9. Health Manpower Study of Selected Health Professions in California. 1976.

Abstract

The need for health personnel in California and recommended targets for expansion of health sciences programs are presented in this report prepared for the California PostsecondarV Education Commission. The report focuses on the role of physicians, mid-level practitioners, nurses, pharmacists, dentists, optometrists, and health sciences education. Supply trends and projections, demand, utilization, clinical training, and special problems, and recommendations are considered for each field. Pertinent national legislation is assessed and it is suggested that the true impact of this legislation will not be felt on the state level for at least a decade. The effect of professional liability insurance rate increases on patterns of physician practice are also presented for the State as well as the impact of national health insurance on health manpower. Health sciences education programs are discussed focusing on the needs of the health care delivery system that are amenable to change via the educational system, and needs of the educational system itself. System changes that are recommended in the education field include improved practitioner attitudes toward primary care, and prevention of disease through patient and health education. Data for health services professionals including: geographic distribution, field, and speciality are appended as well as a list of accredited health professions schools in California and degrees conferred. Abstract

The first 40 pages contain lists of health and hospital occupations, professional health organizations in Kansas, national health professional organizations, educational institutions in the State of Kansas, general hospitals in Kansas, and licensing boards for health professions in Kansas. Where applicable, names of officers, mailing addresses, and some phone numbers are provided. The major portion of the remainder of the handbook provides an introduction to health professions. Each section contains a description of the occupation, comments on the types of position available in that field, and concludes with remarks on the level of education necessary. The second part of this section lists programs for health and hospital occupations available in Kansas. If such training is not available in Kansas, institutions in other states are cited. The type of program, institution name, address, affiliation, tuition, and similar items of interest are listed for each occupation. The handbook also includes information on requirements for certification in the various fields. In concludes with a thirteen-page index on schools of health-related professions in Kansas. This last section, comprising 288 pages, covers health administration, public health, anesthesia, chiropractic, cytotechnology, dentistry, dental assisting, dental hygiene, dietetics, inhalation therapy, medicine, medical assisting and records, laboratory technology, nursing, occupational therapy, operating room technology, optometry, osteopathy, pharmacy, podiatry, practical nursing, prosthetics and orthotics, clinical psychology, radiologic technology, environmental sanitation, social work, speech and hearing, and veterinary medicine. Requirements for certification in the various fields are given. Information for specific programs, in Missouri and elsewhere, includes the name and address of the institution, duration of program, prerequisites, student capacity, tuition, financial assistance, degree or certificate granted, accreditation, and examining boards. Abstract

Abstract: Introduction: Tension‐free mesh repair is currently the gold standard treatment for inguinal hernia. Recent evidence has shown that both open and laparoscopic approaches to inguinal hernia repair can achieve good results. Lots of meshes with different properties are available on the market, but direct comparisons between them are scare. We conducted a prospective randomized controlled trial comparing a partially absorbable lightweight mesh (ULTRAPRO™) and a multifilament polyester anatomical mesh (Parietex™) in laparoscopic total extraperitoneal inguinal hernia repair. Methods: This study was a single‐center, prospective randomized controlled trial to compare the surgical handling and clinical outcomes between two different types of meshes. All operations were performed using a standardized operative protocol. This study was approved by the Institutional Review Board of the Hong Kong East Cluster Health Service in 2009 (reference number: 2009‐087). The study was registered in the Australian New Zealand Clinical Trial Registry (ACTRN12610000031066). Results: From October 2009 to August 2011, 85 laparoscopic total extraperitoneal inguinal hernia repairs were performed. The mean mesh handling time was 152 s for the ULTRAPRO group and 206 s for the Parietex group (P = 0.001). There were three cases of seroma formation in the ULTRAPRO group and nine in the Parietex group (P = 0.02). The overall recurrence rate was 2.5%. Conclusion: It took less time to manipulate the flat mesh (ULTRAPRO) than the anatomical mesh (Parietex) in laparoscopic total extraperitoneal inguinal hernia repair, but the time difference was small. Lightweight mesh and heavyweight mesh offered similar clinical outcomes in terms of discomfort sensation and foreign body sensation during long‐term follow‐up. [ABSTRACT FROM AUTHOR]

Abstract

The youth made up a fifth of Singapore's resident population in 2012. Many are provided with ample educational opportunities preparing them and giving them a head start in technical and professional lifelong employment and a predictable career path. But with changing demographics of both parents working in a nuclear family with small sibship, greater social affluence, increased emphasis on academic excellence, high penetration of Internet and personal social media use, greater awareness of the need for personal space and rights, and the convergence of protective and risk factors impact on youths' emotional and mental well-being vary differently across individuals and society. The government, communities, and parents are responding to these new challenges at the social policy level and work in close partnership with volunteer welfare organisations (VWOs), developing youth social support systems, further evolving the comprehensive national education system, and strengthening family fabric structure and parenting like never before, to mitigate the changing tide. The work that is still evolving as much is needed by given the rising tide, but we are seeing some positive results. A Singapore Youth Resilience Scale was developed to further investigate individual protective factors and understand how incorporating culture and diversity factors would contribute towards resilience among Singaporean youth. Abstract

Background and Purpose: Diffusion tensor imaging (DTI) was used as a noninvasive method to evaluate the anatomy of the corticospinal tract (CST) and the pattern of its degeneration in amyotrophic lateral sclerosis (ALS). Methods. Fourteen patients with ALS and 15 healthy controls underwent DTI. Parameters reflecting coherence of diffusion (fractional anisotropy, FA), bulk diffusion (apparent diffusion coefficient, ADC), and directionality of diffusion (eigenvalues) parallel to (λ∥) or perpendicular to (λ⊥) fiber tracts were measured along the intracranial course of the CST. Results: FA and λ∥ increased, and ADC and λ⊥ decreased progressively from the corona radiata to the cerebral peduncle in all subjects. The most abnormal finding in patients with ALS was reduced FA in the cerebral peduncle contralateral to the side of the body with the most severe upper motor neuron signs. λ∥ was increased in the corona radiata. Internal capsule FA correlated positively with symptom duration, and cerebral peduncle ADC positively with the Ashworth spasticity score. Conclusion: There is a spatial dependency of diffusion parameters along the CST in healthy individuals. Evidence of intracranial CST degeneration in ALS was found with distinct diffusion changes in the rostral and caudal regions. [ABSTRACT FROM AUTHOR]
