PurposeNo approved targeted therapy for the treatment of patients with neuroblastoma RAS viral (v-ras) oncogene homolog (NRAS)-mutant melanoma is currently available.Patients and methodsIn this phase Ib escalation/expansion study (ClinicalTrials.gov identifier: NCT02974725), the safety, tolerability, and preliminary antitumor activity of naporafenib (LXH254), a BRAF/CRAF protein kinases inhibitor, were explored in combination with trametinib in patients with advanced/metastatic KRAS- or BRAF-mutant non-small-cell lung cancer (escalation arm) or NRAS-mutant melanoma (escalation and expansion arms).ResultsThirty-six and 30 patients were enrolled in escalation and expansion, respectively. During escalation, six patients reported grade ≥3 dose-limiting toxicities, including dermatitis acneiform (n = 2), maculopapular rash (n = 2), increased lipase (n = 1), and Stevens-Johnson syndrome (n = 1). The recommended doses for expansion were naporafenib 200 mg twice a day plus trametinib 1 mg once daily and naporafenib 400 mg twice a day plus trametinib 0.5 mg once daily. During expansion, all 30 patients experienced a treatment-related adverse event, the most common being rash (80%, n = 24), blood creatine phosphokinase increased, diarrhea, and nausea (30%, n = 9 each). In expansion, the objective response rate, median duration of response, and median progression-free survival were 46.7% (95% CI, 21.3 to 73.4; 7 of 15 patients), 3.75 (95% CI, 1.97 to not estimable [NE]) months, and 5.52 months, respectively, in patients treated with naporafenib 200 mg twice a day plus trametinib 1 mg once daily, and 13.3% (95% CI, 1.7 to 40.5; 2 of 15 patients), 3.75 (95% CI, 2.04 to NE) months, and 4.21 months, respectively, in patients treated with naporafenib 400 mg twice a day plus trametinib 0.5 mg once daily.ConclusionNaporafenib plus trametinib showed promising preliminary antitumor activity in patients with NRAS-mutant melanoma. Prophylactic strategies aimed to lower the incidence of skin-related events are under investigation.

Ipilimumab is a fully human antibody that blocks CTLA-4 and has proven to extend overall survival in patients with unresectable stage III or stage IV melanoma. There is a need for well-documented pharmacodynamic markers together with potential predictive biomarkers that may allow for pretreatment selection of patients and screening for IRAE. Most of the recently published immune monitoring studies focus mainly on the effect that ipilimumab has on T cell populations. An in-depth immune monitoring study was conducted in advanced melanoma patients undergoing treatment with ipilimumab. The main focus of this work was to analyze the effect of ipilimumab treatment on peripheral blood MDSC populations as well as T cells. Materials and methods Six patients with advanced stage melanoma received ipilimumab treatment at 3 or 10 mg/kg doses as part of double blind randomized trial CA184-169. Twenty-four additional patients received 3 mg/kg doses. Blood samples were collected before treatment (baseline) and at the time of the second and fourth ipilimumab doses. Peripheral blood mononuclear cells were isolated by density gradient centrifugation and stained for flow cytometric analysis within two hours of sample collection. Results In the thirty patients included, median OS was 38 weeks from the start of treatment. Adverse events were observed in 12 patients (40%), including seven grade III-IV (23%). Patients were classified according to their response as PD (progressive disease, 57%), SD (stable disease, 23%) and PR (partial response, 20%). ICOS+CD4+ T cell frequency was significantly increased after Ipilimumab treatment, suggesting an increase in the activation of this cellular population. The endpoint mean frequency of Tregs was significantly lower than the baseline. In addition to this, changes in the suppressive myeloid compartment were also observed, with significant reductions in the frequency of granulocytic MDSCs. The suppressive potential of both granulocytic and monocytic MDSCs was also diminished, as the frequencies of Arg1+ and iNOS+ cells were reduced after treatment. Further analysis showed that the populations of granulocytic MDSCs and CD4+ICOS+ T cell populations presented a statistically significant correlation. Conclusions Ipilimumab treatment can be acting in two very distinct ways: on one hand it is increasing the activation status of T cells and on the other hand, it is decreasing the frequencies of suppressive cells such as Tregs and granulocytic MDSCs. Introduction: Real-life data on vismodegib in advanced basal cell carcinoma (aBCC) are limited. Optimal treatment duration is left to the discretion of the physician., Objectives: To assess the effectiveness, safety and treatment pattern for vismodegib in aBCC in clinical practice., Methods: In this multicenter, non-interventional, prospective study, 49 Swedish patients planned for vismodegib treatment were included. The treatment pattern observed was treatment until remission, allowing unlimited discontinuations/pauses., Results: The majority of patients (93.8%), discontinued at least once during the study. Compared to earlier studies there was a decrease of more than 2 months with actual drug intake, reducing the patients burden and costs, at the same time as a high number of responses were seen (87.8%). Median progression-free-survival was 16.7 months, and 90% of the patients were alive at 13.3 months. Ten patients were re-challenged with vismodegib at recurrence or progression, resulting in five partial remissions and three complete remissions., Conclusions: Clinical response rates with vismodegib for aBCC were comparable to those of similar trials despite a shorter and more intermittent treatment duration. The majority of re-challenges lead to partial or complete remissions.

Despite the success of immune checkpoint blockade in melanoma, the majority of patients do not respond. We hypothesized that the T and NK cell subset frequencies and expression levels of their receptors may predict responses and clinical outcome of anti-CTLA-4 treatment. We thus characterized the NK and T cell phenotype, as well as serum levels of several cytokines in 67 melanoma patients recruited in Italy and Sweden, using samples drawn prior to and during treatment. Survival correlated with low expression of the inhibitory receptor TIM-3 on circulating T and NK cells prior to and during treatment and with the increased frequency of mature circulating NK cells (defined as CD3 - CD56 dim CD16 + ) during treatment. Survival also correlated with low levels of IL-15 in the serum. Functional experiments in vitro demonstrated that sustained exposure to IL-15 enhanced the expression of PD-1 and TIM-3 on both T and NK cells, indicating a causative link between high IL-15 levels and enhanced expression of TIM-3 on these cells. Receptor blockade of TIM-3 improved NK cell-mediated elimination of melanoma metastasis cell lines in vitro . These observations may lead to the development of novel biomarkers to predict patient response to checkpoint blockade treatment. They also suggest that induction of additional checkpoints is a possibility that needs to be considered when treating melanoma patients with IL-15.
