinolaryngologic assessments. Clinical features and therapeutic data were collected and analysed. Results We found that 13/21 (61.90%) of patients had hearing loss with high-frequency impairment in the majority, and most patients presented with vestibular dysfunction as a new finding. The NLRP3 -AID patients with early-onset, cold or stress triggered episodes, red eyes, fatigue, hypopsia and mutations located in the NACHT domain of the NLRP3 protein were more likely to suffer from hearing loss, especially sensorineural hearing loss, perhaps as a result of pathogenic variants of high penetrance. By a series of audiological evaluations, TNF-α inhibitors were confirmed to improve or reverse hearing loss. Conclusions We reported the first cohort of Chinese adult NLRP3 -AID patients with hearing loss and characterized vestibular dysfunction, highlighted the necessity for attention to high-frequency hearing and provided potential alternative treatment. 26. Stereotactic co-axial projection imaging for augmented reality neuronavigation: a proof-of-concept study

Abstract

BACKGROUND: Lack of intuitiveness and poor hand-eye coordination present a major technical challenge in neurosurgical navigation. METHODS: We developed an integrated dexterous stereotactic co-axial projection imaging (sCPI) system featuring orthotopic image projection for augmented reality (AR) neurosurgical navigation. The performance characteristics of the sCPI system, including projection resolution and navigation accuracy, were quantitatively verified. The resolution of the sCPI was tested with a USAF1951 resolution test chart. The stereotactic navigation accuracy of the sCPI was measured using a calibration panel with a 7×7 circle array pattern. In benchtop validation, the navigation accuracy of the sCPI and the BrainLab Kick Navigation Station was compared using a skull phantom with 8 intracranial targets. Finally, we demonstrated the potential clinical application of sCPI through a clinical trial. RESULTS: The resolution test showed that the resolution of the sCPI was 1.3 mm. In a stereotactic navigation accuracy test, the maximum and minimum error of the sCPI was 2.9 and 0.3 mm, and the mean error was 1.5 mm. The stereotactic navigation accuracy test also showed that the navigation error of the sCPI would increase with the pitch and yaw angle, but there was no obvious difference in navigation errors caused by different yaw directions, which meant that the navigation error is unbiased across all directions. The benchtop validation showed that the average navigation errors for the sCPI system and the Kick Navigation Station were 1.4±0.8 and 1.8±0.7 mm, the medians were 1.3 and 1.9 mm, and the average preparation times were 3 min 24 sec and 6 min 8 sec, respectively. The clinical feasibility of sCPI-assisted neurosurgical navigation was demonstrated in a clinical study. In comparison with the BrainLab device, the sCPI system required less time for preoperative preparation and enhanced the clinician experience in intraoperative visualization and navigation. CONCLUSIONS: The sCPI technique can be potentially used in many surgical applications for intuitive visualization of medical information and intraoperative guidance of surgical trajectories.
