18. “Distress is probably the wrong word”: exploring uncertainty and ambivalence in non-clinical voice-hearing and the psychosis continuum.

Abstract

BackgroundMethodsResultsDiscussionNon-clinical voice-hearers (NCVHs) have been the subject of a growing body of psychological research, a primary aim of which is the development of new therapeutic techniques to support those who struggle with voice-hearing. However, relatively little research has examined non-clinical voice-hearing experiences beyond their relationship with clinical voice-hearing.The present study consists of a qualitative re-analysis of 17 semi-structured interviews conducted as part of an NCVH neuroimaging study which included items from the Psychotic Symptoms Rating Scale (PSYRATS) and Positive and Negative Syndrome Scale (PANSS). Results were generated using thematic analysis.Analysis of interview responses showed that participants often experience negative voice content and negative emotion, but have frameworks which normalize a range of voice-hearing experiences. Participants also reported experiences which are not captured by standard clinical scales, as well as reporting comfort with uncertainty and ambiguity surrounding voices.These results indicate that much of the experience of NCVHs may be missed by clinical measures and concepts, suggesting a need to approach them in ways that go beyond typical understandings of the psychosis continuum. Abstract

Summary: Voices in Psychosis: Interdisciplinary Perspectives deepens and extends the understanding of hearing voices in psychosis in a striking way. For the first time, this collection brings multiple disciplinary, clinical and experiential perspectives to bear on an original and extraordinarily rich body of testimony: transcripts of forty in-depth phenomenological interviews conducted with people who hear voices and who have accessed Early Intervention in Psychosis services. Voice-hearing experiences associated with psychosis are highly varied, frequently distressing, poorly understood, and deeply stigmatized, even within mental health services. Voices in Psychosis responds to the urgent need for new ways of listening to and making sense of these experiences. The book addresses the social, clinical and research contexts in which the interviews took place, thoroughly investigating the embodied, multisensory, affective, linguistic, spatial, and relational qualities of voice-hearing experiences. The nature, politics, and consequences of these analytic endeavours is a focus of critical reflection throughout. This volume presents a collection of essays by members and associates of the Hearing the Voice project that were written in response to the transcripts. Each chapter gives a multifaceted insight into the experiences of voice-hearers in the North East of England and to their wider resonance in contexts ranging from medieval mysticism to Amazonian shamanism, from the nineteenth-century novel to the twenty-first-century survivor movement.

Abstract

Summary: This article uses three fictitious case vignettes to raise questions and educate on how clinicians can appropriately approach patients experiencing spiritually significant hallucinations. Religious hallucinations are common but are not pathognomonic of mental illness. They are often intimate experiences for the patient that raise complex questions about psychopathology for clinicians. When assessing a patient with religious hallucinations it is important that clinicians hold at the centre that person's personal experience and create a safe space in which they are listened to and epistemic injustices are avoided. Involvement of chaplaincy services is important not just to support the patient but also to ensure that as clinicians we seek support in understanding the religious nature of these experiences. Abstract

The mRNA-1273 vaccine is effective against SARS-CoV-2 and was granted emergency use authorization by the FDA. Clinical studies, however, cannot provide the controlled response to infection and complex immunological insight that are only possible with preclinical studies. Hamsters are the only model that reliably exhibits severe SARS-CoV-2 disease similar to that in hospitalized patients, making them pertinent for vaccine evaluation. We demonstrate that prime or prime-boost administration of mRNA-1273 in hamsters elicited robust neutralizing antibodies, ameliorated weight loss, suppressed SARS-CoV-2 replication in the airways, and better protected against disease at the highest prime-boost dose. Unlike in mice and nonhuman primates, low-level virus replication in mRNA-1273-vaccinated hamsters coincided with an anamnestic response. Single-cell RNA sequencing of lung tissue permitted high-resolution analysis that is not possible in vaccinated humans. mRNA-1273 prevented inflammatory cell infiltration and the reduction of lymphocyte proportions, but enabled antiviral responses conducive to lung homeostasis. Surprisingly, infection triggered transcriptome programs in some types of immune cells from vaccinated hamsters that were shared, albeit attenuated, with mock- vaccinated hamsters. Triangulating corpus linguistic approaches with other (linguistic and non-linguistic) approaches enhances "both the rigour of corpus linguistics and its incorporation into all kinds of research" (McEnery & Hardie, 2012: 227). Our study investigates an important area of mental health research: the experiences of those who hear voices that others cannot hear, and particularly the ways in which those voices are described as person-like. We apply corpus methods to augment the findings of a qualitative approach to 40 interviews with voice-hearers, whereby each interview was coded as involving ‘minimal’ or ‘complex’ personification of voices. Our analysis provides linguistic evidence in support of the qualitative coding of the interviews, but also goes beyond a binary approach by revealing different types and degrees of personification of voices, based on how they are referred to and described by voice-hearers. We relate these findings to concepts that inform therapeutic interventions in clinical psychology.
