1. Integrating Healthy Nutrition Standards and Practices Into Food Service Contracting in a Large US County Government.

Abstract

PURPOSE AND OBJECTIVES: Although considered a promising model of practice, integrating healthy nutrition standards and practices into a large county governments contracting process with food vendors has not been widely described in empirical literature. We conducted an implementation evaluation project to address this gap. INTERVENTION APPROACH: County of Los Angeles food vendors provide food or meals annually to more than 100,000 employees and millions of clients and visitors. In 2011, the County of Los Angeles Board of Supervisors adopted a policy to integrate healthy nutrition standards and practices into its requests for proposals (RFPs) and contracting process with food vendors. The policy required all contracts awarded to adhere to these new standards. EVALUATION METHODS: In 2011, the Los Angeles County Department of Public Health (DPH) began reviewing RFPs for food services for county departments that procured, served, or sold food. From 2011 through 2021, DPH applied a 4-pronged formative-evaluative approach to help county departments implement the Board of Supervisors policy and ensure that nutritional requirements were appropriately integrated into all RFPs for new and renewing contracts with food vendors. We focused our evaluation on understanding the process and tracking the progress of this policy intervention. Our evaluation included 13 key informant interviews, a 2-part survey, reviews of contract data, and synthesis of lessons learned. RESULTS: Based on reviews and subsequent actions taken on more than 20 RFPs, DPH successfully assisted 7 county departments to incorporate healthy nutrition standards and practices into their food vendor contracts. Implementation of the food policy encountered several challenges, including staffing and training constraints and a limited infrastructure. An iterative approach to program improvement facilitated the process. IMPLICATIONS FOR PUBLIC HEALTH: Although the model for integrating healthy nutrition standards and practices into a government contracting process is promising, more work is needed to make it less resource-intensive and to increase user buy-in.

2. Embedding a Proof-of-Concept Test in an At-Scale National Policy Experiment: Greater Policy Learning but at What Cost to Statistical Power? The Social Security Administration's Benefit Offset National Demonstration (BOND)

Abstract

A randomized experiment that measures the impact of a social policy in a sample of the population reveals whether the policy will work on average with universal application. An experiment that includes only the subset of the population that volunteers for the intervention generates narrower "proof-of-concept" evidence of whether the policy can work for motivated individuals. Both forms of learning carry value, yet evaluations rarely combine the two designs. The U.S. Social Security Administration conducted an exception, the Benefit Offset National Demonstration (BOND). This article uses BOND to examine the statistical power implications and potential gains in policy learning--relative to costs--from combining volunteer and population-representative experiments. It finds that minimum detectable effects of volunteer experiments rise little when one adds a population-representative experiment, but those of a population-representative experiment double or quadruple with the addition of a volunteer experiment.

6. Comparison of Population-Representative and Volunteer Experiments: Lessons from the Social Security Administration's Benefit Offset National Demonstration (BOND)

Abstract

The Benefit Offset National Demonstration (BOND) tested a $1 reduction in benefits per $2 earnings increase above the level at which Social Security Disability Insurance benefits drop from full to zero under current law. BOND included a rare and large "population-representative" experiment: It applied the rule to a nationwide, random sample of beneficiaries. BOND also included a parallel "volunteer" experiment: The same offset rules applied to a nationwide random sample of volunteers. BOND's dual experiments offer a unique opportunity to consider the merits and limitations of these two types of experiments. The findings provide a way to illustrate the limitations of volunteer experiments relative to population experiments when, as in BOND, the goal is to produce impact estimates that are valid for the entire population, not just would be volunteers. We also highlight and compare ethical issues, outreach challenges, and cost considerations for the two types of designs.

8. Phenotypic Evaluation of Rare Cystic Fibrosis Transmembrane Conductance Regulator Mutation Combinations in People with Cystic Fibrosis in Queensland, Australia.

Abstract

Background: Cystic fibrosis (CF) is a multisystem disorder caused by mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene. We describe the distribution of CFTR mutation profiles in sub-tropical Queensland, Australia, and characterise the phenotypes associated with 'rare' CFTR mutation combinations. Methods: We conducted a retrospective observational study to analyse the CFTR mutation profiles of 322 people with CF (pwCF) under the care of a large adult CF centre in Queensland, Australia. Molecular pathology results were available for all identifiable CFTR mutations. The CFTR2 database was utilised to characterise the less common CFTR mutations to define mutation classes and explore associated phenotypic sequelae. Results: In total, eighty-seven different genotypes were identified within our CF cohort, with the most abundant mutation being the F508del mutation, 298/322 (92.5%). Thirty-six pwCF with CFTR mutations are considered to have 'rare' CFTR mutations, and eleven with previously undefined phenotypes. For these eleven pwCF, late diagnosis in adulthood was confirmed in 5/11 pwCF (45.5%) with CFTR modulator therapy only initiated in 5/11 (45.5%). Conclusions: The profile of more common CFTR genotypes within our cohort of adult pwCF living in Queensland, Australia, generally reflects the global predominance of F508del, G542X, G551D, N1303K, and R117H. The phenotypic heterogeneity of disease seen within the eleven pwCF in our cohort with previously undefined CFTR genotypes highlights that rare mutations can also be associated with severe disease and continue to be at risk of delayed diagnosis. Abstract

Background Pseudomonas aeruginosa is a multidrug-resistant pathogen causing recalcitrant pulmonary infections in people with cystic fibrosis (pwCF). Cystic fibrosis transmembrane conductance regulator (CFTR) modulators have been developed that partially correct the defective chloride channel driving disease. Despite the many clinical benefits, studies in adults have demonstrated that while P. aeruginosa sputum load decreases, chronic infection persists. Here, we investigate how P. aeruginosa in pwCF may change in the altered lung environment after CFTR modulation. Methods P. aeruginosa strains (n = 105) were isolated from the sputum of 11 chronically colonized pwCF at baseline and up to 21 months posttreatment with elexacaftor-tezacaftor-ivacaftor or tezacaftor-ivacaftor. Phenotypic characterization and comparative genomics were performed. Results Clonal lineages of P. aeruginosa persisted after therapy, with no evidence of displacement by alternative strains. We identified commonly mutated genes among patient isolates that may be positively selected for in the CFTR-modulated lung. However, classic chronic P. aeruginosa phenotypes such as mucoid morphology were sustained, and isolates remained just as resistant to clinically relevant antibiotics. Conclusions Despite the clinical benefits of CFTR modulators, clonal lineages of P. aeruginosa persist that may prove just as difficult to manage in the future, especially in pwCF with advanced lung disease. 10. Dehiscence : reading, breath and step

Abstract

The dissertation offers its readers a model of how to bring together complex conceptual ideas with personal experience by exploring how reading, breath-orientated meditation and walking in nature may be transformative for self and writing. It is composed in two parts: first, a creative-writing piece — a novella titled Dehiscence — and second, a critical, theoretical piece titled Metaphors and Practices of Self-Becoming. The creative-writing component of the thesis enabled the writer to develop a model of self-becoming based on the metaphor of dehiscence. The critical approach explores how this model was developed and the theoretical ideas underpinning it. The model was in part conceived by the creative writer/researcher's engagement with the writings of Hélène Cixous, Virginia Woolf, Clarice Lispector, Martin Heidegger, Jacques Derrida, Michele Foucault, Jean-Luc Nancy and certain Buddhist scholars, particularly twelfth-century Zen monk, Kuoan Shiyuan. These writers have described through a variety of forms processes of self-becoming and intellectual discovery. This thesis is particularly concerned with how they developed writing styles that accommodate original, personal and spiritual content. In a similar way to Nicholas Royle's employment of the metaphor of veering in his book Veering: A Theory of Literature to 'think afresh' and 'as a productive critical concept', I have chosen the botanical metaphor of dehiscence.1 Dehiscence is a movement whereby the seeds inside a plant grow towards and beyond the outer and encapsulating skin. In the context of this thesis, dehiscence becomes a critically productive metaphor that enables the researcher to slowly trace certain reading and growth journeys. These journeys are towards the edges of knowledge and experience in the hope of discovering something new. Royle writes that, by his sustained and rigorous application of veering as a metaphor in a wide variety of literary texts and genres, he disturbs presuppositions about the literal and the figurative, the physical and the psychological, the external and the internal, the literary and the real. An application of dehiscence as a reading metaphor does not so much seek to disturb, although disturbance may be an effect, but rather to attentively notice that which points beyond dualistic constructions. Twelfth-century Zen poem Ten Bulls depicts a story of spiritual dehiscence via the practice of solitary meditation. The poem and commentary are a model of poetic narration, as well as a source of insight and inspiration. Each poem sets the scene for reflections and analysis of how the practices of breath-orientated meditation, reading and walking in nature assist in the dehiscence of self. The overall arc of self-becoming that this thesis stretches towards in the critical component aims towards love, making it companionable with the narrative trajectory of the creative work. The thesis was developed through an experimental dialogue between creative and critical modes of inquiry. It invites its reader to adjust the lenses of creative and critical modes and to share in a process of unfolding evolution and in the development of a growing sense of awareness, literary embodiment and union.

17. Interprofessional Health Care Delivery: Perceptions of oral health care integration in a Federally Qualified Health Center

Abstract

Purpose: The purpose of this qualitative ethnographic case study was to explore the perceptions of a team of inter-professional healthcare providers regarding how oral health care was integrated into health care provided within a Federally Qualified Health Center (FQHC) in Brighton, Colorado. Methods: Data were gathered through one-on-one, semi-structured personal interviews, which were recorded and professionally transcribed for evaluation. Purposive sampling included physicians, physician assistants, dentists, and dental hygienists. Descriptive analysis was used to describe the sample demographics. An inductive and deductive approach was utilized to assess the qualitative data and subsequently develop themes. Validity was established using triangulation, member checks, and peer review of data and themes by co-investigators. Results: Eight participants (n=8) were interviewed. Subjects were between the ages of 31 and 58 and had been practicing between 5 and 30 years with an average of 13.6 years and had been employed by the FQHC an average of 6.8 years. Thematic analysis revealed seven themes: interprofessional collaboration supports patient care, immediate consultations lead to improved outcomes for all, shared expertise to optimize care delivery, oral health is health, increased communication through collocation, role clarity does not impede team functioning, and mission driven to provide excellent care. These themes support the domains of patient centered care, communication, and the role clarity of the Interprofessional Care Competency Framework and Team Assessment Toolkit (ICCFTAT). Conclusion: Findings from this study can aid FQHC's in the implementation of integrated oral health care delivery systems. Further research is needed to understand how interprofessional health care collaboration (IPHC) affects the team dynamic in FQHC settings. Abstract

The comparability of risk factors, clinical outcomes, and services were examined for Hispanic and non-Hispanic white youths participating in a managed system of care for youths experiencing emotional or behavioral disturbances for at least 6 months. Intra- and inter-group differences were documented for two outcome groups: 34 improvers, whose behavioral indices were within the clinical range at intake and then improved; and 10 deprovers, whose behavioral indices were below clinical range at intake and then deteriorated. The results do not lend themselves to any definitive conclusions, but do suggest that managed system-of-care services may be effective for certain youths. Examination of the youth groups by ethnicity revealed that most Hispanic youths were referred via the probation department, while non-Hispanic whites were referred via mental health, raising the question of bias in agency referrals. Because ethnicity is a significant factor in behavioral and emotional problems and the willingness to seek services, it is critical that agencies seek to provide services before correctional interventions are required. Given the differences in direct therapy hours and the differential sources of referral, it appears that Hispanic and non-Hispanic white clients did not receive equitable care. However, future research should examine how differences in service profiles may result in equity relative to positive outcomes. (Contains 22 references.) (TD)

21. The NIDA Community-Based Outreach Model: A Manual To Reduce the Risk of HIV and Other Blood-Borne Infections in Drug Users.

Abstract

The information in this manual will help community planners, policymakers, and service providers develop and implement programs to better prevent HIV and other blood-borne infections. Findings from more than 30 studies report that community-based outreach is an effective strategy for reaching the drug-using population and for providing them with the means of a behavior change. The model represents community-based outreach as a primary vehicle for accessing out-of-treatment drug users at risk and eliciting their commitments to HIV/AIDS risk reduction, followed by two sessions of risk-reduction counseling and education. Since the model was designed and tested nationally, it can adapt readily to the specific characteristics and needs of any community. Section 1 presents background information on the development of the model. Section 2 outlines the step-by-step procedures for community outreach and personal risk assessment and reduction. 23. Que Dice? Initial Analyses Examining Three Spanish Translations of the CBCL.

Abstract

This study describes preliminary research efforts to examine the validity and utility of three Spanish versions of the Child Behavior Checklist (CBCL), the most extensively used and researched rating scale of children's internalizing and externalizing emotions. The study involved: (1) identification of Spanish items that were significantly different in meaning from the English version; (2) rating by 15 bilingual individuals of the items they judged preferable from all three Spanish versions; and (3) preliminary investigation of CBCL profiles of children whose parents opted to use a Spanish version as compared to those whose parents used the English version. The first analysis indicated that a number of CBCL items required additional clarification and analysis. The rating of individual items on the original Toronto version and the modified Los Angeles and San Francisco versions found only 10 (of 48) items were equally acceptable across all three Spanish versions. Comparison of profiles of 10 Hispanic-American youth who completed the Toronto version with both Caucasian and Hispanic-American youth whose parents/caregivers completed the English version suggested that some differences in Hispanic-American parents' ratings were associated with the language used to solicit the ratings. Abstract

Ovarian reserve and its utilization, over a reproductive life span, are determined by genetic, epigenetic, and environmental factors. The establishment of the primordial follicle pool and the rate of primordial follicle activation have been under intense study to determine genetic factors that affect reproductive lifespan. Much has been learned from transgenic animal models about the developmental origins of the primordial follicle pool and mechanisms that lead to primordial follicle activation, folliculogenesis, and the maturation of a single oocyte with each menstrual cycle. Recent genome-wide association studies on the age of human menopause have identified approximately 20 loci, and shown the importance of factors involved in double-strand break repair and immunology. Studies to date from animal models and humans show that many genes determine ovarian aging, and that there is no single dominant allele yet responsible for depletion of the ovarian reserve. Personalized genomic approaches will need to take into account the high degree of genetic heterogeneity, family pedigree, and functional data of the genes critical at various stages of ovarian development to predict women's reproductive life span.

Abstract

In mammals, there are subsets of genes that are only expressed from one parental allele. This phenomenon, known as genomic imprinting, relies on epigenetic marks to distinguish between the two parental alleles. These epigenetic marks are erased and re-established in the germ line of every generation. Cdkn1c is an imprinted gene in humans and mice. Maternal specific expression of Cdkn1c depends on a germ line DNA methylation mark at a distant cis element (KvDMRl), also known as the imprint control region. When DNA methylation is lost at KvDMRl, Cdkn1c expression is suppressed. On the paternal allele, silencing of Cdkn1c is associated with the expression of a distantly located non-coding RNA (Kcnqlotl/Litl) overlapping the KvDMRl, which is presumed to mediate silencing through the formation of heterochromatin. In somatic cells, long term full silencing of paternal Cdkn1c is maintained by DNA methylation at a CpG island within Cdkn1c. We have shown that embryonic germ (EG) cells, derived from 12.5 dpc primordial germ cells, are a good model for studying the early epigenetic events that lead to heritable silencing of imprinted genes. Cdkn1c expression was suppressed in differentiated EG cells compared to embryonic stem (ES) cells indicating that this domain is silent in EG cells. The Cdkn1c CpG island was hypomethylated in both undifferentiated and differentiated EG cells and was also hypomethylated in ES and androgenetic (AG) stem cells indicating that DNA methylation was not required for silencing Cdkn1c. Suppression of Cdkn1c was likely due to the acquirement of repressive histone marks. H3K27me3 was found enriched at the Cdkn1c promoter in differentiated EG cells and in undifferentiated and differentiated AG cells but was absent in undifferentiated EG cells indicating that this mark was specifically recruited during EG cell differentiation.

Abstract

As shown by IncuCyte Zoom imaging proliferation assays, invasive triple‐negative human breast MDA‐MB‐231 cancer cells treated with sub‐toxic doses (5.0–20 μM, 72 h) of [GaQ3] (Q=8‐hydroxyquinolinato) caused profound morphological changes and inhibition of cell migration, which were likely due to terminal cell differentiation or similar phenotypical change. This is the first demonstration of potential use of a metal complex in differentiation anti‐cancer therapy. Additionally, a trace amount of Cu(II) (0.20 μM) added to the medium dramatically increased [GaQ3] cytotoxicity (IC50~2 μM, 72 h) due to its partial dissociation and the action of the HQ ligand as a Cu(II) ionophore, as shown with electrospray mass spectrometry and fluorescence spectroscopy assays in the medium. Hence, cytotoxicity of [GaQ3] is strongly linked to ligand binding of essential metal ions in the medium, for example, Cu(II). Appropriate delivery mechanisms of such complexes and their ligands could enable a powerful new triple therapeutic approach for cancer chemotherapy, including cytotoxicity against primary tumour, arrest of metastases, and activation of innate and adaptive immune responses. 50. UV irradiance effects on komodo dragon (Varanus komodoensis) vitamin D3, egg production, and behavior: A case study.

Abstract

Modifications to UV irradiance for indoor housed herpetofauna can affect behavior and physiology. Low ultraviolet B (UVB) irradiance can result in vitamin D3 deficiency resulting in calcium metabolism disorders including metabolic bone disease and immune suppression. High UVB can result in skin and eye issues, which can be severe enough to cause shock and death. Using tools available for the assessment of UV light, including Ferguson zones and the UV working tool designed by the British and Irish Association of Zoos and Aquaria, we redesigned lighting in our indoor komodo dragon (Varanus komodoensis) habitat to better suit the UV requirements of this species, while studying changes in behavior and physiology. We measured serum vitamin 25‐hydroxy D3 values in one male and one female komodo dragon before and after they were housed in indoor and outdoor habitats. We also measured behavior changes in our male komodo as he moved from an outdoor habitat, to an indoor habitat with changing UV irradiance. Our female komodo showed a 98% increase in vitamin D3 values after being moved outdoors, and laid her first clutch of eggs. Our male dragon's vitamin D3 remained consistent 200 days after moving inside. He did show increased activity when higher UV irradiance was available. Importantly, we found the UV lamps we used stopped producing desired UV irradiance within 3.5 months of regular use. We suggest all animal care facilities develop UV monitoring programs to research output and longevity of UVB lamps used in indoor herpetofauna habitats. Research Highlights: Our female komodo dragon (Varanus komodoensis) showed an initial rise in serum vitamin D3 values once moved from an indoor habitat to an outdoor habitat. Our male dragon showed changes in his activity patterns suggesting that improved indoor UV irradiance conditions may stimulate activity patterns similar to those seen when housed outdoors.
