2. Gender medicine teaching increases medical students' gender awareness: results of a quantitative survey

Background: Knowledge about gender implications of health is insufficiently integrated into university teaching in Germany. Gender awareness represents a key competence to integrate this knowledge into the medical practice. This study is the first survey of the gender awareness of medical students in a cross-sectional design in Germany.Methods: From April to July 2021, a quantitative cross-sectional survey in an online format using the "Nijmegen Gender Awareness in Medicine Scale" (2008) was conducted at four German universities (Charité Berlin, Friedrich-Schiller-University Jena, Ludwig-Maximilians-University Munich, and the University of Cologne) with a varied implementation of teaching gender medicine. Students indicated their agreement or disagreement with assumptions and knowledge about the influence of gender in everyday medical practice (gender sensitivity), as well as gender role stereotypes towards patients and physicians (gender role ideology). Results: The 750 included participants showed relatively high gender sensitivity and low gender role stereotyping towards patients and physicians. The curricular implementation of gender medicine in the universities showed a significant influence on the students' gender sensitivity, as well as on their gender role stereotyping towards patients. Students who reported having taken classes in gender medicine or stated a definite interest in doing so showed significantly higher levels of gender sensitivity. Cis-males showed significantly lower gender sensitivity and significantly higher gender role stereotyping.Conclusion: Implementation of gender medicine in the medical curriculum, attending courses on gender education as well as one's gender and interest have a significant impact on medical students' gender competencies. Abstract

In dieser Veröffentlichung wird der Einfluss von verteilten Antrieben an einem Flügelsegment auf die Tragflächenaerodynamik experimentell und numerisch untersucht. Für die numerische Modellierung werden die Bereiche Propellerebene, Propellernachlauf und Tragfläche betrachtet. Dabei wird für die Modellierung der Strömung innerhalb der Propellerebene eine Blattelement-Impuls-Theorie (BEMT), für den Propellernachlauf ein auf der Rankine’schen Strahltheorie basierendes Propellerstrahl- Kontraktionsmodell und zur Modellierung der Tragflächenaerodynamik ein Mehrfachtraglinienverfahren verwendet. Zusätzlich werden Experimente durchgeführt, um Versuchsdaten zu gewinnen und die theoretischen Modelle für eine spätere Verwendung in der Flugzeugvorentwurfsumgebung MICADO validieren zu können. Die implementierten Modelle werden mit Ergebnissen aus Windkanalversuchen für ein rechteckiges Flügelsegment mit drei verteilten Antrieben verglichen. Dazu wird ein modulares Flügelkonzept (TETRIS) zur Messung von dynamischen Drücken und aerodynamischen Lasten entwickelt. Die modulare Bauweise erlaubt für die betrachtete Reynoldszahl von ungefähr 5105 eine flexible Anordnung der Komponenten und damit die Untersuchung unterschiedlicher Propeller-Flügel- Konfigurationen mit nur einem Windkanalmodell. Im Vergleich mit den Messdaten konnte gezeigt werden, dass die numerischen Ergebnisse mit dem entwickelten Propellermodell insbesondere im linearen Auftriebspolarenbereich eine geringe Abweichung zu den experimentellen Ergebnissen aufweisen. Die lokalen Auftriebsverläufe können zudem mit einem hohen Detailgrad abgebildet werden, wenngleich signifikante Abweichungen zu den experimentellen Daten auftreten. Diese können primär auf eine Unterschätzung der tangentialen Komponente der durch den Propeller induzierten Geschwindigkeit zurückgeführt werden.

Abstract

This article provides expert opinion on the use of cardiovascular magnetic resonance (CMR) in young patients with congenital heart disease (CHD) and in specific clinical situations. As peculiar challenges apply to imaging children, paediatric aspects are repeatedly discussed. The first section of the paper addresses settings and techniques, including the basic sequences used in paediatric CMR, safety, and sedation. In the second section, the indication, application, and clinical relevance of CMR in the most frequent CHD are discussed in detail. In the current era of multimodality imaging, the strengths of CMR are compared with other imaging modalities. At the end of each chapter, a brief summary with expert consensus key points is provided. The recommendations provided are strongly clinically oriented. The paper addresses not only imagers performing CMR, but also clinical cardiologists who want to know which information can be obtained by CMR and how to integrate it in clinical decision-making

The case of ethical chicken production is analyzed from a systems perspective as well as from the perspective of knowledge management prerequisites of actors involved in the development. We identify the dual purpose chicken approach in the phase of sectoral fermentation, which challenges the overall orientation for the actors involved in innovation and knowledge activities. We draw conclusions for tools that could facilitate the innovation process in order to handle not-knowing, insecurity and ambiguity.

Abstract

Die enantioselektive Synthese bleibt zentrales Thema in metallorganischen, metallfreien and bioorganischen Transformationen sowie in der industriellen organischen Chemie. Glanzpunkte zielmolekulorientierter Synthesechemie waren ein Zugang zu C60, die Route zum Diazonamid A und die Totalsynthese von Vinblastin. Mit „Pyrrolysin” wurde die 22. proteinogene Aminosaure entdeckt. Trotz der immer hoheren Zahl an Testsubstanzen besteht in der medizinischen Chemie ein Engpass an klinisch erfolgreichen Wirkstoffen.

Abstract

This article provides expert opinion on the use of cardiovascular magnetic resonance (CMR) in young patients with congenital heart disease (CHD) and in specific clinical situations. As peculiar challenges apply to imaging children, paediatric aspects are repeatedly discussed. The first section of the paper addresses settings and techniques, including the basic sequences used in paediatric CMR, safety, and sedation. In the second section, the indication, application, and clinical relevance of CMR in the most frequent CHD are discussed in detail. In the current era of multimodality imaging, the strengths of CMR are compared with other imaging modalities. At the end of each chapter, a brief summary with expert consensus key points is provided. The recommendations provided are strongly clinically oriented. The paper addresses not only imagers performing CMR, but also clinical cardiologists who want to know which information can be obtained by CMR and how to integrate it in clinical decision-making

Abstract

Hematite (alpha-Fe2O3) nanoparticles of two different shapes but of same size (ca. 40 nm) were dispersed in PEDOT:PSS matrices in various concentration ranges (0-7 wt%) to study the consequent changes in conductivity in the dark and under solar illumination conditions. Within a distinct range of concentration, a distinct increase in the conductivity was observed for both spherical and cubical particle population. We ascribed this effect to the generalized Poole-Frenkel theory of conduction in conjunction with the basic depletion width properties of heterojunctions and electrostatic dipole moments, and verified our assumptions through data fitting. A difference in conductivity between sphere-and cube-based alpha-Fe2O3-PEDOT: PSS nanocomposites was also observed and ascribed to the electrostatic edge effect on the nanoparticles. The dispersion of alpha-Fe2O3 nanocrystals was confirmed by high-resolution electron microscopy, whereas the electrical properties and modulations thereof were followed by recording current-voltage characteristics.

Abstract

Impaired ventricular myocardial mechanics are observed in patients with repaired tetralogy of Fallot (rTOF). Effects of pulmonary valve replacement (PVR) on ventricular remodeling are controversial. The objective was to assess the impact of surgical PVR on ventricular mechanics in pediatric patients after rTOF. Speckle-tracking analysis was performed in 50 rTOF children, aged 12.6 ± 3.3 years, pre-operatively and 14.5 ± 2.2 months post-PVR. Early post-operative studies 2.2 ± 0.6 months post-PVR were performed in 28 patients. Cardiac magnetic resonance (CMR) pre- and post-PVR was collected. Mid-term post-PVR right ventricular (RV) longitudinal strain increased above pre-operative strain (-19.2 ± 2.7 to -22.0 ± 3.0%, p < 0.001) with increases observed in individual RV segments. Left ventricular (LV) strain did not differ at medium-term follow-up. LV and RV longitudinal strain was reduced early post-operatively, followed by recovery of biventricular systolic strain by mid-term follow-up. CMR RV end-diastolic indexed volumes correlated with RV strain pre-operatively (r = 0.432, p = 0.005) and at mid-term follow-up (r = 0.532, p = 0.001). Volume-loaded RVs had reduced early RV basal longitudinal strain compared to pressure-loading conditions. Reversed basal counterclockwise rotation was associated with lower mid-term global LV and basal RV strain compared to patients with normal rotation. An increase in mid-term global and regional RV strain beyond pre-operative values suggests positive RV remodeling and adaptation occurs in children post-PVR. Patients with larger pre-operative RV volumes had lower RV strain post-operatively. The impact of LV rotation on RV mechanics highlights the presence of ventriculo-ventricular interactions. These findings have important clinical implications in pediatric rTOF patients towards identifying pre-operative factors that predict RV post-operative remodeling. [ABSTRACT FROM AUTHOR]
