Because patients are exposed to one or more food allergens during treatment, OIT is associated with adverse events and can be a cumbersome process for children, their caregivers, and clinicians. However, there have been an overwhelming number of studies that show high efficacy in both single- and multi-allergen OIT, and that quality of life is greatly improved for both patients and their families after undergoing immunotherapy. 3. Efficacy of 18F-FDG PET/CT in investigation of elevated CEA without known primary malignancy

Abstract

Aim: To evaluate the efficacy of 18flurodeoxyglucose positron emission tomography/computer tomography (18F-FDG PET/CT) in investigating patients with elevated carcinoembryonic antigen (CEA) and without known primary malignancy, and the impact of PET/CT findings on patient management. Setting and Design: PET/CT scans done in a tertiary hospital between December 2007 and February 2012 for elevated CEA in patients without known primary malignancy were retrospectively reviewed. Materials and Methods: The PET/CT findings, patients' clinical information, level of CEA, histological diagnosis, and subsequent management were retrieved by the electronic patient record for analysis. Statistical Analysis: Data were analyzed using SPSS version 19. Results: One hundred and one PET/CT scans were performed for patients with elevated CEA. Fifty-eight of these were performed for patients with known primary malignancy and were excluded; 43 PET/CT scans were performed for patients without known primary malignancy and were included. Thirty-three (77%) had a positive PET/CT. Among the 32 patients with malignancy, 15 (47%) suffered from lung cancer and 8 (25%) suffered from colorectal cancer. The sensitivity (97%), specificity (82%), positive predictive value (94%), negative predictive value (90%), and accuracy (93%) were calculated. Thirty (91%) patients had resultant change in management. The mean CEA level for patients with malignancy (46.1 ng/ml) was significantly higher than those without malignancy (3.82 ng/ml) (P < 0.05). In predicting the presence of malignancy, a CEA cutoff at 7.55 ng/ml will achieve a sensitivity of 91% and a specificity of 73%. Conclusion: PET/CT, in our study population, appears to be sensitive, specific, and accurate in investigating patients with elevated CEA and without known primary malignancy. In addition to diagnosis of underlying primary malignancy, PET/CT also reveals occult metastases which would affect patient treatment options.Its role in investigating patients with elevated CEA and without known primary, compared with other investigation modalities, remains to be studied.

In this study, we present our findings on the potential application of adipose-derived stem cells (ASCs) as well as induced cardiac-like progenitors (iCPs) derived from ASCs for the treatment of myocardial infarction. Methods and results: Human bone marrow (BM)-derived stem cells, ASCs, and iCPs generated from ASCs using three defined cardiac lineage transcription factors were assessed in an immune-compromised mouse myocardial infarction model. Analysis of iCP prior to transplant confirmed changes in gene and protein expression consistent with a cardiac phenotype. Endpoint analysis was performed 1 month posttransplant. Significantly increased endpoint fractional shortening, as well as reduction in the infarct area at risk, was observed in recipients of iCPs as compared to the other recipient cohorts. Both recipients of iCPs and ASCs presented higher myocardial capillary densities than either recipients of BM-derived stem cells or the control cohort. Furthermore, mice receiving iCPs had a significantly higher cardiac retention of transplanted cells than all other groups. Conclusion: Overall, iCPs generated from ASCs outperform BM-derived stem cells and ASCs in facilitating recovery from induced myocardial infarction in mice. However, despite 20 years of discourse on pain and dementia, pain is still undetected or misinterpreted in people with dementia in residential aged care facilities, particularly those with communication difficulties. Methods: A topical survey typology with semistructured interviews was used to gather attitudes and experiences of staff from 15 RACF across Northern Sydney Local Health District. Results: While pain is proactively assessed and pain charts are used in RACF, this is more often regulatory-driven than patient-driven (eg, prior to accreditation). Identification of pain and need for pain relief was ill defined and poorly understood. Both pharmacological and nonpharmacological regimes were used, but in an ad hoc, variable and unsystematic manner, with patient, staff, and attitudinal obstacles between the experience of pain and its relief.Conclusion: A laborious “pain communication chain” exists between the experience of pain and its relief for people with severe dementia within RACF. Given the salience of pain for older people with dementia, we recommend early, proactive consideration and management of pain in the approach to behaviors of concern. Individualized pain measures for such residents; empowerment of nursing staff as “needs interpreters”; collaborative partnerships with common care goals between patients where possible; RACF staff, doctors, and family carers; and more meaningful use of pain charts to map response to stepped pain protocols may be useful strategies to explore in clinical settings. 9. Quantum interference between fundamentally different processes is enabled by shaped input wavefunctions

Abstract

We present a general framework for quantum interference (QI) between multiple, fundamentally different processes. Our framework reveals the importance of shaped input wavefunctions in enabling QI, and predicts unprecedented interactions between free electrons, bound electrons, and photons: (i) the vanishing of the zero-loss peak by destructive QI when a shaped electron wavepacket couples to light, under conditions where the electron's zero-loss peak otherwise dominates; (ii) QI between free electron and atomic (bound electron) spontaneous emission processes, which can be significant even when the free electron and atom are far apart, breaking the common notion that electron and atom must be close by to significantly affect each other's processes. Abstract

Frequency multiplication of terahertz signals on a solid state platform is highly sought-after for the next generation of high-speed electronics and the creation of frequency combs. Solutions to efficiently generate extreme harmonics (up to the $31^{\rm{st}}$ harmonic and beyond) of a terahertz signal with modest input intensities, however, remain elusive. Using fully nonperturbative simulations and complementary analytical theory, we show that 3D Dirac semimetals (DSMs) have enormous potential as compact sources of extreme terahertz harmonics, achieving energy conversion efficiencies beyond $10^{-5}$ at the $31^{\rm{st}}$ harmonic with input intensities on the order of $10$ MW/cm$^2$, over $10^5$ times lower than in conventional THz high harmonic generation systems. Our theory also reveals a fundamental feature in the nonlinear optics of 3D DSMs: a distinctive regime where higher-order optical nonlinearity vanishes, arising as a direct result of the extra dimensionality in 3D DSMs compared to 2D DSMs. Our findings should pave the way to the development of efficient platforms for high-frequency terahertz light sources and optoelectronics based on 3D DSMs., Comment: 10 pages, 3 figures

14. Terahertz-optical intensity grating for creating high-charge, attosecond electron bunches

Abstract

Ultrashort electron bunches are useful for applications like ultrafast imaging and coherent radiation production. Currently, however, the shortest achievable bunches, at attosecond time scales, have only been realized in the single or very few electron regime, limited by Coulomb repulsion and electron energy spread. Using ab initio simulations and theoretical analysis, we show that highly-charged bunches are achievable by subjecting relativistic (few MeV-scale) electrons to a superposition of terahertz and optical pulses. Using realistic electron bunches and laser pulse parameters which are within the reach of current compact setups, we provide two detailed examples: one with final bunches of ~1 fC contained within sub-400 as durations, and one with bunches of >25 electrons contained within 20 as durations. Our results reveal a route to achieve such extreme combinations of high charge and attosecond pulse durations with existing technology., Comment: 14 pages, 3 figures

Abstract

Rosacea, a common chronic, predominantly centro‐facial dermatosis, has previously been classified into distinct subtypes with a range of morphological signs that overlap with other inflammatory skin disorders. Recently, there has been a move towards diagnosis of clinical phenotypes, largely driven by a better understanding of the pathophysiology of rosacea and clinical trial endpoints. Despite this, treatment remains a challenge. The Australasian Medical Dermatology Group held a Rosacea workshop in November 2022 to develop a practical narrative. 18. Analysis of the novel Lipid transfer protein Anchored at Membrane contact sites (LAM) family

Abstract

Membrane contact sites are dynamic structures where two organelles come into close proximity to regulate and facilitate the flow of material and information between them. One type of inter-organelle communication is lipid exchange, which is essential for membrane maintenance and in response to environmental and cellular stimuli. We recently discovered a new family of Lipid transfer proteins Anchored at Membrane contact sites (LAMs) that is present in all eukaryotes. LAM proteins are integral Endoplasmic Reticulum (ER) proteins containing at least one domain that is structurally similar to the StARkin domain superfamily, a specialised fold that can bind amphipathic ligands such as lipids. The budding yeast, Saccharomyces cerevisiae, has six such proteins: Lam1p-6p. Lam1p-4p are located at contacts between the ER and the plasma membrane (PM), and Lam1p-3p are implicated in retrograde sterol traffic between the ER and PM. The PM contains a high concentration of sterol where it increases rigidity by altering the packing characteristics of the phospholipids in the bilayer. Sterol is also important in the ER, where its levels are low but it is both synthesised and sensed. However, the mechanism by which sterol traffics between the ER and the PM is unknown. This investigation characterises the phenotype of yeast delete LAM strains on Amphotericin B, a sterol sequestering antifungal agent and shows that the conserved StARkin domain of LAM proteins is responsible for resistance against Amphotericin B. Aspergillus fumigatus, a filamentous fungus, has two LAM proteins and the removal of AfLamA causes a severe growth phenotype. Also, in vitro studies indicate that LAM StARkin domains have a clear sterol transfer activity and a mutation that can diminish the function in vivo and in vitro has been identified. These findings present a new candidate protein family for intracellular sterol trafficking.

20. Australian consensus: Treatment goals for moderate to severe psoriasis in the era of targeted therapies - Considerations for paediatric patients

Abstract

BACKGROUND: Treatment goals have been established in Australia to facilitate the management of adults with moderate to severe psoriasis. The Australasian College of Dermatologists sought to determine if and how these adult treatment goals could be modified to accommodate the needs of paediatric and adolescent patients. METHODS: A modified Delphi approach was used. Comprehensive literature review and guideline evaluation resulted in the development of statements and other questions to establish current clinical practices. Two rounds of anonymous voting were undertaken, with a collaborative meeting held in between to discuss areas of discordance. Overall, consensus was defined as achievement of ≥75% agreement in the range 7-9 on a 9-point scale (1 strongly disagree; 9 strongly agree). RESULTS: Consensus was achieved on 23/29 statements in round 1 and 17/18 statements in round 2. There was a high level of concordance with treatment criteria in the adult setting. The limitations of applying assessment tools developed for use in adult patients to the paediatric setting were highlighted. Treatment targets in the paediatric setting should include objective metrics for disease severity and psychological impact on the patients and their family, and be based on validated, age-appropriate tools. CONCLUSION: While the assessment, classification and management of moderate to severe psoriasis in paediatric patients aligns with metrics established for adults, it is vital that nuances in the transition from childhood to adolescence be taken into account. Future research should focus on psoriasis severity assessment scales specific to the paediatric setting.

Abstract

Graphene plasmons (GPs) have been found to be an exciting plasmonic platform, thanks to their high field confinement and low phase velocity, motivating contemporary research to revisit established concepts in light-matter interaction. In a conceptual breakthrough that is now more than 80 years old, \v{C}erenkov showed how charged particles emit shockwaves of light when moving faster than the phase velocity of light in a medium. To modern eyes, the \v{C}erenkov effect (\v{C}E) offers a direct and ultrafast energy conversion scheme from charge particles to photons. The requirement for relativistic particles, however, makes \v{C}E-emission inaccessible to most nanoscale electronic and photonic devices. We show that GPs provide the means to overcome this limitation through their low phase velocity and high field confinement. The interaction between the charge carriers flowing inside graphene and GPs presents a highly efficient 2D \v{C}erenkov emission, giving a versatile, tunable, and ultrafast conversion mechanism from electrical signal to plasmonic excitation.

Abstract

This crowdsourced project introduces a collaborative approach to improving the reproducibility of scientific research, in which findings are replicated in qualified independent laboratories before (rather than after) they are published. Our goal is to establish a non-adversarial replication process with highly informative final results. To illustrate the Pre-Publication Independent Replication (PPIR) approach, 25 research groups conducted replications of all ten moral judgment effects which the last author and his collaborators had “in the pipeline” as of August 2014. Six findings replicated according to all replication criteria, one finding replicated but with a significantly smaller effect size than the original, one finding replicated consistently in the original culture but not outside of it, and two findings failed to find support. In total, 40% of the original findings failed at least one major replication criterion. Potential ways to implement and incentivize pre-publication independent replication on a large scale are discussed.

Embryonic stem cells (ESC) have been explored as tools forstudying development, as well as, potential sources for a large numberof therapies in regenerative medicine. Traditionally, ESC are culturedon tissue culture plastic, however; it has been recently shown that thestiffness of the environmental substrate can direct the cells towardsvarious cell lineages. Our laboratory is specifically interested in examiningthe combined roles of biochemical and physical signaling incardiac and vascular cell fate and patterning these vascular cells intovascular branch-like tress.Using our novel mouse ESC that expresses aGFP reporter under the Tie-2 and an RFP reporter under alpha smoothmuscle actin and serum-free inductionmediums, we examined the roleof stiffness in the diverging fate of Flk-1 + vascular progenitor cells.The results indicate that both of the Flk-1 + vascular progenitor cellsand human umbilical vein endothelial cells (HUVEC) preferentiallyadhere to 10 kPa compared with the 1 kPa, and 34 kPa compared withthe 10 kPa. We also observed both the GFP/Tie-2 + endothelial-likeand RFP + smooth muscle-like cells outgrowths from the Flk-1 +cells, with the GFP/Tie-2 + cells dominating the cultures, supportingthe role of stiffness in vascular fate. 40. Realizing DIII Class Topological Superconductors using $d_{x^2-y^2}$-wave Superconductors

Abstract

In this work, we show that a quasi-one-dimensional $d_{x^2-y^2}$-wave superconductor with Rashba spin-orbit coupling is a topological superconductor (TS). This time-reversal invariant DIII class TS supports two topologically protected zero energy Majorana fermions at each end of the system. In contrast to proposals using s-wave superconductors in which a strong magnetic field and the fine tuning of the chemical potential are needed to create Majorana fermions, in our proposal, the topologically non-trivial regime can be reached in the absence of a magnetic field and in a wide range of chemical potential.
